3.The Analysis of Chronic Prostatitis Symptom score Surveyed in 19 Yeat Old Korean Men in the Area of Teajeon and Chung-nam.

PURPOSE: Chronic prostatitis is a common diagnosis, and the morbidity of this disease results from a constellation of genitourinary symptoms. We surveyed a population of 19 year old Korean men using National I nstitutes of Health Chronic Protatitis symptom Index, evaluated symptomatology associated with chronic prostatitis and gathered epidemiologic data about this disease. MATERIALS AND METHODS: During May 2000, a total of 4339 male residents in the area of Chung-nam including Taejeon city visited the Military Manpower Administered questionnaire. and the impact of symptoms on quality of life. the results were analyzed statistically by PC-SPSS version 7.5. RESULTS: There were statistically significant correlations among every item and domain, respectively by Pearson's correlation analysis (p=0.00). Pain domain significantly contributed to predicting quality of life domain and explained 42% of the variance on quality of life domain (r=0.65,p=0.00). The change in the amount of explained variance was significant when entering urinary symptoms domain (r(2)change=0.19,p=0.00). Overall pain and urinary symptoms domain predicted 61% of the variance in quality of life domain (r=0.78,p=0.00). If a score of 6 in quality of life domain was determined to the cut-off score of the chronic prostatitis, the prevalence of the chronic prostatitis was 3.8%. CONCLUSIONS: the National Institutes of Health chronic prostatitis and pain and urinary symptoms significantly contribute towards explaining variances in quality of life. although there data show much less common prevalence than most nonpopulation studies suggest, we suggest that chronic prostatitis is a common disease in Korean men if one consider that our data were surveyed in only 19 year old men.

4.Conservative Treatment in Thoracolumbar Fracture and Fracture-Dislocations

The thoracolumbar fracture and fracture-dislocations are ever increasing today as the traffic accidents and industrial accidents frequently occur. But the controversy as to the relative values of early surgical instrumentation and conservative means has continued for well over a decade. The duration of this dispute without final resolution suggests an absence of significant differences in the results of surgical and non-surgical method. The purpose of this study is to review the results of conservative treament for 132 patients with thoracolumbar fracture and fracture-dislocations, who were admitted and treated at Yonsei University Severance Hospital from January, 1980 to December, 1984. And we obtained following results. l. In cases of stable fracture without neurologic deficit, especially when the wedging deformity of vertebral body is below 50%, it seems to be better to treat conservatively; that is, immediate postural reduction and after 2 or 3 weeks of bed rest, to start ambulation with back brace or cast. 2. In cases of stable fracture with neurologic deficit, operative treament is necessary when the frac-fragment is protruded into the spinal canal and neural compression sign is evident. But if not so, attempt to treat by conservative means may be done. 3. In cases of unstable fracture without neurologic deficit, it is more retional to treat conservatively by bed rest for a suffient time and then wearing back brace or cast. If the spinal deformity is so severe that it needs spinal fusion, the operation can be performed later on. 4. In cases of unstable fracture with neurologic deficit, spinal instrumentation and fusion do nothing to enhance neural recovery but are done to provide anatomic spinal alignment and stability to allow early the patient mobilization and rehabilitation.

5.Change of 24-hour Urinary Citrate Level after Dietary Manipulation with Lemon Juice in Urolithiasis Patients.

PURPOSE: Citrate is a well recognized inhibitor of the formation of calcium oxalate and calcium phosphate stones. Hypocitraturia is a common etiology of recurrent calcium nephrolithiasis, with an incidence of 19 to 63%. Potassium citrate therapy can be a useful therapeutic approach for the management of calcium nephrolithiasis. But pharmacological treatment of hypocitraturic calcium nephrolithiasis requires taking too many tablets, or numerous crystal package or liquid supplements throughout the day. This cumbersome regimen often decreases patient compliance. We administered dietary citrate via lemon juice to stone former and evaluated the change of citrate levels. MATERIALS AND METHODS: The prospective study included 7 women and 8 men with documented recurrent or multiple urinary stone disease. None of the subjects suffered from renal impairment, urinary tract infection and other metabolic disorder. Controls comprised 6 voluntary men. They had no previous stone history and no evidence of stone. Patients ingested total 1 liter of lemon juice(containing 4.0gm/L.citrate) divided at 6 hours interval without strict diet restriction. Urine specimens were obtained for urinary citrate levels after 2-3days of lemon juice therapy and compared to pre-lemon juice baseline values. RESULTS: All 15 patient showed increased urinary citrate levels during lemon juice therapy. Average urinary citrate levels increased from 146+/-109mg/day at baseline to 453+/-226mg/day during treatment(p<0.05). Urinary citrate levels during treatment increased up to those of control group(351+/-265mg/day) and did not show significant difference (p>0.05). Urinary pH increased from 5.9+/-0.4 at baseline to 6.8+/-0.6 during treatment(p<0.05). No patient complained of gastrointestinal discomforts. CONCLUSIONS: Citrate supplementation with lemon juice increased urinary citrate levels and urinary pH. Lemon juice is well tolerated dietary source of citrate and would be beneficial in the control of calcium urolithiasis.

