In the period from 1995 to 2001, the rate of self medication without medical advice was in the trend of reduce. The health care trended to increase and to stabilize in the communal health station. The cost of treatment had increased rapidly in all forms of health care, may be due to the increasing price of medicines, the use of many medicaments and of high technique equipments of diagnosis. Authors proposed the support to communal health station for promoting the capacity of health care in this level most closed to the population, the most economized for people, especially for the poor. The assignment of qualified physicians to basic health care level and the health insurance will play a very important role.

3.Prevalence of the first and second molar caries and risk factors among students of Khuong Thuong primary school, Dong Da, Hanoi, 2005

Background: Dental caries are the most common diseases in Vietnam and the World. There are some risk factors influencing diseases.Objectives: (1) describe dental caries prevalence in first and second molars, and (2) risk factors of the first molars. Subjects and method: The cross sectional study was applied with sample size of 377 students in Khuong Thuong primary school in 2005. Structured questionnaries were used to collect information on individual characteristics, eating habits and dental hygienic practices. Results: prevalence of the below first molars was highest (35,3%-36,5%). The prevalence of the above first molars was 8,4%. The logistic regression analysis show that students who had poor tooth brushed practice have higher dental caries prevalence significantly. Hygiennic dental education is important and needed to reduce dental caries.Conclusion:The prevalence of the first molars was higher than the prevalence of the second molars. The prevalence of the first below molars was higher than that in the first above molars. Wrong tooth brushing was a determinant of dental caries.

4.Sex ratio at birth in Viet Nam 2006

Background: In recent years the sex ratio at birth in Viet Nam is imbalanced, with the number of male births being higher than that of female births. Objectives: To describe the trend and distribution of sex ratio at birth by time and geography, and investigate the relationship between sex ratio at birth and abortion ratio in 2006. Subjects and method: The study was conducted in 3.840 communal health stations, 723 district hospitals/polyclinics and 132 provincial and central hospitals with total of 1,095,064 births, occupied 78% of births over the nation in 2006. Result: The male/female sex ratio varies between areas, with the ratio being higher in rural than in urban and higher in the North \u2013East and North-Central regions than in the rest of country (p<0.05). Conclusion: There has been the clear imbalance of sex ratio at birth since 2002 (varying from 110 to 113), as well as the significant imbalance of sex ratio at birth by provinces. High imbalance of sex ratio at birth is related to the high abortion rate in these provinces. Viet Nam needs to implement strong measures/actions to avoid the same situation as in China and India.

5.Assessment of result of interlocking intramedullary SIGN nailing in treatment of femoral shaft fracture in adult patients at Viet Duc hospital from 2004 to 2005.

Background: Femoral shaft fracture, in company with the development of morden society and the increase in means of transport, is increasing in the number of case and the complicated degree of injury. Objectives: 1. To note indication for interlocking intramedullary SIGN nailing in femoral shaft fracture in adult patients. 2. To assess result of interlocking intramedullary SIGN nailing in treatment of femoral shaft fracture in adult patients. Subjects and method: A prospective, descriptive study was conducted in the 40 patients between the ages of 17 and 80 who were dignosed femoral shaft fracture caused by injury and operated for bone union by interlocking intramedullary SIGN nailing at Viet Duc hospital, from April/2004 to July/2005. Results: In this study: 5 patients with I degree fracture (12.5%), 20 with II degree (25%) and most of patients with III and IV degree fracture (62.5%), of which 14 patients with IV degree (35%). Because the SIGN nailing\u2019s structure was solid, so it had a good capacity for patient\u2019s weight. 4 weeks after operation, 85% patients were able to move around without crutches. There was no complications. Conclusion: SIGN nailing enables surgeons to drive interlocking intramedullary nailing for long bone without image intensifier. It also enables both surgeons and patients to appoarch modern, high effective methods without needing expensive machines.

6.Ankle arthroscopy in Viet Duc Hospital: 10 cases report

Background: Ankle arthroscopy has been used in Viet Duc Hospital for 4 years and now has become a routine surgery with about 200 patients, who has had ankle arthroscopy to reconstruct the anterior cruciate ligament or operate ragged meniscectomy. Objectives: Evaluate the result of ankle arthroscopy in Viet Duc Hospital and review the indication and surgical technique of ankle arthroscopy in the literature. Subject and methods: Retrospective study on 10 patients with ankle arthroscopy in Viet Duc Hospital from May 2006 to May 2007. Results: 5 patients with impingement, 4 patients with OCD lesion and 1 case of an old displaced fracture of lateral malleolar. Postoperative evaluation with Olerud Molander, ankle function improves at least 10 points and all the patients are satisfied with the results. Conclusions: Ankle arthroscopy gives good result and can be recommended for other cases.

