1.Observation on effect of 3D-CRT combined with DC-CIK cell in treatment of recurrent smell cell carcinoma in high altitude regions

Objective To investigate the effect of 3‐dimensonal conformal radiotherapy (3D‐CRT ) combined with dendritic cell‐cytokine induced kill (DC‐CIK) cell in the treatment of recurrent small cell lung carcinoma (SCLC) in high altitude regions ,and its influence on VEGF and NSE .Methods Eighty‐five patients with recurrent SCLC in the tumor radiotherapy department of the Qinghai Provincial Fifth People′s Hospital from May 2012 to April 2014 were selected and divided into the control group(41 cases) and observation group(44 cases) according to the random number table .The two groups were respectively treated by 3D‐CRT treat‐ment and 3D‐CRT combined with DC‐CIK treatment cell .The curative effect ,incidence rate of adverse reactions ,levels of VEGF and NSE ,changes of liver and renal function indexes in the two groups were compared before and after treatment .Results The ef‐fective rate and sickness control rate of the observation group were 65 .9% and 79 .5% respectively ,which were higher than those in the control group ,and the differences were statistically significant (P<0 .05) .The levels of serum EGF and NSE ,and pain score at 4 ,8 weeks after treatment were decreased ,the differences were statistically significant (P<0 .05) .The levels of VEGF and NSE , and pain score at 8 weeks after treatment in the observation group were lower than those in the control group ,the differences were statistically significant(P<0 .05) .The incidence rate of adverse reactions in the two groups had no statistically significant differ‐ences after treatment(P>0 .05) .The levels of liver and renal function indexes had no statistically significant differences between before and after treatment(P>0 .05) .Conclusion 3D‐CRT combined with DC‐CIK cell in the treatment of recurrent SCLC in high altitude regions has more obvious curative effect ,moreover has no obvious influence on radiotherapeutic adverse reactions ,and liver and renal functions .

3.Clinical Significance of CK19, CD56 and p53 in differential diagnosis of papillary thyroid microcarcinoma and thyroid papillary hyperplasia

Objective To investigate the expression and diagnostic significance of CK19,CD56 and p53 protein in papillary thyroid microcarcinoma and thyroid papillary hyperplasia.Methods The expressions of CK19、CD56 and p53 protein were detected in 52 cases of papillary thyroid microcarcinoma and 31 of thyroid papillary hyperplasia by iImmunohistochemical methods.Results The positive rate of CK19 expression was 100 % (52/52) of papillary thyroid microcarcinoma and 29.0 % (9/31) in 31 of thyroid papillary hyperplasia.There was significant difference between two groups (P < 0.001).CK56 in 2 cases (3.8 %) of papillary thyroid microcarcinoma appeared mild positive expression,and in 20 cases (66.7 %) of thyroid papillary hyperplasia positive expression (P < 0.001).The positive expression rates of p53 were 69.2 %(36/52) in papillary thyroid microcarcinoma and 6.5 % (2/31) in thyroid papillary hyperplasia (P < 0.001).Conclusion CK19,CD56 and p53 may be important value on differential diagnosis of papillary thyroid microcarcinoma from thyroid papillary hyperplasia,and they are the indispensable markers of differential diagnosis.
4.Expression of Toll-Like Receptor in Peripheral Blood Mononuclear Cells of Rats with Nephrotic Syndrome Induced by Respiratory Syncytial Virus

Objective To explore the expression and the role of Toll-like receptor(TLR3 and TLR4) in rats with nephrotic syndrome induced by respiratory syncytial virus(RSV).Methods SD rats were inoculated intranasally and intraperitoneally with 6?106 plaque for-ming unit(PFU) RSV to construct RSV-induced nephropathy in rat model.Rats were anesthetized and blood was withdrawn from cardiac on day 4,14,30,60 after inoculation.The normal ones without intervention were set as control group.The renal histology was observed by light microscope and electron microscope.The urinary protein collected in 24 hours were measured.Meanwhile,the expressions of TLR3 and TLR4 were detected by indirect immunofluorescence staining and flow cytometry in peripheral blood mononuclear cells of rats.The results were analyzed by SPSS 13.0 softwore.Results After inoculation,the proteinuria increased and under the electron microscope the foot processes of glomerular epithelial cells were fused which resembled human minimal change nephrotic syndrome.Proteinuria reached the peak and the fusion of foot processes were most extensive in rats of RSV at 60 d.The expressions of TLR3 and TLR4 in each group of RSV-induced nephropathy in rat models were significantly higher than those in normal control group(Pa0.05).Conclusions TLR3 and TLR4 in peripheral blood mononuclear cells of RSV-induced nephropathy rat mo-dels had being significantly activated until 60 d after RSV inoculation.TLR signaling pathway may play an important role in nephrotic syndrome of rats induced by RSV.
5.Pigtail catheter mashing thrombosis combined with catheter directed thrombolysis in the treatment of acute iliofemoral venous thrombosis in patients of Cockett syndrome

