1.Results of the management of gastric carcinomas at Viet Duc Hospital in 1993-1998 year period

717 post-operation cases of gastric adenocarcinoma were studied at Viet Duc Hospital from the year 1993 to 1998. Surgical accidents accounted for 0.41%, postoperative complication 7.5%, surgical mortality 3.1%. The curettage of glands did not increase the number of complication and death. 23% survived with a survival duration of 30.29 months pos-surgically. There were different rates of death between the group underwent radical management of the carcinoma by curettage and the one which did not. Curettage had prolonged the postoperative survival duration, without complication and death.

2.Comment on clinical and subclinical features of tophi gout

Background: Tophi gout refers to the condition, where consistently high level of uric acid in the body causes the deposition of uric acid or monosodium urate crystals on the joints. Objective: To describe clinical and subclinical features of tophi gout. Subjects and method: A study conducted in 51 patients with tophi gout (according to Bennett-Wood criteria - 1968) in Department of Rheumatology, Bach Mai hospital, from February 2005 to February 2006. This was a cross-sectional prospective study. Results: The average time of tophi appearance lasted 5.3 \xb1 5.7 years. The average number of tophi was 4.5 \xb1 3.9. The most common positions of tophi were tarso - metatarsal - phalangeal joints and elbow joints (43-47%). 100% had gouty arthritis (in which 66.6% > 4 joints), 50-55% involved tarso - metatarsal - phalangeal joints (43.1% had deforming joints). On X-ray, 62.7% cases has injuries, para-articular erosions and overhanging margins (62.5%), joints space narrowing (71.9%), proliferation osseous change (43.8%). Renal stones examined by ultrasound were 41%, renal failure 24.6%. Blood analysis showed hyperuricemia (88.2%), hypertriglyceridemia (54.9%), hyperglycaemia (13.6%). Conclusions: Common features tophi gout were injuries in tarso - metatarsal - phalangeal joints; gouty polyarthritis over 4 joints. X-ray images showed para-articular erosions, joints space narrowing, proliferation osseous changes.

3.The profile of airborne fungi at a poultry market

Ensuring environmental hygiene is not respected.\r\n', u'Objectives: To evaluate infectious situation of airborne fungi at a poultry market\r\n', u'Subjects and methods: From 10/2006 to 9/2007 a survey on the profile of airborne fungi was carried out at the Havi poultry - market. Total fungal spores averaged at 2.753 \xb1 576 cfu/m3, the highest sample is 7.120 cfu/m3\u2022 More than 10 different fungi were isolated. \r\n', u'Results: The common fungi are Cladosporium (30.13%), Penicillium (26.86%), Aspergillus (20.97%, composed of A.niger, A fumigatus, A.jlavus, A.clavatus ...), Fusarium (8.15%), some rare fungi (Rhizopus, Rhizomucor, Mucor, Alternaria, Curvularia, Absidia, Bipolaris ... ) 13.89%. The concentration of airborne fungi at the market was higher than at distance of 300 and 1000 m. The concentrations of total fungi and of each species are affected by some meteorological factors: they were higher in the end of spring, early of summer and autumn, lowest in the winter. 4.Study of magnetic resonance imaging (MRI) of the wrist in rheumatoid arthritis

Background: Wrist joint damage is early, common symptom and in diagnostic criteria of rheumatoid arthritis. Objective: to describe characteristics of MRI of the wrist in rheumatoid arthritis. Subjects and methods: This was a describe study. The study included 41 patients who were diagnosed rheumatoid arthritis according to the criteria of American College of Rheumatology- 1987 from January to October - 2006. Results: The average disease duration was 16.4 \xb114.71months. According to Rheumatoid Arthritis Magnetic Resonance Imaging Score (RAMRIS) 2002, damages on MRI included: 100% of patients had synovitis (5.9 \xb1 2.20), 100% of patients had bone erosions (15.3 \xb1 11.06) and 90% of patients had bone edema (7.3 \xb1 5.86), total score 28.6 \xb1 16.58. The correlative coefficient between synovitis and bone edema: r=0.43, P<0.01, between synovitis and bone erosions:r=0.61, P<0.01, between bone erosions and bone edema: r=0.55, P<0.01. MRI revealed bone erosions in 100% of patients having rheumatoid arthritis less six months after symptom onset compared with 0% of patients on radiography. In patients with over 2 years of disease duration, this rate on MRI was 100% of patients compared with 87.5% of patients on radiography. 5.Results of retrograde endoscopic ureterolithotripsy with holmium laser in management of ureteral stones at the Post hospital I Hanoi

