3.Trends and Meta-Analysis of Research on the Operation of Programs for Bereaved Families in South Korea

Purpose:
This study aimed to analyze interventions for bereaved families and evaluate their effectiveness, with the ultimate goal of supporting evidence-based nursing for bereaved families.
Methods:
Research trends were identified based on a search of domestic databases from January 2000 to December 2022, and a meta-analysis was conducted on interventions for bereaved families. Forty-five papers were selected, and information was extracted on participants, research design, and interventions. A meta-analysis of seven papers was performed, and the effect size was calculated.
Results:
Fourteen papers dealt with interventions for middle-aged women who had lost their spouses, 20 used qualitative research methods, and 20 were on art therapy programs. Thirty studies had fewer than 10 participants, and most interventions had 60~120 minutes per session and 9~16 sessions in total.There were seven randomized controlled trials, and all studies included in the quality evaluation showed a low risk of bias. Four papers measured grief as an outcome, and the effect size was -1.9577 (95% CI: -2.9206 to -0.9947), indicating that the treatment significantlydecreased grief (P<0.001). Six papers measured depression as an outcome, and the effectsize was -1.6775 (95% CI: -2.1835 to -1.1716), showing that the treatment significantly decreased depression (P<0.001).
Conclusion
Intervention programs for bereaved families were shown to be effective in relieving grief and depression. However, programs should be developed that target middle-aged men who have lost their spouses and children who have lost their parents. Randomized controlled trials should also be conducted on interventions to reduce grief and depression.

Upper gastrointestinal tract angiodysplasia is rare, but it is an important cause of upper gastrointestinal bleeding. We experienced a case of hemorrhagic angiodysplasia of duodenal bulb in a patient under chronic hemodialysis due to chronic renal failure. Repeated upper GI endoscopy revealed an angiodysplasia at the posterior wall of the duodenal bulb. It was successfully treated by bipolar electrocoagulation. We report a case of angiodysplasia in chronic renal failure patient with the review of the literatures.

5.Retroperitoneal Hematoma Caused by Ruptured Pancreaticoduodenal Artery Aneurysm.

A ruptured aneurysm of the pancreaticoduodenal arteries is an emergency condition, with a high mortality rate. Therefore, early diagnosis and adequate management are needed. Recently, treatment with a transcatheter arterial embolization has decreased the mortality rate. A 68-year-old man presented with persistent abdominal pain and dyspnea. Contrast abdominal computed tomography revealed a large retroperitoneal hematoma, with an actively bleeding focus. An urgent celiac and superior pancreaticoduodenal arteriogram showed a contrast extravasation, about 15 X 8 mm in size, with a saccular aneurysm of the distal anterior superior pancreaticoduodenal artery; therefore transcatheter arterial embolization was performed. The patient's condition improved without complication after the embolization.

6.Hepatitis C Viral Markers in the Recipients Before and After Kidney Transplantation.

BACKGROUND: Hepatitis C virus (HCV) has been identified as one of the most frequent causative agent of posttransplant non-A, non-B hepatitis, but the significance of anti-HCV antibodies after transplantation remains controversial. In the present study, we performed anti-HCV and HCV-RNA RT-PCR (HCV PCR) in the kidney recipients to assess the incidence and the outcome of HCV markers after transplantation. MATERIALS AND METHODS: In randomly selected 95 patients' paired sera (before and after transplant samples, respectively), we performed anti-HCV test by Abbott HCV EIA 3.0. We also performed HCV PCR in 80 paired sera of the 95 patients. We evaluated the incidence of anti-HCV and HCV PCR and compared the results in the kidney recipients between anti-HCV test and HCV PCR before and after transplantation. RESULTS: In the recipients' sera before transplantation, 16 (16.8%) among 95 sera were anti-HCV positive and 27 (33.8%) among 80 sera were HCV RNA positive. Among the 80 pretransplant sera performed HCV PCR, 23 (28.8%) discordant results were noted between anti-HCV and HCV PCR, and 17 sera among these were HCV PCR positive and anti-HCV negative. A seroconversion from anti-HCV negative to positive after transplantation was observed in 10 sera, but a conversion from positive to negative was not observed. In case of HCV PCR, a conversion from negative to positive was observed in 21 paired sera, and positive to negative in 13 paired sera. CONCLUSIONS: Our study indicated that disapperance of anti-HCV antibodies after transplantation in kidney recipients was rare. The overall concordance rates between anti-HCV test and HCV PCR in the recipients before and after renal transplantation were lower than other non-transplanted groups reported, and it may be due to the immunosuppressive therapy or the changes in immunoregulatory function of the patients. Further study such as follow-up liver function tests or liver biopsy will be needed for accurate decision about posttransplant HCV status of kidney recipients.

7.Functional Brain Mapping Using H2 15O Positron Emission Tomography ( II ): Mapping of Human Working Memory.

