3.Neuronal apoptosis and the change of apoptosis-related proteins in ovariectomied rats and the effect of App17-mer peptide

AIM: To observe the expression of apoptosis-related proteins in hippocampal neurons of (ovariectomized) (OVX) rats and explore the neuroprotective mechanism of the App17-mer peptide. METHODS: Female Wistar rats were randomly divided into three groups. Bilaterally ovariectomized rats with injection of App 17P peptide (3.5 ?g in 0.1 mL/per rat, three times a week) formed the experimental group (17P+ OVX group). Anti-AIF, Bcl-2 and Bax antibodies were applied in the immunohistochemistry experiment. TUNEL was employed to detect apoptosis. RESULTS: The number of apoptotic neurons was clearly higher in hippocampal and cortex in OVX group than that in OVX+17P group. Immunohistochemistry demonstrated the increased expression of AIF, Bax in hippocampal neurons of OVX group. OVX group showed a significantly reduced expression of Bcl-2 in hippocampal neurons. Hippocampal tissue from OVX group showed the increased expression of AIF, Bax, and showed diminished expression of Bcl-2, treating with App17-mer peptide normalized the expression of these proteins. CONCLUSIONS: The expression of apoptosis-related proteins were abnormal in the OVX rats, App17-mer peptide normalized these changes. Estrogen deficiency induced neuronal apoptosis, and App17-mer peptide diminished apoptosis.

5.Chronic graft versus host disease related polymyositis:a case report and literature review

Objective To summarize the diagnostic and therapeutic experience of a patient with chronic graft versus host disease (cGVHD) related polymyositis (PM) after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods A patient with acute lymphocytic leukemia in com- plete remission received sibling allo-HSCT,and cyclosporine and methotrexate were adopted to pre- vent GVHD.Results Eleven days after HSCT,WBC>0.5?10~9/L,13 days after HSCT,PLT>20?10~9/L;27 days after HSCT,chromosome analysis of bone marrow cells showed 99% donor type. Seventeen days after HSCT,Ⅰ~0 acute GVHD of skin occurred,and it was cured by intravenous injec- tion of dexamethasone and methotrexate.Eight months after HSCT,cGVHD of liver happened.Al- though treated by tacrolimus and azathioprine,enzymes of liver were still elevated.At last,tacrolimus combined with sirolimus were used,and enzymes of liver subsided gradually.However,the serum creatine phosphokinase (CK) began to rise from 9 U/L to 3010 U/L,and fatigue all over the patient occurred.Finally,the symptom relapsed,and disability involved with the origin of limbs appeared. The electromyogram and magnetic resonance imaging of concerned muscles confirmed the PM diagno- sis.Although treated with methylprednisolone and plasma exchange,the patient died due to asphyxia, while the CK as high as 21 010 U/L.Conclusion PM is a rare kind of manifestations of cGVHD. When the key muscle tissue was involved,the prognosis is poor.
6.Pulsed radiofrequency alleviated neuropathic pain by down-regulating the expression of substance P in chronic constriction injury rat model

Background:Pulsed radiofrequency (PRF),as a non-invasive treatment of neuropathic pain (NP),has been widely administered clinically.Previous studies have shown that PRF has the potential to improve hyperalgesia in animal models of NP.However,there have been few reports to clarify whether the mechanism of PRF treatment of NP involves intervention in the expression of substance P (SP).Therefore,this study administered PRF treatment to chronic constriction injury (CCI) model rats and observed the sciatic nerve mechanical pain threshold and SP expression in the spinal cord to explore the mechanism of PRF treatment.Methods:A total of 96 Sprague-Dawley rats were randomly divided into the sham-surgery-sham-treatment group (S-S group),the sham-surgery-PRF group (S-P group),the CCI-sham-treatment group (C-S group),and the CCI-PRF group (C-P group).The C-S group and the C-P group underwent sciatic nerve CCI,while the other groups received a sham operation.At 14 days after the operation,the C-P group and the S-P group were treated with PRF for 300 s.We recorded the hindpaw withdrawal threshold (HWT) and the thermal withdrawal latency (TWL) of rats in the various groups at baseline,before treatment (0 days),and at 1,7,14,and 28 days after treatment.L4 to L6 spinal cord tissues were taken before treatment (0 days) and 1,7,14,and 28 days after treatment.The transcription and translation of SP were measured by quantitative polymerase chain reaction and Western blotting,respectively.Results:The HWT and the TWL in the C-P group 28 days after PRF treatment were significantly higher than those in the C-S group (95% confidence interval [CI]:5.84-19.50,P < 0.01;95% CI:2.58-8.69,P =0.01).The expression of SP in the C-P group 28 days after PRF treatment was significantly lower than that in the C-S group (95% CI:1.17-2.48,P < 0.01).Contusions:PRF may alleviate CCI-induced NP by down-regulating the expression of SP in the spinal cord of CCI model rats.
7.Comparative study on computed orthopantomography and film radiographic techniques in the radiography of temporomandibular joint

