1.Preliminary clinical study of sacroiliac activity in ankylosing spondylitis patients using quantitative dynamic contrast enhanced MRI

Objective To investigate the diagnostic values of DCE-MRI quantitative evaluation of the activity of sacroiliac joints in ankylosing spondylitis.Methods Forty two patients (36 male and 6 female) with ankylosing spondylitis in our hospital were enrolled prospectively according to the standard of diagnostic criteria for ankylosing spondylitis revised in 1984.All of them were evaluated with the blood sedimentation,C-reactive protein and ankylosing spondylitis activity index (BASDAI).We divided the patients with ankylosing spondylitis into stationary phase group and active phase group.We analyzed each of the MRI images of the two patients groups using the extended Tofts model to determine the quantitative parameters of DCE-MRI,such as contrast transfer coefficient (Ktrans),reflux constant (Kep),extravascular extracellular volume fraction (Ve),plasma volume fraction (Vp).And then the differences of quantitative data between the two groups were compared.Spearman correlation analysis was performed between the parameters with statistically significant difference and BASDAI scores.Results Among the 42 patients,22 patients (18 male and 4 female) were in active phase group and the other 20 patients (18 male and 2 female) were in stationary phase group.Ktrans,Kep,Ve were (0.750± 1.130)/min,(1.008±0.732)/min,0.460± 1.735;(0.163±0.401)/min,(0.505 ± 0.902)/min,0.345 ±3.460 for active phase group and the stationary phase group,respectively.The results of the active phase group was significantly higher than the stationary phase group (Z=3.727,2.317,3.696,respectively;all P<0.05).The Vp had no statistically significance for the active phase group (0.125 ± 0.310) and the stationary phase group (0.160 ± 0.329) (Z=1.209,P>0.05).Strong correlations existed between Ktrans,Kep,Ve and BASDAI score,the correlation coefficients were 0.714,0.430,and 0.676 (P<0.05).Conclusions Quantitative DCE-MRI parameters can evaluate the activity of the ankylosing spondylitis.Strong correlations exist between Ktrans,Kep,Ve and BASDAI score.
2.Detection Rate of Oral Candida albicans with Different Sampling Methods among Elderly:A Comparative Study

3.Design of added equipment for transporting the wounded on the truck

Based on new material and technic, the design of general structure, stretcher fixture, vibration isolation, carriage baffle fixture, ergonomical parameters selection for a new kind of stretcher suspension named added equipment for transporting the wounded on the truck is carried out. The results of mechanical performance experiment, reliability experiment, road adaptability test, road comfort test, application experiment by troops indicate that the added equipment is safe, reliable, convenient for use and fairly comfortable to persons lying on it. Thus a new kind of equipment fit for the army to transport the wounded on the truck under the field condition is developed.
4.Research of cochlear coiling pattern and orientation in general population by CT 3D reconstruction.

OBJECTIVE:
To perform morphometric analysis of bilateral cochleae in all subjects based on computed 3-dimensional reconstruction tomographic data and assist the surgeon in diagnosing the inner ear abnormality or surgical strategies.
METHOD:
Two hundred normal developed cochleae from 100 patients were divided into 5 groups according to age. Morphometric analysis of bilateral cochleae was performed in all subjects by 3D reconstructions and 2D multiplanar reformation. The length and width of the cochlear base, the length within the cochlear base, the height of the cochlea, the angle between the first and second turn of the cochlea, and the cochlear orientation within the cranial base were measured and compared in different age, sex and bilateral groups.
RESULT:
The length of the cochlear base was (8.56 +/- 0.52)mm, the width was (6.63 +/- 0.56)mm, the length within the cochlear base was (7.33 +/- 10.56)mm, the height of the cochlea was (3.76 +/- 0.28)mm, and the angle between the first and second turn of the cochlea was (15.82 +/- 2.78)degrees. All index above did not change significantly in different aging, sex or side (P > 0. 05). Variability in the angle between the first and second turn of the cochlea was considerable, and a smaller angle (from the midsagittal line) was showed in the older age groups than the younger groups (P < 0.05).
CONCLUSION
3D and 2D volume rendering enables us to evaluate the features of cochlear morphology and orientation that may assist the surgeon in diagnosing the inner ear abnormality or surgical strategies.

