1.Effect of high dose metformin on serum HE4, LPA and regulatory T cells in patients with ovarian cancer

Objective To analysis the effect of high dose metformin on serum HE4, LPA and regulatory T cells in patients with ovarian cancer.Methods 80 cases of unilateral ovarian cancer patients were given surgery and conventional chemotherapy ,were divided into four groups according to single metformin dose: (group A:0.25g each time, B group:0.25g each time, group C:0.25g each time; group D: conventional treatment only) and three times a day oral administration of metformin and two weeks in a row , before and after the treatment of detection each serum HE4, LPA, transformation growth factor beta 1 (TGF -beta 1), interleukin -10 (IL-10) content, and CD4 +CD25 +CD127 regulating T cell percentage.ResuIts Compared with B, C, and D group, the efficacy of patients in group A was better, as follows: Serum HE4 content decreased significantly ( P<0.05 ); serum LPA content decreased significantly ( P<0.05 ); blood CD4 +CD25 +CD127 regulatory T cell percentage decreased significantly ( P<0.05 ); serum TGF beta 1, IL-10 content decreased significantly ( P<0.05 ) .The results were statistically significant .ConcIusion High dose of metformin can reduce serum HE4 , LPA content, reduce the adjusting the percentage of T cells and related cytokines in patients with ovarian cancer , and play a positive role in inhibit cancer cell proliferation and invasion .
4.Effectiveness of special rectification activity on clinical antimicrobial use in a tertiary first-class hospital

Objective To investigate the application of antimicrobial agents in a tertiary first-class hospital before and after implementing special rectification activity on clinical antimicrobial use.Methods Data about antimicrobial use in a hospital in Sichuan Province between January 2011 and December 2013 were collected and analyzed retro-spectively,data included antimicrobial varieties,the ratio of antimicrobial prescription in outpatient department to emergency department,percentage of antimicrobial application expense in the total hospitalization expense,antimi-crobial use rate and use density,antimicrobial use rate and use density of special use antimicrobial agents,microbial detection for patients receiving restricted and special antimicrobials.Results Before implementing rectification of an-timicrobial use,there were 88 kinds of antimicrobial agents in hospital,after implementing rectification,there were 50 kinds of antimicrobial agents.The ratio of antimicrobial prescription in outpatient department to emergency de-partment,and the percentage of antimicrobial expense in the total hospitalization expense were both lower than be-fore rectification (both P <0.05 ).Antimicrobial use rate and antimicrobial use rate of special use antimicrobial agents before and after rectification were all statistically different(all P <0.05).Compared with before rectification, microbial detection rate of specimens from patients receiving restricted and special use antimicrobial agents improved significantly (both P <0.05 ).Perioperative antimicrobial prophylactic use rate in patients with class Ⅰincisional operation after rectification was lower than before rectification,while percentage of antimicrobials used 30 min-2 h before operation was higher than before rectification (both P <0.05).Conclusion After three year special rectifica-
tion,antimicrobial use become more rational,but management still needs further and continuous improvement.

7.Andrology in the 21st century: keeping pace with the time.

8.Efficacy of different kinds of artificial tears treatment in patients with xerophthalmia after phacoemulsification combined with intraocular lens implantation

AIM:To explore efficacy of the different kinds of artificial tears treatment in patients with xerophthalmia after phacoemulsification combined with intraocular lens implantation.METHODS:Totally 280 patients (280 eyes) with xerophthalmia after operation in our hospital from January 2015 to June 2016 were selected.According to the different treatment methods, they were divided into control group (n=70;treated with tobramycin and dexamethasone eye ointment 3 times per day for 1wk, levofloxacin eye drops 3 times per day for 1wk, pranoprofen eye drops 4 times per day for 1mo), polyacrylic acid group (n=70;besides the treatment of control group, polyacrylic acid was used 4 times per for 1mo), polyethylene glycol group(n=70;besides the treatment of control group, polyethylene glycol was used 4 times per for 1mo) and sodium hyaluronate group (n=70;besides the treatment of control group, sodium hyaluronate was used 4 times per for 1mo).The tear film break up time (BUT), Schirmer Ⅰ test (SⅠt), symptoms of dry eye and corneal staining in four groups were observed.RESULTS:(1) BUT:The BUT of the four groups significantly increased after treatment(P<0.05);that of polyacrylic acid group, polyethylene glycol group, and sodium hyaluronate group was different compared with control group(P<0.05);the BUT in sodium hyaluronate group was significantly higher than the other groups after 2wk of treatment(P<0.05).(2) SⅠt:SⅠt of the four groups significant increased after treatment(P<0.05);that of polyacrylic acid group, polyethylene glycol group, and sodium hyaluronate group was different compared with control group(P<0.05);and the SⅠt in sodium hyaluronate group was significantly higher than other groups (P<0.05).(3) Xeroma score:the scores of dry eye significantly decreased after treatment in the four groups(P<0.05);that of polyacrylic acid group, polyethylene glycol group, and sodium hyaluronate group was different compared with control group(P<0.05);and the scores of the sodium hyaluronate group after 3wk was significantly lower compared with other groups(P<0.05).(4) Corneal staining score:the scores significantly decreased after treatment in the four groups(P<0.05);at 1 and 2wk after treatment the corneal staining score had not statistically different among the four groups (P>0.05);sodium hyaluronate group was significant lower than other groups in corneal staining score at 3wk and 1mo after treatment(P<0.05).CONCLUSION:Artificial tears in the treatment of xerophthalmia after cataract phacoemulsification combined with intraocular lens implantation has better clinical efficacy, which contains sodium hyaluronate may be the better than others.
9.Comparison of four methods for sperm counting.

