Objective To obtain the elasticity value of non-mass-like breast lesions with supersonic shear wave elastrography (SWE), in order to observe the value of quantitative elastography with SWE in differential diagnosis of non-mass-like breast lesions. Methods SWE was performed in 80 non-mass-like breast lesions. Taking pathologic results as reference, quantitative elasticity value of the lesions were performed. The diagnostic accuracy, sensitivity and speciifcity were calculated. Results In the 80 non-mass-like breast lesions, 37 lesions (46%) were malignant and 43 lesions (54%) were benign. The max and mean elasticity value of malignant lesions were (106.28±46.39) kPa and (51.02±30.06) kPa, and the max and mean elasticity value of benign lesions were (37.13±18.22) kPa and (26.44±15.62) kPa. There was statistical differences between malignant and benign lesions in max and mean elasticity values (t=15.328, 18.149, both P<0.05). Taking 61.25 kPa as the threshold of max elasticity value and 40.65 kPa as the threshold of mean elasticity value, the diagnostic accuracy, sensitivity and speciifcity were 70.53%, 66.83%, 51.22%and 68.34%, 65.81%, 50.63%, respectively. When max elasticity was combined with conventional ultrasound (US), the diagnostic accuracy, sensitivity and speciifcity were 84.17%, 92.28%and 68.39%, respectively. The diagnostic accuracy and speciifcity signiifcantly increased compared with conventional US (χ2=5.217, 9.652, both P<0.05). When mean elasticity was combined with conventional US, the diagnostic accuracy, sensitivity and speciifcity were 82.35%, 90.66%, and 63.35%, respectively. The diagnostic accuracy and speciifcity signiifcantly increased compared with conventional US (χ2=5.084, 8.686, both P<0.05). Conclusions The diagnostic accuracy and speciifcity of SWE for non-mass-like breast lesions are not high. But when SWE is combined with conventional US, the diagnostic accuracy and speciifcity increase signiifcantly, which is very helpful for the diagnostic of non-mass-like breast lesions.

Objective To explore the clinical nursing of hip developmental dislocation in infants with Ferguson operation.Method The nursing strategy was performed including observation of the surgical incision,skin care,care of cast and instructions to their parents. Result No pressure sores occurred in all the 34 cases of hip developmental dislocation.1 case had delayed allergy reaction because of use of contrast media during operation,1 cast syndrome,and 19 edema of perineum of different degrees including 6 severe edemata,which were cured using wet compress with Adlerika.Conclusion It is of great importance for the reduced incurrence of operation complications and promising operative effect to observe the conditions and treat timely problems in postoperative nursing.

The acceptance assessment is an important part of research project management.By analyzing the content,methods,process and model of the current acceptance assessment of medical research projects in China,we summarized and elaborated the main problems in the medical research management.By referring to the mature practices of some other countries,we proposed the solutions to improve the quality of acceptance assessment in China,i.e.,balancing the qualitative and quantitative assessment,improving the quality of peer-review,constructing information platform,introducing independent third party assessment and construing the overall process evaluation system.Our study may provide important reference for constructing the high-quality evaluation system of medical research projects in China.

Objective To analyze the influences of lacrimal secretion and tear film after interferon-alpha therapy in patients with chronic viral hepatitis.Methods Sixty patients with chronic viral hepatitis who would accept the interferon-alpha therapy,including 30 cases of hepatitis B and 30 chronic viral hepatitis C,visited our department of ophthalmology and were enrolled in this study.All cases received eye examination,including medical history,slit lamp examination,corneal fluorescein staining ( FL) ,tear film break-up time ( BUT) and schirmer test before inter-feron-alpha therapy and two months after the therapy.Results In the 60 cases of chronic viral hepatitis,there were 19 dry eye cases (31.67%) and 41 non-dry eye cases (68.33%).In dry eyes group,9 of them (47.37%) were hepatitis B patients and 10 of them were hepatitis C patients (52.63%) .There was significant difference in FL grade (t=-5.288,P<0.05),BUT (t=5.799,P<0.05),Schirmer Ⅰ(t=5.319,P<0.05) test in hepatitis B before and two months after interferon-alpha therapy.Statistical significance had been found before and two months after therapy in FL grade (t=-9.103,P<0.05),BUT (t=5.682,P<0.05),SchirmerⅠtest (t=4.592,P<0.05). The incidence of dry eye syndrome in hepatitis B patients increased from 26.70%to 53.30%two months after treated with interferon-alpha,while the incidence of dry eye syndrome in hepatitis C patients increased from 33.30% to 60.00% two months after treated with interferon-alpha.Conclusion Dry eye syndrome is one of common complica-tions in patients with chronic viral hepatitis.Interferon-alpha therapy would influence the lacrimal secretion and dam-age tear film,which increased the dry symptoms in patients.We should pay attention to the side effects of interferon-alpha therapy.

