1.A Case of Melanocytoma of the Optic disc.

A case of melanocytoma of the optic disc(os), a 43 year-old male who had hypertensive retinopathy, was discovered in routine funduscopy on Feb. 5' 75. His visual acuity and visual field revealed normal findings. Funduscopic findings showed a round dark pigmented mass occupied one disc diameter in lower-temporal quadrants.

2.Clinical Study for Variation of Ocular Tension after Retrobulbar Injection.

We had studied correlations of changes of ocular tension after retrobulbar anesthesia and digital massage in 100 cases which was received the cataract operation. The results were as follows. 1) The mean value of intraocular pressure reduction after retrobulbar injection and digital massage was 8.26 mmHg. 2) The effects of lowering intraocular pressure in administration of hyperosmotic agents group was almost same in non-hyperosmotic agents group. 3) Reducing intraocular pressure didn't seem to have any effect to the vitreous loss.

4.The Effect of alpha-MSH on Long-term Histologic Changes after Acute Ischemic Kidney Injury in Rats.

BACKGROUND: Ischemic acute renal failure (ARF) is increasingly recognized as involving chronic functional and structural sequelae. Ischemia reperfusion injury (I/R) plays a major role in delayed graft function and long-term changes after kidney transplantation, also. The present study was designed to evaluate the long-term changes after acute ischemic injury and whether renoprotection by alpha-MSH in the acute ischemic stage, could reduce the long-term sequelae. METHODS: In control group, ischemia/reperfusion injury was induced in male Sprague-Dawley rats by clamping Lt. renal pedicle for 15, 45, 60 minutes after removal of Rt. Kidney and followed by reperfusion. The animals in alpha-MSH group were injected alpha-MSH prior to reperfusion and then every day for 1 week. We measured BUN, creatinine at 24 hour after I/R injury, and in each group, 6 animals were sacrified at 1 week and 4 weeks after I/R injury to evaluate apoptosis, ED1, PCNA, and histopathologic changes. RESULTS: At 4 weeks after I/R injury, the remnant structural damage such as apoptosis, ED1 stained cells, MT (+) tubulointerstitial fibrosis were observed. alpha-MSH could reduce the initial functional injury, and apoptosis, ED1 stained monocyte, MT (+) tubulointerstitial fibrosis, also. CONCLUSION: Renal function was recoverd at 4 weeks after I/R injury, but structural sequelae such as apoptosis, fibrosis were remnant. alpha-MSH could attenuate initial functional damage and remnant fibrosis.

6.TT Virus (TTV) Infection in General Population, Chronic Liver Diseases, Hemodialysis, and Transfused Patients.

BACKGROUND: TT virus (TTV), isolated initially from a Japanese patient with posttransfusion hepatitis of unknown etiology, was suggested to be a new causative agent of hepatitis. However, it has been found to infect both healthy and diseased individuals and numerous studies have raised questions about its pathogenic role in hepatitis. In order to study its prevalence and clinical impact on hepatitis, we assessed the frequency of TTV DNA. METHODS: Serum samples were obtained from 60 cases of the controls, 77 cases of chronic liver diseases, 44 cases of hemodialyzed patients, and 65 cases of transfused patients. TTV DNA was detected using nested polymerase chain reaction and alanine aminotransferase (ALT), aspartate aminotransferase (AST), and hepatitis B surface antigen (HBsAg) were measured. RESULTS: TTV DNA was detected in 41.7% of the controls, 51.9% of patients with chronic liver diseases, 68.2% of hemodialyzed patients and 61.5% of transfused patients. Comparison between patients with or without TTV revealed no significant differences in AST, ALT, and HBsAg test results. CONCLUSION: The prevalance of TTV infection in patients with chronic liver diseases was similar to that in the controls. TTV infection was not related to abnormal liver function findings and HBsAg positivity. We found no relationship between TTV infection and chronic liver diseases.

7.The effects of hydrogen sulfide under sevoflurane administration against ischemia and reperfusion injury in isolated rat heart.

BACKGROUND: Hydrogen sulfide (H2S) produces a protective effect against myocardial ischemia and reperfusion injury. Sevoflurane, which is used for anesthesia in cardiac problem patients, also has a protective effect. This study is designed to reveal the effects of H2S under sevoflurane using rat hearts. METHODS: The hearts were Langendorff-perfused, subjected to 30 minutes ischemia and 60 minutes reperfusion. Group I was a control group. The other groups were pretreated for 15 minutes before ischemia as follows: 1.6% sevoflurane for group S; 18.5 microM H2S S for group H; and 1.6% sevoflurane and 18.5 microM H2S simultaneously for group HS. Hemodynamics and the infarct size were measured. RESULTS: Group HS presented depressed hemodynamics during pretreatment. LV function in group HS achieved better recovery than group I after reperfusion. The infarct size of groups S, H and HS was smaller than group I, while there were no differences between groups S, H and HS. CONCLUSIONS: Exogenous H2S did not enhance the preconditioning effects of sevoflurane. Rather, the results suggest that H2S under sevoflurane might depress hemodynamics.

