3.Personality characteristics, depression and anxiety for patients with essential hypertension.

BACKGROUND: Majority of hypertension is essential in type, so its clear cue usually is not found, and the patients are tend to be persisted of psychopathy such as emotional tension, depression, anger, anxiety and psychologic conflict, also not to be adapted to stressful event. So we decide to survey the personality characteristics of essential hypertensive patients and factors of psychopathology. METHODS: Patient group-50 persons(male 23, female 27) are selected in the course of antihypertensive medication, being in or outpatient treatment at the dept. of F.M. or I.M. in Kae-jung, Dae-sung or Jung-ang Hospital from August 1, 1995 to July 31, 1996. Control group 52 persons(male 25, female 27) are selected. Exclusision criteria are any clinical disease hystory, over 140/90mmHg of BP, under 20-year old age or no cooperation. We assessment of the MMPI, BDI and STAI results about both group. RESULTS: Comparing hypertensive group with normal control group, significantly higher F, Hs, D, Hy, Pd, Pt and Sc scales of MMPI and BDI(p<0.01), and anxiety scale of STAI(p<0.05). There were no significant differences in all scales comparing between both sexs in the hypertensive and control groups, except comparing hypertensive male with hypertensive female in Pt scale(p<0.05) and control female with control male in Hy scale(p<0.05). CONCLUSIONS: Comparing essential hypertensive group with normal control group, significant higher score was found in scales of Hypochondriasis, Depression, Hysteria, Psychopathic deviate, Psychasthenia, Schizophrenia and anxiety. And in the treatment of hypertensive patient with antihypertensive drugs, appropriate additive psychotherapy appears useful in the case of revealing psychopathy.

4.Surgery Alone or Postoperative Adjuvant Radiotherapy in Rectal Cancer: With Respect to Survival, Pelvic Control, Prognostic Factor.

PURPOSE: To find out the role of postoperative adjuvant radiotherapy in the treatment of rectal cancer by comparing survival, pelvic control, complication rate, and any prognostic factor between surgery alone and postoperative radiotherapy group. MATERIALS AND METHODS: From Feb. 1982 to Dec. 1996 total 212 patients were treated by radical surgery with or without postoperative radiotherapy due to rectal carcinoma of modified Astler-Coller stage B2-C3. Of them, 18 patients had incomplete radiotherapy and so the remaining 194 patients were the database analyzed in this study. One hundred four patients received postoperative radiotherapy and the other 90 patients had surgery only. Radiotherapy was performed in the range of 39.6-55.8 Gy (mean: 49.9 Gy) to the whole pelvis and if necessary, tumor bed was boosted by 5.4-10 Gy. Both survival and pelvic control rates were calculated by Kaplan-Meier method and their statistical significance was tested by Log-rank test. Multivariate analysis was performed by Cox proportional hazards model. RESULTS: 5-year actuarial survival rate (5YSR) and 5-year disease-free survival rate (5YDFSR) of entire patients were 53% and 49%, respectively. 5YSRs of surgery alone group and adjuvant radiotherapy group were 63% vs 45%, respectively ( p=0.03). This difference is thought to reflect uneven distribution of stages between two treatment groups ( p<0.05 by x2-test) with more advanced disease patients in adjuvant radiotherapy group. 5YSRs of surgery alone vs adjuvant radiotherapy group in MAC B2+3, C1, C2+3 were 68% vs 55% ( p=0.09), 100% vs 100%, 40% vs 33% ( p=0.71), respectively. 5YDFSRs of surgery alone vs adjuvant radiotherapy group in above three stages were 65% vs 49% ( p=0.14), 100% vs 100%, 33% vs 31% ( p=0.46), respectively. 5-year pelvic control rate (5YPCR) of entire patients was 72.5%. 5YPCRs of surgery alone and adjuvant radiotherapy group were 71% vs 74%, respectively (p=0.41). 5YPCRs of surgery alone vs adjuvant radiotherapy group in B2+3, C1, C2+3 were 79% vs 75% ( p= 0.88), 100% vs 100%, 44% vs 68% ( p=0.01), respectively. Multivariate analysis showed that only stage was significant factor affecting overall and disease-free survival in entire patients and also in both treatment groups. In view of pelvic control, stage and operation type were significant in entire patients and only stage in surgery alone group but in adjuvant radiotherapy group, operation type instead of stage was the only significant factor in multivariate analysis as a negative prognostic factor in abdominoperineal resection cases. CONCLUSION: Our retrospective study showed that postoperative adjuvant radiotherapy could improve the pelvic control in MAC C2+3 group. To improve both pelvic control and survival in all patients with MAC B2 or more, other treatment modality such as concurrent continuous infusion of 5-FU, which is the most standard agent, along with radiotherapy should be considered.

5.A Case of Marjllin's Ulcer.

