1.Surgical treatment and diagnosis for insulinoma

Objective Discussion of diagnosing insulinoma.Methods Retrospective analysis based on 45 cases of insulinoma patients that are collected by General Surgery Department in the First Affiliated Hospital of Zhengzhou University from June 2008 to October 2014.Results Forty-five cases of patients with insulinoma including 41 cases (91.11%) of functional insulinoma and 4 cases (8.89%) of non-functional insulinoma.Apply selective arteriography,preoperative ultrasound,intraoperative ultrasound and CT to diagnosis.Forty-five cases of patients with insulinoma,in which 18 cases with laparoscopic simple tumor resection,6 cases with laparoscopic splenectomy and tail of the pancreas resection;45 cases of insulinoma with laprotomy,in which 8 cases with pancreaticoduodenectomy,12 cases which the tumors were located in the neck,neck-line of the pancreas with local excision of the pancreas and pancreatic tail-jejunal Roux-en-Y anastomosis,one cases of laparotomy found no placeholder.Conclusions Surgery removal is currently the most successful way to eliminate insulinoma.And localization diagnosis can determine the outcome of the surgery.Co-operation of surgery removal and localization diagnosis can be the optimal approach to diagnose certain insulinoma.
2.Value of PET/CT for diagnosis of recurrence and metastasis of differentiated thyroid carcinoma

Objective To analyze the value of PET/CT for diagnosis of recurrence and metastasis of dif-ferentiated thyroid carcinoma ( DTC) .Methods 65 patients with DTC in Thyroid Surgery Department of the First Affiliated Hospital of Zhengzhou University from Dec .2005 to Jan.2015 were collected.Based on the values of serum thyroglobulin(Tg),the patients were divided into 2 groups.29 cases with positive Tg were the observation group and 36 cases with negative Tg were the control group .All patients underwent PET/CT examination one year after surgery.Patients with positive findings from image studies underwent surgical excision and postoperative pathological diagnosis .Clinical data of the 2 groups were then summarized and discussed .Results There was no significant difference on the general conditions between the 2 groups(P>0.05).Tg values were statistically dif-ferent 6 months and 1 year after surgery between the 2 groups(P<0.05).Tg values of both groups decreased 1 year after surgery, but without statistical difference (P>0.05).9 cases in the observation group and 1 case in the control group had positive findings on image study .All the cases with cervical lymph nodes presented as high uptake of 18 F-FDG.All patients who had positive findings on PET/CT underwent surgical removal of the abnormal lymph nodes which confirmed by pathology as lymph node metastasis .Conclusion PET/CT is very important for diagno-sis of recurrence and metastasis on Tg(Tg>2μg/L)positive,131I Dx-WBS imaging and US negative patients.
3.Cone beam CT quality assure procedure and the analysis of results

Objective To study the image quality control system to ensure that equipment meet clinical needs.Methods It was scanning the Catphan504 phantom with models of high quality head,standard dose head and pelvis,we could get the results of CT numbers linearity,uniformity,spatial resolution,contrast resolution.Using T test to compare different scanning technique results.Results The standard dose head scanning technique was better than the pelvis scanning technique in CT numbers linearity test,and gets the best result in uniformity test.The result of CT numbers uniformity was higher in the standard dose head scanning than the high quality head and the pelvis scanning (9.7 ±3.9 vs.17.9 ±5.3,P =0.00 and 9.5 ± 4.0 vs.31.1 ± 5.7,P =0.00).The result of contrast resolution was higher in the pelvis scanning than the high quality head and the pelvis scanning (5.6 ± 0.1 vs.1.3 ± 0.5,P =0.00 and 6.0 ± 1.0 vs.1.3 ± 0.5,P =0.00).The result of spatial linear distance was very accurate,the range was 4.98 -5.06 cm.Conclusions The results of spatial linearity test are stable and accuracy,but CT numbers linearity and uniformity test are affected by the scanning technique significantly for device.To spatial resolution test and contrast resolution test,we need to set the standard and tolerance according to each linear accelerator specialty.
4.A daily quality assure procedure for the on board imager and analysis of the results