Objective To compare the computed orthopantomography(COPT)with Sh?ller radiography(SR),film orthopantomography(FOPT)and other traditional radiographic techniques in the radiography of temporomandibular joint(TMJ).Methods Ninty-eight cases were randomly divided into 3 groups,and the open and close positions of TMJs of both sides were examined with SR,FOPT,and COPT, respectively.The satisfactory rates of the X-ray pictures were statistically analyzed with Pearson chi-square in SPSS10.0,and the satisfactory rates were analyzed with q test between the groups.Results One hundred and forty-four of the open and close positions of 144 TMJ pictures of the COPT group,128 of 128 of the FOPT group,and 6 of 120 of the SR group were satisfactory in the mandible ramus of the TMJ,with satisfactory rate being 100%,100%,and 5%,respectively(P0.01),respectively between FOPT and COPT groups.The difference was not statistically significant.The exposure was as follows:COPT,99—113 mAs;FOPT,210—225 mAs;and SR,48—75 mAs.Therefore,COPT and FOPT were superior to SR in the pictures of the mandible ramus,coronoid process,and incisure,but inferior in the joint space pictures.The satisfactory rates of the condylar process and articular tubercle were same in the 3 groups.The exposure of the FOPT group was greater than that of the COPT and SR groups.Conclusion COPT is superior to SR and FOPT in TMJ radiography,and should be applied widely in the clinic.
8.Application of molecular absorbent recycling system in auto liver transplantation

Objective To investigate the application of molecular absorbent recycling system (MARS) in auto liver transplantation (ALT).Methods The clinical data of 46 patients who underwent ALT in Zhongnan Hospital of Wuhan University from September 2014 to August 2015 were retrospectively studied.The patients were randomly divided into the MARS group (n =30) and the hemodialysis group (n =16).In the MARS group,20 patients were male and 10 were female with an average age of (15 ±67) years,and the median age was 46.In the hemodialysis group,8 were male and 8 were female with an average age of (22 ± 54) years.A heparinized left iliac vein was linked to a centrifugal pump of a MARS and an output tube was linked to a left jugular vein.Before the anhepatic phase,physical pretreatment was carried out by ligating the porta hepatis and inferior vena cava (IVC) to adapt the patient to the anhepatic phase.Subsequently,a bypass was established followed by ligation of the IVC.The duration of operation,length of stay in ICU after operation,one-year survival rate and hepatorenal function were studied.Results The duration of operation in the MRAS group and in the hemodialysis group was not significantly different (P >0.05).The length of stay in ICU after operation in the MARS group was (12.0 ±3.0) d,which was significantly shorter than that of the hemodialysis group (20.0 ± 2.0) d (P < 0.05).The one-year survival rate in the MARS group was (100%),which was significantly higher than that of the hemodialysis group (93.8%,P <0.05).Conclusion The application of MARS in ALT shortened the length of stay in hospital and improved one-year survival rate,resulting in better outcomes than the conventional method.

10.Clinical analysis and treatment strategy of ten cases of pancreatic duct stent displacement complicated with acute pancreatitis

Objective To investigate clinical features and treatment strategy for pancreatic duct stent displacement complicated with acute pancreatitis.Methods Ten cases of pancreatic duct stent displacement complicated with acute pancreatitis were retrospectively analyzed.All the cases were confirmed by lab examination,X-ray examination,endoscopy and computed tomography (CT) examination.After operation,the clinical symptoms of patients were observed.Time consumed for stent removing and blood amylase level before endoscopic retrograde cholargio-pancreatography (ERCP) and 1st,4th,7th day after ERCP were recorded.Clinical features,the time of stent displacement,time of clinical cure and therapeutic strategy were summarized.Results The average age of the ten cases (four male and six female) was 55.9 years.All the displaced pancreatic duct stents were pancreatic duct stent with side wing,length five to seven cm.Obstruction was observed in one case.The end of the pancreatic duct stent of two cases dislocated at the neck of pancreats,seven cases at the head of pancreas and one at the body of pancreas.Ten cases presented with abdominal pain,seven with nausea and vomiting and one with fever.The average time of the occurrence of pancreatic duct stent displacement was 2.9 months.All the stents were successfully taken out,which of two cases were removed with balloon,seven cases with forcep and one with snare,and the average time duration was 17.7 min.After the stents taken out,pancreatic duct stent was replaced in one case and the left nine cases received nasal-pancreatic drainage treatment.After the operation,no severe complications such as pseudocyst of pancreas,pancreatic abscesses,pancreatic necrosis and gastrointestinal bleeding were observed in all the patients.Average blood amylase level at 1st,4th and 7th day after ERCP was 508 U/L,137 U/L and 86 U/L,respectively.The average time of recovery was 6.7 days,and the average time of keeping the nasal-pancreatic tube was 8.6 days.Conclusions For patients with pancreatic duct stent displacement complicated with acute pancreatitis,it is safe and effective to take out the stent and perform pancreatic duct drainage by ERCP as soon as possible,which could improve the symptoms in a short time.
