1.Clinical and Histopathological Observation on Benign Melanocytic Tumor and Malignant Melanoma ( 1974 ~ 1984 ).

This clinical and histopathological study was performed with 147 cases of benign mielanocytic tumors and 19 cases of malignant melanomas, which were obtained as surgical specimens from 1974 to 1984 at Department of Clinical Pathology, Catholic Medical College. The results were as follows: 1, In 89 cases of acquired benign melanocytic nevi, the average age of intradermal type(64 cases) was 34. 7 years and that of compound type(24 cases) was 24. 6 years. 2. In 30 cases of congenital nevus, nevus cells were present in the lower two thirds of reticular layer of the dermis in 93. 3% and in the subcutis as well in 3.3%. A case of giant congenital nevus with balloon cell appearance was found. 3. Of the 147 benign melanocytic tumors, a pigmented spindle cell nevus and a desmoplastic nevus were observed. 4. Blue nevi were 11 in number and excised from the face in 7, buttock in 2, shoulder in 1, upper arm in 1, and all were common type histopathologically. 5. Twelve malignant melanornas which were possible to be re-examined histopathologically were composed of 5 nodular type, 3 acral lentiginous type, 1 superficial spreading type and 3 metastatic malignant melanoma.

2.Acquired, Bilateral Nevus of Ota-like Macules (ABNOM) Associated with Ota's Nevus: Case Report.

Ota's nevus is mongolian spot-like macular blue-black or gray-brown patchy pigmentation that most commonly ocurrs in areas innervated by the first and second division of the trigeminal nerve. Acquired, bilateral nevus of Ota-like macules (ABNOM) is located bilaterally on the face, appears later in life, is blue-brown or slate-gray in color. It is not accompanied by macules on the ocular and mucosal membranes. There is also debate as to whether ABNOM is part of the Ota's nevus spectrum. We report an interesting case of ABNOM associated with Ota's nevus. A 36-yr-old Korean women visited our clinic with dark bluish patch on the right cheek and right conjunctiva since birth. She also had mottled brownish macules on both forehead and both lower eyelids that have developed 3 yr ago. Skin biopsy specimens taken from the right cheek and left forehead all showed scattered, bipolar or irregular melanocytes in the dermis. We diagnosed lesion on the right cheek area as Ota's nevus and those on both forehead and both lower eyelids as ABNOM by clinical and histologic findings. This case may support the view that ABNOM is a separate entity from bilateral Ota's nevus.

3.Blue Nevus Hidden within the Nevus of Ota.

A 3-year-old boy presented with bluish patch and scattered blue spots on the left side of his face. After several sessions of laser treatment, the azury patch in the periorbital area became even darker. Histopathology showed many bipolar, pigment-laden dendritic cells scattered in the papillary and upper reticular dermis. Immunohistochemically, these cells were positive for S100, SOX-10, melan-A, P16, and HMB-45. The positive rate of Ki-67 was less than 5%. Finally, the lesion was diagnosed with nevus of Ota concurrent with common blue nevus. Therefore, for cases of the nevus of Ota with poor response to laser treatment, the possible coexisting diseases should be suspected.

4.Development of Multiple Pigmented Nevi Within Segmental Nevus Depigmentosus.

Nevus depigmentosus is a stable and well-circumscribed congenital hypomelanosis that may be in an isolated, dermatomal or systemic form. An 18-yr-old Korean man with segmental nevus depigmentosus developed multiple pigmented nevi which were present only within the confines of the leukoderma. Histologic and electron microscopic studies rendered a diagnosis of nevus depigmentosus with dysplastic nevus to the patient. The genetic alteration of melanocytes in the hypopigmented lesion is assumed to have resulted in the development of multiple pigmented nevi.

6.Clinical and Histopathological Observation on Acquired Melanocytic Nevus ( 1975 ~ 1985 ).

