1.A Case of Woringer-Kolopp Disease.

Woringer-Kolopp disease, so called pagetoid reticulosis, is most likely represent a chronic localized from of reticulosis. A 24-year old male patient visited with asymptomatic slowly enlarging, scaly single patch on his left thigh for 10 years duration. Histopathologic studies were taken with H-E stain, D-PAS, transmission electronmicroscopy and monoclonal antibody(OK6) tissue stain. Infiltration of atypical cells showed in the lower portion of epidermis like pagetoid patterned, none in dermis. It seems to be that the cell in this case probably originated from the stimuIated T-lymphocyte.

2.A Case of Lichen Aureus.

Lichen aureus is a rare disorder classified as a pigmented purpuric dermatosis. This case report is of a 46-year-old female in whom a coin sized, gold tinted, erythematous maculopapule developed on the right lower quadrant of the abdomen l0 years ago which showed characteristic histological findings of lichen aureus consisting of lymphohistiocytic infiltration in the upper dermis and extravasation of red blood cells.

3.A case of nevus comedonicus-treated by carbon dioxide laser.

At twenty two year old female with nevus comedonicus and an increased tendency to form keloids is described. Although example of successful treatment of nevus comedonicus are readily found in the literature none propose reasonable therapeutic models for keloid formers. As our patient clearly demonstrated this tendency. We initated a t,rial with the carbon dioxide laser due to it's ability to cause less thermal damage. Subsequent to completion of this t,rial, the lesion has imprcived with the acceptable cosmetic results and less comedogenic activity.

4.Surgical Treatment of the Acute Anterior Cruciate Ligament Injuries

The anterior cruciate ligament(ACL) is one of the major stabilizers of knee motion. The ACL does not heal once torn, unlike many other biological tissues. Its absence can lead to severe joint instability which can lead to disruptions of secondary restraints around the knee and then progress to degenerative joint disease. Thus many authors advocate surgical intervention for a torn ACL in order to restore stability to the joint. Yet, there is still controversy as to best treatment for the ACL injuries. It is desirable to be considered for the patients age, the severity of the injury, and the future activity level of the patient to decide treatment. We analyzed thirty ACL acute injuries of twenty seven patients who had an ACL injury and treated surgically between Apr. 1986 and Dec. 1991 at St. Pauls Hospital. The results were as follows: 1. There were twenty five males and two females. The mean age was 33.4 years ranged, 15 to 52 years. 2. The anterior cruciate ligaments were torn at proximal portion in twenty two cases, at midportion and distal portion in four, respectively. 3. Two knees had isolated ACL injuries and twenty eight had combined injuries. 4. Twenty one out of twenty six cases treated with primary repair, and three out of four cases treated with reconstruction had good-excellent results.

6.Intestinal Total IgA and Rotavirus - specific IgA Detection in ICR Mouse Model.

In a murine model system, enhancement of the total IgA and rotavirus-specific IgA of suckling mice was measured by ELISA with the intestinal fluid following oral administration of murine rotavirus EC (EDIM-Cambridge) strain. In the EC strain-administered group, the geometric mean titers (GMT) of total IgAs were 512 and 91 at 1 and 2 week postinfection, respectively. On the other hand, the GMTs of the rotavirus-specific IgAs were 108 and 3 at the same periods, respectively. Thus increase in the total IgAs was 64 folds and that in the rotavirus-specific IgAs was 43 folds compared with the negative control group. As the maximal titers of both the total and rotavirus-specific IgAs were observed at 1 week decreasing until 2 weeks after infection, it is evident that the GMT of the total IgA implies that of rotavirus-specific IgA. In our ELISA system, whose specificity was verified by Western blot analysis, the total IgA in the administered group was determined to be 40-400 ng per 1 ml of the intestinal fluid. Therefore it is concluded that determination of the rotavirus-specific IgA in murine models can be a sensitive indication of rotavirus infection, and will be another promising tool in viral challenge experiments in vaccine development.

7.The Neurologic Manifestations of Systemic Lupus Erythematosus.

Systemrc lupus erythematosus (SLE) is an autoimmune, inflammatory, chronic disorder characterized by multiorgan involvement including the nervous system. The neurologic manifestations of SLE vary in clinical signs and clinical course. We recently reviewed medical records of 198 patients with SLE who had been admitted to Severance hospital between 1980 and 1990 to see neurological features with the relationship of non-neurological features of SLE and prognosis. Among 198 cases with SLE sixty-seven patients (33.8%) had significant neurologic and/or psychiatric manifestations dunng the course of their disease. CNS lupus was found in 48 cases and symptoms were seizure, mental deterioration, psychiatric manifestations and focal neurologic deficit in decreasing order of frequency. Myelopathy was noted in 4, peripheral neuropathy in 5, and rline patients developed myopathy. Eight had typical meningitis ;four of these patients had aseptic meningitis, three had tuberculous meningitis, and one had bacterial meningitis. In half of the patients with CNS lupus, the neurologic symptoms occurred during the first one year of their illness. Other major organs such as the kidney, heart, lung, skin and joints were more commonly affected in CNS lupus. The prognosis of CNS lupus is relatively poor, especially in patients with seizure which is followed by altered mentality.

9.Median Somatosensory Evoked Potentials in Cerebrovascular Disease.

Median nerve somatosensory evoked potentials (SSEPs) were studied in 98 cases of cerebrovascular disease(CVD), and compared with the SSEPs of 100 healthy subjects. According to the clinical and radiological (brain CT and/or MRI) findings, the patients were divided into four groups; 21 cases with brainstem lesion, 33 with thalamic lesion, 32 with subcortical lesion, and 12 with cortical lesion. Comparing with the results of SSEPs obtained from 100 cases of normal subjects, abnorrnal SSEPs were found 7 of 21 patients in brainstem lesion, 17 of 33 in thalamic lesion, 15 of 32 in subcortical lesion, and 9 of 12 in cortical lesion. There are no statistical significance betueen the lesion site and rate of abnormal SSEPs findings. The 48 patients of CVD exihibited abnormal findings with the patterns of(1) absence ol conical responses in 32 cases(32.7%), (2)decreasing the ratio of amplitude between non-affected and affected cortex in 21 cases (21.4%), (3) prolonged inter-peak latency (IPL) between P/Nl3-N19 in 10 cases(10.2%), (4)distorted shapes in 8 cases(8.7%), and (5) low amplitudes of affected cortical potentials in 6 cases (61%). There were 9 cases who revealed abnormally high amplitude of cortical potentials over the non-affected hemisphere. These findings are suggesting that alteration of amplitude are more prominent than prolonged IPL between P/Nl3-Nl9 in SSEPs of CVD. The changes of SSEPs look more related to the deficit position and vibration sense rather than the pin-prick.

10.Repetitive Nerve Stimulation Test in Myotonic Patients.

In 17 myotonic cases (16 of myotonic muscular dystrophy and 1 of congenital myotonia), 13 cases showed significant decremental responses on RNS study. The decremental responses had the tendency of more prominence on faster stimulating rate, and in the proximal limb muscles. This result suggests that the RNS test is useful but not specific for the disorder of neuromuscular junction.