Background: Untill now, many minimal invasive methods have been applied in treating ureteral stones such as the endoscopic ureterolithotripsy technique of Perez-Castro and Martinez-Pinero (1980), the tetroperitoneal laparoscopic ureterolithotomy technique of Wickham (1979) and so on. Retrograde endoscopic ureterolithotripsy with holmium laser has been applied in management of ureteral stones at the Post hospital I Hanoi since 2003. Objective: To show results of retrograde endoscopic ureterolithotripsy with holmium laser in management of ureteral stones at the Post hospital I Hanoi. Subjects and method: A retrospective study was conducted in 183 patients who underwent retrograde endoscopic ureterolithotripsy with holmium laser at the Post hospital I Hanoi, from July/2003 to July/2005. Results:In 183 patients, female was more than male (54.7% versus 45.4%); the mean age was 45.4 (range 14-77). There were total 212 lithotrity stones, of which the rate of left, right, upper-third, middle-third and lower-third ureteral stone was respectively 51.9%, 48.1%, 43.2%, 22.4% and 34.4%. The mean size of ureteral stone was 15.1mm x 11.17mm and the average time of ureterolithotripsy was 48.1 minutes (range 15 -120). Conclusion: The rate of success for etrograde endoscopic ureterolithotripsy with holmium laser was high (92.9%), especially the success rate for lower-third ureteral calculi (100%) and the success rate for upper-third ureteral stones (84.8%). However, 5/165 patients (3%) who had follow-up examination for 2-4 years after surgery had ureteral stenosis. This complication was resulted from burned mucous membrane during endoscopic ureterolithotripsy with laser.

6.Results of mitral valve reconstruction in Viet Duc hospital

Background: Mitral valve diseases are the most frequent in the cardiac valvular acquired diseases. Valvuloplasty is a very good surgical treatment, also its techniques are difficult. Objective: (1) To describe features of valvular lesion and technique of valvuloplasty. (2) To evaluate the effects of these operations. Subject and Method: 95 cases of mitral valve reconstruction were studied and described retrospectively about pre- operative features, mitral valvuloplasty, and post-operative results. Results: Pre-operative cardiac failure >=2 nd level (NYHA) is 96.8%, with 100% of mitral valve insufficiency, 28.4% associated with valvular stenosis. Mean age of patient is 34.5 +/- 13.2. Each case must do an average 4/15 valvuloplasty techniques, the most frequent are: mitral valvular ring (67.4%), commissurotomy (57.9%), cordage section (45.3%)\u2026 Early post \u2013 operative result is very good: no mortality, mitral valvular function is improved significantly than pre-operative (p<0.001), no severe mitral stenosis or insufficiency. Long-term post operative control (3 months, 3 years, 5 years post \u2013 operation) present the stability of this valvuloplasty with high survival frequency (96.8% in 3 months, 96.2% in 3 years, 93.4% in 5 years), in the 78.5% ranges a good level, re-operation is only required in 3 cases with valvular replacement. Conclusions: Although the difficult technique of valvuloplasty \u2013 especially in rheumatic lesion, mitral valve reconstruction is a good and safe technique to treat the mitral valve diseases.

7.Change of estrogen concentration in the menstrual cycle

Background: Many research works showed that menstruation is caused by the sudden reduction of the estrogens and progesterone to low levels of secretion at the end of the menstrual cycle. However, it should be investigated to which levels the concentration of these two hormones sharply reduces to cause menstruation. Objective: To determine estradiol concentration at the beginning of menstruation, the second day of menstruation and the ending of menstruation during the menstrual cycle. Subject and methods: 30 women with the normal menstrual cycle and 30 women with the long menstrual cycle were used as subjects to determine estradiol concentration at the day at the beginning of menstruation, the second day of menstruation and the last day of menstruation during the menstrual cycle. Result and Conclusion: In the women group with the normal menstrual cycle, the estradiol concentration: at the day at the beginning of menstruation is 159.93 \xb1 51.29 pmol/L, at the second day of menstruation is 170.58 \xb1 46.54 pmol/L and at the lastday of menstruation is 177.55 \xb1 46.42 pmol/L. In women in the group with long menstrual cycle, the estradiol concentration: at the day of beginning of menstruation is 154.55 \xb1 61.60 pmol/L and at the last day of menstruation is 154.55 \xb1 61.60 pmol/L.

