1.Clinical study of chemotherapy combined with radiotherapy in the treatment of limited-stage small cell lung cancer

Objective To evaluate the curative effect of chemotherapy combined with radiotherapy for limited-stage small cell lung cancer(LD-SCLC).Methods 34 cases of patients with LD-SCLC were rand- mized into interdigitating chemoradiotherapy group and sequential chemoradiotherapy group.All patients re- ceived four to six cycles of alternating CE(C:carboplatin 300 mg/m~2 d_1;E:etoposide100 mg d_(1~5)and CAP(C: cyclophosphamide 600 mg/m~2 d_(1,8);A:adriamycin 40 mg/m~2 d_1;P:cisplatin 50 mg d_(3~5)chemotherapy,three weeks for a cycle.All patients also received thoracic radiotherapy with conventional fraction,2.0 Gy per frac- tion,five fractions per week.The total dose was 60 Gy.Results In interdigitating chemoradiotherapy group, CR was 64.7%,PR was 29.4%,NR was 5.9% and PD was 0 after chemo-radiation therapy.In sequential chemoradiotherapy group,CR was 23.5%,PR was 52.9%,NR was 23.5% and PD was 0 after chemothera- py,and CR was 58.8%,PR was 35.3%,NR was 5.9% and PD was 0 after radiotherapy.Conclusion The therapeutic effect of chemotherapy combined with radiotherapy for LD-SCLC was satisying.There was no sig- nificant difference in the effect between the two groups.
2.Comparison of Efficacy and Safety of Docetaxel Combined with Carboplatin and Paclitaxel and Cisplatin in the Treatment of Advanced Ovarian Cancer

OBJECTIVE:To compare the efficacy,safety,vascular endothelial growth factor(VEGF)and matrix metallopro-teinase-2 (MMP-2) of docetaxel combined with carboplatin and paclitaxel combined with cisplatin (DDP) in the treatment of ad-vanced ovarian cancer. METHODS:120 patients with advanced ovarian cancer were randomly divided into docetaxel combined with carboplatin group(60 cases)and paclitaxel combined with DDP group(60 cases). Docetaxel combined with carboplatin group received 70 mg/m2 Docetaxel injection,intravenous infusion of 1 h,d1;50 mg/m2 carboplatin injection,intravenous infusion of 1 h,d2. Paclitaxel combined with DDP group received 135 mg/m2 Paclitaxel injection,intravenous infusion of 24 h,d1;30 mg/m2 DDP for injection,intravenous infusion,d3;60 mg/m2 Paclitaxel injection (a maximum of 2.0 m2) by intraperitoneal infusion,d8. 3-week was regarded as 1 treatment course,and it lasted 6 courses. Clinical efficacy,VEGF,MMP-2,progression-free survival, overall survival before and after treatment,mortality rate within 2 years of treatment and the incidence of adverse reactions in 2 groups were compared. RESULTS:There were no significant differences in the objective response rate,disease control rate,mortal-ity rate,incidence of adverse reactions between 2 groups(P>0.05). The progression-free survival in docetaxel combined with car-boplatin group was significantly longer than paclitaxel combined with DDP group,the difference was statistically significant (P<0.05). Before treatment,there were no significant differences in VEGF and MMP-2 level between 2 groups(P>0.05). After treat-ment,VEGF and MMP-2 level in 2 groups were significantly lower than before,and VEGF at different time points and MMP-2 level after 4 weeks,8 weeks and 12 weeks of treatment in docetaxel combined with carboplatin group were lower than paclitaxel combined with DDP group,the differences were statistically significant(P<0.05). CONCLUSIONS:Docetaxel combined with car-boplatin and paclitaxel combined with DDP shows similar efficacy and safety in the treatment of advanced ovarian cancer,but docetaxel carboplatin combined with is superior to paclitaxel combined with DDP in reducing VEGF and MMP-2 and improving pro-gression-free survival.
3.Different concentrations of icariin for bone defect repair:disputes and exploration

