1.Effect of Electromagnetic Navigated Ventriculoperitoneal Shunt Placement on Failure Rates.

OBJECTIVE: To evaluate the effect of electromagnetic (EM) navigation system on ventriculoperitoneal (VP) shunt failure rate through comparing the result of standard shunt placement. METHODS: All patients undergoing VP shunt from October 2007 to September 2010 were included in this retrospective study. The first group received shunt surgery using EM navigation. The second group had catheters inserted using manual method with anatomical landmark. The relationship between proximal catheter position and shunt revision rate was evaluated using postoperative computed tomography by a 3-point scale. 1) Grade I; optimal position free-floating in cerebrospinal fluid, 2) Grade II; touching choroid or ventricular wall, 3) Grade III; tip within parenchyma. RESULTS: A total of 72 patients were participated, 27 with EM navigated shunts and 45 with standard shunts. Grade I was found in 25 patients from group 1 and 32 patients from group 2. Only 2 patients without use of navigation belonged to grade III. Proximal obstruction took place 7% in grade I, 15% in grade II and 100% in grade III. Shunt revision occurred in 11% of group 1 and 31% of group 2. Compared in terms of proximal catheter position, there was growing trend of revision rate according to increase of grade on each group. Although infection rate was similar between both groups, the result had no statistical meaning (p=0.905, chi-square test). CONCLUSION: The use of EM navigation in routine shunt surgery can eliminate poor shunt placement resulting in a dramatic reduction in failure rates.

2.Two Cases of Cytomegalovirus Infection Developed in Pediatric Acute Lymphoblastic Leukemia Patients

A 14 year-old boy with acute lymphoblastic leukemia (ALL) on maintenance chemotherapy presented with vision-threatening cytomegalovirus (CMV) retinitis. Treatment with intavitreal ganciclovir injection (2 mg/0.1 mL) followed by oral ganciclovir resulted in successful resolution of CMV retinitis. Another 13 year-old boy with ALL on maintenance chemotherapy presented with prolonged fever with no response to antibiotics administration. CMV and real-time PCR revealed positive result and a titer of 2,618,700 copies/mL, respectively. Ganciclovir was used for more than the approved duration of treatment, but viral titer frequently recurred with elevated liver enzymes and fever. In these 2 cases of CMV infection, a high index of suspicion and prompt management is important in children receiving ALL chemotherapy.

3.The Role of Serum Pepsinogen in the Detection of Gastric Cancer.

The incidence of gastric cancer is very high in Japan, Korea, and China. Reducing the morbidity and mortality associated with gastric cancer requires early diagnosis, which can be facilitated by applying gastroscopy more frequently in high-risk groups. A strategy of population screening for gastric cancer is currently being adopted in Korea, Japan, and the Matsu region of Taiwan, but using different screening methods. In addition, the history of pepsinogen (PG) in research as a gastric cancer biomarker has varied, in that the use of serum levels of PGI and PGII and the PGI/PGII ratio as gastric cancer screening tools was introduced in Japan before 1990, but in Korea the first research results were only reported in 2008. This review first evaluates the physiology of PG, followed by the usefulness or limitations of serum PG testing with regard to the detection of gastric cancer. Finally, the factors affecting the efficacy of PG tests as a gastric cancer biomarker (i.e., Helicobacter pylori infection status, gender, histopathologic features, and cancer location and depth) are evaluated. It was found that the strategies used to increase the efficacy of PG tests should be individualized in each country according to the seroprevalence of H. pylori.

4.Identification of differentially expressed miRNAs associated with chronic kidney disease-mineral bone disorder

The purpose of this study is to characterize a meta-signature of differentially expressed mRNA in chronic kidney disease (CKD) to predict putative microRNA (miRNA) in CKD-mineral bone disorder (CKD-MBD) and confirm the changes in these genes and miRNA expression under uremic conditions by using a cell culture system.PubMed searches using MeSH terms and keywords related to CKD,uremia,and mRNA arrays were conducted.Through a computational analysis,a meta-signature that characterizes the significant intersection of differentially expressed mRNA and expected miRNAs associated with CKD-MBD was determined.Additionally,changes in gene and miRNA expressions under uremic conditions were confirmed with human Saos-2 osteoblast-like cells.A statistically significant mRNA meta-signature of upregulated and downregulated mRNA levels was identified.Furthermore,miRNA expression profiles were inferred,and computational analyses were performed with the imputed microRNA regulation based on weighted ranked expression and putative microRNA targets (IMRE) method to identify miRNAs associated with CKD occurrence.TLR4 and miR-146b levels were significantly associated with CKD-MBD.TLR4 levels were significantly downregulated,whereas pri-miR-146b and miR-146b were upregulated in the presence of uremic toxins in human Saos-2 osteoblast-like cells.Differentially expressed miRNAs associated with CKD-MBD were identified through a computational analysis,and changes in gene and miRNA expressions were confirmed with an in vitro cell culture system.
5.Application and effectiveness of a nutrition education program based on the 2020 Dietary Reference Intakes for Koreans for undergraduates in Gyeongsangnam-do and Gyeonggi-do

