Journal of Medical Research 2007;52(5):7-11
Background:Hypovolemia is a common cause of hypotension and low cardiac index (CI) in cardiac surgery but no hemodynamic parameters reflect this status well. The accurate diagnosis of hypovolemia is important because the wrong treatment will cause ineffectiveness and bad consequences such as severe heart failure, pulmonary edema, ... Objectives: To evaluate the performance of diagnostic characteristics of the trendelenburg 300 test for hypovolemia in cardiac surgery. Subjects and method: The prospective, cross ?sectional and randomized controlled trial (RCT) study was conducted on 30 patients (18 males, 12 females and average age 47,17 \xb1 13,93) undergoing valvular repair/replacement or coronary revascularization. The Swan ?Ganz catheters were placed in 20 patients and PiCCO catheters in 10 patients. Trendelenburg 300 test is considered positive if blood pressure (BP), central venous pressure (CVP), CI and intrathoracic blood volume (ITBV) increase. Results: The hypovolemic status in cardiac surgical patients is diagnosed if BP and/or CI increase in trendelenburg 300 position (Se 87.5% and 65.63%; Sp 100% and 75%, area under ROC 0.83 and 0.81, respectively). Conclusion: The increases in BP and CI responding to trendelenburg 300 position are good indicators of hypovolemia in cardiac surgery.

Journal of Medical Research 2008;54(2):1-5
Background: Heparan-sufate interacting protetin (HIP) has been known to be up-regulated and expressed in various human cancer cell lines at both transcript and protein levels. HIP’s expression was related to the differentiation status and cancer development. Objective: Using a semi-quantitative PCR method to determine HIP transcript levels in benign prostatic hyperplasia (BPH), prostatic intraepithelial neoplasia (PIN) and prostate cancer tissues. Subjects and methods: 30 samples of BPH, 12 samples of high-grade PIN, and 40 samples of prostate cancer were collected from patients at Viet Duc Hospital and Friendship Hospital. Total RNA was extracted from BPH, PIN and prostate cancer tissues; cDNA synthesis by reverse transcript - polymerase chain reaction (RT - PCR); HIP transcript determination using semi-quantitative PCR. Results: There was significant difference in HIP transcript levels. HIP transcript was very highly up-regulated in the prostate cancer tissues. The up-regulation of HIP transcript was lower in PIN, and lowest in BPH. HIP transcript levels in benign samples were 1/2 and 2/3 compared with cancer and PIN samples, respectively (P< 0.05). These indicated that up-regulation of HIP transcript may be an early event in tumorigenesis. Conclusions: Levels of HIP transcript were different between tissues of prostate cancer, PIN, and BPH. HIP may be a marker for pre-cancer of the prostate.

Journal of Medical Research 2008;56(4):104-111
Background: Metabolic syndrome is characterized by a group of metabolic risk factors in one person but not a real disease. For this reason, metabolic syndrome is underestimated by clinicians and individuals. Metabolic syndrome that was not detected and treated timely can cause serious diseases. Objectives: 1) To investigate the abnormal blood biochemical parameters related to metabolic syndrome. 2) To determine the abnormal ratio of these parameters. 3) To screen the metabolic syndrome among the staffs of Ha Noi Medical University. Subjects and method: 229 staffs underwent laboratory tests to diagnose for the metabolic syndrome. Abnormal blood biochemical parameters were defined as follows: Fasting blood glucose level increased 6.1mmol/l or more. Triglycerides level is 2.3mmol/l or more, HDL-cholesterol decreased to less than 1.03mmol/l for men and less than 1.24mmol/l for women. Total cholesterol level is 5.2mmol/l or more. CT to HDL-C ratio is more than 3.63 and LDL cholesterol level is 3.5mmol/l or more. Results. Rate of subjects with glucose intolerance or blood fasting glucose higher than 6.1mmol/L is 20.09%. Rate of subjects with dyslipidemia is 61.14%. Average levels of blood glucose, triglycerides and cholesterol increase with age. Number of staffs of Ha Noi Medical University have metabolic syndrome in 2007 are 32 persons (14%). M/F rate is equal and most common in persons over 45 years of age. Conclusion:There are significant number of staffs at the Ha Noi Medical University that had abnormal blood glucose and cholesterol levels related to metabolic syndrome.

Journal of Medical Research 2008;56(4):98-104
Background: Heparansulfate Interacting Protein (HIP) is up-regulated in various human cancer cell lines at both transcript and protein levels. HIP expression is related to the differentiation status and cancer development. Objectives: To determine HIP in benign prostatic hyperplasia, prostatic intraepithelial neoplasia and prostate cancer tissues. Materials and method: Western blot method was used to determine HIP expression in 3 different types of prostate tissue, including 11 prostate cancer samples, 2 benign prostatic hyperplasia samples and 11 prostatic intraepithelial neoplasia samples. Results. HIP was particularly up-regulated in prostate cancer and prostatic intraepithelial neoplasia, indicating that up-regulation of HIP expression may be an early event in tumorgenesis. Conclusion: The expression of HIP was different between cancer, prostatic intraepithelial neoplasia tissue and benign prostatic hyperplasia. HIP may serve as a prognostic marker for prostate carcinoma.

Journal of Medical Research 2007;53(5):124-129
Background: Sodium bicarbonate is highly alkaline and may be helpful in relieving symptoms of gout. Sodium bicarbonate helps to adjust your body's pH level, slightly creating an alkaline-forming state that lessens the likelihood of gout attacks. We can prepare sodium bicarbonate in several ways to treat gout symptoms. Objective: To examine the efficacy of sodium bicarbonate for treatment of gout. Subjects and method: A study was conducted in 54 patients with gout at Department of Rheumatology of Bach Mai hospital, from September 2006 to March 2007. Patients were divided into two groups, group 1 included 29 patients treated with intravenous sodium bicarbonate and group 2 included 25 patients treated with oral sodium bicarbonate. This was a descriptive, cross-sectional study. Results: No significant changes showed between serum uric acid level, 24-hour urine uric acid level at before and after treatment in 2 groups. Gout patients with renal failure had increased level of 24-hour urine uric acid after treatment (p < 0.05). There were significantly increased level of urine volume, urine pH and decreased level of urine specific gravity after treatment. The effect of sodium bicarbonate was not different between intravenous usage and oral usage. Conclusion: Sodium bicarbonate could change urine environment, limit crystallization of urine uric acid of gout patients.

Journal of Medical Research 2008;58(5):1-7
Background: Identifying the factors which affect the invasion and metastasis process takes a vital role in the treatment of cancer. This is the first study on the relationship between the expression of matrix metalloproteinase (MMP – 9), invasion and metastasis of nasopharyngeal carcinoma (NPC) in Vietnam. Objectives: To evaluate the significance of matrix metalloproteinase – 9 in predicting the metastasis potential of NPC. Subject and Method: The expression of MMP – 9 was studied by immunohistochemical analysis in 33 nasopharyngeal carcinoma sections and the associations of MMP-9 with lymph nodes metastasis and clinical features. Results: Positive immunostaining for MMP-9 was observed in 94.94% of the NPCs, 39.4% of the cases being extensively positive (+++). The expression of MMP-9 correlated significantly with cervical lymph nodes metastasis (p<0.05). 5/16 cases (31.25% of the cases) without cervical lymph nodes metastasis expressed MMP-9 with 2+ or 3+ level, non-significant of MMP-9 expression with T classification and clinical stage. Conclusions: The expression of MMP-9 often goes along with lymph nodes metastasis in nasopharyngeal carcinoma.
