4.The Modified Uvulopalatopharyngoplasty(UPPP)to Prevent the Complications and Destroy of UPPP.

Objective To improre typieal proeedure of UPPP,increase clinical effects and deerease complioations caused by the operation.Methods Twenty eight patients with obstructive sleep apnea syndrome who had been confirmed by PSG in our hospital from 2003 to 2005 were involved in this study.1)improving stripping method:the fat and tonsil were removed by electric knife.2)improving molding method:the muscle of uvula was maitained,but the fat was removed;mucous and the tissue under it were double-layer sutured.3) the data before and 6 months after UPPP were analyzed by questionnaire and polysomnography(PSG)detection.Result No fissuration occurred.The distance between soft palate and posterior wall was 3?0.6 before operation and it was 6?0.8 6 months after operation.The effect is 100% based on the questionnaire investigation and PSG detection.Conclusions 1)Hemostasis of electric knife was reliable;2)Double-layer suture of mucous and the tissue under it can avoid hemorrhage and insure molding of pharyngeal cavity;3)The results of the uvula preservation can improve the effect of the UPPP by increasing the nasopharyngeal and oropharyngeal cavity,and avoiding velopalatal insufficiency.
5.THE SIGNIFICANCE OF CHANGES IN TISSUE SUPPRESSORS OF CYTOKINE SIGNALING IN RATS WITH SEPSIS INDUCED BY CECAL LIGATION AND PUNCTURE

The aim of this study was to observe the changes in tissue suppressors of cytokine signaling (SOCSs) mRNA expression, and to investigate their potential role in the pathogenesis of sepsis induced by cecal ligation and puncture (CLP). Fifty four Wistar rats were randomly divided into three groups: normal control group ( n =6), CLP induced sepsis group ( n =36) and recombinant bactericidal/ permeability increasing protein (rBPI 21 ) treatment group ( n =12). Tissue samples from the liver, lung and kidney were collected to determine SOCS1 and SOCS3 mRNA expressions. Meanwhile, tissue endotoxin and TNF ? levels were also determined. The aim of this study was to investigate changes in high mobility group 1 (HMG 1) protein in postburn Staphylococcus aureus infection and its potential signaling mechanism. A total of 61 male Wistar rats were randomly divided into four groups as follows: normal control group ( n =6), scald control group ( n =9), postburn sepsis group ( n =36) and rapamycin (RPM) treatment group ( n =10). Tissue samples from the liver and kidney were collected to determine tumor necrosis factor ? (TNF ?) and HMG 1 mRNA expression. 7.MULTIPLE ORGANINJURY BY HIGH CONCENTRATED PLATELET-LEUCOCYTE AGGREGATIONS IN RED BLOOD CELL CONCENTRATION AND ITS COUNTERMEASURE

To study the formation and change of platelet leucocyte aggregation (PLA) in red blood cell concentrations(CRCs) stored at 4℃ and the preventive effectiveness of filtration, 8 units of CRCs were each divided into two equal parts in two integrative bags and two paired groups were formed: control group and filtration group. The following items of control group and filtration group were detected weekly. Platelet and leukocyte counts, concentration and percentage of platelet lympocyte (P lym), platelet monocyte (P mon), platelet neutrophil (P neu) aggregatoins, and concentration of total PLA. The results showed that rate and concentration of PLA in the control group were significanly higher than those in the circulating blood. The rates of P lym and P mon in CRCs stored at 4 ℃ were two to five times of those before storage, and the rate of P neu in CRCs stored at 4 ℃ was ten to twenty times of that before storage. PLA in the CRCs separated from plasma rose to a high level at the first day of preparation. The concentration of PLA in CRCs reached the highest level at one week after stored at 4 ℃ and then decreased, but a high concentration was retained all the while. P neu is the major part (about 80%~90%) of whole PLA in CRCs. PLA was not detectable by flow cytometry during the whole storage course. The results suggested that high concentrated PLA was detected in CRCs stored at 4 ℃, which could be prevented by leucocyte depleting filtration.
8.EXPRESSION OF c-jun mRNA AND PROLIFERATION OF INTESTINAL MUCOSA EPITHELIAL CELLS

The aim of this study was to investigate the effect of total parenteral nutrition(TPN), infection, or growth hormone on changes in proliferation of intestinal mucosa epithelial cells and expression of mucosa c jun . Cecal ligation and puncture was used to replicate intra abdominal infection model, and transjugular intracaval catheterization was chosen to give TPN. Experimental rats were divided into four groups: normal control, TPN control, TPN plus sepsis group, and TPN plus growth hormone group. Changes in mucosa thickness, villus height, crypt depth , mucosa c jun mRNA expression and epithelial cell proliferation index were observed. The results showed that all of mucosa thickness, villus height, crypt depth of intestinal mucosa decreased, proliferation of epithelial cells were markedly inhibited in TPN group. Sepsis states aggravated the above changes in intestinal mucosa. Corresponding to mucosa atrophy, expression of c jun reduced obviously. Growth hormone could promote growth of intestinal mucosal cell, improve proliferation of epithelial cells and expression of mucosa c jun. It suggested that changes in c jun expression appeared to be associated with different proliferation states of intestinal mucosa epithelial cells. c jun mRNA could be used as an important marker to reflect mucosa epithelial cell proliferation .
9.Inhibitory effects of argatroban on instant blood-mediated inflammatory reaction after islet transplantation in vitro

Objective To investigate the inhibitory effects of Argatroban on the instant bloodmediated inflammatory reaction(IBMIR)after islet transplantation.Methods Rat islets were isolated and purified rat islets,and were divided into blank control group,control group and experimental group.In the control group,the blood and the islets were directly mixed,and in the experimental group the Argatroban was added to the mixture based on the control group.while the blank control group was added with blood alone without the islets.Each group was reacted at 37℃for 60min,and then the content was filtered through trap valve of 70 μm.The residual thrombus and tissues were filtered by the trap valve in both the experimental and control groups,detected by the thinprep cytologic test(TCT),and the filtrate received blood routine test,and the function of islet was determined using insulin releasing test.Results The number of blood platelets,white blood cells,mononuclear cells,and lymphocytes percentage in the experimental group were significantly higher than in the control group after 60 min(P<0.05).Under the environment of the high and low concentrations of glucose,the insulin release in experimental group was significantly increased as compared with the control group,and the insulin release index of former was 2.25±0.18,significantly higher than that of the latter 3.36±0.18(P<0.05).The residual thrombus and tissues had few islet cells in the control group,the structure was damaged seriously,the capsule was not intact,and there were a large mumber of micro-thrombosis around the islets formed by red blood cells.But there were a large number of islet cells in the experimental group.the structure was intact in a mass,and no obvious micro-thrombosis around the islets was found.Conclusion Argatroban can effectively inhibit IBMIR in vitro,and alleviate the damage to the islet cells.
10.A New Technique for Composite Graft Preparation in Aortic Root Replacement

We report the Lampshade Technique : a new technique using Carbo-Seal Valsalva (Sorin Biomedica, Saluggia, Italy) to facilitate preparation of a composite graft. A Bentall operation and an ascending aorta replacement were performed with a composite graft using a Carbo-Seal Valsalva. This new technique can be considered useful as it can reduce the time required for preparing a composite graft, and create a skirt portion for continuous suturing to prevent bleeding.