6.The Effect of the Administration of Nitroglycerin and Atropine on the Pattern of Left Ventricular Diastolic Filling as Assessed by Doppler Echocardiography in Normal Human Subjects.

BACKGROUND: The diastolic transmitral flow velocity pattern has been commonly used to assess left ventricular(LV) diastolic function. The effects of multiple factors(such as, LV preload, afterload and heart rate, etc.) make difficulties in accurate interpretation. METHODS: In order to investigate the diastolic transmitral filling patterns according to the changes of the proload or heart rate, we studied 27 normal subjects with pulsed Doppler echocardiography after the administration of nitroglycerin(0.6mg/tab.) sublingually or atropine(0.5mg/amp.) intravenously. RESULTS: 1) After nitroglycerin administration, the folowings were obtained. ; The systolic blood pressure and LV diastolic filling time(DFT) decreased by 10.1% and 15.3%, respectively(p<0.001), compared with baseline data. The ratio of peak early to late diastolic transmitral flow velocities (E/A) and time-velocity integrals(TVIE/TVIA) decreased by 10.3% and 14.8%, respectively(p<0.01). The early diastolic filling time(Time E) was unchanged. Therefore, we suggest that time E is helpful, compared with the increments of the preload or the diastolic dysfunction. 2) After atropin administration, the heart rate, peak late diastolic transmitral flow velocity(PA) and percent atrial contribution(%AC) significantly increased by 43.6%, 25.1% and 41.4%, respectively(p<0.001). The E/A, TVIE/TVIA and DFT significantly decreased by 42.9%, 38.9% and 43.0%, respectively(p<0.001) compared to the data before drug administration. 3) The heart rate correlated negatively to the E/A, TVIE/TVIA and DFT. It was correlated positively to %AC(r=+0.63; p<0.001). The normalized E/A ratio by DFT(E/A/DFT) didn't correlate. Therefore, E/A/DFT is helpful on the exclusion of the influences of heart rate by the administration of the atropine. CONCLUSION: The decrement of preload or the increment of heart rate changes the diastolic transmitral flow velocity patterns. Therefore, when the diastolic function is assessed by interpretation of the Doppler transmitral flow velocity pattern with pulsed Doppler echocardiogram, the potential influences of preload and heart rate must be taken into account.

7.A Study of Case-Based Adult Advanced Cardiac Life Support(ACLS) course in Korea.

BACKGROUND: To describe the fast experience of case-leased advanced cardiac life support(ACLC) course in Korea. METHODS: We have given case-based ACLC course to 13 nurses(6 emergency nurses, 4 coronary care unit muses, 3 professors) and 17 physicians(4 emergency physicians, 12 emergency residents, 1 intem). We performed the case-based ACLS course according to 1992 American Heart Association guidelines and recommendations for advanced cardiac life support by american ACLS instructors(1 pulmonologist, 4 critical care nurses). We performed final theoretical written test and 2 times written survey (immediate and 100th day after the course) about the course. RESULTS: On final written test, all practitioners answered at leasts 70% of the questions correctly. There was no significant difference between nurses and physicians(86.2+/-3.6 of physicians and 82.5+/-6.8 of nurses, p=0.06). 90%of participants considered that case-based advanced cardiac life support was acceptable. 100th day after the course, 93%of participants answered that ACLS course have been helpful on his/her job and also want retraining of ACLS course. 70%of participants considered that ideal ACLS training committee in Korea is the Korean Society of Emergency Medicine. CONCLUSION: Case-based ACLS course is a useful educational method far physicians and nurses in Korea. In the future, we should organize Korean resuscitation committee and then make guidelines for ACLS, and then continuously educate physicians and nurses.

Active infective endocarditis, involving native, is often complicated by conduction abnormalities. These conduction disturbances are considered to represent an extension of the infection from the valve to the annulus, and to the surrounding myocardium. We describe a case of a 59-year-old woman who presented with dyspnea due to staphylococcal endocarditis, which was complicated by a complete heart block.

10.Serum hormone and testicular histologic changes in spinal cord injured men.

A quantitative assessment of the testicular biopsy, and serum hormone levels( testosterone, prolactin, and follicle-stimulating and luteinizing hormones) were studied in 10 spinal cord injured men and comparison of these parameters to normal controls were performed. The mean number of Sertoli cells, mature spermatids, tubular diameter and tubular wall thickness were determined in both groups and analyzed. In the spinal cord injury group the mean number of mature spermatids per tubule was significantly lower and the mean number of Sertoli cells per tubule was significantly higher than in controls(p<0.05). Serum testosterone and luteinizing hormone values were significantly higher in the spinal cord injury group than in controls(p<0.05). These significant differences of the quantitative parameters of the testicular biopsy and hormonal values that may contribute to the reproductive dysfunction.
