4.A clinical and mycological study of superficial fungal diseases(vii).

BACKGROUND: Superficial fungal infections are common diseases acid the prevalence of superficial fungal diseases are not static but change under the influence of various forces such as climate, migration of peoples and development in prophylaxis and therapy. OBJECTIVE: To investigate the present status of superficial dermatiphytoses and some changes on the frequency of the diseases, sex distributions and varieties of the casiative organisms. MEHTODS: The clinical and rnycological aspects of superficial fungal diseases of 3,222 patients from July 1989 to June 1992 were analyzed. Results : 1) The incidence of superficial fungal diseases was 25.4% of total out-patients. 2) The superficial fungal diseases showed a higt incidence in the third(26.4%) and the fourth decade(24.3%). 3) The ratio of male to female was 2.11:1(2,188:1,034). 4) The monthly prevalence of super ficial fungal diseases was high inhuly(17.8%), August(15.2%) and June(1.7%). 5) The incidence of superficial fungal diseases in order of frequency were tinea pedis ,28.1%, tinea cruris(19.5%), onychomycosis(15.6%), tinen orporis(11.5%), tinea manus(7.8%), tinea versicolar(7.8%), candidiasis(7.4%) and tinea capitis(2.3%), 6) Coexisting fungal infections were found in 644 patients(19.9%) and the case of tinea pedis with onychomycosis was the most common. 7) The positive rate of KOH scraping was 54.0% and the positive rate of culture on ordinary Salourauds dextrose agar media was 42.7%. 8) Trichophyton rubr,im was the most common causative orantism of superficial dermatophytes(38.4%), and other in decreasing frequency were Candida albicans(36.9%), Trichophyton mentagrophytes(23.6%) Microsporum canis(0.7%), Trichophyton ferrvgiteum(0.3%) and Epidermophyton floccosum(0.1%). CONCLUSION: These results are not significantly different from those iif previous studies. But the incidence rate of the superficial pungal diseases(25.4%) were higher thar, previous studies and showed a high incidence in young male patients and in the summer. The positiv rate of KOH scraping(54.0%) was lower than previous studies. The most common causative organism was Trichophyton rubrum(38.4 %).

5.A Case of Henoch-Schoenlein Purpura with Necrotizing Vasculitis in the G-I Tract.

Henoch-Schoenlein purpura is a syndrome of acute systemic allergic vasculitis involving the small vessels of skin and multiple organs, characterized by a symmetrical, non-thrombocytopenic, painless purpura, nephritis and gastrointestinal manifestations. Although GI involvement is about 70%, endoscopic and histopathological finding of the GI tract in Henoch-Schoenlein purpura is rarely reported and necrotizing vasculitis in GI tract biopsy has not been reported yet. We report a case of a 16-year-old male patient, who complained of palpable purpura, vomiting and epigastric pain with necrotizing vasculitis of the duodenum on histopathological examination.

6.Hemorrhoidectomy Specimens: Necessity for Routine Pathologic Evaluation.

PURPOSE: The aim of this study was to determine the necessity for routine pathologic evaluation of hemorrhoidectomy specimens. METHODS: Between March 1998 and February 2001, 280 patients (185 males, 95 females) underwent a hemorrhoidectomy at Seoul National University Hospital. All patients had grade III~IV hemorrhoids, and the mean age of the patients was 51 years (range: 21~74 years). All hemorrhoidectomy specimens were examined with a hematoxylin and eosin stain of one representative section by a pathologist. We performed a retrospective analysis regarding the pathologic results for the hemorrhoidectomy specimens. RESULTS: Two hundred sixty-seven specimens (267, 95.4%) had typical hemorrhoids reported as external and internal hemorrhoids, external hemorrhoids, hemorrhoidal varices, and thrombi. Ten patients (10, 3.2%) had additional benign pathologes such as fibroepithelial polyps (6 cases), a flat condyloma (1 case), hypertrophied papillae with a condyloma, like papillomatosis and keratosis (1 case), dyskeratotic squamous cells with koilocytotic atypia (1 case), and an inflammatory polyp (1 case). Interestingly, three patients (3, 1.1%) had carcinomas in the hemorrhoidectomy specimens. Two patients had squamous- cell carcinomas; one suffered from delayed wound healing after a previous hemorrhoidectomy, and the other had indurated lesions on the hemorrhoids. One patients who had under gone a low anterior resection due to stage-C rectal cancer 7 months before had a adenocarcinoma. CONCLUSIONS: Because of the possibility of unsuspected anal cancer, we recommend pathologic examination of hemorr hoidectomy specimens, especially in cases of suspected indurated lesions within the hemorrhoids, delayed wound healing after a previous hemorrhoidectomy, or previous history of colon cancer.

