[Objective]To compare spontaneous correction of the unfused thoracic curves after selective anterior versus posterior fusion in Lenke5 adolescent idiopathic scoliosis(AIS). [Method]A total of 72 Lenke5 AIS patients were rescruited from May 1997 to October 2005.Out of them,40 received selective anterior fusion(group A) and 32 received selective posterior fusion(group B).All had a minimum of 2-year follow-up.[Result] No complication were found in both groups at the latest follow-up.The thoracic curve was corrected from 30? to 16? for group A,33? to 18? for group B.Both groups had a better spontaneous correction of the unfused thoracic curves. The correction rate had no significant difference between groups. However, the thoracic curve was increased in four patients(2 in each group;group C),which resulted in trunk imbalance. The thoracolumbar/lumbar thoracic(TL/L:T) Cobb's ratio averaged 1.09 in the four patients whereas 1.59 in other 68 patients(group D).The flexibility of the thoracic curve had significant difference in group C and D(34.2% vs 57.3%).[Conclusion]Both of the surgical treatments can get a better spontaneous correction of the unfused thoracic curves. It is important to evaluate the.thoracolumbar/lumbar–thoracic(TL/L:T) Cobb's ratio and the flexibility of the thoracic curve before selective fusion.

Objective To investigate the causes and therapy of complications of microvascular decompression (MVD) for hemifacial spasm (HFS). Method Totally 255 cases of HFS underwent MVD in our hospital from December 2000 to March 2000. Among the cases,207 patients were followed up for more than 1 year, the occurrence and recovery of their postoperative complications were reviewed in this study. Results In this series,no patient died;cerebrospinal fluid leakage was detected in 15 patients (7.2%) in one week after the operation;facial palsy occurred in 48 cases (23.2%);hearing deficit was shown in 20 patients (9.7%);and tinnitus was found in 12 cases (5.8%). The patients were followed up for 1 to 7.3 years with a mean of (4.2?2.7) years, during the period, none of the cases had cerebrospinal fluid leakage or intracerebral infection, and the cases who had facial palsy and tinnitus recovered completely;only 10 patients remained hearing deficit (including unilateral deaf in 6) during the follow-up.Conclusion Cerebrospinal fluid leakage, facial palsy, hearing deficit, tinnitus are the major complications of MVD,among which cerebrospinal fluid leakage, facial palsy, and tinnitus can be improved after treatments, but hearing deficit is non-reversible.

Objective]The clinical efficacy of TCM directed through drug treatment of pain for the first time with the temperature needle shoulder-hand syndrome after caused by stroke.[Methods]The 90 cases were randomly divided into treatment and control groups. The treatment group was directed through medicine with traditional Chinese medicine acupuncture treatment temperature ,the control group using only warm acupuncture.Respectively, before treatment, the first 30 days of oral grading score(VRS) was assessed for limb pain.[Results]After treatment ,the patient's limb pain scores were significantly lower than before treatment, treatment group decreased more significantly than the control group, the difference was statistically significant(P﹤0.05). [Conclusion]TCM directed through drug treatment and needle with warm warm acupuncture alone can significantly ease the pain of limb ,but TCM directed through drug treatment is better than warm needle acupuncture alone.

Our country has been using maturity grading method, which was proposed by Grammum in 1979, to evaluate the placental function. However, this method is subjective to consequence because it totally depends on the observation and experiences of clinicians. With the development of ultrasound technology, therefore, we reviewed more novel applications in other aspects of placenta (such as blood flow, vascularization, etc). Over the past years, scholars in the world have done a lot of research around these topics. In this review we introduce placental maturity grading with B-mode ultrasound, placental vascularization qualitative and quantitative analysis with three-dimensional Doppler ultrasound and placental volume measurement, respectively.

Chinese Journal of Endocrinology and Metabolism 2012;28(1):81-84
Abnormal metabolism in vivo of uric acid leads to many diseases.Recent discovery shows that glucose transporter-like protein-9,which belongs to glucose transporter family not only transports glucose, but also plays an important role in the process of uric acid transport, which is also affected by pH,glucose, estrogen, etc. The variation of glucose transporter-like protein-9 results in metabolic diseases. Therefore, the study of glucose transporterlike protein-9 in uric acid transport and related drug will provide new ideas to control the development of hyperuricemia and related cardiovascular disease.

Objective To study the effect of mechanical ventilation guided by esophageal pressure on hemodynamics and oxygen metabolism of severe acute pancreatitis (SAP) swine model with intra-abdominal hypertension (IAH) Methods By self-controlled study,SAP model was made by infusing sodium taurocholate (5%) into the pancreatic duct in 6 domestic swine.Mechanical ventilator mode was volumeassist control with tidal volume 10 ml/kg ; FiO2 40% and PEEP 5 cm H2O (routine ventilation) given to SAP model swine. After 3 h ventilation,pneumo-peritoneum was made with N2 gas to increase the intraabdominal pressure (IAP) to 25 mm Hg in SAP swine.Three more hours later,PEEP was adjusted as the measurement of esophageal pressure (Pes guided ventilation) to such a level that trans-pulmonary pressure stayed above 0 cm H2O during end-expiratory occlusion.During the investigation period, heart rate (HR),cardiac output index (CI),central venous pressure (CVP),mean arterial pressure (MAP) and pulmonary arterial wedge pressure (PAWP) were continuously recorded with the aid of Swan-Ganz catheter and ECG monitor, and oxygen partial pressure of artery (PaO2) and carbon dioxide partial pressure of artery (PaCO2) were measured by blood-gas analysis. In addition, systemic oxygen delivery (DO2) and systemic oxygen consumption (VO2) were calculated by using the data of blood-gas analysis of arterial and central venous blood. Results No swine model was subjected to barotrauma. After routine mechanical ventilation, there were significant differences in HR,CI,MAP,CVP,PAWP,Ppeak,Pplat,Pes,pulmonary compliance (Cstat),PaO2,and DO2 between SAP and IAH in swine (all P < 0.05).Compared with routine ventilation,however,PaO2 and Cstat improved significantly with lower CI and increased Pplat after PEEP adjusted according to measurements of esophageal pressure (all P < 0.05).The lactate decreased significantly after esophageal pressure guided ventilation (all P < 0.05).There were no significant changes in PaCO2,HR,MAP,CVP and PAWP in IAH swine after mechanical ventilation with routine parameters (all P > 0.05).Conclusions There were remarkable effects on oxygen metabolism in response to mechanical ventilation guided by esophageal pressure. In case of clinical application of mechanical ventilation, the results of this study are in favor of setting transpulmonary pressure according to measurements of esophageal pressure in SAP patients with IAH in an early stage.