Marjolin is ulcer may be defined briefly as a cancer arising in a bum scar. A 41-year-old female had a Marjolin is ulcer(12x10x3cm) on her right lower leg. This ulcerated tumor has been present for two and half months and the induction period sinnc her bum was 29 years. The pathologic diagn.osis was squamous cell carcinoma, grade 2. Roentgenograms of her right lower leg revealed several soft tissue masses densities which, in part, horizontally connected with the periosteum of the tibia. So she was treated by above-knee amputation with dissection of several, unenlarged inguinal nodes which showed no evidence of metastases. She developed large, single, fixed metastatic node in right inguinal area one month after amputation and dissection was done.

6.Treatment of Uncomplicated Male Gonococcal Urethritis with Ofloxacin.

Ofloxacin, one of the new quinolone derivatives, is found to be highly effective against both PPNG and non-PPNG urethritis. The objective of this trial was to establish the efficacy of two ofloxacin regimens in the treatment of uncomplicated male gonococcal urethritis. A total of 138 patients with uncomplicated gonococcal urethritis seen at the VD Clinic, Choong-Ku Public Health Center, in Seoul in the period of March Jun 1988, were subjected to this study. The patients were divided randomly into two groups and assigned to one of two treatment regimens of afloxacin, 400mg, PO and 200mg, PO, twice a day for 2 days. No failure case was seen in both groups. Both of these ofloxacin regimens were found to be highly effective and safe in the treatment of gonococcal urethrithis. It is suggested that, because of high rate of PPNGs among circulating N. gonorrhoeae, they can be recommended as the first line treatment for gonorrhea in Korea.

7.A Case Report of Osler-Rendu-Weber Syndrome.

The Osler-Rendu-Weber syndrome is characterized by multiple telangiectasic lesions usually involving the mucous membranes, face and distal extremities. It is a congenital malformation inherited as an autosomal dominant trait and the lesions usually appear during adulthood. The major symptoms are recurrent epistaxis and gastrointestinal bleeding, but they may cause intracranial hemorrhage at the white matter of the brain stem, cerebellum and diencephalon. We report a case of typical autosomal dominant trait Osler-Rendu-Weber syndrome associated with intracranial hemorrhage at the right basal ganglia.

8.Electron Microscopic Study on Differentiation of Tracheal Epithelium in Human Fetus.

The human trachea is normally lined by a pseudostratified ciliated columnar epithelium where ciliated, goblet, intermediate and basal cells are mainly represented. However the fetal tracheal epithelium was found to be composed of ciliated, non-ciliated and basal cells. The present study was designed to characterize the development of ciliated cells in the fetal trachea at mid (19 weeks) and last (32 weeks) trimester of gestation. At 19 weeks of gestation, the tracheal epithelium, 35 µm in height, was composed of surface, intermediate and basal layers. The surface cells were subdivided into ciliated, immature ciliated, non-ciliated, granule containing and goblet cells. The ciliated cells covered approximately half of the luminal surface area. The immature ciliated cells contained basal bodies, but the apical membrane was not invested with cilia. The granule containing cells contained numerous dense granules, 0.3-0.7 µm in diameter, in the apical cytoplasm. The goblet cells contained less electron dense granules, 1-2 µm in diameter, in the apical cytoplasm. The cells in intermediate layer were relatively undifferentiated and contained poorly developed organelles. Submucosal gland were well differentiated and were composed of the mucous and serous cells. At 32 weeks of gestation, the tracheal epithelium, 50µm in height, was also composed of surface, intermediate and basal layer. The surface cells were composed of ciliated, non-ciliated and goblet cells. The ciliated cells, dominant type of surface cells, were subdivided into mitochondria-rich cells (type I) and smooth endoplasmic reticulum-rich cells (type II). The non-ciliated cell were of three subtypes : mitochondria-rich cells (type A), glycogen and microfilament-containing cells (type B) and cells with bulging apical surface into the lumen (type C). Small granule containing cell appeared in the basal layer. These cells contained clear vesicles, 50 ㎚m in diameter, and dense granules, 100-300 ㎚m in diameter. Submucosal gland were well developed and consisting of mucous, serous and myoepithelial cells. These results indicate that the cell populations of the tracheal epithelium at late stage of pregnancy have essential features similar to those of adult. and show that the different steps of ciliogenesis could be identified.

9.Immunohistochemical Study on Cytokeratin Expression in Tracheal Epithelium of Human Fetus.

