Panniculitis*

Conflicting results have been reported, as to whether or not wound repair is influenced following treatment with low doses of prednisolone, insulin, or thyrotrophic hormone. The present studies were undertaken to evaluate and compare the influence of above hormones on the wound repair process and their target organs in rats at different times after operation. The influence of the above hormones were observed as measurement of the tensile strength of healing skin incisions and the histologic finding of the wound sites and the target organs staining with H-E, PAS, and Masson's trichrome method. Saline 0. 1 ml were injected daily intramusculary in control group: prednisolone acetate 0. 2ml (4mg/kg/day) in prednisolone treated group: regular zinc insulin 0.1 ml(1.5IU/kg/day) in insulin treated group; thyrotrophin 0.1 ml (0.4IU/kg/day) in TSH trcated group. The observation period wer at 4th day, 7th day, 14th day and 21st day of post-operation. The results were as follows: 1. Prednisolone treated group: (1) The tensile strength were decreased at 4th and 7th day than control groups, but there were no statistically significant difference at 14th and 21st day from control group. (2) In histologic findings of the wound sites at 4th and 7th day, the repair processes were suppressed, but at 14th and 21st day, there were no difference from control group. (3) In histologic finding of adrenal cortex, the atrophic changes was observed from early stage. 2. Insulin treated group: (1) There were no statistical difference from control group in tensile strength at each observation period. (2) In histologic findings of the wound sites, the pictures of the repair processes were similar with control group at each observation period. (3) Atrophic change of islets of Langerhans in pancreas was observed at each observation period. 3. TSH treated group: (1) The tensile strength were increased at 4th, 7th, and 14th day than control group, but there was no statistical difference at 21st day frorn control group. (2) In histologic findings of the wound sites, the repair processes were aecelerated at 4th and 7th day than control group, but there were no defferences at 14th and 21st day from control group. (3) Hyperplastic change of acini in thyroid gland was observed through entire observation period.

The purpose of this study is to investigate th effect of the blood pressure self monitoring on the reduction of essential hypertension. This study had been conducted from March to July, 1995. Six persons who lived in Song-pa Gu, Seoul were selected for the experimental participants. They were consist of 5 males and 1 female. The samples measured their blood pressure by automated sphygmomanometer in supine position every morning(6-7AM) and night(10-11PM) for 6 weeks. The researcher measured blood pressure of samples in supine position by mercurial shpygmomanometer 2 times a week for 6 weeks. The samples' subjective stress was measured by VAS before and after the experiment. Results were as follows : 1. The average age of the samples was 53 (range from 49 to 60) and the average hypertension history of the samples was 56 months (range from 1 month to 10 years). 2. The self measuring samples' blood pressure of the morning showed the tendency of gradual reduction during the 2nd week and the 5th week. 3. The self measuring samples' blood pressure of the night showed no variation but systolic blood pressure showed slight reduction during the 5th week. 4. The blood pressure measured by researcher showed no variation but it showed the higher blood pressure than the self measuring samples' blood pressure. The conclusion is that the blood pressure self monitoring had effects on the reduction of essential hypertension, especially on the reduction of systolic blood pressure. 5. Generally blood pressure measured by researcher showed the higher blood pressure than the self measured samples' blood pressure. Therefore the medical persons who measure the blood pressure must be more careful when they measure blood pressure. I suggest that, first the client who want to know their real blood pressure should compare the blood pressure measured by clinic with the self monitoring method. 2nd the patient of essential hypertension who take anti-hypertensive agents should be selected in sampling. 3rd the experiment of the lasting duration of reduction of blood pressure self monitoring should be conducted.

