2.Mechanism of tanshinone II A in inhibiting transformation of aortic valvular myofibroblast to osteoblast-like phenotype.

Aortic valve calcification (AVC) is a pathological process correlated with multiple disease causes and actively regulated by cardiac valve cells. In this study, porcine aortic valve myofibroblasts cultured in vitro were treated with 50 μg z L(-1) of pathological factor tumor necrosis factor α (TNF-α). Tanshinone II A (TSN) with the concentration of 50 mg x L(-1) and TNF-α were combined in incubating cells for 72 h (3 d) and 120 h (5 d). The Western blotting and Real-time PCR were adopted to detect the changes in smooth muscle α actin (α-SMA), bone morphogenetic protein 2 ( BMP2), alkaline phosphatase (ALP) in cells, and expressions of key effect proteins GSK-3β and β-catenin on Wnt/β-catenin signal pathway. According to the findings, TNF-α can significantly increase the expression of myofibroblasts α-SMA and add the transformation activity to them, with nearly no expression of BMP2, ALP and mRNA in the control group and the TSN group but significant increase in their expressions in the TNF-α group (P < 0.01), which showed osteoblast-like phenotype. Moreover, TNF-α down-regulated the expression of up-streaming regulator GSK-3β and mRNA expression (P < 0. 01) , notably increased the expression of key effect protein β-catenin, but with no significant difference in mRNA with the control group and the TSN group. The result demonstrated that TSN showed a certain inhibitory effect on TNF-α's pathological impact (P < 0.05) in a time-dependent manner. Inflammatory factor TNF-α may promote the transformation of aortic valvular myofibroblasts to osteoblast-like phenotype by activating Wnt/β-catenin signal pathway in aortic valvular myofibroblasts, so as to cause AVC. Tanshinone II A can have a preventive effect in AVC by activating GSK-3β proteins and regulating signal transduction of Wnt/β-catenin signal pathway.

Objective To evaluate the expression of Toll-like receptor(TLR)on the monocyte- derived dendritic cells(DC)from chronic hepatitis B(CHB)patients and to analyze the expression pro- file and significance of the TLR such as TLR3,TLR4,TLR?,TLR8 and TLRg,which are associat- ed with immune response to viral infection.Methods Peripheral blood mononuclear cell(PBMC) centrifugated by the hydroxyethyl starch(HES)centrifugation were cultured and induced into DC by granulocyte-maerophage colony stimulating factor(GM-CSF)and interleukin-4(IL-4),and their mor- phology and phenotype were detected by the inverted microscope and flow cytometry respectively. Monocyte-derived DC were obtained from 10 chronically hepatitis B virus(HBV)-infected patients and 15 healthy volunteers.TLR3,TLR4,TLR7,TLRS,TLR9 expression on immature and mature DC were analyzed by FACS Calibur.DC was pulsed with HBcAg on day 3 and 5,then DC maturation and ability to process HBcAg and to stimulate autogeneic T cells were evaluated.Results Monocyte- derived DC developed different TLR expression patterns as they went through different maturation stages.TLR7,TLR8 expressions on immature DC and TLR3,TLR7 expressions on mature DC were lower in CHB than in control(for TLR7,TLR8 expression on immature DC:75.9%,1.0%vs 98.4%,15.4%,P

6.Research advance on the pathogenesis of T-ALL induced by notch 1 activating mutations.

T-cell acute lymphoblastic leukemia (T-ALL) is the hematological malignancy of bone marrow characterized by the rapid proliferation and subsequent accumulation of immature T lymphocyte and mainly occurs in children and adolescents. In 1991, a kind of activating mutation of Notch 1 was found in a subset of T-ALL with chromosomal translocation t(7;9) for the first time. During the past 20 years since then, understanding of the relationship between Notch 1 activating mutation and T-ALL has been deepened and widened. This review briefly discusses the four main subtypes of Notch 1 activating mutations, also focuses on how these mutations change the normal signaling pathways and genes expression during their participation in the pathogenesis of T-ALL, and how these insights will promote the development of newly targeting therapies for patients with this aggressive form of leukemia.

7.Application of Endoscopic Vein Harvesting in Obese Patients Undergoing Coronary Artery Bypass Grafting

This study aims to evaluate the clinical outcomes of endoscopic vein harvesting (EVH) for coronary artery bypass grafting (CABG) in obese patients.Totally,153 obese patients who underwent EVH (n=81) or standard bridging technique (SBT,n=72) in CABG surgery from May 2012 to October 2014 in our hospital were enrolled in this retrospective non-randomized controlled study.The general situation of operation,postoperative complications and short medium-term outcomes were analyzed.The baseline characteristics were similar between these two groups (P>0.05).There were no statistical differences in total operation time (226±28 min vs.224±30 min,P>0.05),number of damaged vessels (0.12±0.05 vs.0.16±0.06,P>0.05) and short medium-term outcomes including revascularization rate (1.25% vs.2.78%,P>0.05),vessel dysfunction rate (11.25% vs.11.11%,P>0.05) and mortality (0.00% vs.0.00%,P>0.05).Use of EVH was associated with significant reduction of total harvesting time (41±6 min vs.63±11min,P<0.05),incision length (4.4±1.1 cm vs.18.2±4.5 cm,P<0.05) and postoperative lower extremity complications (P<0.05).EVH can reduce the risk of wound complications,whereas does not influence short-and medium-term outcomes in obese patients.It can be considered a reliable procedure of harvesting vessel conduits for obese patients undergoing CABG.
8.Clinical,neuroimaging and myopathological features of MELAS:a retrospective analysis of 31 patients

