1.Clinical observation on tuina plus foot bath with Chinese medicine for diabetic foot in early stage

Objective:To observe the clinical effect of tuina plus foot bath with Chinese medicine for patients with diabetic foot (DF) in early stage.Methods:A total of 70 patients with early-stage DF were randomly allocated by the random number table into two groups,with 35 cases in each group.Patients in the control group received conventional medication,while patients in the observation group received tuina plus foot bath with Chinese medicine on the basis of conventional medication.The clinical efficacy was compared after 2 courses of treatment.Results:After treatment,intra-group comparisons of ankle-brachial index (ABI) showed statistical significance in both groups (both P<0.05).The curative rate was 83.3% in the observation group,with the total effective rate of 96.7%,versus 29.4% and 76.5% in the control group,respectively,and the between-group comparisons showed statistical significance (both P<0.05),indicating a better effect in the observation group.Conclusion:Tuina plus foot bath with Chinese medicine has a good therapeutic effect for DF patients in early stage.
2.Comment and Analysis for Prescriptions of Chinese Herbal Pieces in an Integrative Traditional Chinese and Western Medicine Hospital

Objective:To review and analyze the prescriptions for Chinese herbal pieces in an integrative traditional Chinese and western medicine hospital to explore the trend and problems in clinical application for Chinese herbal medicines and provide reference for dispensing in pharmacy of traditional medicine and rational drug use. Methods: Totally 1200 prescriptions for Chinese herbal pieces in our hospital were surveyed from January to December in 2014. The irrational use was analyzed, and reasonable suggestions were provided for the clinical application. Results:The problems in the prescriptions for Chinese herbal pieces included uncompleted diagnosis,incompatible diagnosis and medication,nonstandard Chinese name,unclear footnote,irrational usage or dosage and incompati-bility. The prescriptions including above 15 varieties of Chinese herbal pieces in one accounted for 65. 70%, and those with average dose above 250 g accounted for 40. 25%. Conclusion:Regular inspection and quality supervision on the prescription for Chinese herb-al pieces should be strengthened to improve the quality of prescriptions and guarantee the safety and economy of the medication.
3.Observation on therapeutic effect of acupuncture at abdomen acupoints plus tuina for lumbar intervertebral disc herniation

Objective:To observe the clinical efficacy of acupuncture at abdomen acupoints plus tuina for lumbar intervertebral disc herniation (LIDH).Methods:A total of 70 patients with LIDH were randomized into an observation group and a control group,with 35 cases in each group.The observation group was treated with acupuncture at abdomen acupoints plus tuina,while the control group was treated only with tuina treatment.The clinical efficacy was observed after one course of treatment.Results:The cure rate and the total effective rate of the observation group were 83.3% and 96.7%,respectively.The cure rate and the total effective rate of the control group were 39.4% and 78.8%,respectively.There were significant differences in the cured rate and the total effective rate between the two groups (both P<0.05).There was no significant difference in Japanese Orthopedic Association (JOA) score between the two groups before treatment (P>0.05).After treatment,the JOA scores of both groups increased significantly,and the intra-group differences were statistically significant (both P<0.05);the JOA score of the observation group was significantly higher than that of the control group (P<0.05).Conclusion:Acupuncture at abdomen acupoints plus tuina has a better therapeutic effect than tuina alone in the treatment of LIDH.
4.Altered of zinc finger proteins expression in FL cells following benzo a pyrene treatment.

OBJECTIVETo understand benzo[a]pyrene (B[a]P) mediated cellular responses, and to provide clues to explore molecular mechanism of mutagenesis and carcinogenesis induced by B[a]P.
METHODSTwo-dimensional electrophoresis (2-DE) was used to investigate the protein expression levels of FL cells after B[a]P exposure, matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF-MS) combined with database search was applied to identify the differentially expressed proteins.
RESULTStatistical analysis showed that the volumes of 47 protein spots were altered after B[a]P treatment (P<0.05) and 23 proteins were successfully identified, including zinc finger proteins, SWI/SNF related protein, Bromo domain containing domain and other proteins.
CONCLUSIONThese affected proteins may be involved in the cellular responses to B[a]P exposure, and may mediate the B[a]P induced mutagenesis and carcinogenesis.

5.Proteomic analysis of cellular responses to low concentration of N-methyl-N'-nitro-N-nitrosoguanidine in human amnion FL cells.

OBJECTIVETo investigate the protein profile after treatment of low concentration of N- methyl-N'-nitro-N-nitrosoguanidine (MNNG) in human FL cells.
METHODSAfter MNNG treatment, whole cellular proteins were separated using two-dimensional gel electrophoresis and visualized by silver staining; the digitized images were analyzed with 2D analysis software. The differentially expressed protein spots were identified by matrix-assisted laser desorption ionization time of flight mass spectrometry (MALDI-TOF-MS).
RESULTMore than 60 proteins showed significant changes in MNNG-treated cells compared to control cells (DMSO treatment). There were 18 protein spots detected only after MNNG treatment, while 13 protein spots were detected only in the control cells. Moreover, the levels of another 31 proteins were either increased or decreased in MNNG-treated FL cells. And some of the proteins were identified by MALDI-TOF-MS.
CONCLUSIONThere are significant alterations of protein profile after MNNG attack.

