1.Investigation of hemostatic disorders in patients with congenital heart diseases undergoing open heart surgery at Hue Central Hospital

Background: Open heart surgery with cardiopulmonary bypass (CPB) causes haemostatic abnormalities which result in postoperative excessive bleeding. Objectives: To investigate haemostatic disorders before - after CPB and postoperative bleeding. Subjects and methods: Sixty congenital cardiac patients with and without cyanosis were recruited consecutively. Hematology and coagulation tests were done 1 day before operation, 15 minutes after protamine administration, 2 and 6 hours after the operation in the intensive care unit (lCU). Mediastinal chest tube drainage (MCTO) was measured for the first 6h in the ICU. Results: Significant differences between 2 groups could be found for red blood cells, hemoglobin, hematocrit, fibrinogen, D-dimer, fibrinogen degradation products (FOP), von Kaulla and platelet aggregation to epinephrine before operation (p < 0.05). There were not significant differences in platelets, PT, APTT, platelet aggregation to adenosine diphosphate (AOP) in the acyanotic and cyanotic patients (p > 0.05). Most hematology parameters were decreased significantly and hemostasis measurements were prolonged after operation in both groups (p < 0.05). There was also the significant difference in MCTO between 2 groups (p < 0.05). Background: Hemophilia is the most common clotting disorder in the hereditary blood clotting disorders causing harm to health and psychology. The disease can lead to disability and leave the burden on families and society as well as the development of race\r\n', u"Objectives: To study the familial characteristics of haemophiliacs treated at Regional Hematology and Blood Transfusion Center of Hue Central Hospital. Subject and method: This was a prospective study. It included 48 patients diagnosed and treated Haemophilia A and B at Regional Hematology and Blood Transfusion Center of Hue Central Hospital from 7/2005 - 8/2007. Results: In 48 patients, there were 23 patients who had obviously familial history (included 12 families). They were siblings, cousins, maternal grandfathers or mother's brothers. Among 67 haemophiliacs, 23 haemophiliacs had been studied (34.32%), 30 haemophiliacs died of the disease (44.77%). Most of them died at childhood, below age of 15 years (80.64%). Conclusion: Numbers of deaths in the family was not related to the severity of the disease. The age of clinical detection, morphology, number, site, characteristics of haemorrhage as well as the level of articuar injures were not completely the same between the haemophiliacs of the same family. \r\n", u'\r\n', u'

Background: A/H5N1 influenza virus spreads from birds to humans and cause influenza diseases with high mortality rate. Vaccination is the most effective way to protect communities from pandemic, reduce morbidity and mortality. The study of creating A/H5N1 influenza vaccines in conformity with Vietnam was the urgent need. Institute of Vaccine\u2019s Achievement (IVAC) studied production of inactivated influenza vaccine for human on egg-grown from reassortants NIBRG-14. Objectives: In order to produce experimentally A/H5N1 influenza vaccine for human in accordance with WHO requirements and set up a viable process for production of the vaccines. Subjects and method: 10 days embryonated eggs and NIBRG-14 strains were served to the study with LAL method to check endotoxin, Kijehdal method to test total protein. Results: IVAC had produced successfully 5 lots of absorbed vaccine A/H5N1 (FLUVAC) using NIBRG-14 strains and embryonated eggs. Initially, production and quality control processes had been set up at IVAC by applying the recommendations of WHO. Conclusion: The success of the study was a basis of the approval of the government to establish a influenza vaccine manufacturing facilities.

4.Application of microplate almar blue assay for rapid detection of drug resistance of M. tuberculosis strains

Background: Recently, microplate almar blue assay has been used commonly in detecting gen related to tuberculosis drug resistance, which provides results after 5-7 days with lower cost compared to traditional methods. Objective: To evaluate the application of microplate almar blue assay for rapid detection of drug resistance of M. tuberculosis strains. Subject and Method: A microplate-based assay which uses Alamar blue reagent - an oxidation reduction dye (MABA), was used for the determination of the anti-tuberculosis drug (isoniazid-INH, rifampicine-RIF, streptomycine-STR and ethabuton=EMB) resistance of 96 M. tuberculosis strains isolated from Vietnamese patients in comparison to those obtained by conversional method. Result: MABA showed to have high sensitivity and specificity in testing the sensitivity to individual anti-tuberculosis drugs (from 82.4% for STR to 93.3% for - INH and from 82.5% for EMB to 98.4% for STR; respectively), as well as for the multi-drug resistant M.tuberculosis (86.4% of sensitivity), highly correlated with the result determined by proportion method. Conclusion: MABA reveals the advantage in shortening test time, in simple performance and lower cost compared with the conversional culture based methods.

