Objective To study the perinatal outcomes of pregnant women when their total stage of labor over 24 hours after the application of new partogram.Methods A retrospective case-control study was conducted on 3 991 pregnant women who received routine prenatal care and intrapartum management according to new partogram and delivered at full-term with alive babies at Beijing Obstetrics and Gynecology Hospital from October 1,2014 to January 31,2015.Among these 3 991 women,85 delivered after 24-hours' labor and all were nulliparas,while five of the 85 converted to cesarean section (CS group) and 80 delivered vaginally (study group).Another 160 primigravidas were randomly selected from those vaginal deliveries with the total stage of labor less than 24 h as control group.The general conditions,maternal and fetal perinatal outcomes were compared between the study and control group and between the study and CS group.The duration of labor was compared among different groups of women in the study group.Independent sample t-test,Rank-sum test,Chi-square test or Fisher's exact test were applied for statistics.Results The durations of first stage,second stage,third stage and total stage of labor were all longer in the study group than in the control [25.7(23.8-26.8) vs 10.4(5.9-13.9) h,1.4(0.6-1.8) vs 0.8(0.4-1.0) h,0.14(0.08-0.17) vs 0.11(0.07-0.13) h,27.2(24.9-26.8) vs 10.4(5.9 14.0) h,Z=12.525,4.359,3.528 and 12.588,all P＜0.01].The proportions of intrapartum interventions and application of labor analgesia were both higher in the study group than in the control [97.5％(78/80) vs 32.5％(52/160),42.5％(34/80) vs 11.9％(19/160),x2=90.764 and 29.071,both P＜0.01].Higher incidences of intrapartum fever [18.8％(15/80) vs 5.6％(9/160),x2=10.208],episiotomy [50.0％(40/80) vs 25.6％(41/160),22=14.172],assisted vaginal delivery [26.3％(21/80) vs 10.0％(16/160),x2=10.800],postpartum hemorrhage [32.5％(26/80) vs 8.1％(13/160),x2=23.284],and cervical laceration [15.0％(12/80) vs 3.1％(5/160),~=11.427] were shown in the study group than in the control (all P＜0.01).There was no statistically significant difference was found in the rate of fetal distress,neonatal asphyxia and neonatal intensive care unit (NICU) admission between the study and control group (all P＞0.05) except for newborns' birth weight [(3 542±388) vs (3 431 ±368) g,t=2.162,P=0.032].However,the incidences of postpartum hemorrhage,intrapartum fever and neonatal admission to NICU were comparable between the study (n=20) and CS group (both P＞0.05).Within the study group,the durations of second stage of labor in women with postpartum hemorrhage,assisted vaginal delivery and episiotomy were significantly longer than those without [1.8(0.9-3.1) vs 1.2(0.5-1.5) h,2.0(0.8-3.4) vs 1.2(0.5-1.6) h,1.7(0.6-2.6) vs 1.1(0.5-1.5) h,Z=2.168,2.756 and 1.891,all P＜0.05].Conclusions After the application of new partogram,vaginal delivery remains possible for pregnant women whose total stage of labor over 24 hours without increase of neonatal morbidity.However,the incidences of postpartum hemorrhage,episiotomy and assisted vaginal delivery may rise up.

Along with several decades development,the urinary and male genital system neoplasm has gained distinguished research achievements in China.The research output is being historically high.However,our ability of treatment and diagnosis cannot meet people's requirement of health due to the serious prevalent situation.It is necessary for China to take actions to keep up with the advanced world level.

Objective To investigate the effects of Hui-hui Gan-song Yin(HGY) on the accumulation of extracellular matrix of rat glomerular mesangial cells(MCs) induced by high glucose.Methods The 40 SD rats were randomly divided into normal control group(distilled water), glurenorm group(10 mg/kg), HGY high-dose group(10 g/kg) and HGY low-dose group(5 g/kg), 10 rats in each group.The rats in each group were treated with corresponding drugs, twice a day.After 3 days, the serum containing each drug were prepared to culture rat MCs in vitro.The MCs were divided into the normal control group( 10% serum of rats in normal control group ) , high glucose group ( 30 mmol/L glucose +10% serum of rats in normal control group), glurenorm group(30 mmol/L glucose+10% serum of rats in glurenorm group), HGY high-dose group(30 mmol/L glucose+10% serum of rats in HGY high-dose group) and HGY low-dose group(30 mmol/L glucose+10% serum of rats in HGY low-dose group).The fibronectin(FN), ColⅠand ColⅣ levels were detected by Western blot.Results Compared with normal control group, the expression of FN, ColⅠand ColⅣ in high glucose group increased(P<0.01).The HCY suppressed the protein expression of FN, ColⅠand ColⅣ significantly(P<0.05).Conclusion The serum containing HGY could suppressed protein expression of FN , ColⅠand ColⅣ and inhibit the accumulation of extracellular matrix of MCs induced by high glucose, which could protect glomerulus and delay the development of diabetic nephropathy.

