Diabetic retinopathy (DR) is one of the most significant microvascular complications of diabetes,and its incidence increases with the prolongation of DM;meanwhile,it can lead to severe vision loss.Recently,the pathogenesis of DR has been one of the focuses both at home and abroad,but the basic mechanisms are not fully elucidated.More and more scholars believe that DR may be a chronic and low degree of inflammatory response.Moreover,it is well established that microglia is a monocyte-macrophage,with antigen-presenting function.Especially in the retina,as microglia is activated,followed by the release of a large number of inflammatory factors,microglia involves in DR inflammatory response.Understanding the activation mechanism of microglia can provide a new perspective for the prevention and treatment of DR.Therefore,recent advances in activation of microglia in diabetic retinopathy and its intervention mechanism will be reviewed in the article.

Objective To analyze the factors related to glare occurred after the phakic intraocular lens (P-IOL) implantation,and to raise the standard for the P-IOL selection.Design Retrospective case series.Participants 104 patients with high myopia underwent P-IOL implantation in Beijing Tongren Hospital.Methods In 104 cases,22 cases (22 eyes) were implanted angle supported Phakic6 designed accord with corneal symmetry,54 cases (54 eyes) were implanted iris fixed Verisyse,and 28 cases (28 eyes) were implanted posterior chamber PRL.Selection of the diameter of P-IOL optics depended on P-IOL diopters.The pupil diameter,the P-IOL optic center position,the pupil and corneal center position were measured with anterior segment optical coherence tomography (OCT).The contrast sensitivity (CS) under different condition (day,day and glare,night,night and glare) and in different space frequency (1.5,3,6, 12,18c/d)were examined with Optec6500 system.Main Outcome Measures Glare incidence,CS,and the influence of pupil diameter and P-IOL location to glare.Results None of the 104 P-IOL eyes occurred glare in day time.59 eyes (56.73%) complained of glare in dime condition.The glare incidence was 63.18% (14/22) in Phakic6 eyes,55.56% (30/54) in Verisyse eyes,and 53.27% (15/28) in PRL eyes (P=0.691).CS of all surgical eyes were improved in day time with or without glare luminous environment (P=0.000).And the CS of halo-complaining eyes was lower than of no halo eyes in night time with or without glare luminous environment (P=0.000).Glare oc- curred more frequently in eyes with the pupil diameter larger than P-IOL diameter in clime condition after surgery (10 in Phakic6 eyes, 20 in Verisyse eyes,12 in PRL eyes,all P

Objective To investigate the relationship between C-reactive protein and clinical significance of Tokyo Guidelines for acute cholangitis.Methods Analyzed 739 cases with acute cholecystitis in Xianyang Central Hospital of Shaanxi province.Records of patients diagnosed with acute cholecystitis were screened between September 2013 and October 2015.The criteria of Tokyo guidelines were used in grading the severity of acute cholecystitis.Patients were divided into 3 groups,Group Ⅰ (n =450),Group Ⅱ (n =260) and Group Ⅲ (n =29).C-reactive protein values at the time of admission were analyzed and compared among the groups.Results Mean C-reactive protein levels of groups were found to be significantly different,18.74 mg/L in Group Ⅰ,133.67 mg/L in Group Ⅱ,and 257.43 mg/L in Group Ⅲ.C-reactive protein values among the groups were found to be highly and significantly correlated with the disease grade (P ＜ 0.01).After evaluating C-reactive protein levels according to the grade of the disease,Group Ⅱ was distinguished from Group Ⅰ with a cutoff C-reactive protein level of 72.54 mg/L,and from Group Ⅲ with a value of 195.85 mg/L.Those results were found to be statistically significant (P ＜ 0.01).Conclusion C-reactive protein can be accepted as a strong predictor in classifying different grades of the disease,and treatment can be reliably planned according to this classification.

