1.Effect of BL-21 (Wei-Yu) acupoint stimulation on gastric motility following preanesthetic treatment in dogs.

In acupuncture practice of animals, preanesthetics sometimes are needed. The purpose of this study was to select the ideal chemical restraint at acupuncture for gastric motility. Nine healthy mixed breed dogs weighed 10-21 kg and aged 1-3 years old were used in this study. Two EMG surface electrodes were placed between the seromuscular and the mucosal layer of pylorus. Twenty minutes after feeding normal gastric motility was recorded for 60 minutes using physiograph (Narco-Biosystem). Then preanesthetic treated-gastric motility was observed for 30 minutes. Preanesthetics used were xylazine, diazepam, and acepromazine. Acupuncture needles were inserted to BL-21 (Wei-Yu) acupoint, and then changes of gastric motility were recorded for 60 minutes. The gastric motility following xylazine administration (1 mg/kg, IV) was markedly decreased. BL-21 (Wei-Yu) acupoints stimulation did not alter xylazine-induced depression of gastric motility. The diazepam (1 mg/kg IV) treated-gastric motility was increased mildly 20 minutes after drug administration. BL-21 (Wei-Yu) acupoint stimulation after diazepam administration enhanced gastric motility significantly. The gastric motility following acepromazine (0.3 mg/kg, IM) administration was not changed compared with normal gastric motility. Application of traditional acupuncture at BL-21 (Wei-Yu) significantly increased the gastric motility. Based on these results, acepromazine and diazepam could be acceptable chemical restraints for acupuncture therapy of gastric motility, but xylazine couldn't be.

3.A Case of Infantile Nyofibromatosis.

Infantile myofibromatosis is an uncommon, benign, probably hamai tomatous proliferation of myofibroblasts. This lesions are most often present before the age of 2 years, show a male predomenance and may be multiple or solitary. The solitary lesions arise in the dermis, subcutis, or deep soft tissue, most commonly in the head and neek region or trunk. The elinical coirse is benign and spontaneous regression is not infrequent. A female newborn presented with a turmr lesion on the upper portion of the right eyelid. Physical examination showed a firm, non-tender, fixed, erythematous, 3 x 3cm sized tumor. Histologic examinatign revealed a spindle cell tumor in the deimis with a whorled arrangemant and grouped fescicles. There was no evidence for potential visceval and bony involvement. Follow-up examination at 6 months of aige revealed a remarkable degree of spoitaneous involution.

4.Pedunculated Lipofibroma.

A 45-year-old woman had a bean-sized, asymptomatic, soft nodule on the right buttock for 4 years. Histopafhologic evaluation revealed acanthosis and a central core of adipose tissue with thin papillary and reticular dermis containing eccrine glands. Staining with alcian blue showed a slight increase of mucopolysaccharides in the reticular dermis. We made the diagnosis of pedunculated lipofibroma as a solitary form of nevus lipomatosus superficialis.

5.Health-Promoting Lifestyle Patterns and Health Perception in Elders Using Welfare Service Centers.

PURPOSE: The purpose of this study was to identify health promoting lifestyle patterns (HPLP) and health perception (HP), and related factors, in elderly people using welfare service centers. METHODS: Three hundred elders were interviewed using a structured questionnaire through convenience sampling at two welfare centers. Data were analyzed using the SPSS WIN 12.0 program. RESULTS: Most of the elders had some kind of diseases, such as hypertension, diabetes, or osteoarthritis. The mean HPLP score was 2.53, and significant differences were found according to the elders' general characteristics. The sub-domain nutrition had the highest mean score, and stress management and exercise, the lowest. The mean HP score was 3.38, and significant differences were found according to education level and number of underlying diseases. HPLP showed a correlation with HP. CONCLUSION: The results indicate that elders have poor practices in stress management and exercise reflecting need for community based stress management programs for welfare service centers elderly clients.

6.Central Retinal Artery Occlusion after Cervical Spine Surgery in Prone Position under General Anesthesia: A case report.

Central retinal artery occlusion occurs rarely as a complication of spine surgery under general anesthesia in prone position, but is quite tragic. The suggested causes are hypotension during anesthesia and increased external ocular pressure by headrest, sand bag or others. We experienced a case of left central retinal artery occlusion following cervical spine surgery under general anesthesia using a horseshoe headrest. The patient was 53 years old male whose medical history was non remarkable except dislocation of cervical spine. He was positioned prone after induction. The vital signs were stable during opreration. At the recovery room, he presented left visual field disturbance and investigations revealed that left central retinal artery occlusion occured. This case demonstrates that proper positioning of the head on an adequate head rest and contineous cautious inspection during surgical procedure are important to prevent retinal damage.