OBJECTIVETo evaluate the accuracy and precision of 4 methods including Hemacytometer, Makler chamber, Cell-VU chamber, and computer-aided semen analysis for determining sperm concentration.
METHODSLatex bead solutions with concentrations known as (35 +/- 5) x 10(6)/ml and (18.0 +/- 2.5) x 10(6)/ml and semen samples (n = 54) were counted by the above 4 methods and the results were then compared.
RESULTSMean bead concentrations for Hemacytometer, Makler, Cell-VU chambers and CASA were (44.84 +/- 4.86), (52.36 +/- 7.78), (39.70 +/- 4.76), (28.53 +/- 2.06) x 10(6)/ml respectively for the standard solution containing (35 +/- 5) x 10(6)/ml, and (21.04 +/- 1.87), (24.54 +/- 3.67), (19.09 +/- 2.02), (14.62 +/- 0.95) x 10(6)/ml respectively for a standard solution containing (18 +/- 2.5) x 10(6)/ml. The results of Cell-VU chamber were consistently similar and close to the standard solutions, while those of Hemacytometer, Makler chambers were overestimated, and those of CASA were underestimated. The coefficients of variation for Hemacytometer, Makler, Cell-VU chambers and CASA were 10.81%, 14.86%, 12.80%, and 7.22% respectively for a higher standard solution, while 8.89%, 14.96%, 10.58%, and 6.50% respectively for a lower standard solution. CASA has the lowest CV%. When semen samples were counted, the results of Hemacytometer, Makler, Cell-VU chambers and CASA were (76.98 +/- 59.90), (63.89 +/- 53.84), (45.28 +/- 34.52), (41.96 +/- 31.93) x 10(6)/ml respectively. There wasn't any significant difference either between Cell-VU chamber and CASA (P = 0.71), or between Hemacytometer and Makler chamber (P = 0.14), while there was significant difference between Cell-VU chamber or CASA and Hemacytometer or Makler chamber (P < 0.05 or P <0.01).
CONCLUSIONWhen counting semen sample, there wasnt any significant difference between Cell-VU chamber and CASA. Each laboratory can select its own proper method for manual or computer-aided analysis.

10.WHO Laboratory Manual for the Examination and Processing of Human Semen: its applicability to andrology laboratories in China.

The 5th edition of WHO Laboratory Manual for the Examination and Processing of Human Semen (2010) represents a comprehensive revision. This article aims to explore the applicability of this manual to andrology laboratories in China mainland in view of sperm count analysis, sperm motility analysis, sperm morphology analysis, sperm function analysis, anti-sperm antibody and seminal plasma biochemical marker analysis, and quality assurance and quality control of semen analysis. The authors deem that its recommendation to the analysis method and lower reference limit of sperm concentration may be a little arbitrary and lack of evidence-based support, that the revised grading sperm motility, the strict criteria and the very low cut-off value of 4% morphologically normal spermatozoa for the evaluation of sperm morphology are not applicable to andrology laboratories in China mainland, that the sperm function markers need to be supplemented, and that the determination methods of anti-sperm antibody and seminal plasma biochemical markers are incompatible with the status of Chinese andrology laboratories. However, its recommended methods for quality assurance and quality control of semen analysis have a significant directive role in China mainland. It is worth to point out that the WHO manual ignored the data obtained from Chinese which accounts for approximate 20% of the world population. Thus, given the importance of the WHO manual, its general applicability should be evaluated in China.