BACKGROUND:As one of the main active ingredients in epimedium, icari n plays an important role in bone defect repair. Sustained and effective concentration of icari n in vivo is essential for bone damage repair.
OBJECTIVE:To recommend the research progress of epimedium glycoside for bone repair and to explore the pharmaco-active concentration of icari n.
METHODS:A computer-based online search of CNKI database (http://www.cnki.net/) and PubMed database (http://www.ncbi.nlm.nih.gov/PubMed) from January 2000 to October 2013 was performed for related articles of the effect of icari n for bone defect repair and bone damage repair. The key words were“icari n, concentration, bone”in Chinese and English. After repeated articles were excluded, 76 related articles were screened out and 44 of them met the inclusive criteria.
RESULTS AND CONCLUSION:The icari n-released scaffold materials can induce the osteogenic differentiation of bone marrow-derived mesenchymal stem cells, promote the viability of osteoblasts and inhibit the resorption of osteoclasts, thus repairing bone tissue. It is certain that icari n promotes cellular dif erentiation, however whether it can promote cellular proliferation remains unclear. The pharmaco-active concentration of icari n ranges from10-8 to 10-5 mol/L, but clinical trial has not yet been carried out, and specific drug concentration is uncertain, which needs further exploration.
4.Effect of pre-treatment with intravenous cisatracurium besilate on sufentanyl-induced cough

Objective To observe the effect of intravenous cisatracurium besilate pre -treatment on sufentanyl -induced cough ( SFIC ) . Methods Two hundred patients scheduled for elective surgery were ran-domly divided into two groups.Patients in treatment group received intra-venous cisatracurium besilate 0.01 mg? kg-1 and those in control group were given 0.9%NaCl 1 mL through the left median cubital veins.Five minutes later, the patients were injected with sufentanyl 0.4μg? kg-1 in two seconds.The timing and numbers of coughing were recorded and classified according to numbers.The data of blood pressure, heart rate, respiration, SpO2 and adverse reactions of patients were recorded at the moment of injection of 0.9%NaCl or cisatracurium besilate, the injection of sufentanyl and one and two minutes after injections.Results The in-cidence of cough was 11.0%( 11/100 ) in treatment group and 35%(35/100) in control group (P<0.05).The incidence of moderate and severe cough was significantly higher in the control group ( 71.4%, 25/35) than in treatment group (36.4%,4/11)(P<0.05).There was no significant difference between two groups on the timing of cough, blood pressure and heart rate. Conclusion Cisatracurium besilate pre-treatment can effectively reduce the incidence and severity of SFIC.
6.Changes of rabbit IL-1 and TNF-alpha, etc cytokines in response to acute normovolemic hemodilution with HAES-balanced solution as diluting agent.

AIMTo observe effect of acute normovolemic hemodilution(ANH) with HAES-balanced solution as diluting agent on levels of cytokines including IL-1, IL-2, IL-6 and TNF-alpha in rabbit serum so as to provide theoretical basis for clinical application.
METHODSA total of 20 healthy adult rabbits were enrolled in the study and randomly divided into two groups (10 rabbits per group), i.e., control group (Group C) and HAES group (Group H). Under anesthesia of the rabbits, we performed incision of trachea, high-frequency jet ventilation and liberation of femoral artery and femoral veins. Group C was free from hemodilution. Group H was injected with dilution (2-fold of blood letting volume) via femoral veins during blood letting of the femoral artery. 6% HAES-steril plus compound solution of sodium lactate, with crystal/gel ratio of 2:1, blood letting volume = TBV x (Ho-Hf)/Hav. All blood was transfused back 60-120 min after blood letting. Venous blood was collected before blood letting (T0) and 30 min (T1), 60 min (T2), 120 min (T3) and 24 h(T4) after blood letting to detect Hb and Hct and measure level of IL-1, IL-2, IL-6 and TNF-alpha in serum.
RESULTSIn Group H, levels of IL-1, IL-2, IL-6 and TNF-alpha in serum were increased from T1 after ANH, reached peak at T3 but showed decrease at T4, with significant difference compared with Group C at T1, T2, T3 and T4 (P < 0.01) and significant difference compared with those before ANH (P <0.01). In Group C, there was no significant difference upon IL-1, IL-2, IL-6 and TNF-alpha in serum at different time points.
CONCLUSIONANH with HAES-balanced solution as diluting agent can up-regulate the levels of cytokines IL-1, IL-2, IL-6 and TNF-alpha in rabbit serum. In the meantime, ANH may arouse eustress with low intensity and short action time, which exerts effect of enhancing immune function of the organisms.

