3.A Study of Luteinizing Hormone in Patients with Infertility and Recurrent Spontaneous Abortion.

OBJECTIVE: To evaluate luteinizing hormone(LH) in patients with infertility and recurrent spontaneous abortion. MATERIAL AND METHOD: LH was tested by solid phase immunoradiometric assay based on monoclonal and polyclonal anti-LH antibodies. RESULTS: Of 100 infertile patients, the number of less than 5 mIU/ml, 5~10 mIU/ml, greater than 10 mIU/ml in LH level was 67(67%), 22(22%), 11(11%), respectively. Of 100 patients with recurrent spontaneous abortion, the number of less than 5 mIU/ml, 5~10 mIU/ml, greater than 10 mIU/ml in LH level was 79(79%), 18(18%), 3(3%), respectively. There was a significant difference between patients with infertility and recurrent spontaneous abortion only in the group with LH level greater than 10 mIU/ml(p=0.325). CONCLUSIONS: High LH in the follicular phase is known to decrease pregnancy rate and increase abortion rate. But in this study the incidence of high LH in patients with recurrent spontaneous abortion was low. On the contrary, there was a significant increase of LH in infertile patients. So recurrent spontaneous abortion does not seem to be related to high LH level.

Chusid et al proposed diagnostic criteria of hypereosinophilic syndrome (HES) that remain valid today. These were, (1) a sustained peripheral blood eosinophil count of more than 1500/L present for longer than 6 months ; (2) no evidence of other apparent causes for eosinophilia, and (3) presumptive signs of parenchymal organ involvement. Any organ system may be affected in HES, but the most severe clinicopathological involvements are of the heart and nervous system. Although multiple organ systems may be involved, the most common cause of morbidity and mortality is cardiac involvement with extensive fibrous thickening of the endomyocardium and overlying thrombus. We report a case of acute peri-myocarditis with eosinophilia, which was confirmed as eosinophilic myocarditis by endomyocardial biopsy, with literature review.

6.Transcranial Approach for Removal of Pleomorphic Adenoma of Lacrimal Gland.

Pleomorphic adenoma in lacrimal gland is a rare disease. Injuries to capsule surrounding the tumor lead to recurrence or malignant transformation of tumor by seeding of tumor cells into adjacent tissues. Therefore complete removal of tumor without any injury to capsule is important. We present 2 cases of pleomorphic adenoma of lacrimal gland with symptoms of exophthalmos, hypophthalmos and supraduction limitation. We performed frontatemporal craniotomy and removed orbital roof for the exposure of the tumor located at superior portion of orbit. We removed the tumor with intact capsule avoiding any damage to superior levator palebral muscle and superior rectus muscle microscopically. Periosteum of superior orbital roof was repaired and frontal bone was repositioned. As a result, patients had no symptoms of exophthalmos, hypophthalmos and supraduction limitation. There are no complications of ptosis, diplopia, dry eye syndrome, symblepharon and scar on the face. Transcranial approach for removal of pleomorphic adenoma of lacrimal gland provides wide exposure of the tumor for complete removal of the mass without any damage to surrounding tissues and good cosmetic effects.

7.Rathke's Cleft Cyst: Case Report.

Symptomatic Rathke's cleft cysts may be confined to the sella turcica or extended upward into suprasellar area. Like the more common cystic tumor of this region, the craniopharyngioma, they produce symptoms by compression of the surrounding structures. We report a rare case of a symptomatic Rathke's cleft cyst. The importance of the intraoperative differentiation between the cyst and a craniopharyngioma is discussed.

8.Intrathecal and Epidural Morphine for Postoperative Pain Control after Lumbar Laminectomy.

A number of reports have shown that intrathecal and epidural administration of morphine provides acute or chronic pain relief. We report the use of morphine for postoperative pain control in 84 lumber laminectomy patients. These patients were compared to the 24 control patients. Single dose of morphine in 0.5ml saline was administered under the direct vision into the intrathecal or the epidural space just before the closure of the lumbar operative wound. Intrathecal group was divided into 4 subgroups 0.2mg, 0.5mg, 1mg and 2mg and epidural group was also divided into 2 subgroups 2mg, 5mg, respectively. Under the visual pain analogue scale, postoperative pain control were statistically significant in intrathecal 0.5mg, 1mg and epidural 2mg, 5mg group, respectively(P<0.01). Respiratory depression was occurred in 2 cases of intrathecal 2.0mg group and nausea with vomiting was occurred in 10 cases but prutitus was not occurred. Urinary Retention was developed between 86.7% and 100% of study group compared to 45.8% of control group.
