1.Analysis and strategy on the less approved results of lumbar discectomy

Objective To study the relationship of surgical procedures with clinical effect in herniat-ed lumbar disc in order to improve operative methods and obtain a better outcome.Methods Retrospec-tive analysis was carried out in7235patients with herniated lumbar disc,who had been operated by removal of nucleus pulposus using small incision and fenestration in our hospital since1983,313cases of whom re-ceived second operation because of postoperative complications.There were187males and126females aging from27to62years(mean,45.8years),the incidence of morbidity was4.32%.During the same period,552patients,who had less approved operative result primarily treated in other hospitals,were admitted to our hospital.There were317males and235females aging from31to64years(mean,46.0years).Results The postoperative complications could be divided into short term(within1month after operation)and mid-dle to long term(more than1month after operation)groups.The total446cases with short term compli-cation were133cases primarily treated in our hospital and313cases in other hospital,including of re-herni a-tion or incomplete nuclear extirpation in77treated with second operation,discitis in106treated with con-servative therapy for 67and second operative management for 39,canal hematoma compression in76treated surgi cally,multi-level herniation and leakage in21and mislocalization in59re-operated,injury of nerve root or cauda equina in85treated with nerve exploration,release and anastomosis,residual materials in canal in17with removal of foreign body,venous thrombus in2,and arachnoiditis in3.The total424cases with middle to long term complications were185cases primarily treated in our hospital and239cases in other hospitals,including of nerve root adhesion in159,recurred disc herniation in122,segmental instability in81,and ia-trogenic spinal stenosis in62.Conclusion Many factors may influence the outcome of herniated lum bar disc which can be abolished by sufficient preparation,careful operation and proper managememt.
2.Studies on Preparation of Herba EpimediiTotal Flavonoids Phyt osomes and Their Pharmaceutics

Objective: To optimize preparation techniques for Herba E pimedii flavonoid phytosomes (EFP) and explore their suitable pharmaceutics. Methods: To optimize the preparation conditions by means of unifo rm design and step regression, prepare Herba Epimedii total flavonoid phytosomes by means of solvent evaporation and investigate the accumulative dissolution of different ratios of EFP-PVP precipitates by means of dissolution release. Resu lt: The optimized preparation conditions are as follows: solvent-tetrahydrofura n, lecithin to PVP-2.5 times, temperature-40 ℃ and reaction-3 hours. Oil/wa ter apparent partition coefficient of icariin was enhanced more than 4 times by phospholipid. The accumulative dissolution of Herba Epimedii flavonoids of EFP- PVP precipitate was significantly higher than that of its physical mixture and H erba Epimedii extract tablet. Conclusion: Phospholipid can ef fectively enhance the oil/water apparent partition coefficient of icariin, and P VP can improve the dissolution of Herba Epimedii phytocomes, but the pharmacokin etics needs further study.
3.Comparative test on puncture coring of two different needles used with the implantable drug-supplying device.

A comparison test on the number of puncture coring is conducted using the normal needle and the Huber needle in the injection area of the implantable drug-supplying device separately. The result indicates that the number of coring using the Huber needle is much less than that using the normal needle during the puncturing. So it is suggested to popularize the Huber needle in the drug transfusion of the implantable drug-supplying device.

4.Relationship of proprioception, cutaneous sensitivity and strength to postural stability among older adults

Objective To explore the impacts of proprioception, cutaneous sensitivity and strength on dynamic and static postural stability.Methods From June to November, 2020, a total of 164 elderly participated in this study. Independent parameters included proprioception of knee flexion/extension and ankle dorsi/plantar flexion with proprioception measurement; the cutaneous sensitivity of great toe, first and fifth metatarsals, arch, and heel with monofilament; and strength of ankle dorsi/plantar flexion and hip abduction with isokinetic test. The Berg Balance Scale (BBS) and center of pressure (COP)-based postural stability tests were conducted to represent dynamic and static postural stability. Exploratory factor and multivariable linear regression analyses were used to explore the relationship of each generated factor to postural stability outcomes.Results Proprioception was significantly correlated to the score of BBS (r=-0.449, P<0.001) and the root mean square (RMS) of COP in mediolateral direction (r=0.254, P=0.004). Cutaneous sensitivity was significantly correlated to the RMS of COP in the anteroposterior direction (r=0.281, P=0.002). Strength was significantly correlated to the score of BBS (r=0.493, P<0.001).Conclusion Proprioception and strength are related to dynamic postural stability, while proprioception and cutaneous sensitivity are associated with static postural stability.
5.Effects of epimedium total flavonoids phytosomes on preventing and treating bone-loss of ovariectomized rats.

