2.A clinical and mycological study of superficial fungal diseases(vii).

BACKGROUND: Superficial fungal infections are common diseases acid the prevalence of superficial fungal diseases are not static but change under the influence of various forces such as climate, migration of peoples and development in prophylaxis and therapy. OBJECTIVE: To investigate the present status of superficial dermatiphytoses and some changes on the frequency of the diseases, sex distributions and varieties of the casiative organisms. MEHTODS: The clinical and rnycological aspects of superficial fungal diseases of 3,222 patients from July 1989 to June 1992 were analyzed. Results : 1) The incidence of superficial fungal diseases was 25.4% of total out-patients. 2) The superficial fungal diseases showed a higt incidence in the third(26.4%) and the fourth decade(24.3%). 3) The ratio of male to female was 2.11:1(2,188:1,034). 4) The monthly prevalence of super ficial fungal diseases was high inhuly(17.8%), August(15.2%) and June(1.7%). 5) The incidence of superficial fungal diseases in order of frequency were tinea pedis ,28.1%, tinea cruris(19.5%), onychomycosis(15.6%), tinen orporis(11.5%), tinea manus(7.8%), tinea versicolar(7.8%), candidiasis(7.4%) and tinea capitis(2.3%), 6) Coexisting fungal infections were found in 644 patients(19.9%) and the case of tinea pedis with onychomycosis was the most common. 7) The positive rate of KOH scraping was 54.0% and the positive rate of culture on ordinary Salourauds dextrose agar media was 42.7%. 8) Trichophyton rubr,im was the most common causative orantism of superficial dermatophytes(38.4%), and other in decreasing frequency were Candida albicans(36.9%), Trichophyton mentagrophytes(23.6%) Microsporum canis(0.7%), Trichophyton ferrvgiteum(0.3%) and Epidermophyton floccosum(0.1%). CONCLUSION: These results are not significantly different from those iif previous studies. But the incidence rate of the superficial pungal diseases(25.4%) were higher thar, previous studies and showed a high incidence in young male patients and in the summer. The positiv rate of KOH scraping(54.0%) was lower than previous studies. The most common causative organism was Trichophyton rubrum(38.4 %).

5.A Case of Henoch-Schoenlein Purpura with Necrotizing Vasculitis in the G-I Tract.

Henoch-Schoenlein purpura is a syndrome of acute systemic allergic vasculitis involving the small vessels of skin and multiple organs, characterized by a symmetrical, non-thrombocytopenic, painless purpura, nephritis and gastrointestinal manifestations. Although GI involvement is about 70%, endoscopic and histopathological finding of the GI tract in Henoch-Schoenlein purpura is rarely reported and necrotizing vasculitis in GI tract biopsy has not been reported yet. We report a case of a 16-year-old male patient, who complained of palpable purpura, vomiting and epigastric pain with necrotizing vasculitis of the duodenum on histopathological examination.

Intradiploic cyst, non-neoplastic process simulating neoplasm, is rare in skull. The pathogenesis of this lesion remains obscure. But, results of the hemorrhage into the diploic space after trauma in newborn and young children may attribute as one of the major cause to form a intradiploic cyst. A case of intradiploic cyst in a 7 months-old male, which involved right parietal vertex, is presented with reviews of the literatures. Diagnosis was made with the plain skull X-rays, Brain CT, surgical and pathological findings.

7.Antibody-dependent Cell-mediated Cytotoxitity as a Prognostic Indicator in the Medical Treatment of Graves' Disease.