8.Results of treating flail chest by technique of open fixation

Background: Flail chest is a severe condition of thoracic trauma, and it requires diagnosis and treatment as soon as possible. From year 2000, we developed an improved technique of open fixation in order to adapt the situations of Vietnam health settings. Objectives: To report preliminary results of application of improved open fixation technique in Viet Duc Hospital from 2001 to 2006. Subjects and method: This descriptive, prospective and retrospective study involved 19 patients with flail chest due to closed thoracic trauma or multiple traumas, treated by open fixation technique. The parameters included features of patients, characteristics of this technique and postoperative progression.Results: Of 19 patients, there were 16 males and 3 females, mean age: 47.8 years. All patients presented obvious signs of flail chest preoperatively, but only 9 cases were treated by open fixation technique and pleural drainage immediately. This technique canbe applied in all sugical settings with single local anaesthesia, and any kind of common surgical threads. The most suitable initial pulled weight is 2000 g. All cases of flail chest required pleural drainage. All patients had good outcomes with open fixation technique, no death. There were 3 patients with postoperative complications, all were severe multiple trauma patients. Conclusion: Improved open fixation technique applied in Viet Duc Hospital is simple, effective and safe, and can be widely used in all surgical settings.

9.Some remarks on lymph node dissection and recovering of gastrointestinal circulation after gastrectomy caused by cancer at B15 department of Military Central Hospital N.108

Background: Gastric cancer is a common disease in the world. For treatment, gastrectomy is still considered as the first choice. However, recovering of postoperative gastrointestinal circulation remains a controversial issue. Objectives: To assess the early results of 2 methods as gastroduodenostomy (Billroth I type) and gastrojejunostomy (Billroth type II) in recovering of gastrointestinal circulation. Subjects and method: A descriptive, retrospective study was conducted on 98 patients (61 males, 37 females, mean aged 48.1\xb112.7), were confirmed diagnosed with gastric cancer, treated in Military Central Hospital N.108 from May, 2001 to December, 2005. Results: Tumor positions were seen in 1/3 lower and 1/3 middle of gastric organ (59.2% and 40.8%, respective). There was no significant difference for invasive level of tumor between 2 groups (P>0.05). 62.2% patients with invasive level of tumor in stage of T4 in both 2 groups. Generally, the lesion between 2 forms of ulcer and rough differed significantly. The patients with and without metastatic lymph nodes were 57 and 41 patients, respective. All of patients were conducted with lymph node dissection at the stage of DIII-DIV. The difference between 2 groups for surgical time was no statistic significant. Conclusion: Billroth type I method combined with lymph nodes dissection was a safe and feasible technique in treatment of gastric cancer.

10.Noise pollution, occupational failing hearing and occupational hearing loss among auto-engineering workers in Hue city

Background: Noise pollution has seriously negative effects on worker\u2019s health, especially their hearing ability. Occupational hearing loss is one of 21 insured occupational diseases. Occupational hearing loss is common in workers in transpostation, construction, energy heavy and light industry areas. Objectives: 1. To determine the degree of noise pollution in Thua Thien Hue Unity Auto Mechanical Joint Stock Company. 2. To evaluate occupational failing hearing and occupational hearing loss among workers in the company. 3. To promote preventive measures which imnimize occupational failing hearing and occupational hearing loss. Subjects and method:A cross-sectional, descriptive study was carried out in 254 employees in Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company. Results and Conclusion: 72.7% the employees of Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company have exposed to environmental noise at the level higher than the permitted one. The rate of occupational failing hearing among these people is rather high (33.1%), and occupational hearing loss rate is 11.8%. There is a increased rate of occupational hearing loss according to the employee\u2019s age and seniority. Proposed preventive measures should be immediately conducted (P < 0.05).\r\n', u'
