1.A Case of Cutaneous Metastasis from Pancreatic Adenocarcinoma.

Cutaneous metastases from internal carcinomas are relatively rare, especially from pancreatic carcinoma. Pancreatic carcinomas are usually adenocarcinomas which arise in the head of the gland, and are known to rapidly metastasize to the lymphatic system by permeation and embolization. We report a case of cutaneous metastasis from pancreatic adenocarcinoma. in 74-year-old male patient who have two pea sized, slight erythematous nodules on the lower abdomen and posterior side of the neck.

2.A Case of Cystic Teratoma on the Floor of the Mouth in Neonate.

Cystic teratoma on the floor of the mouth is infrequent disease and known as congenital origin. Teratoma has no sex distribution. Treatrnent of the cystic teratoma is completely surgical excision and recurrence is rare. The authors have experienced a case of cystic teratoma on the floor of the mouth in neonate. We reported this case with review of literatures.

3.Studies on the Renal Inulin Clearance and Inulin Space in Post-hemorrhagic Anemic Men.

Applying the constant infusion method for eight males, series of three or four clearance periods for each subject were followed by the measurement of Inulin space. The subjects had lower hematocrit values ranging between 20.4 and 43.0%. They gave frequent blood donations over several years and were in a state of chronic post-hemorrhagic anemia. Inulin in plasma and urine was treated with the resorcinol method. The following results were obtained. 1. Subjects with lower hematocrit value showed lower value of renal inulin clearance. Both of them correlated each other . 2. Subjects with the hematocrit value of less than 30% showed a renal clearance value of 89ml/ min/1.73 square meter. 3. Inulin space also had correlation with the hematocrit value. Inulin clearance was great with the subject whose inulin space had expanded.

4.Analysis of Serum Zinc and Copper Levels in Alopecia Areata.

BACKGROUND: As co-factors of metalloenzymes, zinc(Zn) and copper(Cu) have a considerable effect on nearly all the metabolism that takes place in organs of the body, including the skin. OBJECTIVE: The purpose of this study was to evaluate zinc and copper status in alopecia areata patients. METHODS: We measured serum levels of zinc and copper in 30 cases of alopecia areata patients and 10 normal individuals by atomic absorption spectrometry. Furthermore, we studied the significance of the ratio between serum levels of the two metals. RESULTS: The serum levels of zinc were significantly lower in alopecia areata patients than in healthy controls. The serum levels of copper were slightly lower in alopecia areata patients than in healthy controls. Ratios of Cu/Zn were slightly higher in alopecia areata patients than in healthy controls. But their differences were not statistically significant. CONCLUSION: We suggest that a serum Zn assay should be included in the chemical assessment of patients with alopecia areata.

5.A Case of Lichen Aureus.

Lichen aureus is a rare disorder classified as a pigmented purpuric dermatosis. This case report is of a 46-year-old female in whom a coin sized, gold tinted, erythematous maculopapule developed on the right lower quadrant of the abdomen l0 years ago which showed characteristic histological findings of lichen aureus consisting of lymphohistiocytic infiltration in the upper dermis and extravasation of red blood cells.

6.Long-term Results and Complications using Scleral-fixated Posterior Chamber Intraocular Lenses.

The scleral fixation of a posterior chamber intraocular lens is known to be a technique for managing cataract, aphakia, lens dislocation, pseudophakic bullous keratopathy with incomplete zonular or capsular support. We performed a retrospective review of longterm results and complications of 86 patients(103 eyes) who had a minimum of 12 months follow-up after scleral fixation of a posterior chamber intraocular lenses. At the last examination, mean postoperative corrected visual acuity is 0.7 +/- 0.28 and 62 eyes(57.4%) had a vision of 0.8 or better. Especially in aphakia, postoperative corrected visual acuity was significantly improved. The complications related to scleral fixation were exposed scleral fixation suture, glaucoma, cystoid macular edema, subluxation of intraocular lenses, retinal detachment, herpetic keratitis, vitreous hemorrhage and vitreous opacity. The other factors influenced on visual acuity were age-related macular degeneration, optic atrophy, diabetic retinopathy and clinical significant macular edema, and corneal opacity. Though these longterm results suggest that overall prognosis for visual acuity is quite good, careful assessment preoperatively and proper management are important.

7.Occult Hepatocellular Carcinoma Metastasized to Heart: A case report.

