1.Effects of dexmedetomidine on acute lung injury induced by hind limb ischemia-reperfusion in rats

Objective To explore whether dexmedetomidine(Dex)could mitigate acute lung in-jury induced by hind limb ischemia reperfusion(I/R).Methods One hundred and twenty SD rat weigh-ting 250-300 g were allocated to receive hind limb I/R,I/R plus Dex (25 μg/kg Dex was intraperitoneal in-jection 30 min before ischemia)and norm control group,and each group was further divided into five sub-groups:before operation and 4 h ischemia followed by 0,1,3 and 6 h reperfusion.After euthanization,lung W/D weight ratio,PaO2 ,SOD,MDA and TNF-α were determined.Results Compared with group C, hind limb I/R injury significantly increased serum TNF-α concentration and W/D ratio,with significantly decreasing PaO2 level at any time of reperfusion in the group I/R,the concentration of MDA increased and the SOD activity decresed at 1,3,6 h of reperfusion.Conversely,W/D ratio as well as the concentration of TNF-αin the serum of the I/R plus group Dex were significantly lower than those of the I/R at 6 h of reperfusion,PaO2 level increased respectively;the concentration of MDA decresed at 1,3,6 h of reperfusion and the SOD activity increased at 3,6 h of reperfusion in I/R plus group Dex.Conclusion Dex mitigates a-cute lung injury induced by unilateral hind limb I/R in rats.The mechanisms may involve attenuating oxida-tive stress and inhibiting inflammatory response.
2.Effects of ischemic preconditioning on oxidative stress and mitochondrial function in young and old rat myocardium with ischemia/reperfusion

[ ABSTRACT] AIM: To study the protective effect of ischemia preconditioning ( IPC ) on ischemia/reperfusion ( IR)-damaged myocardium in young and old rats .METHODS: Male Wistar rats aged at 3 months ( young ) and 20 months ( old) were used to establish myocardial IPC model and IR model with the method of Langendorff heart perfusion . The rats were divided into young ischemia/reperfusion ( YIR) group, young ischemic preconditioning ( YPC) group, old ischemia/reperfusion ( OIR) group and old ischemic preconditioning ( OPC) group.Transmission electron microscopy was used to observe the ultrastructural changes of myocardial tissue and myocardial mitochondria .The myocardial infarction area was determined by TTC staining .The lactate dehydrogenase ( LDH) content in coronary effluent fluid and the levels of su-peroxide dismutase ( SOD) and malondialdehyde ( MDA) in myocardial tissues were detected by the method of colorimetry . The levels of nitrated and carbonylated proteins in myocardial tissue were measured by ELISA .The myocardial cell apopto-sis was analyzed by TUNEL assay .The mitochondrial respiratory function and mitochondrial permeability transition pore o-pening induced by calcium load were evaluated by oxygen electrode method .RESULTS: Compared with YIR group , the myocardial infarction area in YPC group was obviously smaller , SOD activity in myocardial tissues increased , LDH activity in coronary effluent fluid and the content of MDA decreased , and the levels of nitrated and carbonylated proteins in the car-diac tissues reduced .In YPC group, the mitochondrial membrane structure appeared intact , cristae of the mitochondria showed close arrangement , and the matrix was compressed under the electron microscope .Myocardial mitochondrial respir-atory control rate , state Ⅲoxygen consumption and the P/O ratio in YIR group all significantly increased , proton leak de-creased, mitochondrial swelling induced by calcium distinctly reduced , and myocardial apoptosis rate declined .No signifi-cant difference of the above indexes between OIR group and OPC group was observed .Compared with YPC group , myocar-dial ultrastructural damage increased clearly , cardiac oxidative stress increased , mitochondrial respiratory function de-clined, and cell apoptosis and necrosis increased in OPC group .CONCLUSION:Ischemic preconditioning has protective effect against myocardial IR injury in young rat hearts , while old rat hearts were less sensitive to ischemic preconditioning , leading to bluntness of cardioprotection with IPC in aging hearts .This may be related to mitochondrial injury and severe cel-lular apoptosis caused by increase of cardiac oxidative stress levels in the aging ischemic preconditioning heart .
3.Formula Optimization of Shengmai Dispersible Tablets by Uniform Design

