1.Clinical analysis of 49 cases of necrotizing pneumonia in children

Objective To explore the clinical characteristics,pathogens,inflammatory biomarkers,therapeutic methods and prognosis of necrotizing pneumonia (NP) in children.Methods The clinical data of children with NP who were admitted to the Department of Pediatrics of Shengjing Hospital of China Medical University from October 2010 to October 2015 were collected.The data included demographic data,laboratory test results,intrapulmonary complications,therapeutic methods and so on were analyzed,retrospectively.Results Forty-nine pediatric patients with NP were enrolled,31 cases were boys,18 cases were girls;the average age of the patients were 2.5 years (4 months-13 years).The average febrile time were (15.08 ± 5.92) d,and the hospital stay was (21.19 ± 10.83) d,respectively.The median value of peripheral blood leukocyte count was 17.7 × 109/L,the average of neutrophils ratio was (67.62 ± 18.52)%,and the median value of C-reactive protein (CRP) and procalcitonin (PCT) was 97.9 mg/L and 0.54 μg/L,respectively.Nineteen cases had Mycoplasma pneumoniae (MP) infection,16 cases with pneumonia chlamydia infection,9 cases complicated with MP and chlamydia pneumoniae infection,and 8 cases with positive bacterial culture,and 10 cases suffered from the intrapulmonary complications.Forty-seven cases got better prognosis with the treatment of antibiotics,glucocorticoids,intravenous immunoglobulin and other comprehensive therapeutic methods such as bronchoalveolar lavage with fiber bronchoscope and closed thoracic drainage.Conclusions NP is a severe complication of community-acquired pneumonia in children,and the occurrence of NP should be alerted in the children who have persistent high fever,higher inflammatory index and pleural effusion,but the great majority of patients do recover fully after comprehensive treatment.
2.Conflict processing of the frontal cortex and aging change: event-related potentials study.

OBJECTIVETo explore the working mechanism and age-related change of the conflict processing system of the frontal cortex.
METHODSFifteen normal elderly people and 15 youth were performed a modified Eriksen flanker paradigm, while event-related potential (ERPs), which include 32 systerm electroencephalography, reaction time and correct rate were recorded.
RESULTSThe elderly group showed a distinct effect of reaction time and effect of conflict in the respond level compared with the youth group. The elderly group had a longer time window of N380 and same amplitude as the youth group. Low resolution brain electromagnetic tomography (LORETA) showed the bilateral temporal lobe and the dorsolateral prefrontal cortex (especially right) were activated in the youth group, while the left temporal lobe, the left dorsolateral prefrontal cortex and left medial frontal gyrus were activated in the elderly group.
CONCLUSIONWhen conflict stimuli existed in the response level, old people showed frontal interference control hypofunction. N380 reflected the activation of the left dorsolateral prefrontal cortex and the left temporal lobe during response selection and executive control processing in older people.

3.Study on the influences of lacrimal secretion and tear film after interferon-alpha therapy in patients with chronic viral hepatitis

Objective To analyze the influences of lacrimal secretion and tear film after interferon-alpha therapy in patients with chronic viral hepatitis.Methods Sixty patients with chronic viral hepatitis who would accept the interferon-alpha therapy,including 30 cases of hepatitis B and 30 chronic viral hepatitis C,visited our department of ophthalmology and were enrolled in this study.All cases received eye examination,including medical history,slit lamp examination,corneal fluorescein staining ( FL) ,tear film break-up time ( BUT) and schirmer test before inter-feron-alpha therapy and two months after the therapy.Results In the 60 cases of chronic viral hepatitis,there were 19 dry eye cases (31.67%) and 41 non-dry eye cases (68.33%).In dry eyes group,9 of them (47.37%) were hepatitis B patients and 10 of them were hepatitis C patients (52.63%) .There was significant difference in FL grade (t=-5.288,P<0.05),BUT (t=5.799,P<0.05),Schirmer Ⅰ(t=5.319,P<0.05) test in hepatitis B before and two months after interferon-alpha therapy.Statistical significance had been found before and two months after therapy in FL grade (t=-9.103,P<0.05),BUT (t=5.682,P<0.05),SchirmerⅠtest (t=4.592,P<0.05). The incidence of dry eye syndrome in hepatitis B patients increased from 26.70%to 53.30%two months after treated with interferon-alpha,while the incidence of dry eye syndrome in hepatitis C patients increased from 33.30% to 60.00% two months after treated with interferon-alpha.Conclusion Dry eye syndrome is one of common complica-tions in patients with chronic viral hepatitis.Interferon-alpha therapy would influence the lacrimal secretion and dam-age tear film,which increased the dry symptoms in patients.We should pay attention to the side effects of interferon-alpha therapy.
4.Effects of cromolyn sodium on forebrain ischemia-reperfusion injury in gerbils

