1.A Case of Lupus Miliaris Disseminatus Faciei.

A case of lupus miliaris desseminatus faciei, 23 year old male was reported. The multiple minute discrete yellowish brown superficial nodules, 2 to 3 mm in diameter, revealed apple-jelly color under diascopy without any subjective symptoms since June, 1971. The lesions affect lower eyelids, forehead, cheeks, malar region, upper lip, chin and nasolabial folds symmetrically. Histopathological findings revealed conglomerated individual tubercles with prominent central caseation necrosis. He was given 600mg of INH and 12 mg of PAS daily per os for 2 months. The eruptions were healed gradually with pitted scars.

2.A case of lipodystrophy centrifugalis abdominalis infantilis.

We report a case of lipodystrophia centrifugalis abdomina]is infantilis in a 2 year 5 month male. At age 7 months, his parents noticed a bluish macule in the right inguinal region, which gradually became depressed and increased in size over a year to involve the abdomen and flank bilaterally. Regional lymph nodes were enlarged. Biopsy showed absence of subcutaneous fat in the affected area.

3.A case of fixed sporotrichosis treated with itraconazole.

We report a case of fixed sporotrichosis in a 9-year-old boy, who showed;i well-cle fineci erythematous plaque on right periorbital area for 4 months. Histopathologically, chronic granulomatous inflammation with pseudoepitheliomatous hyperplasi was oh served. Fungal cultures grew out the typical moist wrinkled colonies. Therapy was initiated on with 100mg of itraconazole daily with almost complete resolution of the skin lesion 2 months after treatment. I3uring the 4 month follow up period, there was no recurrence.

4.A Case of Klippel-Trenaunay-Weber Syndrome with Congenital Heart Anomaly.

A 17 year old female with Klippel-Trenaunay-Weber Syndrome is presented. Of particular interest was presence of congenital heart anomaly. The nevus flarnmeus invoIved her entire right leg and foot with hypertrophy, elongation and partial ulecration. Elongation of the right leg Iead cornpensatory vertebral scoliosis (Fig.1,2). The cutaneous temperature of affected side was higher than the other side, and. was larger on somatometry and osteometry (Table 1). On auscultation, grade III systolic blowing murmur was heard on left upper sternal border with second heart sound splitting. Electrocardiograrn revealed complete right bundle branch block (Fig. 3). Chest X-ray revealed cardiomegaly, increase 3 hilar shadaws and rnild pulmonary congestion (Fig. 4). Above mentioned signs suggests strongly secundum type of atrial septal defect. Femoral angiography didnt show arteriovenous malformation (Fig. 5). Authors experienced this rare variant of Klippel-Trenaunzy-Weber Syndrome acco- mapanying with congenital heart anamaly, and report this with review of literatures.

5.One Case of Reactive Perforating Collagenosis.

Authers observed one case of typical reactive perforating collagenosis (RPC) which did not seem to have been described in the Korean literature. A 19 year old male patient complained of a skin eruption involving his face, upper and lower extremities, and trunk in June 1973. The skin eruption appeared at the age of 10 and consisted of discrete papules of which the earliest lesion was a pinhead-sized, skin-colord papule, but it became older, it increased in size and developed a small central area of umbilication containing keratinous material. As new papules continued to develop, the older lesions regressed and disappeared with residual scar. No subjective symptom was obtained except intermittent, mild itching sensation which probably due to a irritation. The Keobner's phenomenon was observed on the dorsum of hands. He has acne vulgaris on the face with many of the individual comedones showing development into RPC. He also had a chronic maxillary sinusitis which was seemed to aggravate the skin eruption. In follow up period (about 5 months), the sinusitis was cured with operation, but the skin eruption was not improved. The family history was negative. Histopathological studies revealed typical RPC, i.e., a cup-shaped epidermal depression of which center was lacked the epidermis was noted, and through it, necrobiotic connective tissue, degenerating inflammatory- cells and collagen bundles are extruded to form a plug. Diagnosis was confirmed by clinical appearance and typical histopathologic features. The patient did not responed to topical application of 0. 1% Retinoic acid solution and Vitamin A and D.

6.A Case of Epidermolysis Bullosa Dystrophica.

A case of Epidermolysis Bullosa Dystrophica is reported with review of pathogenesis with literatures. Epidermolysis Bulloaa Dystrophica is a rare genetic disease which is characterized by the formation of blister upon the minor trauma or friction to the skin. A 7-year old boy was referred to our dept. in Aug. 1976 because of a severe bullous eruption primarily on his hands and feet. On admission tense bullae were noted on the dorsa of hands and feet which is usually serous but may be hermorhagic tendency. Soon thereafter tbis bullous lesion tend to break down into painful shallow denudation which may leave depigmented scars and milia after healing. Family history was negative for skin disorders.

8.A Clinical Study of Subacute Thyoidits.