Objective:To explore the formula and preparation technology parameters of Shengmai dispersible tablets. Methods:With the granulation status, disintegration time, friability, taste and and so on as the testing indices, the formula and preparation tech-nology of Shengmai dispersion tablets were optimized by uniform design. Results:The optimized formula of Shengmai dispersible tab-lets was as follows:25% extract powder, 58% MCC, 8% CCMC-Na, 4% CMS-Na, 2% L-HPC, 2% magnesium stearate and 1%sweetener. L-HPC and magnesium stearate were added after the granulation, and the tablet hardness was controlled at 25N. The opti-mized dispersible tablets could disintegrate uniformly within 3 min. Conclusion: The optimization of the prescription and preparation process parameters of Shengmai dispersing tablets is stable and reliable, and has good repeatability, and the process is feasible.
4.Combined intravenous and intrathecal vancomycin in treatment of patients with intracranial infections after craniotomy

Objective To explore the efficacy and safety of combined intravenous and intrathecal vancomycin in treatment of patients with intracranial infection after craniotomy. Methods Clinical data of a total of 60 consecutive patients with intracranial infections after cranial operation admitted to Department of Neurosurgery of Nanfang Hospital of Southern Medical University from June 1st 2013 to June 1st 2015 were retrospectively analyzed. The patients were divided into two groups: intravenous injection only (n = 25) and combined intravenous and intrathecal injection (n = 35). In both groups of patients intravenously given vancomycin hydrochloride 500 kU every 6 hours as well as third or fourth generation of cephalosporins or meropenem. In combined intravenous and intrathecal injection group, in addition to 20 mg vancomycin was slowly injected via lumbar puncture after release of cerebrospinal fluid (CSF) once a day. The clinical efficacy and complications of the two groups were compared. Results The recovery rate in the combined intravenous and intrathecal injection group was significantly higher than that in the intravenous injection only group (94.3% vs. 76.0%, χ2 = 4.220, P = 0.040). Lowering of white blood cell count in combined intravenous and intrathecal injection group was significantly earlier than that of the intravenous injection only group (time to become normal: 8 days vs. 13 days). The time of recovery in combined intravenous and intrathecal injection group was significantly shorter than that of the intravenous injection only group (days: 9.9±0.7 vs. 13.4±1.1, t = -2.716, P = 0.009). There were 3 patients who experienced nerve root irritation symptoms in combined intravenous and intrathecal injection group. Symptomatic treatment was given and injection speed was slowed down for these patients. There were no severe complications, such as coma, epilepsy or death in both groups. Conclusion Combined intravenous and intrathecal injection of vancomycin could be a safe and effective therapy for intracranial infection after craniotomy.
5.Optimization of Enzymatic Extraction Technology of Polysaccharides from Schisandra chinensis by Central Composite Design-response Surface Methodology

OBJECTIVE:To optimize enzymatic extraction technology of polysaccharide from Schisandra chinensis. METH-ODS:Using pH value of enzymatic extraction solution,the amount of enzyme,extraction temperature as response factor,S. chi-nensis polysaccharide as response value,on the basis of single-factor experiments,3-factor,5-level central composite experimental design was adopted for the experiment. Validation test was also conducted. RESULTS:The optimal extraction technology was as pH value of 5.7,enzyme dosage of 1.3%,extraction temperature of 53 ℃. In validation test,the extraction rate of S. chinensis polysaccharide was 14.30%(RSD=1.84%,n=6). CONCLUSIONS:The optimized extraction technology is simple,reasonable and stable,and can be used for the extraction of polysaccharide from S. chinensis.
6.Influence of mindfulness-based stress reduction plus micro-class education on the complications and knowledge mastery rate in laparoscopic appendectomy patients with diabetes mellitus