BACKGROUND: The several forms of treatment of Graves disease-thyroidectomy, antithyroid drugs and radioiodide therapy-are in wide use now. But which therapy is best is a matter of debate. Some authors reported that in patients who underwent thyroidectomy, higher titers of serum antimicrosomal antibody were associated with 1) higher formation rates of germinal centers, 2) more lymphocyte infiltration in the thyroid tissue, 3) higher incidence of hypothyroidism, and 4) lower incidence of recurrence. We were interested in the relationship of thyroid autoantibody titers, ADCC(antibody-dependent cell-mediated cytotoxicity) activity and the clinical response to antithyroid medication. METHODS: We measured ADCC activities from patients in Graves disease(n-48), Hashimoto thyroiditis(n=17) and normal control(n=9). The patients of Graves disease were followed up for more than 1 year, and they were grouped into A(n=17, well responsed group to antithyroid medication) and B(n=31, poorly responsed group). We examined ADCC activities of patients' sera by chromium release assay. RESULTS: 1) Mean age of patients with Graves disease was 34.4210.4 years and 15 patients were male(31%). 2) Results of thyroid function tests of the Graves' patients were T 585.9 +/- 255.3 ng/dL, T4 21.3 +/- 12.2 mg/dL, TSH 0.11 +/- 0.06mIU/mL. Concentrations of antimicrosomal antibody, antithyroglobulin antibody and thyrotropin binding inhibitory immunoglobulin were 1279.1 +/- 1486.7 IU/mL, 488.1 +/- 751.1 IU/mL, and 38.5 +/- 33.4U/L respectively. 3) There was no significant difference between levels of thyroid hormones or concentrations of thyroid autoantibodies and ADCC activities in graves patients. 4) The ADCC activity of the Graves patient group(24.49%) was significantly higher than that of the normal control group(3.76%), and significantly lower than that of the Hashimotos thyroiditis group(36.34%). 5) There was no significant difference in ADCC activity between group A(18.24 +/- 13.44%) and B(27.91 +20.02%). CONCLUSION: From this results, we suggested that ADCC activity seems to be no value as a prognostic factor in predicting the response to antithyroid drugs in Graves disease patients. But, further studies, larger number of patients and long-term follow up, are needed.

8.Angiographic Analysis of Ruptured Anterior Communicating artery Aneurysms.

The authors reviewed angiographic findings of 96 cases of ruptured anterior communicating artery aneurysms that were operated at the Department of Neurosurgery of Hanyang University Hospital from Jan. 1986 to June 1990. The results were summarized as follows ; 1) Asymmetricity of caliber of A1 was observed in 71.9%(69/96) and which was 3 times more frequent than that of reported incidence of control group(2~23%). 2) The incidence of predominance of left A1 was 54.2%(53/96) and that of right A1 was 17.7%(17/96). Among them, predominant A1 was operated as afferent artery of aneurysm in 94.2%(65/69). 3) Of 69 cases in which a unilateral A1 showed predominance, the direction of aneurysm was opposite to the side of predominant A1 in 66.7%(46/69) and ipsilateral to the predominant A1 in only 2.9%(2/69). 4) Above findings suggest that it is desirable to approach the anterior communicating artery aneurysm via the side of predominant A1 when the direction of aneurysm is uncertain on cerebral angiogram.

9.Coronary Angiographic Findings and Risk Factors in Acute Myocardial Infarction Patients before after Age 50.

BACKGROUND: A number of studies in western countries have shown that coronary artery disease(CAD) and younger patients with acute myocardial infarction(AMI) is less extensive but more. closely related to cigarette smoking. However, there are some similarities between the characteristics of the CAD and the risk factors analyzed by age subsets in patients with AMI in Korea. METHODS: Pre-discharge coronary angiograms and risk factors in 194 patients with first AMI(male/female : 154/40,79.4%/20.6%) were analyzed. A comparison was made between the lesion characteristics of the infarct related artery(IRA) and the CAD risk factors. Those under 49 years old (Group I, n=54, male/female : 50/4 : 92.6%/7.4%) and those over 50 years old ( Group II, n=140, male/female : 104/36 : 74.3%/25.7%). RESULTS: There are 54 and 140 patients in groups I and II respectively. 92% of group I and 74.1% of group II were male (p<0.01). At the time of study, more group I patients were smoking cigarettes and drinking alcohol(over 3 times per week 22.6% vs 11.5%, p<0.01) and insignificant stenosis was more common in group I than in group II. In both groups, the most common IRA was the left anterior descending artery and concurrently the anterior wall was the most common morphological type of coronary stenosis of IRA was concentric irregular in group I(27.7%) and eccentric smooth in groupII(31.5%). Although group I had the larger minimal cross sectional area of coronary stenosis(0.97+/-1.16mm2 and 0.54+/-0.80mm2, respectively, p<0.05, reference diameters 2.71+/-0.60mm and 2.66+/-0.67mm, respectively),the coronary lesion was shorter in group I patients(3.39+/-4.06cm and 5.49+/-6.13cm respecitively, p=0.03). CONCLUSION: These findings reveal that coronary atherosclerotic lesions of IRA in patients with AMI in the younger age group are less severe. However, they are more closely related to cigarette smoking and alcohol intake, than cases in the older age group.