Objective To reproduce a rat model of collagen-induced arthritis with anemia,and to probe the effects of thalidomide on hemoglobin of these rats.Methods 128 male Wistar rats were randomly divided into 5 groups:Ⅰ(normal control group,n=24),Ⅱ(model group,n=26),Ⅲ(model group with high dosage of thalidomide,n=26,200mg?kg-1?d-1),Ⅳ(model group with low dosage of thalidomide,n=26,100 mg?kg-1?d-1)andⅤ(model group with methotrexate(MTX),n=26,2.7mg?kg-1?w-1).Every group was divided into 6 sub-groups(14,21,28,35,42 and 60d)according to different time points.The rat models with collagen-induced arthritis and anemia were reproduced by multiple subcutaneous injections at intervals of emulsion of collagen Ⅱ and Freund's complete adjuvant.Different kinds of treatment were given to the rats of different groups beginning on the 12th day after the first injection.The levels of hemoglobin in rats of group Ⅰ,Ⅱ,Ⅲ,Ⅳ and Ⅴ were determined,and serological characteristics,morphologic changes,and serum iron content,total iron binding capacity,and pathological changes in bone marrow in rats of group Ⅰ and Ⅱ were observed.Results On the 28th day after the first injection,anemia appeared in rats of group Ⅱ,in which the hemoglobin level was lower significantly compared with that of group Ⅰ(P

Normal wound healing is a very complicated process. As the wound covering, the dressing can protect the wound, accelerate the healing process and prevent infection during wound healing. Chitosan-based nanofibers have shown great prospect in biomedical dressings due to their unique biological and physicochemical properties. In this paper, the characteristics, materials, application and evaluation of chitosan-based electrospun nanofiber dressings are reviewed, and its application prospects are also prospected.

Using bioactive compounds 7-hydroxy flavone, salicylaldehyde, cinnamic acid and 4-amino-5- mercapto-1,2,4-triazoles as starting materials, three new types of flavone derivatives containing 1,2,4-triazole structure were synthesized via different step reactions. These new compounds were characterized by 1IHNMR, ESI-MS, IR and elemental analysis. Their scavenging effects on the superoxide radical (O2·-), hydroxyl radical (·OH), DPPH · radical and their total reduction activities were tested. The results showed that all of the compounds possessed some antioxidative activity at the concentration of 0.5 mg · mL(-1), but the scavenging ability of the target compounds was lower than that of the standard compound Vc.

Objective To investigate the necessity of dilution in samples with high concentrations of D-Dimer and optimum dilution multiple.Methods Quality control products and calibration were detected by using Sysmex CS5100 for precision evaluation,including within-batch and between-run precision.Calibration were detected for validation of linear range and clinical reportable.Samples with D-Dimer<5 mg/L and fibrinogen degradation products(FDP)<20 μg/mL were serially diluted and detected to calculate recovery rate.Samples with D-Dimer>5 mg/L and FDP>20 μg/mL were also serially diluted and detected to calculated recovery rate.Results Within-batch and between-run coefficients of variation were both less than 3%.Within the scope of 0.207-5.170 mg/L,the linear distribution was fine.The clinical reportable range was 0.207-165.440 mg/L.For samples with D-Dimer<5 mg/L and FDP<20 μg/mL,no antigen excess phenomenon was found,and gradient dilution was not necessary.For samples with D-Dimer>5 mg/L and FDP>20 μg/mL,there was obvious antigen excess phenomenon,and gradient dilution was required.Conclusion For samples with D-Dimer>5 mg/L and FDP>20 μg/mL,dilution should be performed to ensure the accuracy of detected results.

Objective To discuss the optimal imaging conditions of spatio-temporal image correlatin (STIC). Methods Conventional 2D fetal echocardiography and 3D STIC volume collection were performed in 130 fetuses. The images obtained using two methods were scored and compared. Results Complete 3D volume collection with the initial view of four chamber view were abtained in 121 fetuses, the successful rate was 93.13%. There was no significant differences in total score between the two methods. Significant difference was found between the score of different condition groups.The highest scores mostly appeared in lateral four chamber heart in both method groups. The highest scores mostly appeared in 25-29 gestational weeks with 2D ultrasound, while 20-24 gestational weeks with 3D ultrasound, scores in long axis views of vessels were statistically different. Conclusion STIC has advantages over conventional ultrasonography, but needs conditions control and analysis skills.

AIM:To investigate morphologic and functional changes of small intestinal mucosa and proliferating cell nuclear antigen in postoperative portal hypertension patients with single or combined administration of Gln and rhGH.METHODS:Twenty-nine portal hypertension patients with surgical treatment were prospectively randomized to four groups as follows:① Gln group(n=6);② rhGH group(n=8);③ Gln+rhGH group(n=7)and ④ control group(n=8).A standard solution for TPN was given three days after operation for a week.The concentration ratio of urinary lactulose and mannitol(L/M),the villus height and crypt depth and PCNA index of small intestinal mucosa were compared.RESULTS:A week after TPN postoperation,the increased ratios of L/M in Gln+rhGH group were less than those in control group(P0.05).CONCLUSION:This study suggest that Gln together with rhGH reduce the intestinal permeability and protect the mucosa integrality in postoperative portal hypertension patients,but not in single treatment.