Objective:To discuss the influence of mindfulness-based stress reduction plus micro-class education on the complications and knowledge mastery rate in surgical patients with diabetes mellitus.Methods:A total of 105 patients, diagnosed as diabetes mellitus complicated with appendicitis in the Affiliated Hospital of Chengde Medical College were selected from January 2019 to January 2020. They were hospitalized for laparoscopic appendectomy and were randomly divided into the control group ( n = 52) and the study group ( n = 53) in accordance with the random number table. The patients in the control group were given routine nursing care, and the patients in the study group were given mindfulness decompression therapy combined with micro-classroom education. The blood glucose control and psychological emotion of the two groups before and after operation, and the postoperative complications and the mastery rate of disease knowledge of the two groups were compared. Results:There was no significant difference in blood glucose indexes between the two groups at baseline ( P>0.05); FBG, HbA1c and other indicators in the two groups were improved during operation and 24h after operation, but FBG (7.38±0.54) mmol/L, HbA1c (6.39±0.21)% and FBG (6.90±0.52) mmol/L and HbA1c (6.10±0.39)% in the study group were lower than those in the control group [(8.16±1.21) mmol/L, (7.53±1.05)%, (7.60±0.57) mmol/L, (6.50±0.41)%], the difference was statistically significant ( t value was 6.789-13.264, P < 0.05); there was no significant difference in SAS and SDS scores between the two groups at baseline ( P > 0.05), until discharge, the SAS and SDS scores of the treatment group were 35.81±5.49 and 42.08±4.91 respectively, which were significantly lower than those of the control group (42.21±5.53, 6.51±4.72) respectively, compared with the corresponding scale scores of the control group, the difference between the two groups after treatment was statistically significant ( t value was 5.386, 4.265, P < 0.05). Compared with 17.31% (9/52) of the control group, the incidence of complications in the study group decreased to 5.66% (3/53), there was significant difference ( χ2 value was 6.789, P < 0.05). The qualified rate of disease knowledge mastery in the study group (98.11%,52/53) was significantly higher than that in the control group (86.53%, 45/52), and the difference was significant ( χ2 value was 5.062, P < 0.05). Conclusion:The mindfulness-based stress reduction plus micro-class education can effectively control the laparoscopic appendectomy patients blood glucose, stabilize the mental emotions, increase the illness knowledge mastery degree, keep in good mood, reduce the postoperative complications and promote the fast recovery.
7.The effect of 18β-sodium glycyrrhetinic acid on the nasal mucosa epithelial cilia in rat models of allergic rhinitis.

OBJECTIVE:
To investigate 18β-sodium glycyrrhetinic acid impact on nasal mucosa epithelial cilia in rat models of allergic rhinitis (AR).
METHOD:
AR models were established by ovalbumin-induction. Wister rats were randomly divided into groups as normal group, model group, budesonide (0.2 mg/kg) group and sodium glycyrrhetinic acid (20 mg/kg and 40 mg/kg) group after the success of AR models. At 2 weeks and 4 weeks after treatment, the behavioral changes of rats were observed and recorded, and nasal septum mucosae were collected after 2 week and 4 week intervention, and the morphological changes of nasal mucosae were observed by electron microscope.
RESULT:
Model group developed typical AR symptoms, the total score in all animals was > 5. With budesonide and sodium glycyrrhetinic acid treatment, the AR symptoms were relieved, and the total scores were reduced significantly (P < 0.01). Compared with the model group: after 2 weeks' intervention, thick mucous secretions on the top of columnar epithelium cilia in rat nasal mucosa was significantly reduced, and cilia adhesion, lodging, shedding were relieved in budesonide group and sodium glycyrrhetinic acid group, the relieve in budesonide group was slightly better than that in sodium glycyrrhetinic acid group; after 4 week intervention, Cilia adhesion, lodging, shedding were completely vanished, and the cilia were ranged in regular direction in budesonide group and sodium glycyrrhetinic acid group. Cilia in sodium glycyrrhetinic acid (20 mg/kg) group was more orderly, smooth than that in budesonide group and sodium glycyrrhetinic acid group (40 mg/kg), and the condition of cilia in sodium glycyrrhetinic acid group (20 mg/kg) was similar to the normal group.
CONCLUSION
18β-sodium glycyrrhetinic acid is effective to restrain the pathological changes of nasal mucosa cilia in rat models of AR.