Objective Performing a daily quality assure (QA) program to get variation and error range of on board imager (OBI) system,so that the OBI system can meet the needs of clinical treatment.Methods The daily QA program including: mechanical accuracy,2D/2D Shift calculations accuracy,couch motion accuracy.Results The max deviation was-0.7 mm in lcft-right (LR) dircction and 0.8 mm in superior-inferior (S1) direction in Linac& OBI isocenter accuracy check.The max deviations in 4 blades (x1,x2,y2,y1) position accuracy check were:-2.1 mm,2.2 mm,± 1.7 mm,-2.1 mm.In OBI mechanical arms position accuracy check,31% standard data was 85.2 cm with 0 mm deviation; 69% standard data was 85.1 cm with 1 mm deviation.In LR,SI and anterior-posterior direction,2D/2D shift calculations accuracy was 0.46 mm,1.35 mm,-0.04 mm and couch motion accuracy was-0.1 mm,0.3 mm,0.2 mm,respectively.Conclusions By performing the daily QA program,it could be found whether OBI works properly and satisfies the clinical use.The physicist can pay more attention to the parameters which change frequently,and adjust the frequency of the parameters which are stable,so that working efficiently.
6.Porhyromonas endodontalis liopolysaccharide regulates interleukin-23 expression in mouse osteoblasts

Objective:To investigate the effects of Porhyromonas endodontalis(P.e)liopolysaccharide(LPS)on the expression of IL-23mRNA and protein in mouse osteoblasts MC3T3-E1 and the role of phosphatidylinositol 3-Kinases (PI3K)signaling pathway in this process.Methods:MC3T3-E1 cells were treated with different concentrations of P.e LPS for different hours,or pretreated with LY294002,a special PI3K inhibitor.The IL-23 mRNA and protein expression levels were detected by real-time PCR and Western blot respectively.Results:The level of IL-23 mRNA increased in MC3T3-E1 cells with the increase of concentration and the treatment time of P.e LPS(P <0.05).The IL-23 protein expression was increased by P.e LPS in a time dependent manner(P <0.05).The mRNA and protein of IL-23 decreased(P <0.05)after pretreatment with LY294002.Conclusion:P.e LPS can induce the expression of IL-23 mRNA and protein in MC3T3-E1 cells,and the PI3K signaling pathway may play a part in this process.
7.Changes of mitochondria in human placenta in intrauterine growth restriction

Objective To explore the mechanism of intrauterine growth restriction (IUGR) via observing the change of mitochondria in IUGR placenta. Methods Placenta samples were collected from 30 singleton pregnancies at the time of elec-tive caesarean section. Fifteen of them were appropriate for gestational age and 15 were IUGR. Mitochondrial morphology was observed by transmission electron microscopy, DNA copies were analyzed by real-time quantitative PCR and membrane potential was assayed by lfow cytometry. Results Signiifcant morphological changes of placental mitochondria were observed under transmission electron microscopy in IUGR, mitochondrial DNA copies in IUGR placenta were signiifcantly increased (P<0.01) and membrane potential decreased dramatically (P<0.01). Conclusions It is suggest that impaired mitochondrial function in IUGR may involve in IUGR pathogenesis.
8.Identification and molecular mechanism study of a case with B(A)02 allele

Objective To identify and investigate B(A)02 allele in a patient. Methods Serological tests were performed with standard serological methods in a patient with B(A)02 allele. DNA sequences of all seven exons and exon-intron boundaries of ABO gene were analyzed by polymerase chain reaction (PCR), direct DNA sequencing and sequencing after gene cloning. In order to analyze the allele, PyMOL software was used to establish 3D model of Glycosyltransferases B (GTB). Results The serological results showed the characteristics of B(A) phenotype. DNA analysis revealed that ABO gene of the individual was heterozygous of B(A)02/O01 allele. 700C>G mutation was identified in B101 allele, which resulted in the amino acid substitution P234A in GTB. Through the analysis of the 3D structure of GTB, it was speculated that the P234A replacement affected the intermolecular forces of the 234 amino acid and Met-266, thus changed the conformation of the donor-binding pocket of GTB,that made GTB capable of recognizing and tranferring the GalNac to the H antigen, which can lead to the formation of the weak A antigen on membrane of red blood cells. Conclusion The P234A replacement can affect the spatial conformation of the specific recognition region conformed by Met-266 and Ala-268 residues, which leads to the antigenicity change of the ABO blood group.

10.The inhibitory effect of quercetin on in vitro activation of T lymphocytes

AIM: To investigate the inhibitory effect of quercetin on in vitro activation of T lymphocytes by polyclonal activators with CD69 expression as an activation marker. METHODS: After being separated from lymphoid nodes of a C57BL/6 mouse, the lymphocytes were exposed to polyclonal activators (PDB or Con A) with or without quercetin. Then they were harvested at 2 h, 6 h and 24 h, respectively. The expressional rates of CD69 on T lymphocytes were assessed by two-color immunofluorescent staining and flow cytometry, and the inhibitory rates of quercetin at different time points were estimated. RESULTS: Quercetin had no effect on the expressional rate of CD69 on T lymphocytes under resting states. After the stimulation with PDB or Con A, the expressional rates of CD69 on T lymphocytes in the present of quercetin (10 ?mol/L) showed significant decrease compared with those of control groups at different time points (P