Objective To investigate the effects of cromolyn sodium(CS)on forebrain ischemiareperfusion injury in gerbils.Methods Twenty-four male gerbils weishing 55-70 g were randomly divided into 3 groups(n=8 each):group Ⅰ sham operation;groupⅡ I/R and group Ⅲ CS 50 mg/kg+I/R.Forebrain ischemia was produced by occlusion of bilateral common carotid arteries for 10 min and confirmed by isoelectricity of EEG. CS was injected via lingual vein at 0 and 1 h of reperfusion.All the animals were killed at 2 h of reperfusion for determination of cerebral cortex injury score,cerebral water content[(wet weight-dry weight)/wet weight×100%],cerebral MDA content,SOD activity and histamine content.Results The cerebral cortex injury 8core,cerebral water content, MDA content and histamine content were significantly increased while SOD activity was signiiieantly decreased in I/R group(Ⅱ)aa compared with sham operation group(Ⅰ)(P<0.05 or 0.01).CS significantly attenuated the I/R-induced changes(P<0.05 or 0.01).Conclusion CS can attenuate the forebrain ischemia-reperfusion injury by reducing histamine and oxidative response.
5.Analysis of related risk factors and control measures of senium patients with type 2 diabetes

Objective To explore the related risk factors and control measures for senium patients with type 2 diabetes and to provide basis for making control and prevention measures.Methods A retrospective case-control study was conducted.548 cases of type 2 diabetes patients in our hospital were selected as case group and other 640 cases of normal people who accepted physical examination in our hospital were selected as control group.Results Exposure rates of smoking,drinking,obesity,hypertension,hypedipidemia and family history in case group were obviously higher than those of control group(P <0.05).Conclusion Smoking,drinking,obesity,hypertension,hyperlipidemia and family history were risk factors of type 2 diabetes.Comprehensive intervention measures related to the risk factors,such as maintaining a good way life,were importantto prevent and control type 2 diabetes.
6. Cardiac magnetic resonance in evaluating cardiac function: The status and advance

Clinical evaluation of cardiac function is routine and important. Cardiac magnetic resonance (CMR) cine sequence can obtain dynamic images of the whole cardiac cycle. Based on echocardiographic techniques, CMR has been used to establish a set of scanning sequences and an evaluation index system to evaluate cardiac function, including ejection fraction, ventricular mass index, strain, interventricular septum curvature ratio, tricuspid annular plane systolic excursion and vascular compliance. Understanding the principles, significance and indications of these indicators is helpful to optimize the scanning sequence and make timely and effective diagnosis, which is of great significance for clinical treatment and prognosis evaluation. This review sums up the status and progress of clinical application of CMR in evaluation of cardiac function.
7.The significance and expression of HIF-1α and COX-2 in the retinal neovascularization

AIM: To investigate the expression and significance of hypoxia-inducible factor -1α (HIF-1α)and Cyclooxygenase-2(COX-2) in the retinal neovascularization.METHODS: Mouse model of hyperoxia-induced ischemic retinopathy were established. A novel flucorescein-dextran perfusion method has been developed to assess the vascular pattern. histological methods were used to count blood vessel profiles in the inner retina with HE staining.HIF-1α and COX-2 expression in the retina was determined by immunohistochemical method.RESULTS: Lots of neovascularization were seen in hyperoxia group. The number of nuclei of proliferative retinal vessels increased significantly as compared with normal control group (P<0.001). The expression of HIF -1α was noted in the ganglion cells and retinal blood vessels. The expression of COX-2 was noted in the inner nuclear layer and ganglion cells and retinal blood vessels.CONCLUSION: There are the expression of HIF-1α and COX-2 in the retinal neovascularization and they have correlation.
8.The relationship between thrombin activatable fibrinolysis inhibitor and coronary heart disease