8.Optimization of Extraction Process for Total Lignans from Fructus Chinensis by Orthogonal Test

Objective:To optimize the extraction process of the total lignans from Fructus chinensis. Methods:The comprehensive weighted score method for the extraction rate and the extraction content was regarded as the index by orthogonal experimental method, the effects of ethanol concentration, extraction duration, amount of ethanol and extraction times on the extraction of the total lignans from Fructus chinensis were studied. The chromatographic separation was performed on a Unitary C18 column (250 mm × 4. 6 mm,5μm). Methanol(A)-water(B) was used as the mobile phase (0-10 min:70% A;10-13 min:70% A-85% A;13-30 min:85% A). The detection wavelength was set at 217 nm, the column temperature was maintained at 30℃,the flow rate was 1. 0 ml·min-1 and the sample size was 10μl. Results:The extraction conditions of the total lignans were as follows:14-fold amount of 85% ethanol was used for 3 times extraction with 2. 5 h for each time. Under the above conditions, the extraction rate of Schisandra lignans was 1. 10%, and the extraction content was 22. 99%. Conclusion:The extraction and purification technology for Schisandra lignans and the total poly-saccharides is stable and reliable, which provides scientific basis for the reform of Shengmai new dosage forms.
9.The update of AJCC/TNM staging system may change the post-operative assessment of differentiated thyroid cancer

Objective To investigate the post-operative mortality risk staging changes of differentiated thyroid cancer resulted from the changes of the AJCC/TNM cancer staging system and to explore its clinical significance. Methods This study included 2 553 patients accepting surgical therapies in Department of Thyroid Surgery from January 2013 to February 2017. Overall,2 553 patients were staged according to the 7th and 8th editions of the AJCC/TNM cancer staging system respectively. Results Compared with the previous staging outcomes,548 patients(21.46%)were downstaged according to the 8th edition of the staging system. Of these 548 patients,438 (17.16%)migrated from an advanced stage(Ⅲ/Ⅳ)to stage Ⅰ/Ⅱ. The difference of the staging outcomes was significant(P<0.01). Because of the change of age cut off,364 patients(14.26%)were downstaged to stage I. Of these 364 patients, 318(12. 46%) migrated from an advanced stage(Ⅲ/Ⅳ) to stage I. Conclusion For the changes of the AJCC/TNM cancer staging system,particularly for the change of age cut off,many patients with DTC would be referred to lower stages, and therefore they would receive less aggressive post-operative management strategies.
10.The predictive value of metabolic syndrome in patients with acute coronary syndrome after percutaneous coronary intervention

Objective To investigate the predictive value of metabolic syndrome in patients with acute coronary syndrome (ACS) after percutaneous coronary intervention (PCI).Methods A total of 660 patients with ACS admited to cardiovascular department,first affiliated hospital of zhengzhou university were enrolled in this study from January 2009 to June 2010.The enrollment criteria were:the stenosis degree were above 75% in at least one coronary artery by coronary angiography and successful PCI procedure.Exculsion criteria were:liver and renal insufficiency,malignancies and valvular heart diseases.The relevant clinical data and labtory examination were recorded after admission. The patients were followed up by outpatients interview or telephone from March to June 2011 and adverse cardiovascular events were recorded.The patients were divided into MS and non-MS groups,and basic clinical data were compared between two groups.The proportion difference between two groups were tested by chi square. Multivariate logistic regression was established to analyze the factors related to progonosis.The survival ratio was estimated using the Kaplan-Meier method.Statistical significance was established at a P value of less than 0.05.Results ①A total of 606 (91.7%) patients successfully accepted follow-up.Mean follow-up time were ( 14.3 ±1.7 ) months.95 patients experienced adverse cardiovascular events ( 15.7% ).②There were 393 patients (64.96% ) satisfied the definition of metabolic syndrome.The patients in MS group were with higher BMI,SBP,DBP,blood glucose and disordered lipid (all P < 0.05 ),with less fale patients (P =0.016),less current somking (P =0.008 ) and with higher platelet (P =0.037 ). The incidence of adverse cardiovascular events in two groups were 17.81% and 11.79% ( P > 0.05 ). ③ Multivarite logistic regression revealed that the predictors of adverse cardiovascular events were age [ OR =2.628,95% confidence interval (CI) 1.395 ~ 4.954,P =0.003 ],New York Heart Association (NYHA) ≥ 3 grade ( OR =2.310,95% CI 1.095 ~4.870,P =0.028) and left ventricular ejection fraction (LVEF) ( OR =4.328,95% CI 1.955 ~9.580,P < 0.001 ).However,MS was not related with prognosis ( OR =1.170,95% CI 0.583 ~ 2.345,P =0.659 ).④The cumulative survival rates of no adverse cardiovascular events in the two groups were no significant difference ( P > 0.05 ).Conclusions MS is a risk factor with coronary heart disease.Howerer,it has no relationship with adverse cardiovascular events in patients with ACS after PCI.