Objective To evaluate pigtail catheter mashing thrombosis combined with catheter directed thrombolysis in the treatment of acute iliofemoral venous thrombosis complicated by Cockett syndrome in the left lower limbs.Method Data of 137 cases of acute iliofemoral venous thrombosis complicated with Cockett syndrome in left lower limb by interventional therapy from January 2007 to October 2012 were analyzed retrospectively.Inferior vena cava filters were placed in all of the patients.Patients were divided into two groups:Group A (n =81) treated with catheter directed thrombolysis only,Group B (n =56) treated with pigtail catheter mashing thrombosis combined with catheter directed thrombolysis.After operation,patients were treated by anticoagulation with urokinase and heparin calcium,and then warfarin for 6 to 1 2 months.Results The thrombolysis time in group B was significantly shorter than that in group A (P <0.01),the dosage of urokinase was significantly less than that in group A(P < 0.01).The venous patency score in group B after therapy was significantly better than in group A (P < 0.01).121 patients were followed up for 10-60 months.There were no pulmonary embolism.Conclusions Pigtail catheter mashing thrombosis combined with catheter directed thrombolysis in the treatment of acute iliofemoral venous thrombosis complicated with Cockett syndrome in left lower limb can improve thrombolytic efficiency,shorten thrombolysis time,reduce the use of urokinase.
6.Activation of NF-?B in airway epithelial celland modulation mechanism of NAC

Aim The expression of NF-?B activation and the effect of antioxidant (N- acetylcysteine, NAC) on NF-?B activity in human airway epithelial cell was assessed. Methods Using the TNF-?,the airway epithelial cell strains of normal subject(16HBE) and tumor patient (H292) was activated and using Western-Blot and ELISA the expression of NF-?B and IL-8 were detected. Results It was found that the activity of NF-?B could be stimulated by the TNF-? and increase with the amount of TNF-? with the peak occurring at 2 to 4 hours after stimulation and then decreasing at six hours. At the same time, the level of IL-8 was elevated, but decreased with inhibition of NF-?B activity by NAC, that means the action of NAC has a dose-dependent effect. Conclusion NAC not only blocks the signal transmission activated by NF-?B, but also anticipates the transcription modulation of expression of many cell factors and inflammatory mediums. It suggests that NAC may play a role in the anti-inflammatory treatment of respiratory diseases.
7.Effect of combined therapy of xiaoke shen'an capsule and western medicine in diabetic nephropathy.

OBJECTIVETo observe the therapeutic effect of xiaoke shen' an capsule on diabetic nephropathy.
METHODSNinety patients with diabetic nephropathy were randomly divided into two groups, the patients in the control group (CG) were treated with conventional western medicine, and those in the treated group (TG) were treated with combined therapy of xiaoke shen'an capsule and conventional western medicine. The treatment course of both groups was 8 weeks, and the therapeutic effect related indexes were measured before and after treatment.
RESULTSThe curative rate was 85.0% and 73.3% in CG and TG respectively, it was better in TG than that in CG (P < 0.05). Indexes such as fasting blood glucose, glycosylated hemoglobin, blood pressure, quantity of 24h urinary protein, urinary albumin excretion rate, renal function, blood lipids and hemoreheologic parameters were significantly different before and after treatment in TG (P < 0.05 or P < 0.01). As compared with CG, some of these indexes after treatment were superior to those in CG (P < 0.01 or P < 0.05).
CONCLUSIONCombined therapy of xiaoke shen'an capsule and conventional western medicine has definite therapeutic effect on diabetic nephropathy.

8.Effects of nano-lead exposure on learning and memory as well as iron homeostasis in brain of offspring rats.

OBJECTIVETo investigate the effects of nano-lead exposure on learning and memory and iron homeostasis in the brain of the offspring rats on postnatal day 21 (PND21) and postnatal day 42 (PND42).
METHODSTwenty adult pregnant female Sprague-Dawley rats were randomly divided into control group and nano-lead group. Rats in the nano-lead group were orally administrated 10 mg/kg nano-lead, while rats in the control group were administrated an equal volume of normal saline until PND21. On PND21, the offspring rats were weaned and given the same treatment as the pregnant rats until 42 days after birth. The learning and memory ability of offspring rats on PND21 and PND42 was evaluated by Morris water maze test. The hippocampus and cortex s amples of offspring rats on PND21 and PND42 were collected to determine iron and lead levels in the hippocampus and cortex by inductively coupled plasma-mass spectrometry. The distributions of iron in the hippocampus and cortex were observed by Perl's iron staining. The expression levels of ferritin, ferroportin 1 (FPN1), hephaestin (HP), and ceruloplasmin (CP) were measured by enzyme-linked immunosorbent assay.
RESULTSAfter nano-lead exposure, the iron content in the cortex of offspring rats on PND21 and PND42 in the nano-lead group was significantly higher than those in the control group (32.63 ± 6.03 µg/g vs 27.04 ± 5.82 µg/g, P<0.05; 46.20 ±10.60 µg/g vs 36.61 ± 10.2µg/g, P<0.05). The iron content in the hippocampus of offspring rats on PND42 in the nano-lead group was significantly higher than that in the control group (56.9 ± 4.37µg/g vs 37.71 ± 6.92µg/g, P<0.05). The Perl's staining showed massive iron deposition in the cortex and hippocampus in the nano-lead group. FPNl level in the cotfex of offspring rats on PND21 in the nano-lead group was significantly lower than that in the control group (3.64 ± 0.23 ng/g vs 4.99 ± 0.95 ng/g, P<0.05). FPN1 level in the hippocampus of offspring rats on PND42 in the nano-lead group was significantly lower than that in the control group (2.28 ± 0.51 ng/g vs 3.69 ± 0.69 ng/g, P<0.05). The escape latencies of offspring rats on PND21 and PND42 in the nano-lead group were longer than those in the control group (15.54 ± 2.89 s vs 9.01 ± 4.66 s; 6.16 ± 1.42 s vs 4.26 ± 1.51 s). The numbers of platform crossings of offspring rats on PND21 and PND42 in the nano- lead group were significantly lower than those in the control group (7.77 ± 2.16 times vs 11.2 ± 1.61 times, P<0.05; 8.12 ± 1.51 times vs 13.0 ± 2.21 times, P<0.05).
ONCLUSIONn Nano-lead exposure can result in iron homeostasis disorders in the hippocampus and cortex of offspring rats and affect their learning and memory ability.