7.Protein array technology applied in high throughput monoclonal antibody generation.

UNLABELLEDTo reduce the huge labor-cost in the screening in traditional monoclonal antibody generation, We established a new system for monoclonal antibody generation integrating with protein array. BALB/c mice were immunized by eight recombinant proteins respectively, and the positive hybridoma cells were obtained by cell fusion and ELISA screening. All the eight kinds of positive hybridoma cells were mixed, cloned, screened by protein array, and definite dilution cloned.
RESULTS175 single cell clones were obtained by complex cloning, and 119 of those were positive clones. Then 8 positive cell lines were generated by the following 2 rounds definite dilution cloning. By comparing with the traditional method, we got 8 monoclonal antibodies using the combined protein array screening and multiplex cloning method in 1 cycle, and fewer amounts of antigens were used. As a result, the combined protein array and multiplex cloning method could be used as an economical, rapid and simple tool applying in high throughput monoclonal antibody generation.

8.Medullary ventrolateral nitric oxide mediates the cardiac effect of electroacupuncture at "Neiguan" acupoint on acute myocardial ischemia in rats.

Experiments were performed on male Sprague-Dawley (SD) rats anesthetized with a mixture of urethane and chloralose. A rat model of acute myocardial ischemia (AMI) was made by ligation of the left anterior descending branch of the coronary artery (LAD). After the LAD ligation, the ischemia area of the left ventricular wall became somewhat pale immediately. Under a light microscope, the pathological examination revealed that all the cells were swollen and in red color when the cardiac section was stained with hematoxylin basic fuchsin picric acid (HBFP), which indicated a typical change in the myocardial ischemia. In the AMI model, it was found that cardiac functions were markedly attenuated, such as decreases in the heart rate (HR), mean arterial pressure (MAP), left ventricular systolic pressure (LVSP), maximal rate for left ventricular pressure rising and declining (+/-dp/dt(max)), velocity of contractile element (V(CE)) and total area of cardiac force loop (L(0)), and an increase in the left ventricular end diastolic pressure (LVEDP). In such AMI rats, application of electroacupuncture (EA) at "Neiguan" acupoints (Pe 6) for 20 min could obviously improve the above-mentioned cardiac functions. After microinjection of nitro-L-arginine (L-NNA), an inhibitor of nitric oxide synthase (NOS), was made into the rostral ventrolateral medulla (RVLM), the curative effect of EA on myocardial ischemia was reduced significantly or abolished, while after microinjection of normal saline of the same volume was made into the RVLM, the improving effect of EA remained. These results suggest that the effect of EA on myocardial ischemia is possibly mediated by the nitric oxide (NO) in the RVLM.

The syndrome assessment tool for dementia used nowadays needs to be optimized in content, reliability, and validity. In this paper, the authors reviewed some problems correlated with the design of Chinese medical assessment questionnaire on the basis of Chinese medical theories by combining the common requirements for questionnaire development.

The constituents in 95% ethanol extract of the root of Rosa cymosa Tratt were purified by column chromatography techniques, leading to isolation of eleven triterpenes. Their structures were elucidated by spectroscopic data as pomolic acid (1), fupenzic acid (2), ursolic acid (3), euscaphic acid (4), arjunic acid (5), tomentic acid (6), 3β-E-feruloyl corosolic acid (7), 1β-hydroxyeuscaphic acid (8), myrianthic acid (9), cecropiacic acid (10), and ilexoside B (11). Among them, compounds 3, 6-8, 10 and 11 were obtained from this plant for the first time, and compounds 7 and 10 were obtained from this genus for the first time.
