156 patients with facial paralysis treated in hospital No103 from 1991 to July 2002 were studied by technique of descriptive observation X ray photography at Schiller position to detect mastoid malformation to find the cause and to determine the treament. In 60% of cases, the cause was not identified, especially Charles Bell paralysis can caused by local dysturbance of vasculority. In such cases vitamine B can administered in combining with acupuncture. In case of pain, high dose of prednisolon can be used with dilatater medicines. Background: High glucose level can cause many serious complications, especially for type 2 diabetic patients. Poorly blood glucose control in type 2 diabetic patients usually induces serious complications. Objective: (1) To evaluate blood glucose control state and (2) To describe affected factors to blood glucose control in type 2 diabetic patients. Subjects and method: We carried out a survey on blood glucose control in 104 type 2 diabetic patients (based on HbA1c results), admitted to Dept of Endocrinology, Bach Mai Hospital from December 2005 to April 2006. 73.1 % of patients is female, average age is 63.4 years old, 57.7% has hypertension, and 62.5% has diabetes for \ufffd?5 years. Results:Blood glucose is poorly controlled in 80.8% (HbA1C > 7.5%), and good controlled in only 12.5% patients (HbA1C < 6.5%). Several affected factors are identify, such as patients were not educated and/or do not perform correctly nutritional and exercise therapy (48.1 %); not compliant with drug treatment; or not measure regularly blood glucose; and long duration of diabetes (p < 0.01). Conclusions: (1) Blood glucose in majority of type 2 diabetic patients is poorly controlled. (2) Factors affected are non - compliance with therapy, irregularly blood glucose measurement and long duration of diabetes.

Background: According to statistical data of the Vietnam Ministry of Health, there were 124,223 people with positive HIV, 23,661 AIDS patients and 13,649 AIDS- related deaths up to April 30th 2007. The major transmission route is injection. Objectives: (1) To compare the characteristics of drug use in users with different time. (2) To describe the health issues and health service access of young male drug users. Subjects and method: A cross-sectional survey was carried out using a combination of medical anthropology and epidemiology methods. 1.115 young male drug users in Ha Noi between September 2004 and September 2005 were divided into 4 groups of the equivalence number, but with different drug using history. Results and conclusion: The newer drug users (within 2 years) started using drug later than the older ones, more of them started using by injecting; they transfer from smoking/sniffing to injecting as well as trying and using more different types of drugs together in shorter time than others. The proportion of HIV (+) infection in these people was higher, which states the necessity of intervention to help them enhance their knowledge to prevent HIV infection amongst themselves. Mental health care is also needed for drug users, the demand for mental health care increases with the time of drug usage.

Background: Although there are many advances in the treatment of Acute Respiratory Distress Syndrome (ARDS), the mortality rate is still high (40%-60%). The new strategy \u2018protective ventilation\u2019 helped to lower the fatality rate and shortening the duration of mechanical ventilation in comparison with traditional modes. However, there is controversy about the effects of protective ventilation. Objectives: (1) To estimate the change of arterial blood gas and hemodynamic in the first 48hrs in the patients suffering from ALI or ARDS under protective ventilation. (2) To discover the disadvantages of this ventilation mode. Subjects: 30 ALI and ARDS patients (22 males and 8 females) treated at ICU, Viet Duc Hospital between April 2005 and October 2005. Method: Prospective and self-control study. Protective ventilation was applied for these patients. Artery blood gas, hemodynamic and disadvantages of protective ventilation were monitored during first 48hrs. Results: Protective ventilation significantly lowers airway pressure (p < 0.001). There was no change of PaCO2 (p > 0.05), and PaO2 was improved clearly after 12hrs of ventilation (p < 0.05). There were no significant changes of mean arterial pressure, heart rate and central venous pressure. 83.3% patients needed a high dose of sedation during protective ventilation. Conclusion: Protective ventilation lowered airway pressure, improved PaO2 and had no hemodynamic effect. However, patients required a high dose of sedation during this mode of ventilation.

Background: In recent years, Vietnam has applied four methods (morphology, cell chemistry, immune marker classification, cyto genetic) in diagnosis and used multi-chemotherapy in treatment for acute myelogenous leukemia (AML)\r\n', u'Objectives: To initially determine some fusion gene transcripts in the acute myelogenous leukemia patients by applying PCR technique. Subject and method: The study included 19 patients with acute myelogenous leukemia treated in National Institute of Hematology and Blood Transfusion and Bachmai Hospital from April 2007 to August 2007. RNA were extracted from leukemic cells and PCR for AML1/ETO, CBFP/MYH11, PMR/RARa fusion transcript was done. Results: Number of male patients was 6 (32%), female patients was 13 (68%). The average age of these patients was 32.67 \xb113.62. There were three M4, M4eo patients with AML1/ETO gene (accounting for 16%), two M2, M4 patients with CBF/MYH1 gene and type F of genetic modification accounting for 11%), two M3 patients with PMR/RAR\u03b1 and Bcr3 of genetic modification (accounting for 11%). Conclusion: Results of the study did not differ significantly from other researches in the world. Background: This study aimed to identify the location of the optic foramen in relation to the anterior sphenoid sinus wall, which is essential information for surgeons in planning and performing endoscopic transnasal surgery. Methods: Computed tomography scans of 200 orbits from 100 adult patients with no abnormalities were examined. The results included the location of the optic foramen in relation to the anterior sphenoid sinus wall and the distance between them, as well as the distance from the optic foramen and the anterior sphenoid sinus wall to the carotid prominence in the posterior sphenoid sinus. Results: The optic foramen was anterior to the anterior sphenoid sinus wall in 48.5% of orbits, and posterior in the remaining 51.5%. The mean distance from the optic foramen to the anterior sphenoid sinus wall was 3.82 ± 1.25 mm. The mean distances from the optic foramen and the anterior sphenoid sinus wall to the carotid prominence were 7.67 ± 1.73 and 7.95 ± 2.53 mm, respectively. Conclusion The optic foramen was anterior to the anterior wall of the sphenoid sinus in approximately half of the orbits examined in this study, and posterior in the remaining half. The mean distance from the optic foramen to the anterior sphenoid sinus wall of the sphenoid sinus was 3.82 ± 1.25 mm.

Background: In Vietnam, there are a number of studies on the application of ATRA in treating acute promyelocytic leukemia (AML \u2013 M3) but they have still faced with certain difficulties. Objectives: (1). Study PML/RAR alpha fusion gene on the patients diagnosed with AML \u2013 M3. (2). Study the index of hematology of the PML/RAR alpha positive group. Subject and Method: 21 patients with acute promyelocytic leukemia (M3) were studied. All patients were examined with morphology, coagulation and cytogenetic tests and RNA were extracted from leukemic cells and PCR for PML/RAR alpha fusion transcript. Result and conclusion: PML/RAR alpha positive in 67% including 4 patients which were not discovered t(15; 17) by cytogenetic technique. Rates of three subtype (bcr1, bcr2 and bcr3) of PML/RAR alpha were 7 patients (50%), 3 patients (21,5%) and 4 patients (28,5%), respectively. WBC and bone marrow cells of PML/RAR alpha positive group were 5.08+/-3.87 and 155.82+/-106.21. D \u2013 Dimer level was 1954.89+/-1575.28; 93% of patients in the PML/RAR alpha positive group had DIC.