6.Therapeutic Observation of Warm Needling at Yintang (GV 29) plus Ginger-partitioned Moxibustion for Allergic Rhinitis

Objective To observe the clinical efficacy of warm needling at Yintang (GV 29) plus ginger-partitioned moxibustion in treating allergic rhinitis.Method Seventy patients with allergic rhinitis were randomized into a treatment group and a control group,35 cases each.The treatment group was intervened by warm needling at Yintang (GV 29) plus ginger-partitioned moxibustion;the control group was treated with conventional medication.After 4-week treatment,the clinical efficacies of the two groups were compared.Result The markedly effective rate and total effective rate were respectively 83.9% and 96.8% in the treatment group,versus 56.3% and 75.0% in the control group,and the between-group differences were statistically significant (P<0.05).Conclusion Warm needling at Yintang (GV 29) plus ginger-partitioned moxibustion is an effective approach in treating allergic rhinitis.
7.Impact of fish oil enriched total parenteral nutrition on elderly patients after colorectal cancer surgery.

BACKGROUNDPolyunsaturated omega-3 fatty acids may beneficially influence healing processes and patient outcomes. The aim of this research was to study the clinical efficacy of fish oil enriched total parenteral nutrition in elderly patients after colorectal cancer surgery.
METHODSFifty-seven elderly patients with colorectal cancer were enrolled in this prospective, randomized, double-blind, controlled clinical trial. All patients received isocaloric and isonitrogenous total parenteral nutrition by continuous infusion (20 - 24 hours per day) for seven days after surgery. The control group (n = 28) received 1.2 g/kg soybean oil per day, whereas the treatment group (n = 29) received 0.2 g/kg fish oil and 1.0 g/kg soybean oil per day. Blood samples were taken pre-operatively, and at days one and eight after the operation. The plasma levels of CD4, CD8, CD4/CD8, interleukin 6 (IL-6) and tumor necrosis factor α (TNF-α) were measured. Clinical outcomes were then analysed.
RESULTSPatient characteristics were comparable between the two groups. At day eight post-surgery, IL-6, TNF-α and CD8 titres were lower in the treatment group when compared to the control group; these results reached statistical significance. In the treatment group, there were fewer infectious complications and incidences of systemic inflammatory response syndrome (SIRS), and shorter lengths of hospital stay were observed. The total cost of medical care was comparable for the two groups. No serious adverse events occurred in either group.
CONCLUSIONSFish oil 0.2 g/kg per day administrated to elderly patients after colorectal surgery was safe and may shorten the length of hospital stay and improve clinical outcomes.

8.A Cross-sectional Study Assessing Predictors of Essential Medicines Prescribing Behavior Based on Information-motivation-behavioral Skills Model among County Hospitals in Anhui, China.

BACKGROUNDThe self-consciousness and practicality of preferentially prescribed essential medicines (EMs) are not high enough in county hospitals. The purposes of this study were to use the information-motivation-behavioral skills (IMB) model to identify the predictors of essential medicines prescribing behavior (EMPB) among doctors and to examine the association between demographic variables, IMB, and EMPB.
METHODSA cross-sectional study was carried out to assess predictive relationships among demographic variables and IMB model variables using an anonymous questionnaire administered in nine county hospitals of Anhui province. A structural equation model was constructed for the IMB model to test the instruments using analysis of moment structures 17.0.
RESULTSA total of 732 participants completed the survey. The average age of the participants was 37.7 ± 8.9 years old (range: 22-67 years old). The correct rate of information was 90.64%. The average scores of the motivation and behavioral skills were 45.46 ± 7.34 (hundred mark system: 75.77) and 19.92 ± 3.44 (hundred mark system: 79.68), respectively. Approximately half (50.8%) of respondents reported that the proportion of EM prescription was below 60%. The final revised model indicated a good fit to the data (χ2 /df = 4.146, goodness of fit index = 0.948, comparative fit index = 0.938, root mean square error of approximation = 0.066). More work experience (β = 0.153, P < 0.001) and behavioral skills (β = 0.449, P < 0.001) predicted more EMPB. Higher income predicted less information (β = -0.197, P < 0.001) and motivation (β = -0.204, P < 0.001). Behavioral skills were positively predicted by information (β = 0.135, P < 0.001) and motivation (β = 0.742, P < 0.001).
CONCLUSIONThe present study predicted some factors of EMPB, and specified the relationships among the model variables. The utilization rate of EM was not high enough. Motivation and behavior skills were crucial factors affecting EMPB. The influence of demographic variables, such as income and work experience, on EMPB should be fully appreciated. Comprehensive intervention measures should be implemented from multiple perspectives.

9.Effects of early Xuebijing injection(血必净注射液) treatment on the change of prognosis in patients with multiple traumas

Objective To observe the clinical efficacy of moxibustion plus tuina in treating early-stage diabetic foot. Method Seventy patients with early-stage diabetic foot were randomized into a treatment group and a control group, 35 cases each. The control group was intervened by conventional medication, while the treatment group was additionally intervened by moxibustion plus tuina. After 2 treatment courses, the changes of ankle bronchial index (ABI) in the two groups were observed, and the clinical efficacies were compared. Result Patients in the treatment group showed significant improvement in symptoms, and the total effective rate was 96.9%. The total effective rate was 75.8% in the control group. The difference between the two groups was statistically significant (P<0.05). Conclusion Moxibustion plus tuina is safe and effective in treating diabetic foot. With significant Chinese medicine feature and acceptable to patients, this method is a good choice in treating early-stage diabetic foot.