This clinical and histopatholgical study was performed with 149 cases of acquired melanocyticnevi, which were obtained as surgical specimens from 1975 to 1985 at Depa.rtment of Clinical Pathology, Seoul Red Cross Hopital. The results were as follows: 1) Histologic subdivision of melanocytic nevus into junctional(,J), IJC, compound (C,"), ICI, and intradermal(I) is used. IJC is a lesion in an intermediate state between ,junctional and compound nevus, and ICI, between compound and intradermal nevus. 2) The age distribution of the excisional melanocytic nevus peaks during the third decade of life, with 47% of patients between 21 40 year old of age. 3) Of l49 specimens with excisional melanocytic nevus, the number of junctional, IJC, compound, ICI and intradermal nevus were 17, 4, 21, 32 and 75 in each group And so, it is likely that evolution of the melanocytic nevus have an increased opportunity for surgical excision. 4) Among the 17 cases of junctional nevus, one case is found in 5th and the other one in 7th decade. 5) In distvibution of melanocytic nevus, head including face is the most common site among the body area and then trunk, neck, lower and upper extremity in decreasing orders.

7.Clinical Study of Halo Nevi.

BACKGROUND: There have been few clinical studies of halo nevi. OBJECTIVE: The purpose of this study was aimed at evaluating the clinical and histopathologic features of halo nevi and correlation between halo nevi and vitiligo. METHODS: The medical records of 40 patients with halo nevi and biopsy specimens of 30 patients with halo nevi were reviewed. RESULTS AND CONCLUSION: 1.The ratio of male to female patients was 1:1.5. 2.The mean age of the onset was 20.1 years (male : 11.5, female : 25.9). 3.Multiple halo nevi were present in 32.5%(male : 25%, female : 37.5%). 4.The areas in which the lesions developed were head and neck(39.1%), back(32.8%), anterior chest(12.5%), abdomen(10.9%), groin(3.1%), and lower extremity(1.6%) in descending order. 5.The mean duration of each color in central nevus was as follows: black(2.7 years), pink(3.5 years), gray(4.9 years) 6.The most common pathology of the central nevus was intradermal(80%). 7.The halo nevi associated with vitiligo were 21 cases(52.5%) out of 40 patients. Among them, the cases with non-segmental vitiligo were 13(61.9%), the cases with segmental vitiligo were 8(38.1%). 8.Out of the 21 cases with halo nevi associated with vitiligo, the cases with halo nevi prior to vitiligo were 4(19.1%), concurrent onset 12(57.1%) and the cases with halo nevi after vitiligo were 5(23.8%).

8.Clinical Analysis of Benign Eyelid and Conjunctival Tumors.

PURPOSE: To assess the occurrence and characteristics of benign eyelid and conjunctival tumors. METHODS: We histopathologically analyzed 80 consecutive cases of benign eyelid (56 cases) and conjunctival (24 cases) tumors between April 2000 and November 2002. All eyelid and conjunctival tumors were confirmed histopathologically. Age, sexual distribution, occurrence and location were analyzed. RESULTS: The mean ages were 42.32+/-16.27 SD in benign eyelid tumors, and 29.71+/-18.60 SD in benign conjunctival tumors. The sexual distribution revealed 23 males and 33 females in benign eyelid tumors, and 12 males and 12 females in benign conjunctival tumors. About the location, lower lid was most frequent site of benign eyelid tumors, responsible for 27 (48.2%) of the 56 cases, and medial conjunctiva was the most frequent site of benign conjunctival tumors, responsible for 14 (58.3%) of 24 cases. Shave biopsy was performed in eyelid margin tumors. Excisional biopsy was performed in other eyelid tumors and conjunctival tumors. The frequency of benign eyelid tumors was noted in this order: intradermal nevi 25 cases (44.6%), seborrheic keratoses 9 cases (16.1%), and compound nevi 6 cases (10.7%), and frequency of benign conjunctival tumors was noted in this order: compound nevi 7 cases (29.2%), and intradermal nevi 6 cases (25.0%). CONCLUSIONS: From the Results of this study, we hope that this report provides a basic source for diagnosis and therapy of the benign eyelid and conjunctival tumors.

10.Multiple eccrine nevus with depressed patches.

A 2-year-old girl had variably-sized, relatively well demarcated, depressed brownish patches on the left shoulder, back, and both legs from the age of 1 month. The biopsy specimen from the left shoulder showed an increase in the number of normal intradermal eccrine ducts and coils, which was compatible with pure eccrine nevus. The clinical manifestation of our case, including brownish depressed patches, was unique compared to other reported cases.
