1.Effects of Health Literacy and Knowledge on Diabetic Self-care in the Elderly with DM Living Alone.

PURPOSE: The aim of this study was to evaluate the level of self-care adherence in the elderly with diabetes mellitus (DM) who have lived alone, and to investigate the association between health literacy, diabetic knowledge and self-care adherence. METHODS: Descriptive research using the cross-sectional approach was conducted. Data was collected by using a convenience sampling of 201 participants who aged sixty-five years old or more and have lived alone. Statistical analysis was conducted by using an independent t-test, one-way ANOVA, Scheffé test, Pearson correlation coefficient and stepwise multiple regression analysis. RESULTS: Mean scores of health literacy, diabetic knowledge and adherence of self-care were 4.41±3.54 points out of a possible 12, 6.22±2.55 points out of a possible 15, 58.56±17.28 points out of a possible 112, respectively. Adherence of self-care showed a statistical association with health literacy (r=.26, p<.001) and diabetic knowledge (r=.30, p<.001). Statistically significant factors in the regression model were monthly income, diabetic knowledge and exercise. CONCLUSION: The level of self-care adherence in the elderly with diabetes living alone was relatively low. Diabetic knowledge was an important factor to improve the level of self-care adherence for the elderly with diabetes living alone.

2.Atypical Proliferating Muinous Tumor arising in a Mature Cystic Teratoma.

The borderline tumor is not benign but has low malignant potential, which accounts for 10-15% of all ovarian tumors. The mucinous borderline tumors make up approximately 40% of all borderline tumors. About 5% of mature cystic teratomas of ovary include some with mucinous cystadenoma. The malignant transformation is occurred in about 1-3% of patients who have a mature cystic teratoma. There has been reported frequently about the malignant transformation of a mature cystic teratoma. After the first description of a patient with mucinous borderline tumor associated with a mature cystic teratoma in 1988, the borderline tumorous change of a mature cystic teratoma has been reported rarely. This report presents a patient with atypical proliferating mucinous tumor arising in a mature cystic teratoma.

3.Bone Forming Gene Therapy (Immune Animal Model in Ex Vivo Gene Therapy for Spinal Fusion with Type 5 Adenoviral Delivery of the LIM Mineralization Protein-1 cDNA).

STUDY DESIGN: In vivo study to determine the immune effects to adenoviral vector encoding LMP-1 cDNA in rabbit. OBJECTIVE: To quantify the immune effect of Ad5-LMP-1 in the rabbit during the therapeutic gene transfer. SUMMARY OF LITERATURE REVIEW: One of the major limitations in the use of adenoviral vector for gene therapy is the immune response and it made the poor transduction efficiency when re-administrated. Adenoviral antigen plus those derived from transgene expression in transduced cell contribute to cellular, humoral and non-specific immune response constitutes barriers to successful gene therapy. Therefore, the animal immune model will be mandatory to study the immune impact. MATERIALS AND METHOD: We i.v. injected Ad5-betaGal to total 24adult NZW rabbits; 1x108, 1x109, 1x1010, 1x1011v.p. to each 6 rabbits allowed them to develop immune response. Six non-immunized animals were used as control. Adenovirus antibodies were measured at 0, 4, 8, 16, 20 weeks. Group I. 6 control rabbit underwent spinal arthrodesis at 4 weeks (n=3) and 16 weeks (n=3) with 4 million cells and MOI of 4. Group II. 6 rabbit underwent spinal arthrodesis at 4 weeks after injection of 108 p.f.u virus (n=3) and 16 weeks (n=3). Group III. six 109 immunized rabbits, Group IV. six 1010 immunized rabbits, Group V. six 1011 immunized rabbits, underwent spinal arthrodesis at 4 and 16 weeks after injection. Total anti-Ad Ig and neutralizing antibody titer was measured on the 0. 4. 8, 16, 20 weeks after injection. RESULTS: Group I. All 6 non-immunized rabbits had solid spine fusions at 4 and 16 weeks. Group II. All 3immunized rabbits had not spine fusions at 4 weeks and all three had solid spine fusion at 16 weeks. Group III. None of them (n=6) immunized rabbits had spine fusion at 4 and 16weeks, but some bone formation was observed at 16 weeks. Group IV, V. None of them immunized rabbits had bone formation. The anti-Ad5 Ig and neutralizing Ab were detected and peaked at the 4weeks and significantly dropped off 16 weeks after injection. CONCLUSION: This experiment revealed that a small dose of adenovirus elicited an enough immune response that inhibited the bone formation. Because majority of human posses the Ab against adenovirus, it will be mandatory to overcome immune response in adenoviral vector gene therapy.

4.Anti-Tumor Effect of IDF-11774, an Inhibitor of Hypoxia-Inducible Factor-1, on Melanoma

Melanoma is one of the most aggressive skin cancers. Hypoxia contributes to the aggressiveness of melanoma by promoting cancer growth and metastasis. Upregulation of cyclin D1 can promote uncontrolled cell proliferation in melanoma, whereas stimulation of cytotoxic T cell activity can inhibit it. Epithelial mesenchymal transition (EMT) plays a critical role in melanoma metastasis. Hypoxia-inducible factor-1α (HIF-1α) is a main transcriptional mediator that regulates many genes related to hypoxia. CoCl2 is one of the most commonly used hypoxia-mimetic chemicals in cell culture. In this study, inhibitory effects of IDF-11774, an inhibitor of HIF-1α, on melanoma growth and metastasis were examined using cultured B16F10 mouse melanoma cells and nude mice transplanted with B16F10 melanoma cells in the presence or absence of CoCl2-induced hypoxia. IDF-11774 reduced HIF-1α upregulation and cell survival, but increased cytotoxicity of cultured melanoma cells under CoCl2-induced hypoxia. IDF-11774 also reduced tumor size and local invasion of B16F10 melanoma in nude mice along with HIF-1α downregulation. Expression levels of cyclin D1 in melanoma were increased by CoCl2 but decreased by IDF-11774. Apoptosis of melanoma cells and infiltration of cytotoxic T cells were increased in melanoma after treatment with IDF-11774. EMT was stimulated by CoCl2, but restored by IDF-11774. Overall, IDF-11774 inhibited the growth and metastasis of B16F10 melanoma via HIF-1α downregulation. The growth of B16F10 melanoma was inhibited by cyclin D1 downregulation and cytotoxic T cell stimulation. Metastasis of B16F10 melanoma was inhibited by EMT suppression.