Objective To investigate the clinical,neuroimaging and myopathological features of mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes(MELAS).Methods The clinical manifestations,neuroimaging and myopathological features of 31 patients with MELAS diagnosed in our Neuromuscular Center in the recent 7 years were retrospectively analyzed.A3243G point mutations were analyzed by RFLP method in 10 patients.Results ①Clinical features:There were 18 male patients and 13 female patients.The age of onset ranged from 3 to 43 years,averaging 21.9 years.The averaged duration was 4.9 years.Thirteen patients in this group had family history of maternal inheritance pattern.The main clinical manifestations included short stature(26 patients),recurrent headache and vomiting(24 patients), muscle weakness(22 patients),epileptic seizure(21 patients),cognitive decline(19 patients),visual disturbance(17 patients),sensorineural deafness(16 patients),ataxia(6 patients),psychiatric symptom (8 patients),external ophathalmoplegia(2 patients)and diabetes mellitus(9 patients).The serum CK level was slightly elevated in 6 patients,and the fasting blood lactic acid was increased in 15 of the 18 detected patients.②Neuroimaging features:The stroke-like lesions were mostly confined to cerebral cortex, including temporal lobe(24 patients),occipital lobe(21 patients),parietal lobe(12 patients)and frontal lobe(4 patients).Three patients had deep white matter involvement.Migrating stroke-like lesions were confirmed in 4 patients by repeated cranial CT/MRI examination.In addition,cerebral atrophy(17 patients)and bilateral basilar ganglion calcification(11 patients)were found.③Myopathological features: Scattered ragged red fibers(RRF)in various number were found in all the patients by MGT staining.Other founding included strongly SDH-reactive blood vessel(27 patients),COX enzyme deficiency(19 patients), and mild to moderate lipid storage in RRF(20 patients).④MtDNA analysis showed 9 patients with A3243G point mutation in all the detected 13 patients.Conclusion The clinical and neuroimaging features may offer important clue to the diagnosis of MELAS,but a definite diagnosis of MELAS relies on the myopathology and mtDNA mutation analysis.
9.Effect of pre-treatment with intravenous cisatracurium besilate on sufentanyl-induced cough

Objective To observe the effect of intravenous cisatracurium besilate pre -treatment on sufentanyl -induced cough ( SFIC ) . Methods Two hundred patients scheduled for elective surgery were ran-domly divided into two groups.Patients in treatment group received intra-venous cisatracurium besilate 0.01 mg? kg-1 and those in control group were given 0.9%NaCl 1 mL through the left median cubital veins.Five minutes later, the patients were injected with sufentanyl 0.4μg? kg-1 in two seconds.The timing and numbers of coughing were recorded and classified according to numbers.The data of blood pressure, heart rate, respiration, SpO2 and adverse reactions of patients were recorded at the moment of injection of 0.9%NaCl or cisatracurium besilate, the injection of sufentanyl and one and two minutes after injections.Results The in-cidence of cough was 11.0%( 11/100 ) in treatment group and 35%(35/100) in control group (P<0.05).The incidence of moderate and severe cough was significantly higher in the control group ( 71.4%, 25/35) than in treatment group (36.4%,4/11)(P<0.05).There was no significant difference between two groups on the timing of cough, blood pressure and heart rate. Conclusion Cisatracurium besilate pre-treatment can effectively reduce the incidence and severity of SFIC.
10.Effect of Ent-11α-hydroxy-15-oxo-kaur-16-en-19-oic-acid on human gastric cancer cells and its mechanism.

OBJECTIVETo investigate the apoptosis-inducing effect of Ent-11α-hydroxy-15-oxo-kaur-16-en-19-oic-acid (5F), a compound isolated from Pteris semipinnata L (PsL), on human gastric cancer SGC7901 cells and explore its mechanism.
METHODSThe inhibitory effect of 5F on SGC7901 cells was observed by MTT assay and flow cytometry, and the changes of the expression of Bcl-2 and Bax in SGC7901 cells following 5F exposure were evaluated by Western blot analysis.
RESULTS5F inhibited the proliferation of SGC7901 cells in a concentration- and time-dependent manner, and the cell apoptosis induced by 5F was confirmed by Annexin V-EGFP staining and caspase-3 activation assay. The cell apoptosis induced by 5F was associated with decreased Bcl-2 and increased Bax expressions.
CONCLUSION5F exposure induces apoptosis in SGC7901 cells by activating mitochondrial apoptotic pathways.
