1.Additive effects of the variants in the peroxisome proliferator activated receptor γ2 and uncoupling protein 2 genes on type 2 diabetes

Objective To investigate the additive effects of uncoupling protein 2(UCP2) gene 3′-untranslated region(3′-UTR) 45-base pair insertion/deletion( I/D) variation and peroxisome proliferator activated receptor( PPAR)γ2 gene Pro12Ala variation on type 2 diabetes(T2DM) in Chinese Han popula-tion.Methods The UCP2 gene 3′-UTR I/D variation and PPARγ2 gene Pro12Ala variation were exam-ined by polymerase chain reaction-restriction fragment length polymorphism ( PCR-RFLP) in 490 type 2 dia-betes subjects and in 585 control subjects .The additive effects of the two gene mutations were analyzed . Results ⑴The frequency of PPARγ2 gene Pro12Ala variation in type 2 diabetes was not significantly dif-ferent from that in control subjects.(χ2 =0.058, P =0.809).In T2DM group, A12 allele carriers had larger waist circumference than Pro12Pro genotype carriers;Glucose stimulated insulin secretion by oral glu-cose tolerance test (OGTT) was significantly lower in carriers of the Ala12Ala or Pro12Ala genotype com-pared with Pro12Pro genotype.( Z =2.222, P =0.026; Z =2.051, P =0.040; Z =2.079, P =0.038 ) .⑵There wasn't significant difference among 3 genotypes with 3′-UTR I/D variation in UCP2 gene (χ2 =2.311 , P =0.315 ) .In control group , Glucose stimulated insulin secretion by oral glucose tolerance test ( OGTT) was significantly higher in carriers of the I/D or I/I genotype compared with D/D genotype ( Z =1.997 , P =0.046 ) .⑶The genotype carriers with Pro/Ala +Del/Del were the greatest relation to T2DM (OR=1.22, 95%CI 1.078~1.386).Conclusion Though the UCP2 gene mutation alone or the PPARγ2 gene mutation alone is not associated with T 2DM, the possible additive effects of the two micro genes increase the occurring of T 2DM.
2.Efficacy of montelukast sodium in the treatment of stable chronic obstructive pulmonary disease

Objective To investigate the application value of montelukast in patients with stable chronic obstructive pulmonary disease (COPD).Methods 64 patients with stable COPD in Jinqiu Hospital of Liaoning Province from December 2015 to December 2016 were randomly divided into observation group and control group according to the digital table,32 cases in each group.The two groups were given routine treatment,including cough,phlegm,oxygen therapy,smoking cessation,rehabilitation treatment,regular use of long-acting bronchodilators,on this basis,the observation group was treated with montelukast sodium.After 12 weeks of treatment,the patients were assessed for airway improvement and quality of life before and after treatment,and recorded the two groups of acute exacerbation and adverse drug reactions.Results The two groups had improved lung function and quality of life after treatment,but after treatment,the FEV1,FVC and FEV1 actual/FEV1 prediction of the observation group were significantly higher than those of the control group,the differences were statistically significant (t =2.49,1.77,1.85 all P < 0.05).After treatment,the 6 minutes walking distance of the observation group was longer than that of the control group,Borg score of the observation group was lower than that of the control group,the differences were statistically significant (t =1.83,6.66,all P <0.05).After treatment,the SGRQ score in the observation group was lower than that of the control group,the difference was statistically significant[(51.15 ± 3.14)points vs.(54.83 ± 4.03)points,t =3.94,P < 0.01].Conclusion Montelukast can effectively improve the stability of COPD pulmonary function and quality of life,and with high safety.
3.Study on the risk factors for coronary artery lesions in children with Kawasaki disease in Lanzhou

Objective To investigate the risk factors for coronary artery lesions (CALs) in children with Kawasaki disease (KD) in Lanzhou. Methods One hundred and seventy-four children with diagnosed KD were divided into CAL group and non-CAL group based on the existence of concurrent CALs. The age, gender, fever duration, intravenous immunoglobulin (IVIG) start time, IVIG dose, C-reactive protein (CRP), serum albumin, erythrocyte sedimentation rate (ESR), platelet (PLT), red blood cell count (RBC), hemoglobin and so on were compared. Results Among the 174 children, 46 children (26.44%) were complicated by CALs and 128 children were not. The differences of average fever duration, IVIG starting time, IVIG dose, PLT, CRP, ESR and RBC were statistically signiifcant (P<0.05). Conclusions When KD children has the fever durations>10 d, start of IVIG af-ter 10 days of fever, increase of PLT, CRP and ESR and decrease of RBC, clinicians should be alert to the risk of concurrent CAL.
4.A novel citrinin derivative from the marine-source fungus Penicillium citrinum.