Background: HIV/AIDS pandemic really become danger to mankind on the earth. Objectives: Description epidemiological characteristics of HIV/AIDS patients. Studying clinical expression, learning aboutdemands of infected patients and community care for infected patients. Subjects and method: 71 HIV/AIDS people in Thua Thien Hue were studied in 2006. Using descriptive method to give epidemiological characteristics and behaviour of HIV/AIDS people. People were determined HIV/AIDS infection based on the criteria of Ministry of Health. The information were collected by target study. Results: HIV/AIDS people mainly from the age of 20 to 39 (83,10%), males were 52,10% and in various levels of education and all kinds of occupation. The rate of sexually transmitted infection was 84,5% and infection transmission was 15,5%. The rate of sexually transmitted infection within the last 12 months was 57,7%. 63,4% of infected people used condom while having sex with their spouses or partners. 1,4% of infected people using drug infection within the last month. 38% of the infected people had manifestations of AIDS, 77,8% of HIV/AIDS were treated with antiviral drugs. Families and communities had positive attitudes and behaviors to the HIV/AIDS people. Infected people being remoted from the society was 12,7%, 88,7% of them receiving supports from their communities such as disease treatment, spirit and material supports and job opportunities. Conclusion: Strengthen communication activities to change behavior of HIV/AIDS people and in communitiy to reduce alienation, discrimination for infected people. Execute socialization in care and support for HIV/AIDS people.

OBJECTIVES:
The prevalence of diabetes mellitus is rapidly increasing in Vietnam, particularly among adults aged over 45 years. This study estimated trends in diabetes and prediabetes prevalence and determined risk factors in Vietnamese adults (over 45 years).
METHODS:
A cross-sectional study was conducted based on data from an annual diabetes screening program among people aged 45-69 years in an urban city in central Vietnam (Da Nang). Joinpoint regression analyses were performed to calculate the annual percentage change and ptrend-values. Multinomial logistic regression analysis was used to determine risk factors.
RESULTS:
In total, 3,765 men and 9,149 women were included in this analysis. The age-adjusted prevalence of diabetes and prediabetes in 2017 was 11.4% and 52.9%, respectively. The prevalence of diabetes was higher in men (15.1%) than in women (10.3%), but that of prediabetes was similar in both genders (53.4% vs. 52.8%). The prevalence of prediabetes significantly increased during the study period, whereas no upward or downward trend for diabetes was observed. The prevalence of obesity, abdominal obesity, hypertension, and dyslipidemia showed no obvious trend. Obesity, a high waist-to-hip ratio, hypertension, more severe abdominal obesity, and dyslipidemia were significantly associated with a higher risk of diabetes and prediabetes.
CONCLUSIONS
Diabetes and prediabetes were more prevalent among people aged over 45 years than in the general population. Da Nang has experienced a marked increase in the prevalence of prediabetes. These findings have significant implications regarding the need for nationwide public health interventions and management aiming at diabetes prevention and control.

8.The Therapeutic Role of Nanoparticle Shape in Traumatic Brain Injury : An in vitro Comparative Study