As a critical part of national scientific and technology system reform, construction of national innovation system is a fundamental measure of innovation driven development strategy.Medical science and technology innovation system (MSTIS) is an important component of national innovation system (NIS).It is essential to elaborate the content, characteristics and process of the MSTIS.Based on the development status of Chinese economy and society and the framework of NIS, we evaluated the characteristics of medical science and technology development, and ultimately concluded and elaborated the content and characteristics of MSTIS from the following aspects: innovation force, innovation subjects, innovation activities, and innovation environments.We also prospected the future process of MSTIS, i.e., promotion of innovation force, ability construction of innovation subjects,scientific plan of innovation activities, and improvement of innovation environment.

Objective To evaluate the diagnostic value of clinical laboratory indexes on judgment of hypovolemia in the patients with nephrotic syndrome (NS).Methods The blood volume of each 50 cases of healthy adult men and women was assessed with indocyanine green-pulse dye densitometry (ICG-PDD).The normal range of blood volume and the cut-off value of hypovolemia were determined.The blood volume of 81 patients with NS was also measured with ICG-PDD and then these patients were divided into the hypovolemic group (21 cases) and the non-hypovolemic group (60 cases) according to the cut-off value of hypovolemia.The test data of clinical laboratory indexes of the patients in the two groups were compared,and the indexes with statistic difference were screened out.Their diagnostic values on judgment of hypovolemia were evaluated by receiver operating characteristic (ROC) curve analysis,and finally the indexes with high judgment accuracy were selected.Results ①The cut-off values of hypovolemia are ＜ 52.9 ml/kg for the male and ＜ 52.5 ml/kg for the women,which were determined with ICG-PDD.②The five clinical laboratory indexes,including orthostatic heart rate (OHR) increase ＞ 10 bit per minute,fractional excretion of sodium (FENa) ＜ 1,transtubular potassium gradient (TTKG) ＞ 60％,blood urea nitrogen/serum creatinine ratio (BUN/Scr) ＞ 20,and urine specific gravity (SG) ＞ 1.020,were used to judge the hypovolemia in NS patients in this study.ROC curve analysis showed that the diagnostic accuracy in judgment of hypovolemia by one index or two indexes combination only belonged to medium level.However,the diagnostic accuracy in judgment of hypovolemia by the following three indexes combination,i.e.OHR increase+FeNa+BUN/Scr,FeNa+ BUN/Scr+SG,OHR increase+TFKG+BUN/Scr,or ORG increase+FeNa+TTTKG,reached high level.Conclusion This study obtained the cutoff value of Chinese adults hypovolemia are ＜ 52.9 ml/kg for the male and ＜ 52.5 ml/kg for the women,which are determined with ICG-PDD,through evaluation we recommend applying the above four specific combinations of three indexes for diagnosis of hypovolemia in NS patients.