We have experienced a case of occult hepatocellular carcinoma metastasized to the heart in a 41 year old female. The hepatocellular carcinoma invaded the hepatic vein, grew in a snakelike fashion up the inferior vena cava and reached the right atrium. Ultrastural examination in this case confirmed the hepatocellular origin of the tumor.

8.Surgical Treatment of the Anterior Tibial Spine Fracture: Surgical Indication and Results

The management of fractures of the intercondylar eminence of the tibia is uncomplicated when the fracture has minimal displacement(type I) or when only anterior one-third or half of the eminence is elevated(type II)(Meyers and Mckeever 1959, 1970). The treatment of complete separation(type III) has been controversial. Many authors however recommended conservative treatment even for the completely separated fracture if the fracture fragment is not rotated. Two cases of non-union of the tibial spine fracture we experienced were; one in type IIl, and the other type II. They were treated conservatively by cast immobilization. One patient had severe anterolateral rotatory.instability and had lateral meniscal tear, and the other had transverse ligament impingement at the fracture gap which interfered the reduction of the fragment and also bony union. Through the clinical experience and the two listed non-union cases, we drafted a therapeutic plan for the avulsed tibial spine fractures. The authors recommend open reduction and internal fixation of the avulsion fracture of the tibial spine in following circumstance; 1) all of the type K complete separation injuries 2) tibial spine fracture with positive Lachman test and soft end point 3) fracture with associated ligament injury. We applied the above surgical indications for 13 cases. The authors reviewed 25 cases of the anterior tibial spine fracture patients treated at the Department of Orthopaedic Surgery, Catholic Unviersity Medical College during the period from October 1982 to August 1986 and the following results were obtained. 1. The cases were classified into 3 different categories according to the Meyers and Mckeever classification; Type I(7 cases, 29%), Type II(10 cases, 40%), Type III(8 cases, 31%). 2. Associated ligament injuries on the same knee were rupture of LCL for 7 cases(28%) and MCL for 6 cases(24%). 3. Twelve out of thirteen open reduction and internal fixation cases had excellentresult in minimum 6 months follow-up period.

9.The Causes of Metallic Failure and Loosening of MOSS Transpedicular Spinal Instrumentation

With the increasing use of pedicular system to fix the spine, many complications are being reported. Recently many systems are available to fix the spine. However, each system has own advantages and disadvantages. The causes of metallic failure of Modular Segmental Spinal (MOSS) instrumentation on 42 consecutive patients undergone in Kang-Nam St. Mary's hospital since 1989 were reviewed. The specific aim of this investigation was to assess causes of metallic failure and loosening of this system on various spinal disorders. Of these, 8 cases had metallic failure and loosening. Breakage of screw and rod developed in 6 cases and dislodgement of rod from screw in 2 cases. In case of degenerative spondylolisthesis (unstable phase) with stenosis, however, the complications were closely correlated to expansile decompressive laminectomy to widen the narrowed spinal canal and the instrumental distraction to gain normal intervertebral disc space at the operation. Bony union and back pain were not correlated to metallic failure and loosening. Therefore, the main causes of metallic failure and loosening were (1) preoperative instability undergone expansile decompressive laminectomy including total bilateral facetectomy that aggravated preoperative instability, and (2) forceful instrumental dis traction. In cases needed these requirements, combined anterior interbody fusion or posterior interbody fusion should be added, heavier rods and screws larger than 3.5mm, 4.0mm in diameter, respectively, should be used. In addition to postoperatively sufficient bed rest and immobilization using rigid braces should be recommended to reduce these complications.

10.Treatment of Symmetrical Thumb Polydactyly by Bilhaut-Cloquet Operation

Since 1985, 7 symmetrical thumb polydactylies were treated with Bilhaut-Cloquet operation (V-shaped wedge out). As for Wassel's classification, two cases were type I (duplicated distal phalanx but not epiphysis) and two type IV (duplicated proximal and distal phalanx), and three type II (duplicated distal phalanx and epiphysis). Preoperatively, thumb polydactylies must be symetric and cut articular surfaces be accurately approximated. As for Wassel's type IV polydactyly, preoperative angiogram was done for detection of any vascular anomalies and tried not to damage the anomalous digital arteries of thumb. There was no postoperative complications. All patients and their parents were much satisfied with the postoperative results, but somewhat limited thumb motions were remained. In type I (2 cases) the average IP joint range of motion was 39°, in type II (3 cases) the average IP joint range of motion was 22°, and in type IV (2 cases) the average range of motion of IP joint was 21° and MP joint was 26°.