OBJECTIVETo observe the effects of Epimedium total Flavonoids Phytosomes on preventing and treating bone-loss of the castrate osteoporosis rat model.
METHODThe osteoporosis model was established with 4-month-odl panther's rats, their ovaries on both sides castrated. Dual energy X-ray scanning was used to determine the bone density, and immunity and ELASA were used to assay concentration of estradiol and IL-6 in serum respectively, then determine their effect.
RESULTThe BMP and E2 of high dosage group nilestriol group and normal group are higher than those of model group (P < 0.01), while their content of IL-6 is apparently lower than that of model group(P < 0.01).
CONCLUSIONThe osteoporosis model was established successfully and the using of EFP can improve the bone density, enhance E2 level and decrease the IL-6 concentration in serum.

6.The technique study and primary clinical application of inverted Y-shaped self-expandable metal airway stent

Objective To investigate the feasibility and primary therapeutic effect of inverted Y-shaped self-expandable metal stent for complex airway stenosis.Methods On the standpoint of the peculiar anatomic structure and the pathological changes of complex airway stenosis,we designed the inverted Y-shaped self- expandable metal stent.Under the fluoroscopic guidance,7 stents were implanted in 7 cases of airway complex stenosis.Results The inverted Y-shaped self-expandable metal stents were placed seccussfully,with instantaneous relief of dyspnea and improvement of living quality.Conclusion The placement of inverted Y- shaped self-expandable metal stent is feasible and safe for treating airway complex stenosis.(J Intervent Radiol, 2007,16:92-94)

Hemagglutinin and neuraminidase, two important glycoproteins on the surface of influenza virus, play a considerable role in the entry and release stage of the viral life cycle, respectively. With in-depth investigation of influenza virus glycoproteins and the continuous innovation of drug discovery strategies, a new generation of glycoproteins inhibitors have been continuously discovered. From the point of view of medicinal chemistry, this review summarizes the current advances in seeking small-molecule inhibitors targeting influenza virus glycoproteins, hoping to provide valuable guidance for future development of novel antiviral drugs.
8. Influencing factors of dose coverage of unplanned irradiation of internal mammary lymph node drainage area in patients receiving radiotherapy after mastectomy

Objective:
To investigate the influencing factors of the dose coverage of unplanned internal mammary lymph node (IMN) irradiation in patients receiving chemotherapy after mastectomy.
Methods:
Clinical data of 138 patients receiving radiotherapy in the upper and lower lymph node drainage area of the thoracic wall and clavicle [three-dimensional conformal radiotherapy (3DCRT), field-in-field forward intensity-modulated radiotherapy (F-IMRT) or inverse IMRT (I-IMRT)] were retrospectively analyzed. The IMN was delineated according to the Radiation Therapy Oncology Group (RTOG) criteria. The unplanned irradiation dose of the IMN was obtained. The correlation between the IMN irradiation dose, clinical characteristics and specific parameters of radiotherapy during the unplanned irradiation was statistically analysed.
Results:
The mean dose of unplanned IMN irradiation was 32.85 Gy (range: 2.76-50.93 Gy). In total, 7.3% of breast cancer patients obtained the therapeutic dose of≥ 45 Gy. Body weight, body mass index (BMI), body surface area (BSA) and thoracic transverse diameter (DT) were lower, whereas the planning target volume of IMN (VIMN) included in the chest wall PTV (IMNin) and the ratio of IMNin to VIMN were higher compared with those of their counterparts with insufficient therapeutic dose. 9.Analysis of RPS6KA3 gene mutation in a Chinese pedigree affected with Coffin-Lowry syndrome.

OBJECTIVE:
To identify potential mutations of the CLS gene in a Chinese pedigree affected with Coffin-Lowry syndrome.
METHODS:
Whole exome sequencing was applied to detect potential mutation in the proband, and the result was verified by Sanger sequencing.
RESULTS:
The proband was found to carry a c.966_967delAA (p.Arg323Thr fs*11) deletional mutation in the RPS6KA3 gene. The same mutation was also found in his mother.
CONCLUSION
The c.966_967delAA (p.Arg323Thr fs*11) deletional mutation of the RPS6KA3 gene probably underlies the disorder in this pedigree.

10.Adult-Onset Still's Disease with Thrombotic Thrombocytopenic Purpura.

Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder that affects multiple organs and is fatal in some cases. Thrombotic thrombocytopenic purpura (TTP), another rare disease, is characterized by multiple organ failure and microangiopathic hemolytic anemia. TTP is an extremely rare complication of AOSD. We report a 59-year-old woman who presented with TTP that manifested after she was diagnosed with AOSD. Initially, her clinical manifestations improved with glucocorticoid therapy. However, her disease worsened and was accompanied by acute kidney injury, thrombocytopenia, hemolytic anemia, petechiae, and impaired consciousness. These clinical findings led to a diagnosis of TTP. This is the first report of AOSD complicated by TTP in Korea. Awareness of the possible development of TTP as a complication of AOSD is important for early diagnosis and treatment.