7.Educational Needs of Drinking Prevention for Primary School Students.

PURPOSE: To provide basic data for developing problematic drinking prevention problem through the investigation of the educational needs on drinking of primary students. METHOD: Subjects were 630 fourth, fifth, and sixth grades in 5 primary schools in Seoul. Data were collected from November to December 2000 by using structured questionnaire. RESULT: 1) Among the students under inquiry, 41.2% answered the had educational need about drinking. Educational need about drinking was significantly higher in the students with higher mothers educational level than those of with lower ones. 2) With regard to the appropriate class in providing education, 37.3% selected normal class education. 47.3% of the respondents said experiments would be the most effective education method. 61.8% of the students picked school nurses as the most appropriate person in carrying out the education. As for the most effective media for education, 40.3% chose video/television. The most concerning topic was academic achievement with 51.9% of the surveyed. As for the rate of preference in methods of drinking prevention education, Significant differences were shown in the class, teaching method, instructor, media, memory of education and concerning topic according to the gender. CONCLUSION: The results of the finding suggest that the various programs of drinking prevention education for the primary students should be developed with consider the difference of preference by the grade and the gender.

8.Sudden Cardiac Death.

Sudden cardiac death (SCD) refers to the unexpected natural death from a cardiac cause within a short time period, generally within an hour from the onset of symptoms, in a person without any prior fatal condition. Despite the tremendous advances in the field of cardiovascular medicine, the incidence of SCD continues to rise. In 60 to 80 percent of cases, SCD occurs in the patients with coronary artery disease. Most instances of SCD are thought to involve ventricular tachycardia degenerating to ventricular fibrillation and subsequent asystole. Since the implantable cardioverter defibrillator (ICD) is effective in terminating ventricular tachycardia and fibrillation, the application of ICD has increased markedly. However, the application of ICD needs to be individualized for the patient, similar to drug therapies in LV systolic dysfunction. This review discusses the current understanding on SCD, risk stratification, and management goals for reducing SCD, particularly with the ICD usage.

9.Influence of Clonidine on the Postoperative Analgesic Effect of Epidural Bupivacaine and Fentanyl.

BACKGROUND: Epidural clonidine reduces pain after surgery. The aim of this study was to evaluate the effect of adding low-dose clonidine to continuous epidural local anesthetics and fentanyl on pam relief. METHODS: 100 patients scheduled for gynecologic low abdominal surgery were investigated. All patients were given 10cc of 0.25% bupivacaine with fentanyl 100 ug through epidural catheter. Group I was infused with combined 0.15% bupivacaine and fentanyl 5 ug/ml at a rate of 2cc/hr. Group II was infused with combined 0.15% bupivacaine and fentanyl 5 ug/ml and 150 ug of clonidine at a rate of 2cc/hr. Pain was assessed on a visual analogue pain scale for 2 postoperative days. Changes in blood pressure, heart rate, and incidence of side effects were observed. RESULTS: VAS observed 20min, 1hr, 1day, 2days after operation were significantly lower in Group II than Group I . Blood pressure and heart rates were significantly changed in Group II but not in Group I for 2 hours after epidural injection. The incidence of side effects was similar between Group I and Group II. CONCLUSIONS: Continuous low-dose epidural clonidine infusion reduces blood pressure and heatt rates significantly but enhances postoperative analgesic effect of combined epidural bupivacaine and fentanyl without increased side effects.

10.The Experimental Study on the Usability of Gore-Tex(R) as an Extraocular Muscle Expander.

Expander procedure is a newly developed extraocular muscle weakening technique which has several advantages such as good control of resection margin, easy reoperation, rare postoperative muscular paresis. For the study of usability of Gore-Tex(R) as a muscle expander, fifteen rabbits received expander procedure with Gore-Tex(R) in right eye and silicone retinal band in left eye. At postoperative 1, 3, 5, 7, 12 weeks, histopathologic examination was performed. There was no significant difference in inflammatory reaction in both groups. Inflammatory reaction decreased with time in both groups. In Gore-Tex(R) expander group, muscle regeneration from cut end was observed at the 3rd postoperative week and fibroblast ingrowth into the expander was observed at the 5th postoperative week. These findings were more prominent with time. Adhesion of expander to underlying sclera was not observed in both groups. It is concluded that expander procedure using Gore-Tex(R) will be a new and useful surgical method for the extraocular muscle weakening procedure.