A novel citrinin derivative, penicitrinol L (1), along with two known analogues, penidicitrinin B (2) and pennicitrinone A (3) were isolated from the marine-source fungus Penicillium citrinum. The structure of the new compound was elucidated by spectroscopic methods including one and two-dimensional NMR as well as high-resolution mass spectrometric analysis. Furthermore, compound 1 showed modest cytotoxic activity against HL-60 cell line and compound 3 showed weak cytotoxic activity against A375 cell line.
5.Simultaneous Content Determination of Alantolactone and Isoalantolactone of Radix Inulae in Sanwei Ganlusan by HPLC

Objective To establish a method to simultaneously determine contents of alantolactone and isoalantolactone in Sanwei Ganlusan. Methods Contents of alantolactone and isoalantolactone was determined by HPLC was used with PDAD detector;the column was CAPCELL PAK MG C18 (4.6 mm×250 mm, 5μm);the mobile phase consisted of acetonitrile-0.4%phosphoric (58∶42);the flow rate was 1.0 mL/min;the detection wavelength was set at 225 nm;the column temperature was maintained at 30 ℃. Results The linear range of alantolactone was 0.083-0.517 μg (r=0.999 9), and isoalantolactone was 0.108-0.672 μg (r=0.999 9). The mean recovery of alantolactone was 97.95%, RSD=1.31%. The mean recovery of isoalantolactone was 97.69%, RSD=1.24%. Conclusion The method is accurate and simple in operation, which can be used to simultaneously determine contents of alantolactone and isoalantolactone in Sanwei Ganlusan.
6.Content Determination of Phellodendrine in Leshu Lotion by SPE-HPLC

Objective To establish the method of thin layer identification of Phellodendri Chinensis Cortex and solid phase extraction - high performance liquid chromatography (SPE-HPLC) determination of the content of phellodendrine in Leshu Lotion. Methods Thin layer chromatography (TLC) was used for qualitative identification of Phellodendri Chinensis Cortex in Leshu Lotion and the Bond Elutplexa PCX strong cation-exchange solid phase was used for extraction small column purification. Platisil-NH2 column was as chromatographic column; column temperature was 30 ℃; triethylamine pH 3.82 (phosphoric acid), tetrahydrofuran, acetonitrile was 18:12:70, isocratic elution; flow rate was 1.0 mL/min; detection wavelength was 286 nm. Results TLC could obviously identify active ingredients of phellodendrine in Phellodendri Chinensis Cortex. The phellodendrine had good linear relationship between concentration and peak area within the scope of 1.22–12.2 μg (r=0.9999); the average recovery rate of phellodendrine was 101.18%, RSD was 1.71%. Conclusion This method is simple to operate, with accuracy and repeatability, and can be used for quality control of Leshu Lotion.
7.Combined detection of capsule associated protein 10 and virulence-associated DEAD-box RNA helicase 1 in the clinical diagnosis of Cryptococcus neoformans meningitis

Objective To establish the quantitative detection of capsule associated protein 10 (CAP10)and virulence-associated DEAD-box RNA helicase 1(VAD1)genes in Cryptococcus neoformans (CN) and compare the diagnostic values of single gene test and combined gene test in CN meningitis.MethodsTwenty-three CN meningitis patients with fungal culture or ink staining or CN antigen detection positive in cerebrospinal fluid (CSF) were recruited and patients with craniocerebral trauma were recruited as controls.Standard plasmids were constructed using standard CN strain.Real time fluorescent quantitative polymerase chain reaction (RT-FQ-PCR) was established to detect the mRNA expressions of CAP10 and VAD1 genes in the CSF of patients with CN meningitis,which were compared with the results of CSF ink staining,fungal culture and antigen detection.The diagnostic values of single gene test and combined gene test were compared by chi square test.Results Among 23 CN meningitis patients,22 (95.6%) were CAP10 mRNA positive detected by RT-FQ-PCR,which was significantly higher than both ink staining (16/23,69.6%,x2 =4.167,P<0.05) and fungal culture (15/23,65.2%,x2=5.143,P<0.05),respectively; but not significant different from antigen detection (21/23,91.3%,x2=0.500,P>0.05).There were also no statistical significant differences between combined detection of CAP 10 + VAD1 and CAP 10 or VAD1 single gene test (P>0.05).ConclusionRT-FQ-PCR detection is successfully established using virulence genes as target,which is superior to the conventional methods.

9.Association of CA repeats polymorphism in the promoter region of the insulin-like growth factor- Ⅰ gene with type 2 diabetes

Objective To investigate the association of CA repeats polymorphism in the promoter region of human IGF-I gene with type 2 diabetes in narthen Han nationality. Methods Gender-age matched subjects with type 2 diabetes and normal glucose tolerance (NGT) were enrolled for this study, 147 subjects in type 2 diabetes group and 159 subjects in NGT group. Genomic DNA was extracted by standard methods. PCR, Genescan,Genotyper, and direct sequencing were conducted to screen CA repeats polymorphism in the promoter region of the human IGF-I gene. Results No significant association was observed between any ( CA), repeat genotype and type 2 diabetes. A lowered serum uric acid was seen in genotypes that included alleles with larger than (CA) 20 repeats [(4.18 ± 1.25 vs 4.63 ± 1.36) mg/dl, P = 0.03]. Conclusion Alleles with larger than (CA) 20 repeats may be a protective factor for type 2 diabetes.