6.Expression of C3a, C5a, TXA2, LT in pulmonary injury of infant open heart surgery improved with the modified ultrafiltration technique

Objective To study the mechanism of pulmonary injury and protective effect of modified ultrafiltration on lung function in infant open heart surgery.Methods According to the wishes of parents,40 cases of congenital heart disease were divided into without modified ultrafiltration control group (C) and modified ultrafiltration group (M),and parents signed informed consent.The cardiopulmonary bypass (CPB) was used without ultrafiltration in Group C,while with modified ultrafiltration in group M.The pneumodynamic parameters and C3a,C5a,TXA2,LT were measured at specific time points.Results The static pulmonary compliance (Cstat) and oxygen index (OI) were lower,and alveolar-arteria oxygen difference (AaDO2) was higher after CPB in the two groups(P < 0.05).At T3,T4 and T5 time points,the Cstat and OI in Group M was higher than that in Group C; AaDO2 in Group M was lower than that in Group C (P <0.05).The levels of C3a and C5a were lower after CPB in the two groups; levels of TXA2,LT were higher after CPB in the C groups.At T2,T3,T4 and T5 time points,the TXA2 and LT in Group M were lower than that in Group C(P <0.05).Conclusions The pulmonary injury in pediatric open heart surgery may be concerned with the the alexin(C3a,C5a) activation and I/R.The level of C3a and C5a was considered earlier index of inflammatory reaction and pulmonary injury.Modified ultrafiltration improves pulmonary function due to elevating coloid osmotic pressure and degrading the plasma level of TXA2,LT.
7.Oil of Piper longum unsaponifiable matter prevents cholesterol gallstone formation.

To observe the effect of various doses of oil of Piper longum unsaponifiable matter (OPUM) to cholesterol gallstones in experimental mice. C57BL/6 mice (n = 60) were randomly divided into 6 groups: control group, model group, OPUM (15, 30 and 60 mg x kg(-1)) group and ursodeoxycholic acid (UDCA, 60 mg x kg(-1)) group, administered for 10 weeks. The level of serum lipid and liver function enzymes were tested. The gallbladder was removed and bile was obtained by centrifugation. Next, the levels of the bile total cholesterol (TC), phospholipid (PL) and bile acid (TBA) were measured. The indicators of lipid peroxidation were determined and cholesterol saturation index (CSI) was calculated. The liver histological changes were observed by HE staining. The results showed that serum TC, TG (triglycerides) and AST (aspartate transaminase) contents, gallbladder cholesterol crystallization and CSI increased significantly (P < 0.05). In addition, the activity of SOD decreased significantly and MDA content increased significantly in liver (P < 0.05). HE staining results showed that the hepatic cord disorder and intracellular lipid droplets increased significantly. All results indicate that lithogenic diet lead to the formation of cholesterol gallstones. In OPUM (30 and 60 mg x kg(-1)) group, serum TC, TG and AST content, gallbladder cholesterol crystallization and CSI decreased significantly, the activity of SOD increased significantly and MDA content decreased significantly. HE staining results showed that OPUM can improve the morphology of liver cell, reduce the degree of hepatic cord disorders and restore the cell morphology close to normal. The cause of OPUM prevents cholesterol gallstone formation maybe due to protect the integrity of the liver cells, lower CSI, and reduce cholesterol crystal formation and hence prevent cholesterol gallstone formation.
8.A Soil Water Extraction Method Using Accelerated Solvent Extraction Technique for Stable Isotope Analysis