Background: In many years, National Institute of Malariology, Parasitology and Entomology conducted collection, storage and preservation of malaria parasites species \r\n', u'Objective: to evaluate some results of malaria parasite species collected from Daknong province and analysis of drug resistance in P.Palciparum by the polymerase chain reaction.\r\n', u'Subject and method: Malaria parasite species collected from Daknong province in 2006. Thirty-five isolates were confirmed to be resistant with chloroquin by in vitro test. Polymerase chain reaction-restriction fragment leng polymorphism were used. \r\n', u'Results: 55 Plasmodium jalciparum. 7 Plasmodium vivax. 4 Plasmodium malariae. 1 Plasmodium ovale samples were collected from the malaria patients. A preliminary analysis of drug-resistant mutations in the Plasmodium jalciparum chloroquine resistance transpory (pfcrt) and P Jalciparummulti-drug resistant genes showed that the change of the order of amino-acid of Plasmodium jalciparum was closely correlated to chloroquine resistance in 35 isolates at the mutant allele 76 of pfcrt gene of chloroquine resistant Plasmodiuntjalciparum isolates. \r\n', u'Conclusion: These results contributed to supplement malaria parasite species that were stored in National Institute of Malariology, Parasitology ad Entomology.\r\n', u'

Background: Hypertensive with impaired fasting glucose is the basic expression of metabolic syndrome. Objectives: To study the IR and the correlation between Fasting Plasma Insulin (FPI) and Fasting Plasma Glucose (FPG) concentration in hypertensive patients with Impaired Fasting Glucose (IFG). Subjects and method: The descriptive, cross-sectional study was carried out on 38 hypertensive patients with IFG (IFG group) and 30 hypertensive patients without IFG (control group). The data were collected and analyzed by SPSS 11.5 software. Results:The FPI and IR index in the IFG group were 14.6+/-5.2 micro mol/ml and 3. 94+/- l.40, respectively, higher than in the control group (9.6+/-4.2 micro mol/ml and 2.17+/-0.99/ respectively) with p<0.00l. There was a positive correlation between FPI and FPG systolic blood and diastolic blood pressure/ with correlation coefficients were 0.4/ 0.48 and 0.46/ respectively (p<0.00l). 9.Situation of extremity injury caused by traffic accident in Viet Duc hospital from 2000 to 2004

Background: Situation of extremity injury caused by traffic accidents and occupational accidents are increasing in Vietnam. Therefore, the prevention and fight against these accidents are one of the key objectives to reduce the burden on the families of victims as well as for society. Objectives: To outline situation of extremity injury patients caused by traffic accident, treated in Viet Duc hospital from 2000-2004. Subjects and method: A retrospective study was conducted on 19.845 patients (14.562 males, 5.283 females, aged from 1-97 years old). The subjects were divided into 3 main groups: alone upper extremity injury, alone lower extremity injury and multiple fractures. Results: Extremity injury was common seen in people of working age (78.1%), men more than women (2.76/1 ratio), left foot more than right foot and it was common been in multiple fracture situation due to complex mechanisms of injury (50.3%), alone extremity injury (49.7%). The lower extremity injury was more common seen than upper extremity injury. Open fracture injury (27.3%), in which open fractures in lower extremity were more common seen. Open fractures in both 2 leg bones was the most common seen, accounted for 57.4% among total of open fractures. Amputation accounted for 8.3% among all case of open fractures and 2.3% among all case of extremity injuries. Conclusion: Extremity injury (lower extremity and upper extremity) caused by traffic accidents is common seen. The average, there are 3 emergency surgeries due to open fracture injury were performed daily in Viet Duc hospital per a total of about 11 traumatic emergency surgeries.

10.Autologous bone marrow transfusion to treat the tibia diaphyseal nonunions

Background: Bone marrow stem cells with their plasticity can be used to replace and repair the other damaged organs and tissues, so they can also be used to obtain bone healing of nonunions. Objective: to evaluate the results of percutaneous autologous bone marrow grafting to treat the tibia diaphyseal nonunions. Subjects and methods: 12 patients with noninfected nonunion of the tibia were diagnosed and treated in Viet Duc Hospital. About 250mL of marrow was aspirated, then separated and concentrated by density gradient centrifugation. The final mononuclear cell mass containing stem cells and progenitors was washed in 30ml of 0.9% NaCL and then injected into the damaged sites. Patients were evaluated by clinical and X-rays examinations with at least 6 months follow-up. Results: None of the patients had post - op complications. Bone union was obtained in eleven of the twelve patients (91,7%) at an average of 15,3 weeks (range, 9 - 30 weeks), the bone marrow grafts used for these patients who had bone union contained a mean of 5,65 \xb1 3,74 x 106 (0,95 - 11,73 x 106) CD34(+) stem cells in total. Conclusions: Percutaneous autologous bone - marrow grafting is a minimally invasive alternative and a simple, effective, safe method for the treatment of the tibia diaphyseal nonunions with the comparative bone healing rate. \r\n', u'\r\n', u'