Purpose:
The 2020 Dietary Reference Intakes for Koreans (KDRIs) serves as a foundation for daily nutrient and energy recommendations aiming to enhance public health and prevent chronic diseases. They act as guidelines for maintaining proper nutrition and overall health.Using KDRIs is crucial for promoting healthier lifestyles and making informed dietary choices. Thus, this study explores the influence of a nutrition education program, based on the 2020 KDRIs, on the nutrition knowledge and dietary habits of undergraduates in Gyeongsangnam-do and Gyeonggi-do.
Methods:
The nutrition education program, designed with diverse instructional materials, was executed across a wide range of universities. The education group (n = 75) engaged in the program for a 6-week instructional period, while the control group (n = 53) underwent the survey without participating in the education program. Nutrition Quotient (NQ) and knowledge assessments were administered to both groups immediately before and after the instructional period.
Results:
Within the education group, the nutrition education program positively impacted responses to NQ practice items, including knowledge of nutrition, daily intake, and portion sizes (p < 0.05). In contrast, there were no significant differences between the before and after responses of the control group for most survey items. Post-program evaluations showed significantly higher self-assessment scores and increased satisfaction levels (p < 0.05), with the satisfaction rate for the education program using the 2020 KDRIs reaching 99.2%.
Conclusion
This study has demonstrated the positive impact of an effective nutrition education program. However, there is a need for the continuous development and implementation of nutrition education programs to sustain these outcomes and further enhance the nutritional education experience.
6.Efficacy of Fecal Microbial Transplantation for Improving Symptoms of Irritable Bowel Syndrome - A Pilot Study for Voluntary Participants in Korea

Background/Aims:
Irritable bowel syndrome (IBS) is a chronic, intractable functional disease. It is inferred that fecal microbiota transplantation (FMT) may have favorable efficacy on IBS by gut microbial modification. The aim of this study was to investigate the efficacy of FMT for improving severity in patients with IBS.
Methods:
Patients who voluntarily wanted FMT were consecutively enrolled. The study subjects were classified by subtype of IBS by the ROME IV criteria. The IBS-symptom severity score (IBS-SSS) was used to evaluate the efficacy of FMT. The subjects completed a questionnaire at baseline week 0 and weeks 4, 12, and 24 after FMT. FMT was performed by esophagogastroduodenoscopy using frozen stock stool solution. If the follow-up IBS-SSS achieved less than 75 points, it was defined as remission. Adverse events were also gathered.
Results:
Twenty-one subjects were included from October 2023 until July 2024. There were 7 patients with IBS-C, 10 patients with IBS-D, 2 patients with IBS-M, and 2 patients with IBS-U type. The mean SSS of the IBS-D group was 244.0±64.2, which was higher than IBS-C group (192.9±85.4). Alleviations in IBS-SSS after FMT were observed in 19 subjects (19/21, 90.5%) at week 4. At week 12, 71.4% (5/7) in the IBS-C group and 20.0% (2/10) in the IBS-D group achieved remission. The remission states were maintained up to week 24 and no serious adverse events were reported.
Conclusions
FMT might be an effective treatment option for improving symptoms of mild to moderate IBS, especially IBS-C.
7.Antioxidant and immune-enhancing effects of Fuyu persimmons and Hachiya persimmons

Purpose:
Persimmon (Diospyros kaki Thunb.) is rich in phytochemicals (e.g., tannin and beta-carotene.), vitamins (e.g., vitamins A and C), minerals (e.g., calcium and potassium), and dietary fiber. Persimmon exhibits efficacy against diseases such as cancer, hypertension, and heart disease. This study examined the antioxidant and immune-enhancing activities of Fuyu (a sweet persimmon) and Hachiya (an astringent persimmon) and evaluated their potential as functional food ingredients.
Methods:
The antioxidant and immune-enhancing activities of Fuyu and Hachiya persimmons were evaluated by extracting them with hot water and ethanol (100%). The antioxidant activity was assessed by measuring the total phenolic content (TPC), total flavonoid content (TFC), 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging activity (RSA), and oxygen radical absorbance capacity (ORAC). Six-year domestic red ginseng was used as a reference for assessing the immune-enhancing activity. Nitric oxide (NO) production, phagocytosis, cytokine levels interleukin [IL]-2, IL-4, IL-10, and interferon-γ), and inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) gene expression were analyzed.
Results:
The hot water extracts of Fuyu and Hachiya persimmon exhibited significantly higher TPC, TFC, DPPH RSA, and ORAC levels than the ethanol extract. The Hachiya persimmon extract generally showed comparable or higher immune-enhancing activity than the Fuyu persimmon extract in NO production, cytokine activity, phagocytosis, and expression of iNOS and COX-2 genes.
Conclusion
Hachiya persimmon possesses superior antioxidant properties to Fuyu persimmon while exhibiting comparable immune-enhancing activity. These characteristics highlight its potential for beneficial applications. Nevertheless, further research is required to elucidate these effects and optimize their use in health-promoting interventions.