Soil water is one of the most important components in hydrological cycle. The stable hydrogen and oxygen isotopes in soil water have been increasingly used in the ecological, environment and hydrological research. In view of different techniques for extracting soil water, there is significant difference in theδD andδ18 O composition. This paper presents a method for analyzing hydrogen and oxygen isotopes in soil water by using elemental analyzer and isotope ratio mass spectrometry with accelerated solvent extraction for sample pretreatment. The conditions are: extraction solvent: dichloromethane, temperature: 100 ℃, pressure of 10. 3 MPa, static time:10 min. The samples were extracted three times, and with cycle values of four, four and three, respectively. Comparing with the added water, the deuterium and oxygen isotope values in the extracted soil water enrich 2. 12‰-4. 58‰ and 0. 17‰-0. 93‰, respectively. The reproducibility of replicate extractions of soil water is around ±0. 89‰ for δD and ±0. 37‰ for δ18 O.
9.Determination of Pantoprazole Sodium Bioadhesive Tablets by HPLC

Objective:To establish a method to determine the contents of pantoprazole sodium bioadhesive tablets. Methods:An HPLC method was adopted. The determination was performed on a HYPERSIL ODS-2 (150 mm × 4. 6 mm, 5μm) column with mobile phase consisting of 0. 01 mol·L-1 dipotassium phosphate solution –methanol (60 ∶40) at the flow rate of 1. 0 ml·min-1 . The de-tection wavelength was set at 289 nm, the column temperature was maintained at 30 ℃ and the sample size was 20 μl. Results:The linear range of pantoprazole sodium was 1. 28-20. 60μg·ml-1,(r=0. 999 8) . The average recovery was 99. 52%(RSD=1. 43%, n=9). Conclusion:The method is simple, accurate and reliable, and can be used for determining the content of pantoprazole sodium bioadhesive tablets.
10.Efficacy assessment of 33 cases of peroral endoscopic myotomy in different achalasia subtypes by esophageal high-resolution manometry

Objective To explore the symptomatic improvement of different achalasia (AC)subtypes after peroral endoscopic myotomy (POEM) procedure.Methods A total of 33 patients received POEM for the first time were enrolled.Patients were evaluated by high-resolution manometry (HRM)and subtyped by Chicago classification criteria.The total scores of symptoms were evaluated by Eckardt scale before and after POEM.Solid food dysphagia,liquid dysphagia,reflux and chest pain were scored by five-point Likert scales and compared by Wilcoxon paired test.Multiple linear regression analysis was performed to analyze key factors affecting the prognosis of symptoms.Results Among 33 AC patients,there were 20 females and the mean age was (44.0± 13.3) years.According to Chicago criteria,four patients had subtype Ⅰ AC,25 patients had subtype Ⅱ AC,and four patients with subtype Ⅲ achalasia.Before operation,the total Eckardt score was 7.5(6.3,10.0),which declined to 1.5 (1.0,3.0) after operation,and the difference was statistically significant (Z=-4.795,P < 0.01).There was no statistically significant difference in the incidence of solid food/liquid dysphagia,reflux and chest pain among subtypies.Solid food dysphagia was the main symptom of each AC subtypes.POEM could improve all the symptoms of subtype Ⅱ AC except for chest pain.Before and after POEM,solid food dysphagia score was 16.0 (12.0,16.0) and 2.0 (1.0,12.0),liquid dysphagia score was 2.0 (0.0,5.0) and 0.0(0.0,0.0),reflux score was 9.0 (6.0,9.0) and 0.0 (0.0,1.0),respectively,and the differences were statistically significant (Z =-3.929,-3.470,1.841;all P< 0.01).The results of multiple linear regression analysis indicated that female and 4 s integrated relaxation pressure (4 s IRP) of 10 mL continuous swallows were positively correlated with symptom scores of solid food dysphagia (β 3.950,P=0.048;β=0.376,P=0.005);While 4 sIRP in solid food swallow was negatively correlated with symptom scores of solid food and liquid dysphagia (β=-0.244,P =0.008;β=-0.118,P =0.037).Conclusion For patients with AC,clinical symptoms could be significantly improved by single treatment of POEM,but there are certain differences in the symptomatic improvement among different subtypes.