7.Angiographic Analysis of Ruptured Anterior Communicating artery Aneurysms.

The authors reviewed angiographic findings of 96 cases of ruptured anterior communicating artery aneurysms that were operated at the Department of Neurosurgery of Hanyang University Hospital from Jan. 1986 to June 1990. The results were summarized as follows ; 1) Asymmetricity of caliber of A1 was observed in 71.9%(69/96) and which was 3 times more frequent than that of reported incidence of control group(2~23%). 2) The incidence of predominance of left A1 was 54.2%(53/96) and that of right A1 was 17.7%(17/96). Among them, predominant A1 was operated as afferent artery of aneurysm in 94.2%(65/69). 3) Of 69 cases in which a unilateral A1 showed predominance, the direction of aneurysm was opposite to the side of predominant A1 in 66.7%(46/69) and ipsilateral to the predominant A1 in only 2.9%(2/69). 4) Above findings suggest that it is desirable to approach the anterior communicating artery aneurysm via the side of predominant A1 when the direction of aneurysm is uncertain on cerebral angiogram.

8.A Study on the Level of Physiologic CSF Reflux into Ventricles.

Using Omnipaque CT cisternogram, author prospectively studied on the level of physiologic CSF reflux into ventricles in 42 cases, who had not history of brain pathology or abnormal findings in neurologic examination. Results were as follows: 1) Age of all cases ranged from 26 to 78 years and male to female ratio was 2.8:1. 2) Of 7 cases used control, 6 cases showed maximum reflux at 2 hours scan and remaining 1 showed at 5 hours. 3) CSF reflux to 4th ventricle were detected in all cases, but reflux to Aqueductus of sylvius, posterior 3rd ventricle, anterior 3rd ventricle, foramen Monro and lateral ventricle were in 17 out of all 42 cases(40.5%), 12/42(28.6%), 3/42(7.2%), 3/42(7.2%), 2/42(4.8%) respectively. 4) Cases with reflux to 4th ventricle could be considered as normal, but authors with reflux above 4th ventricle need follow-up scan after 3 to 6 hours to decide as normal.

Intradiploic cyst, non-neoplastic process simulating neoplasm, is rare in skull. The pathogenesis of this lesion remains obscure. But, results of the hemorrhage into the diploic space after trauma in newborn and young children may attribute as one of the major cause to form a intradiploic cyst. A case of intradiploic cyst in a 7 months-old male, which involved right parietal vertex, is presented with reviews of the literatures. Diagnosis was made with the plain skull X-rays, Brain CT, surgical and pathological findings.

10.Functional Outcome after Ileal J-pouch Anal Anastomosis in Patients with Ulcerative Colitis.

PURPOSE: This study was performed to assess the complications and functional outcomes after a total proctocolectomy and ileal J-pouch anal anastomosis for patients with ulcerative colitis. METHODS: We reviewed the medical records of 30 patients who had undergone a total proctocolectomy and ileal J-pouch anal anastomosis for ulcerative colitis from 1992 to 1999 in our hospital. We used questionnaires or telephone interviews to assess the functional outcomes of the patients. The median duration of follow-up was 23 months after the ileostomy take down. RESULTS: The mean age of the patients at the definitive operation was 35.9 (+/-11.8). The indications for operation were medical intractability (76.7%), suspicious malignancy (13.3%), perforation (6.7%), and hemorrhage (3.3%). The double stapling method was used in 26 patients and the handsewn method in 4 patients. Of the 30 patients, 23 patients completed the functional analysis. Bowel frequency was 6.6 (+/- 2.6) per 24 hours, with 5.1 (+/- 2.1) in the daytime and 1.4 (+/-1.3) in the night. Fourteen patients (60.9%) had relatively mild incontinence, and four patients (17.4%) had to wear pads, especially at night. Eighteen patients (78.3%) were able to discriminate flatus from feces, and only one patient (4.3%) suffered from perianal irritation. Twelve patients (52.2%) had to restrict their diets, and five patients (21.7%) took antidiarrheal medications. Pouchitis occurred in three patients (13.0%). Sexual dysfunction was noted in four patients (17.4%), and urinary urgency in one patient (4.3%). There was no functional difference between the double stapling method and the handsewn method. CONCLUSIONS: The functional outcomes after ileal J-pouch anal anastomosis for patients with ulcerative colitis were satisfactory, irrespective of the method of anastomosis.
