Primary liver cancer occurs frequently in male. It has a fatal outcome within the first 6 months and deaths are mainly due to coma and cachexia. 3 initial symptoms are often seen: anorexia, pain under the right costal margin and diarrhea. Liver cancer occurs frequently on a cirrhotic liver. Mass cancer and nodular cancer are the prevailing forms. Microscopically, hepatocellular carcinoma is the predominant picture. Metastasis to the liver pedicle nodes is often noted and distant metastasis is uncommon

A case of Juvenile Granulosa Cell Tumor (JGCT) of the ovary with unusual pleomorphic histologic and malignant biologic behaviour is described. The tumor occurred in a 10-year-old girl and was associated with clinical features of isosexual pseudoprecosity and a marked elevation of serum estradiol. The mass manifested initially in the right ovary and subsequently involved the contralateral ovary. A multi-organ metastasis developed during a 6-month-interval despite chemotherapy. She received two operations at 6-month interval, and tissues were obtained from the tumor mass. A marked histologic difference was observed between these two samples. The second biopsy showed profound cellular pleomorphism with numerous multinucleated tumor giant cell formation and hyaline bodies. The differential diagnosis from germ cell tumor and the possible factors for the pleomorphism are discussed.

Brain metastasis from primary uterine cervical carcinoma have been described but are extremely rare and usually occur in the presence of widely disseminated diseases. The authors report a case of solitary cerebellar metastasis from uterine cervical carcinoma, which was confirmed by histopathological examination. The patient has shown no evidence of metastatic lesions elsewhere, thus far, but had undergone a radical hysterectomy under the diagnosis of uterine cervical carcinoma 52 months prior to the surgical excision of the cerebellar metastasis.

Study on 171 patients who had metastasis cervical lymph nodes (132 males, 39 females) caused by head – neck neoplasm or unknown causes were diagnosed and surgery treated at Hospital K, Ha Noi from Dec 1998 to Jun 2001. The results showed that: the clinical features can used to evaluate metastasis and to be basic for treatment strategy such as: site, size, density, number, mobility, characters of skin on node’s surface. It is emphasized that cytology biopsy is very effective method in diagnosing metastasis cervical lymph nodes.

During 1994-1997, the nuclear medicine department of Cho Ray hospital diagnosed clinically 1938 patients with the cancer and implemented the scintigraphy to discover the metastasis. 816 patients with the cancer with the high risk of metastasis including the breast cancer, vaginal cancer, lung cancer, prostate gland cancer and thyroid cancer which arranged as kind of cancer and age. The bone scintigraphy help classifying the cancer with metastaisi and the comparision of the rate of metastasis into bone in this study with this rate reported by the Utrecht University of Ha Lan.

Primary malignant melanoma of the esophagus is extremely rare, with less than 200 cases reported in the literature. However, primary malignant melanoma is an aggressive tumor. We report a case of primary esophageal malignant melanoma that presented with a large polypoid mass in the lower esophagus, and subsequently showed multiple metastatic nodules in the subcutaneous layer, skin and retroperitoneum. Primary malignant melanoma should be included in the differential diagnosis of a polypoid esophageal mass, especially in cases where metastases to unexpected areas occur or in cases of unexpected metastatic patterns.

Small cell carcinoma of the ovary is rare malignancy occurring in women under 40 years of age(average, 23 years), which is associated with hypercalcemia in two thirds of cases. Its histogenesis is uncertain, but the possibilities of common epithelial, neuroendocrine, sex cord stromal and germ cell origin are suggested. All reported cases were proved to have rapid fatal course despite various therapy and 5 years suvival rate was only 10%. We report one case of a 20-year old woman with primary small cell carcinoma of the left ovary. The ovary was markedly enlarged and completely replaced by a mass, measuring 21x16x8 cm. Microscopic examination revealed dimorphic population of small and large malignant cell producing immature follicle-like structure which is characteristic of small cell carcinoma of the ovary. These pathological findings were similar to those of granulosa cell tumor, which is required to make differential diagnosis from small cell carcinoma. Immunohistochemical stains for cytokeratin and vimentin were positive, but those for S-100 protein and NSE were negative. One month after the initial operation, the tumor has recurred and the second and the second palliative operation followed by 3 cycles of chemotherapy was done. The patient showed disseminated metastasis at present time.

A metastatic melanoma is a ubiquitous tumor that spares no organ from metastases. Gastrointestinal involvement is a common occurrence in patients with a metastatic melanoma, and post mortem records have shown gallbladder involvement in about 15% of all gastrointestinal metastatic localizations. However, metastasis to the gallbladder as an isolated site is rare. Herein, an unusual case of a young man in whom a single metastatic gallbladder melanoma was the first and only metastatic site of the disease is reported. The clinical presentation, diagnosis, histopathology, prognosis and treatment of metastatic melanomas of the gallbladder are also reviewed.

PURPOSE: We have conducted this study to investigate the role of c-myc and AAT in gastric carcinoma progression and to see if clinical application of its expression in cancer tissue is of help for the diagnosis or in determining prognosis of gastric carcinoma. MATERIALS AND METHOD: The expression of c-Myc and AAT by immunohistochemical method applied to paraffin-embedded tissue sections of endoscopic biopsy materials of 71 cases of gastric carcinoma (24 early and 47 advanced) and immunoreactivities of antigens were correlated with histological differentiation of carcinoma, degree of tumor infiltration of mononuclear cells, serum levels of carcinoembryonic antigen (CEA) and presence of distant metastases. RESULTS: c-Myc in gastric carcinoma tissue was expressed in 24 cases (33.8%), and the rate of immunoreactivity of c-Myc was higher in the advanced carcinoma cases (38.2%) than early carcinoma cases (25.0%), but the difference was not stastistically significant. The elevated c-Myc expression correlated well with the elevation of serum CEA levels (P<0.05), with the presence of distant metastses (p<0.05), especially with peritoneal metastsis (p<0.05). AAT expression in gastric carcinoma was shown in 11 cases (14.1%), and the rate of immunoreactivity of AAT was significantly higher in advanced carcinoma cases (21.3%) than early carcinoma cases (4.2%) (p<0.05). The elevated expression of AAT correlated well with the elevation of serum CEA levels (p<0.05), and showed negative correlation with the degree of mononuclear cell infiltration in tumor area (p<0.05). The increased expression of c-Myc and AAT in gastric carcinoma correlated well (p=0.05, k= 0.31), which suggests the cooperative action of the two in gastric carcinoma progression. CONCLUSIONS: Our findings suggest that c-Myc expression may be a good marker of high grade malignancy in gastric carcinoma, and may be able to be used clinically in predicting distant metastases, especially for peritoneal dissemination. Our data also imply that c-myc, through its proliferative action, may play an important role in the progression of gastric carcinoma in cooperation with AAT which has immunosuppresive action.

The authors retrospectively reviewed the high resolution computed tomographic scans of 19 patients who had hematogenous or lymphangitic metastatic lung lesions. In all patients, the histologic diagnosis for primary cancers and the radiographic manifestations of pulmonary metastasis were established. Certain characteristic findings of metastatic lung tumors on HRCT scans were evident: uneven thickening of bronchovascular bundles, multiple small nodules, thickening of interlobular septum, and the presence of polygonal lines. The nodules are more predominent in hematogenous metastasis. The prevalent site of nodules is cortical portion of lung.