Trachea is lined by a pseudostratified epithelium which usually expresses a complex mixture of stratified as well as simple epithelial-type cytokeratins. In the present work, the cytokeratin expressions was studied immunohistochemically in the tracheal epithelium and gland of human fetus at 14, 26 and 32 weeks of gestation. The primary antibodies used were CK7, 8, 10, 14, 18, AE8, 5D3, MNFl16 and AE3. In PAS-hematoxylin stain, the tracheal eithelium was composed of pseudostratified ciliated columnar type and consisted of surface, intermediate and basal layers regardless of gestational ages. The PAS positive cells, however, were decreased in number in proportion to gestational ages. The tracheal gland was not fully differentiated at 14 weeks of gestation, and had well differentiated secretory portions consisting mucous and serous cells at 26 and 32 weeks of gestation. The mucous cells and luminal border of the duct were positive for PAS stain. The tracheal eithelium showed different immunoreactivity between cartilageous and membranous portions. In general, CK7 and 5D3 were expressed in surface cells, AE8 in intermediate cells, and MNFl16 and AE3 in the cells of all layers. At 14 weeks of gestation, the tracheal epithelium immunoreacted for CK7, AE8, 5D3, MNFl16 and AE3. The premordium of tracheal gland was positive for 5D3, MNFl16 and AE3. The tracheal epithelium at 26 and 32 weeks of gestation showed same staining properties to those at 14 weeks of gestation. The duct cells at 26 weeks of gestation were immunoreactive for CK7, 8, 14, 18, AE8, 5D3, MNFl16 and AE3, and those at 32 weeks of gestation were immunoreactive for CK7, 14, 5D3, MNFl16 and AE3. The acinar cells at 26 and 32 weeks of gestation were positively stained for CK7, 8, 18, 5D3, MNFl16 and AE3. These results suggest that CK7 and 5D3 may serve as useful markers for mature cilated cells, AE8 (CKl3) for immature ciliated cells, and CKl4 for duct cells in tracheal epithelium and gland.

10.Immunohistochemical Study on Cfokeratin Expression in Lingual Epithelium of Human Fetus.

The human oral mucosa has noncornified lining epithelium, cornified masticatory epithelium, and complex epithelium. The epithelium of human tongue shows diverse morphological variations from one site to another, and conflicting reports exist in the literature concerning the type of lingual epithelium. Cytokeratin[CK] have been shown to characterize different type of epithelia. In the present study to clarify intermediate filament patterns of tongue mucosa and lingual gland in human fetus from second trimester of pregnancy and adult, cytokeratin expression was investigated immunohistochemically using antibodies for cytokeratins in the dorsal surface of tongue, taste bud, and lingual gland, and comparison with the expression between fetus and adult was made. The epithelium of the fetal lingual papillae consisted of 4 to 6 layered stratified cells, and that of the inferior surface of tongue consisted of 6-8 layered stratified cells in PAS-hematoxylin stain. The lingual gland was well differentiated and the alveolus was strongly positive to PAS reaction. The dorsal surface of the adult tongue was composed of lingual papillae. The papillae was covered by keratinized stratified squamous epithelium and interpapillary area by nokeratinized epithelium in hematoxylin-eosin stain. The taste bud was present in the fungiform papilla. The lingual gland appeared among the muscle layers near the inferior surtace and was composed of mucous and serous cells. By immunohistochemical stain, the epithelium of the lingual papillae exhibited various staining-intensities for cytokeratin antibodies, and showed same staining patterns bosh epithelium in tip of papilla and interpapillary area in fetus. However, the dorsal epithelium of the adult tongue showed different staining patterns between tip of the papilla and interpapillary area. In fetal lingual papilla CK7 and CK8 were expressed in superficial cells, AE8 in intermediate and superficial cells, CKl4 in basal cells. MNFI116 and AE3 showed a strong reaction in basal and suprabasal cells. The epithelium of the inferior lingual surface reacted positively with AE8 and CK14, MNF116, and AE3. CK7 and CK8, however, were not reacted. In adult lingual papilla CK10 was expressed in superficial cells of the tip of the papilla, AE8 in suprabasal cells of interpapillary epithelium, CKl4 in basal cells of papillary and interpapillary epithelium, CKl9 in superficial cells of interpapillary epithelium, MNFI116 and AE3 in suprabasal cells of papillary and interpapillary epithelium. However, CK7, CK8, CK18, and 5D3 were not expressed in the epithelium of the dorsal tongue. The cells of taste bud in fetus showed positive reactions for CK7, CK8, MNF116, and AE3, but negative reactions with CK10 and AE8. The cells of taste bud in adult were stained with CK7, CK8, CK18, and MNF116, but not stained with CK10 and AE8. In lingual gland of fetus, CK7, CK8, CKl8, 5D3, MNF116, and AE3 were expressed in alveolar cells. Only CK10 gave a negative staining in ductal cells. The mucous cells of the adult lingual gland were reacted with CK7, CK10, CK18, CK19, and MNF116, and the serous cell with CK7, CK19, and MNF116. The ductal cells of the adult lingual gland were stained with CK7, AE8, CK18, CKl9, 5D3, MNF116, and AE3. CK14 was expressed in the cells of intralubular ductule, not in the ductal cells. By electron microscopy, the epithelia of both dorsal and inferior lingual surfaces in fetus consisted of nonkeratinized stratified squamous epithelium. A cell with clear cytoplasm and some dense granules was noted among the basal cells. These results indicate that the epithelium of fetal lingual papillae is non-keratinized type and the epithelium of the papillary tip is keratinized type and interpapillary epithelium is nonkeratinized type in adult, and suggest that the superficial cell containing cytokeratins 7 and 8 in dorsal lingual epithelium of fetus has a similar role to the periderm of fatal skin.