9.Effect evaluation of health education on iodine deficiency disorders prevention in Linxia Hui autonomous prefecture of Gansu province

Objective To evaluate the effect of health education on the target population of iodine deficiency disorders (IDD) in Linxia Hui autonomous prefecture.Methods During 2006-2010,IDD health education was carried out in the target population of Linxia Hui autonomous prefecture,and IDD knowledge of 5th grade pupil and 20-50 years old child-bearing women were surveyed by stratified sampling and questionnaire survey methods to evaluate the effect of health education.Results After health education,IDD knowledge passing rate rose from 55.6% (548/986) and 18.0%(118/657) to 90.0%(1103/1225) and 86.2%(687/797) among pupils and women,respectively.The awareness rates of iodine deficiency in causing goiter,mental retardation and birth born silly children,and the key role of iodized salt in controling IDD were increased from 79.2% (781/986),76.5% (754/986),51.7% (510/986) and 82.3% (811/986) to 88.8% (1088/1225),85.6% (1049/1225),68.7% (842/1225) and 90.0% (1103/1225) among pupils,and from 48.6% (319/657),28.3% (186/657),15.4% (101/657) and 39.6% (260/657) to 86.2% (687/797),70.0% (558/797),46.3% (369/797) and 87.3% (696/797) among women,and all the difference were significant,respectively.Belief rate held by students that health education can prevent iodine deficiency disorders was 87.3% (1069/1225),and 52.9% (648/1225) of the students told their parents about IDD.The rate of consciously purchase of qualified iodized salt was 87.1% (694/797) after health education among women of childbearing age,and the consumption rate of iodized salt was 64.6%(515/797).More than 90% of the people in relevant departments knew iodine deficiency disorders related knowledge,in particular,had a clear understanding of intellectual hazards caused by iodine deficiency,and actively supported and participated in health education in the prevention and control of iodine deficiency disorders.Conclusions Comprehensive health education measures taken through schools and religious organizations has promoted a shift in the behavior,beliefs and attitudes of its consciously purchase and consumption of iodized salt,and integrated knowledge,attitude and practice (KAP).It is a good model of health education through religious organizations in ethnic minority areas.It has promoted the health education feasibility and accessibility,and the effect is more durable.
10.Analysis of iodine nutritional status and thyroid function of pregnant and lactating women in urban areas of Wuwei City of Gansu Province

Objective To find out the iodine nutritional status and thyroid function of pregnant and lactating women in the urban areas of Wuwei City of Gansu Province,and to provide a scientific basis for iodine supplementation.Methods Blood and urine samples of pregnant (52 persons) and lactating women (59 persons) were collected in 2009.Urinary iodine content was measured with arsenic cerium catalytic spectrophotometric.Serum free three triiodothyronine(FT3),free thyroid hormone(FT4),triiodo thyronine(TT3),serum total thyroxine (TT4) and thyroid-stimulating hormone (TSH) were detected with direct chemiluminescence immunoassay.Results The medians urinary iodine of pregnant and lactating women were 274.68,217.88 μg/L.The rates of low urinary iodine (pregnant women below 150 μg/L,lactating women lower than 100 μg/L) were 9.62% (5/52) and 6.78% (4/59).Serum TT3,TT4 levels in pregnant women[(2.48 ± 0.59),(132.18 ± 33.36)nmoL/L] were higher than that in the lactating women[(2.16 ± 0.49),(108.79 ± 28.36)nmol/L,t =-3.123,-3.971,all P < 0.05].Thyroid dysfunction incidence rates of pregnant and lactating women were 17.31% (9/52) and 8.47% (5/59).Thyroid dysfunction was mostly subclinical hypothyroidism.Conclusions Overall iodine nutrition of pregnant and lactating women is in good condition,some individuals have the trend of hypothyroidism.It is necessary to carry out routine monitoring of urinary iodine and thyroid function.
