4.Spitz Nevus in a Giant Speckled Lentiginous Nevus.

Speckled lentiginous nevus is circumscribed patch of hyperpigmentation with smaller, darker pigmented macules or papules within the patch. The background shows histologic features of lentigo simplex. The speckled areas show junctional or compound nevus. Association of speckled lentiginous nevus with multiple spitz nevi was rarely reported. We report a case of multiple spitz nevi arising in a giant speckled lentiginous nevus.

Spitz nevus is a variant of melanocytic nevus which is histopathologically defined as large spindle and/or epithelioid cells. Angiomatoid Spitz nevus is a rare histologic variant of desmoplastic Spitz nevus characterized by prominent vasculature. We present a case of angiomatoid Spitz nevus, celluar type, that has not been reported before. We provide another example to show the remarkable diversity of Spitz nevus.

7.A Case of Pigmented Epithelioid Cell Nevus.

Pigmented spindle cell nevus was described as a variant of spindle and epithelioid cell nevus (Spitz nevus). However, it hs been classified as a separate disease because of the presence of large pigmented cells confined to the epidermis and papillary dermis. Pigmented epithelioid cell nevus is considered as a counterpart to pigmented spindle cell nevus, because it has similar clinicopathologic features to those of pigmented spindle cell nevus, except for tumor cells being entirely composed of large epithelioid cells. Herein, we report a case of a 31 year-old woman with a deeply-pigmented nodule which was entirely composed of large epithelioid cells, and showed many histologic findings of pigmented epithelioid cell nevus.

8.Four Cases of Spitz Nevus.

Spitz nevus is a benign tumor of melanocytic origin. The lesion is a usually asymptomatic, solitary, reddish or pinkish, hairless, dome-shaped, smooth-surfaced, and firm papule or nodule, measuring less than 1.0cm in the greatest diameter. In most instances, it is encountered most commonly on the face and extremities in the first two decades of life. The histologic picture of the lesion reveals numerous uniform nests of spindle cells and epithelioid cells with sparse pigmentation in the dermis. We report herein four cases of Spitz nevus with a review of literatures.

9.Pigmented Spindle Cell Nevus in an Infant.

Pigmented spindle cell nevus (PSCN) was first described by Reed et al. in 1975 as a distinctive form of nevocellular nevus. Although some authors consider PSCN to be a pigmented variant of Spitz nevus, most classify it as a separate disease. We encountered a two year old boy with deeply pigmented macule on the left foot dorsum. It's histologic features were sharply demarcated lateral margin, confinement above reticular dermis and entirely composed of heavily pigmented spindle cells. There were no atypical mitotic fugures or nuclear pleomorphism, and cell size was uniform. The pathologic features were consistent with PSCN.

10.The Expression of HSP105 in Spitz Nevus and Malignant Melanoma.

BACKGROUND: Spitz nevus and malignant melanoma have common features clinically and histologically, and in some cases it is impossible to distinguish between the two. Heat shock proteins (HSPs) serve to protect cells, and are activated by cell injury. Some HSPs are shown to be elevated in many types of cancers. Previous studies have reported the expression of heat shock protein in association with melanoma; however, a similar relationship with Spitz nevi has never been investigated. OBJECTIVE: This study was designed to measure the expression pattern of HSP 105 in both Spitz nevi and melanomas. METHODS: The specimens of 4 of Spitz nevi and 10 of malignant melanomas were analyzed for heat shock protein 105 expression through immunohistochemical staining. RESULTS: Immunohistochemical examination of HSP 105 showed strong expression in malignant melanoma specimens. On the other hand, weak expression was observed in Spitz nevus specimens. The degree of expression of HSP 105 showed a statistically significant difference (p<0.05). CONCLUSION: These findings provide the possibility of using HSP 105 as a effective marker for differentiating between Spitz nevi and malignant melanomas. In support of this, HSP 105 is considered to be a tumor-associated antigen of malignant melanoma.