PURPOSE: To localize and compare the neural basis of verbal and visual human working memory, we erforrned functional activation study using H2O PET. MATERIALS AND METHODS: Repeated H213O PET scans with one control and three different activation tasks were performed on six right-handed narmal volunteers. Each activation task was composed of 13 matching trials. On each trial, four targets, a fixation dot and a probe were presented sequentially and subjects task was to press a response button to indicate whether or nat the probe was one of the previous targets. Short meaningful Korean words, simple drawings and monochromic pictures of human faces were used as matching objects for verbal or visual memory. All the images were spatially normalized and the differences between control and activation states were statistically analyzed using SPM96. RESULTS: Statistical analysis of verbal memory activation with short words showed activation in the left Broca's area, premotor cortex, cerebellum and right cingulate gyrus. In verbal memory with simple drawings, activation was shown in the larger regions including where activated with short words and left superior temporal cortex, basal ganglia, thalamus, prefrontal cortex, anterior portion of right superior temporal gyrus and right infero-lateral frontal cortex. On the other hand, the visual memory task activated predominantly right-sided structures, especially inferior frontal cortex, supplementary motor cortex and superior parietal cortex. CONCLUSION: The results are :consistent with the hypothesis of the laterality and dissociation of the verbal and visual workmg memory from the invasive electrophysiological studies and emphasize the pivotal role of frontal cortex and cingulate gyrus in working memory system.

STATEMENT OF PROBLEM: Proper finishing and polishing enhance both the esthetics and the longevity of restored teeth. Blade finishing technique would be suited for smoothing and finishing. Evaluation of this technique are necessary. PURPOSE: The purpose of this study was to evaluate the blade finishing and polishing procedures on the surface profile and roughness of ormocer-based composite resin and nanohybrid composite resin. MATERIAL AND METHODS: The material included a ormocer-based composite resin (Admira(R) & Admira(R) Flow); a nanohybrid composite resin (Grandio(R) & Grandio(R) Flow). One hundred forty specimens of each group were prepared using a mylar strip and randomly divied into blade finishing and rubber polishing groups (n=10). The average surface roughness (Ra) in micrometers was measured and the surface profile was examined by scanning electron microscopy (SEM) (Magnification X 200). The data were analyzed by Mann-Whitney Test at 0.05 significance level. CONCLUSION: The results of this study indicated that the mylar strip produced the smoothest surface on all materials and among the finishing-polishing methods was not significanct difference (P>0.05). Ormocer-based flowable composite resin performedthe lowest variability in initial surface roughness among the tested materials.

9.The Role of ELISA in Prediction of the Prognosis after Treatment of Neurocysticercosis.

The diagnosis of neurocysticercosis is relatively easy with brain computed tomo-grarhy(CT) and/or magnetic resonance imaging(MRI) and ELISA test of serum and cerebrospinal fluid(CSF). The clinical manifestation of the disease are varied and correlated with the infection sites. The use of praziquantel and/or surgical excision are effective in treatment of the disease. The effect of treatment has been evaluated with imaging studies and changes of clinical manifestation. The authors reviewed 97 neurocysticercosis patients to evaluate the prognostic predictability of serum and CSF ELISA titers after treatment . We classified the patients into two groups. Rebound group was patients which showed higher ELISA titers after praziquantel medication than pretreatment. The responses to treatment were evaluated with the changes on brain CT or MRI. The Initial ELISA titers were lower in rebound group than those of nonrebound group, but three month after treatment, rebound group showed higher titers. The changes of ELISA titers in rebound group were greater in CSF than those in serum. The reduction of cyst on brain CT or MRI was more remarkable in rebound group than in nonrebound group and the prognosis of rebound group was better. Changes of ELISA titers was valuable in predicting the effect of treatment and prognosis.

10.The Outcome of Uvulopalatal Flap in Obstructive Sleep Apnea Syndrome Patients.

BACKGROUND AND OBJECTIVES: Uvulopalatal flap (UPF), which is a modification of UPPP, is established in the treatment of snoring and obstructive sleep apnea syndrome (OSAS) with reduced velopharyngeal incompetence (VPI) risk. This study was performed to evaluate the subjective outcome of UPF among patients with simple snoring or obstructive sleep apnea. MATERIALS AND METHODS: Twenty patients with OSAS were examined (sixteen male, four female) in a prospective manner. Subjects who underwent UPF have completed pre or post operative (after 3 months) polysomnographic (PSG) studies. The parameters of PSG were apnea index (AI), respiratory disturbance index (RDI), mean SaO2, and lowest SaO2. Questionnaires composed of Eworth sleepiness scale (ESS) and snoring symptoms inventory (SSI) were completed by patients and bed partner before and after the UPF. Statistical analysis was performed using the Wilcoxon signed rank test. RESULTS: Subjective questionnaires indicated statistically significant improvements (SSI ; from 60.9 to 37, ESS ; from 11.6 to 7.4). The AI decreased from 18.6 to 9.3, RDI decreased from 29.8 to 16.6 (p<0.05). CONCLUSION: UPF seems to be more effective in mild OSAS than in moderate and severe OSAS. Long term follow up studies with more patients is deemed necessary.