Objective To investigate the effects of PM2.5 on tear film function and corneal epithelial structure in mice.Methods Totally 24 male BALB/c mice (24 eyes) were divided into two groups:group A (with PBS eye drops,n =12),group B (5 mg · mL-1 PM2.5 eye drop group,n =12).PBS and PM2.5 eye drop were given with four times per day for 7 consecutive days in right eye.Tear secretion level was measured with phenol red thread.Break-up time (BUT) of tear film was tested,and corneal fluorescein staining (FL) was scored before therapy and 1 day,4 days and 7 days after droppings and HE staining was performed 7 days after droppings,respectively.Results There was no significant difference in the tear secretion levels,BUT,FL between the groups A and B before treatment (all P ＞ 0.05).At 4 days,7 days after treatment with PM2.5,the mean differences of the group B showed all items significantly changed compared with those before treatment (all P ＜ 0.05).For the group A,there was no statistical change in tear secretion levels,BUT,FL at 7 days after treatment (all P ＞ 0.05).There were statistical differences in all items between group A and B at each time point (all P ＜ 0.05).At 7 days after therapy,the mean layers of corneal epithelial cells in the group A (4 ± 1) was significantly lower than that in the group B (7 ±l) (P ＜0.05).The group B showed that the whole corneal fluorescein staining obviously increased,and corneal epithelial cell layer was thickened.Conclusion PM2.5 can influence tears film function and damage the corneal epithelial structure in mice.

Objective:To evaluate the reliability of several formulae for estimating the quantity of glucose absorption of peritoneal dialysate (GA).Methods:Forty-four patients receiving continuous ambulatory peritoneal dialysis (CAPD) were enrolled in this study. The quantities of GA obtained from actual measurement and estimation were compared to judge whether there was statistical difference between them.Results:The GA quantities estimated by Grodstein formula, Bodnar formula, K/DOQI formula, empirical method A (based on 60% absorption rate) and empirical method B (based on 50% absorption rate in daytime and 80% absorption rate in night) were as follows: 81.3 (64.2, 118.0) g, (97.8±19.7) g, (94.1±25.8) g, 87.1 (76.2, 109.0) g and (89.5±16.0) g, respectively; the actually measured GA quantity was [94.2 (77.5, 111.6)] g. Wilcoxon signed rank test of paired samples showed that only the results of Bodnar formula and K/DOQI formula did not present statistical differences from actually measured result.Conclusion:It can be considered to use Bodnar formula and K/DOQI formula to roughly estimate the GA quantity of CAPD patients, but to accurately understand the individual GA value, actual measurement is still required.

Several kinds of column chromatography method were used to investigate the chemical constituents of the leaves of Lophatherum gracile. The structures of the isolated compounds were identified based on their physicochemical properties and spectral data. A new flavone C-glycoside was isolated and its structure was identified as 3'-methoxyl-luteolin 6-C-beta-D-galactopyranosiduronic acid (1 --> 2) -alpha-L-arabinopyranoside (1).

OBJECTIVETo investigate the effects of early postnatal exposure to dieldrin on striatum synaptic development in lactation, adolescence and adulthood of mice.

METHODSThe pups were divided into 5 groups randomly. Three groups were exposed to dieldrin (0.01% DMSO solution) at doses of 0.2, 2.0 and 20.0 microg/kg and two control groups were exposed to DMSO or saline by intraperitoneal injection of every other day from postnatal days (PND) 3 to PND13. The striatum were isolated from brain in lactation (PND14), adolescence (PND36) and adulthood (PND98). Western blot assay was used to detect the expression levels of striatal synaptic proteins.

RESULTSThe postnatal exposure to dieldrin could reduce the level of growth associated protein (GAP43) of striatum in lactation in a dose-dependent manner. In adolescence, the level of glial fibrillary acidic protein (GFAP) in striatum increased and the levels of tyrosine hydroxylase (TH), GAP43 and post-synaptic density protein 95 (PSD95) decreased with exposure doses. The level of Synapsin I decreased in adolescence male mice. The changes of expression levels of GFAP, TH and PSD95 proteins lasted to adulthood.

CONCLUSIONEarly postnatal exposure to dieldrin could affect the expression level of GAP43 protein in striatum. The expression levels of TH and PSD95 proteins in striatum decreased in adolescence and adulthood. These results indicated that the early postnatal exposure to dieldrin may persistently interfere in the striatal synaptic development.