OBJECTIVES: Subacute thyroiditis is a nonsuppurative inflammation of thyroid gland and is probably caused by a cytopathic virus. Typical clinical symptoms and features of subacute thyroiditis vary widely during the course of illness. It has a clinical course, evolving from hyperthyroidism through a temporary hypothyroidism to recovery. However, the final outcome of this disease remains unpredictive in some patients. Permanet hypothyroidism occurs infrequently. METHODS: Thirty-three patients proven to have subacute thyroiditis at the Ewha Womans University Hospital from September 1993 to November 1995 were studied. We analyzed their clinical features, laboratory findings, and duration of recovery to cha- racterize the course of the disease. RESULTS: 1) Total 33 patients were studied: 31 patients were female and 2 patients were male. Their mean age was 42.6+/-8.3 years old. The peak months were August through October in this study. 2) Initial mean ESR was 73.0+/-35.2mm/hr, mean T3 was 217.3+/-73.9ng/dl, mean T4 was 15.2+/-8.5microgram/ dl, and TSH was 0.06+/-0.09microIU/ml. The positive rates of antithyroglobulin and anitmicrosomal antibodies were 31% and 6% respectively, and TSH receptor antibody was elevated in one patient. 3) In the thyroid scan, 91% showed both lobes nonvisualisation, and 9% showed one lobe nonvisualization. Radioactive iodine uptake(RAIU) at 24 hour was 2.4+/-3.3%. 4) With the predisolone therapy, 90% of patients completely recovered, 57% of these patients had no hypothyroid phase and remaining 33% of them had hypothyroid phase during course of the disease. Three of the patients had permanent hypothyroidim. 5) The average duration of recovery was 3.2+/-1.4 months and it has no correlation with initial thyroid hormone levels, antithyroid antibodies and duration of steroid administration. CONCLUSION: There was no historical, physical, laboratory findings that help us predict those patients likely to have an exacerbation of the disease.

9.A Clinical Study of Postpartum Autoimmute Thyroiditis

Background: Postpartum thyroiditis is a painless, destructive lymphocytic inflammation of the thyroid gland that occurs during the postpartum period and is associated with a high prevalence of serum thyroid autoantibodies. Clinical symptoms and laboratory fi#ndings of postpartum thyroiditis vary widely during the course of illness and the final outcome of this disease remains unpredictive in some patients. The purpose of this study is to investigate the clinical course and the predictors of the outcome of the disease. Methods: Thirty-eight patients proven to have postpartum thyroiditis at the Ewha Womans University Hospital were studied. We analyzed their clinical features, laboratory findings and duration of recovery to characterize the course of the disease. Results: Their mean age was 29.0±3.3 years and the time of diagnosis was 4.9±1.9 months after delivery. The prevailed months of delivery were November through January in this study. Twenty six patients were hypothyroid and 12 were thyrotoxic at initial thyroid function. The positive rates of antithyroglobulin and anitmicrosomal antibodies were 92%, 67% in the hypothyroid group and 75%, 67% in th thyrotoxic group respectively. TSH receptor antibodies were negative in all patients. In the hypothyroid group the titers of antimicrosomal antibodies were significantly higer than the thyrotoxic group. The titers of antimicrosomal antibodies were positively correlated with serum TSH and negatively correlated with serum T4. In all patients, the titers in thyroid function tests returned to the normal range without long-term hypothyroidism. Conclusion: The titers of antimicrosomal antibodies were significantly higher in the hypothyroid group than the thyrotoxic group. Thus the titers of antimicrosomal antibodies can help guide the physician in the care of patients with postpartum thyroiditis who will probably be hypothyroid. In this study, we were surprised that all patients became euthyroid without permanent hypothyroidism.

10.Exploring the Experience of Motivational Interviewing Training to Improve Communication Skills in Parents of Adolescent Children

PURPOSE: The study's aim was to describe the experience of motivational interviewing (MI) training for parents with adolescents. METHODS: Focus group interviews were carried out with 12 mothers in 3 groups who participated in the MI training. The study had a qualitative descriptive design, and qualitative content analysis was used. RESULTS: Data analysis was separated into 4 domains: acceptance, relational skills, self-reflection, and rebuilding relationships. Seven categories and 11 subcategories included (1) enhancing acceptance and empathy, (2) recognizing the importance of the MI spirit and applying core skills, (3) improving self-expression and self-control of negative emotions and behavior, (4) recognizing self-problems, and (5) true communication and recovery of trust. CONCLUSION: Through MI training, participants have experienced not only the enhancement of their communication skills but also personal modifications (in self-acceptance, self-reflection, and self-control) and positive interpersonal relationships (rebuilding relationships). In addition, participants have realized the importance of listening and reflection in such experiences. Reflection training has provided participants meaningful experiences. Our key insight from these findings is that skill is not most important in communication training. MI elements such as acceptance, collaboration, and a respectful attitude are more likely to be used than conventional communication skills.