Objective:
To analyze the relationship between thrombin activatable fibrinolysis inhibitor ( TAFI ) and coronary heart disease ( CHD ), and to provide evidence for the prevention of CHD.
Methods:
The patients with CHD in Fushun Central Hospital in Liaoning Province were selected as the case group, the patients without CHD in the same hospital and period were selected as the control group. The demographic information and clinical examination results ( serum TAFI, lipid, glucose, etc. ) were collected to analyze the association between TAFI and CHD by logistic regression models.The multivariate logistic regression analysis was used to explore the relationship between TAFI and CHD.
Results:
There were 222 cases, including 100 cases of stable angina, 44 cases of unstable angina and 78 cases of acute myocardial infarction, and 222 controls. The median ages of cases and controls were 62 and 57 years old. The results of multivariate logistic regression analysis showed that serum TAFI>22.88 μg/mL ( P75 of controls ) was associated with the risk of CHD ( OR=1.619, 95%CI: 1.011-2.593 ), unstable angina ( OR=2.917, 95%CI: 1.433-5.939 ) and acute myocardial infarction ( OR=2.626, 95%CI: 1.007-6.847 ).
Conclusion
The high level of TAFI is related to CHD, unstable angina and acute myocardial infarction.
9.Effects on the expression of lipopolysaccharide-induced inflammatory cytokines mediated by bovine bactericidal/permeability-increasing protein.

Bactericidal/permeability-increasing protein (BPI) can bind to and specifically neutralize lipopolysaccharide (LPS) from the outer membrane of Gram-negative bacteria. In order to evaluate potent LPS-neutralizing activity of bovine BPI, the full-length coding sequence (1 449bp) or 714 bp N-terminal coding sequence (BPI714) of bovine BPI was transfected into mHEK293 cells and the expression of LPS-induced inflammatory cytokines was studied. First, we constructed the lentiviral expression vectors and generated mHEK293 cells stably expressing recombinant bovine BPI or BPI714. Then, we detected the expression of IL-8, IL-1β, TNF-α, NF-κB-1 and NF-κB-2 genes by real-time PCR at 0, 1, 3, 6, 12, 24, 36 and 48 h post of LPS induction in cells with or without recombinant bovine BPI or BPI714 ectopic expression, respectively. In response to LPS, the robust abundance of inflammatory cytokines including IL-8, IL-1β, TNF-α and NF-κB-2 was observed in wild type mHEK293 cells at eachtime point. On the contrary, mRNA abundance of IL-8, TNF-α and NF-κB-2 in transfected mHEK293 cells showed no significant changes at each indicated time point. Our results demonstrated that recombinant bovine full length BPI or BPI714 down-regulated the expression of inflammatory cytokines and revealed that either of bovine BPI or BPI714 was able to inhibit the immune respond stimulated by LPS. This study provides evidence for further investigating the mechanisms and application of BPI/LPS-neutralizing activity and also documents a reliable approach for analysis of the efficacy of antibacterial proteins.

10.Applications of three-dimensional visualization technology in individualized diagnosis and treatment oh hepatic hemangioma

Objective To investigate the values of three-dimensional visualization technology in the diagnosis and surgical treatment of hepatic hemangioma. Methods Thirty two patients with hepatic hemangioma who had been hospitalized during the period from July 2010 to January 2014 in our hospital were scanned by 64-slice spiral computerized tomography (CT) before surgical treatment. Three-dimensional (3D) reconstruction based on the CT data was carried out to achieve dignosis and surgical planning. Assisted with the 3D model , we chose the best surgical procedure for liver resection, protecting the major blood vessels near hemangioma and retaining normal liver tissue as far as possible. Results The location, size and shape of hepatic hemangioma, vascular variation, and spatial relationship with intrahepatic vessel were shown factually by three-dimensional reconstruction. All the hemangiomas were preoperatively assessed to be resectable. The compliance rate for preoperative surgical planning to actual surgery was 100%. Under assistance of the 3D model during surgery , 14 patients received laparoscopic hepatectomy and 18 underwent hepatectomy. Pringle′s maneuver was applied in 18 patients , with blocking time of (15.32 ± 7.12) min and blood loss of (188.63 ± 66.37) mL. The postoperative complications included pleural effusion in 5 patients and incision infection in one patients. Conclusions Three-dimensional visualization technology for the individualized diagnosis and treatment of hepatic hemangioma helps reduce surgical trauma and incidence of postoperative complications.