Objective:
: To perform a comparative analysis of therapeutic effects associated with two different shapes of ceria nanoparticles, ceria nanorods (Ceria NRs) and ceria nanospheres (Ceria NSs), in an in vitro model of traumatic brain injury (TBI).
Methods:
: In vitro TBI was induced using six-well confluent plates by manually scratching with a sterile pipette tip in a 6×6-square grid. The cells were then incubated and classified into cells with scratch injury without nanoparticles and cells with scratch injury, which were treated separately with 1.16 mM of Ceria NSs and Ceria NRs. Antioxidant activities and anti-inflammatory effects were analyzed.
Results:
: Ceria NRs and Ceria NSs significantly reduced the level of reactive oxygen species compared with the control group of SH-SY5Y cells treated with Dulbecco’s phosphate-buffered saline. The mRNA expression of superoxide dismutases was also reduced in nanoparticle-treated SH-SY5Y cells, but apparently the degree of mRNA expression decrease was not dependent on the nanoparticle shape. Exposure to ceria nanoparticles also decreased the cyclooxygenase-2 expression, especially prominent in Ceria NR-treated group than that in Ceria NS-treated group.
Conclusion
: Ceria nanoparticles exhibit antioxidant and anti-inflammatory effects in TBI models in vitro. Ceria NRs had better antiinflammatory effect than Ceria NSs, but showed similar antioxidant activity.
9.Predictors of twin pregnancy in in vitro fertilization with intracytoplasmic sperm injection cycles with day 3 double embryo transfer

Objective:
The purpose of this study was to identify factors associated with twin pregnancy following day 3 double embryo transfer (DET).
Methods:
This retrospective cohort study incorporated data from 16,972 day 3 DET cycles. The participants were women aged between 18 and 45 years who underwent in vitro fertilization with intracytoplasmic sperm injection (IVF/ICSI) at My Duc Assisted Reproduction Technique Unit (IVFMD), My Duc Hospital, located in Ho Chi Minh City, Vietnam.
Results:
Of the 16,972 day 3 DET cycles investigated, 8,812 (51.9%) resulted in pregnancy. Of these, 6,108 cycles led to clinical pregnancy, with 1,543 (25.3% of clinical pregnancies) being twin pregnancies. Factors associated with twin pregnancy included age under 35 years (odds ratio [OR], 1.5; 95% confidence interval [CI], 1.32 to 1.71; p<0.001) and cycles involving the transfer of at least one grade I embryo. Relative to the transfer of two grade III embryos, the risk of twin pregnancy was significantly elevated following the transfer of two grade I embryos (OR, 1.40; 95% CI, 1.16 to 1.69; p<0.001) or a combination of one grade I and one grade II embryo (OR, 1.27; 95% CI, 1.05 to 1.55; p=0.001).
Conclusion
By analyzing a large number of IVF/ICSI cycles, we identified several predictors of twin pregnancy. These findings can assist medical professionals in tailoring treatment strategies for couples with infertility.
10.Evaluation of skin prick test to screen dust mite sensitization in chronic respiratory diseases in Southern Vietnam

BACKGROUND: In the view of the epidemic growth of sensitization to indoor allergens in Southern Vietnam, there is a requirement to screen large population. OBJECTIVE: To evaluate skin prick tests (SPTs) as predictors of positive specific IgE (sIgE) to dust allergens, among patients with chronic respiratory diseases (CRDs). METHODS: The sensitization to Blomia tropicalis (Blo t), Dermatophagoides pteronissinus (Der p), and Blattella germanica allergens (Bla g) were evaluated among 610 CRD, both SPT (≥4 mm) and sIgE by immuno-CAP (≥0.7 kUA/L). RESULTS: Based on sIgE, 45%, 32%, and 33% of patients with CRD were sensitized to Blo t, Der p, and Bla g, respectively, compared to 19%, 18%, and 13% by SPT. The association between SPT and sIgE was statistically significant, though the Kappa factor was fair (i.e., 0.39 to 0.23). While the specificity of SPT to detect sensitization (compared to sIgE) was >90% among the whole population, the sensitivity was only 34%, 41%, and 24% for Bo t, Der p, and Bla g, suggesting that SPT was not enough sensitive to screen the indoor allergen sensitization. Though, among the < 10 pack-year (PY) smokers, the sensitivity was 43% for Blo t, 52% for Der p, and 61% for Blo t and/or Der p, compared to 27%, 30%, and 35% among the ≥10 PY smokers. The sensitivity/specificity was not associated with the diagnosis of asthma compared to chronic obstructive pulmonary disease. CONCLUSION: In the present circumstance, SPT to dust mites allergens can be used to detect a sensitization among CRD population in Southern Vietnam.