8.Apoptosis induced by adenosine 5'-trisphosphate in mouse leukemic cells.

Extracellular ATP elicits various biological responses and plays a significant role in physiological regulation. Recently, ATP-induced growth inhibitions were reported in some tumor cell lines, but these effects and mechanisms are not well known. This study was conducted to investigate ATP-induced growth inhibition in mouse leukemic (P388D,) cells. ATP inhibited cell growth in a dose-dependent manner as analyzed by MTS assay (IC50: 33.1 muM). Nucleotides other than ATP, such as ADP (37.5 muM) and AMP (33.2 muM) had the same effects as ATP, but adenosine (57.8 muM) showed less effect than ATP. ATP attenuated the cells in G0/G1 and G2/M phases, but increased those in S phase in flow cytometric analysis. Hypodiploid cells (A0), the presumptive findings of apoptosis, were found among the ATP-treated cells. ATP induced DNA fragmentation into 180 ~ 200 bps as measured by electrophoresis. Some apoptotic cells were stained by TUNEL method. ATP increased the intracellular free Ca++ concentration ((Ca++)i) and the increment of (Ca++)i was caused by influx from the extracellular space. These results suggest that extracellular ATP induces growth inhibition through apoptosis.

9.Comparisons of Myocardial Protective Effects of Sevoflurane at Different Concentrations against Ischemia in Isolated Rat Heart.

BACKGROUND: Sevoflurane, a newly developed halogenated inhalation anesthetic agent shows myocardial protective effects against global ischemia like other inhalation agents. We investigated differences between pharmacologic preconditioning effects at various concentrations of sevoflurane. METHODS: Forty male Sprague-Dawley rats were subdivided into 4 groups (each n = 10). All groups underwent the same procedure (Langendorff preparation, 30 minutes ischemia and 60 minutes reperfusion) except for the concentrations of sevoflurane. The control group received no sevoflurane treatment. The sevo 1.6% group was given 1.6% sevoflurane before ischemia, the sevo 205% group was given 2.05% sevoflurane before ischemia, and the sevo 2.5% group was given 2.5% sevoflurane before ischemia. Hemodynamic parameters of all groups were recorded through a thin, saline-filled latex balloon and a transducer. Coronary flows were also measured. All hearts were stained by triphenyl tetrazolium to measure infarct size. RESULTS: The sevoflurane administered groups showed higher left ventricular end systolic pressures and lower left ventricular end diastolic pressures than the control group after ischemia and reperfusion. The dP/dtMAX of the sevoflurane administration groups showed a more rapid recovery pattern after ischemia than the control. But no differences were found between the sevoflurane administered groups. Infarct sizes in the sevoflurane administered groups were smaller than those in the control group, and there were no significant differences between the sevoflurane administered groups. CONCLUSIONS: Sevoflurane (even below one MAC) administration before myocardial ischemia has a superb cardioprotective effects, i.e., rapid recovery of left ventricular fuctions, less stiffness development, and a reduced infarct size. There were no significant differences between the sevoflurane administered groups.

10.A clinical study of borderline malignant tumors of the ovary.

OBJECTIVE: The purpose of this study was to evaluate the clinical and histopathological features, and to find relationship between treatment modality and prognosis of borderline ovarian tumors. METHODS: Ninety-three cases of borderline ovarian tumors that had been treated were reviewed retrospectively. RESULTS: Histologic types of this study group composed of 67 cases of mucinous borderline tumor, 23 cases of serous borderline tumor, 2 cases of mixed type, and 1 case of Brenner tumor. The mean age was 41.6 years (range 14 to 83), and 41 (44.1%) patients were nullipara. The most common chief complaint was palpable mass (39.8%) and asymptomatic cases were presented in 23 patients (24.7%). The cases of elevated serum CA125 (< or =35 IU/ml) were 25.8% in mucinous type and 61.9% in serous type. Surgical staging was completed in 57 cases and other 36 cases were incompletely staged. 45 cases were managed by conservative surgery. There were 4 cases with recurrence, and all of them were related to conservative surgery. Chemotherapy was given to 40 cases. Mean follow-up period was 27.3 months. One patient expired during follow-up, and overall 5-year survival rate was 95.2%. CONCLUSION: Borderline ovarian tumors have a good prognosis. But recurrence rate was high in conservative surgery. In conclusion, careful preoperative evaluations, complete surgical exploration and long-term follow-up after treatment are needed.