OBJECTIVE: To introduce the immune response of CD4~+ T helper lymphocytes (Th) and their cytokines following liver transplantation. DATA SOURCE: A computer-based online search was conducted in Medline from January 1970 to December 2008, Wanfang Database from January 1998 to December 2008, and China Hospital Knowledge Database from January 1998 to December 2008 with the key words of "liver transplantation, rejection, immune Tolerance, T helper lymphocytes, Th1 cells, Th2 cells" in both Chinese and English. DATA SELECTION: Articles about immune response of Th and their cytokines following liver transplantation were included, but reviews and duplicated studies were excluded. MAIN OUTCOME MEASURES: A total of 36 relative articles about immune response of Th and their cytokines following liver transplantation (n=3), immune effect of Th and cytokines on liver transplantation (n=22), and relationship among Th, cytokines, and transplantation immunity (n=11) were included. RESULTS: T helper lymphocyte was of importance to regulate the immune response, with function of supporting body fluid and cellular immune. Th and their cytokines played a very important role in immune tolerance following liver transplantation. The ration of Th1/Th2 was correlated with the immune response following liver transplantation. The increased Th1 secretion might induce rejection of liver tissue. When the ration turned to Th2, the immune tolerance easily occurred. CONCLUSION: Up to now, the theory of relationship between Th1/Th2 ration and transplantation immunity is not used for clinical application. Therefore, how to determine the immune state of organism by detecting and regulating cytokines, adjust Th1/Th2 ration, and induce specific immunologic tolerance of host become hot topics in the future.

Objective To investigate the effect of high mobility group box-1 protein (HMGB1) on immune function of peritoneal macrophages in mice. Methods The peritoneal macrophages were isolated from female BALBc mice in vitro. The macrophages were stimulated with different doses of HMGB1 (1, 10, 100, 1 000ng/ml) for 6 hours, or with 100ng/ml HMGB1 for different duration (0, 6, 12, 24, 48, 72 hours). The phagocytosis ability of macrophages was determined by the neutral red dye uptake assay and optical densities of samples were read at 540nm. The cytotoxicity to L1210 cells was measured with MTT assay and was expressed by absorbance at 570nm. The chemotaxis was assayed by using the Costar Transwell cell culture chamber inserts and the cells on the lower surface of the transwell membrane were fixed and counted. The cells were harvested at the indicated time points, fixed, and stained for indirect immunofluorescence, then the expressions of I-Ak MHC class Ⅱ alloantigen on macrophages were determined with flow cytometry. Objective To observe and measure the facial nerve canal(FNC) in curved planar reformation by multislice spiral CT.Methods High resolution computed tomography (HRCT)of temporal bone was performed in 40 cases (80 ears) by multislice spiral CT. Curved planar reformation of FNC was performed separately in axial, coronal and sagittal plane of multiplanar reconstruction. The morphology and measurement of FNC were studied.Results The mean length of FNC was (29.73?1.07)mm. The mean length of the labyrinthine, tympanic and mastoid segment was (5.6?0.74)mm, (10.66?0.79)mm and (13.47?1.01)mm respectively. The mean diameter of the l-abyrinthine, geniculate fossa, tympanic and mastoid segment was (0.76?0.16)mm, (2.37?0.63)mm, (1.03?0.16)mm and (1.57?0.31)mm respectively. The mean angle of first and second genu was 67.04??9.41? and 114.25??8.44? respectively. The distance between the tympanic segment and ampulla of the lateral semicircular canal was (0.57?0.19)mm. The distance between the tympanic segment and arch of the lateral semicircular canal was (1.59?0.61)mm. The mean angle between the tympanic segment of the facial nerve canal and the lateral semicircular canal was 10.63??3.60?. The tympanic course of the facial nerve canal formed an angle of 34.65??5.39? with the sagittal plane. Dehiscences was found in 89% FNC, 73.7% of them only located in the tympanic portion and 26.3% both in the mastoid and tympanic portions. Conclusion Curved planar reformation of FNC by multislice spiral CT is the good method to study the anatomy of FNC,and in combination with HRCT axial scan and multiplanar reconstrution can obtain more valuable information.

Objective To observe the change in interleukin-18 (IL-18) mRNA expression in cecal ligation and puncture (CLP) induced sepsis, and to investigate its potential role in the pathogenesis of endotoxemia. Methods Fifty-four Wistar rats were randomly divided into normal group (n=6), CLP group (n=36) and recombinant bactericidal/permeability increasing protein (rBPI 21) treatment group (n=12).