This study was performed to investigate the heavy metal contents of freshwater fishes. The samples of 24 species were collected at 7 areas located on the Mankyung River during September in 1987. And then the contents of lead, cadmium, copper and zine were analyzed by atomic absorption spectrophotometer. The results were summarized as follows ; 1. The mean value of lead, cadmium, and copper contents of fishes collected in the downstream were significantly higher than those of upstream. 2. The mean lead content of C. auratus was the highest 1.50+/-0.98 microgram/g in viscera and statistically significant difference from muscle content. 3. The mean cadmium content of C. auratus was the highest 0.087+/-0.054 microgram/g and significantly higher than that of muscle, skeleton and gill. 4. In the copper contents, the viscera of C. auratus was the highest 5.25+/-0.94 microgram/g and significantly higher than that of muscle, skeleton and gill. 5. The mean value of zinc content of C. auratus was shown the order of gill, skeleton, viscera and muscle.

There are several modalities of surgical treatment in lumbar herniated nucleus pulposus (HNP), derangement of lumbar intervertebral disk (DLID), spondylolysis, spondylolisthesis and degenerative spinal stenosis with scoliosis, such as laminectomy, laminectomy and diskectomy, laminectomy with diskectomy and posterolateral fusion, laminectomy with diskectomy and posterior lumbar intebody fusion, anterior diskectomy and interbody fusion, modification with cage instrumentation and decompression with instrumentation and posterolateral fusion. Low back pain due to the instability resulting from the removal of the nucleus pulposus was not infrequent in clinical practice. The purpose of the study was to determine the factors affecting clinical results by evaluating fusion rate, fusing pattern, measurement of spinal canal, correction of deformity. Five hundred and twenty eight cases of lower lumbar spinal diseases including 2S6 cases of lumbar HNP and DLID, 189 cases of spondylolysis and spondylolisthesis and 53 cases of spinal stenosis with scoliosis were studied respectively. The cases were admitted to Severance Hospital department of Orthopaedic Srugery from January 1970 through December 1994, and followed the patients from two years up to 15 years with an average of 2.9 years. It was reviewed clinical records and radiographs. The radiological study included plain x-ray, dynamogram, tomogram, myelography, computed axial tomography and/or MRI. It was evaluated the fusion rate, fusing pattern, area of spinal canal, correction of deformity and clinical results. The rate of solid fusion was 89.5% in lumbar HNP and DLID, 77.3% in spondylolysis and spondy-lolisthesis, and 86.7% in spinal stenosis with scoliosis. The most common type of the fusing pattern was type I in lower lumbar diseases. The satisfying clinical result was 82.6% in lumbar HNP and DLID, 76.7% in spondylolysis and spondylolisthesis and 58.5% in spinal stenosis with scoliosis. Spinal canal measurement was done in 23 cases and the increase of A-P diameter of spinal canal was 0.50+/-0.29mm and it was correlated with satisfying clinical result. Correction of the deformity was evaluated in 53 cases of spinal stenosis with scoliosis and of them 67.9% was corrected and it was correlated with satisfying clinical result. In conclusion the factors affecting the satisfying clinical results in the lower lumbar spinal diseases treated by diskectomy and anterior interbody fusion are solid union, restoration of disk height, correction of the deformity and increment of area of dural sac. Anterior diskectomy and interbody fusion have shown to be a good method of treatment for lower lumbar spinal diseases.

Panax ginseng C.A. Meyer (Araliacea) has been known for thousands of years in the Far East as a remedy effective against a multitude of ailments. Pharmacological investigations have shown that the basic effect in ginseng action is its capacity to increase nonspecific resistance of the organism to various untoward influences. Ginseng saponin has been considered the active principle for ginsengs most therapeutic properties. Recently, the use of cream and soap with ginseng extract has been gradually increasing. The purpose of this study is to evaluate the efficacy of Panax ginseng to Candida albicans in vitro. During this experiment ginseng saponin and powder of Panax ginseng root were diluted serially in Sabouraud's dextrose agar, and candidial suspensions of isolated strains from candidial intertrigo and female genitalia were inoculated into each medium containing different concentrations of test materials. Then their growth was observed for weeks at 25'C. After that we measured the size of colonies grown in various concentrations and eompared them with those of the Sabourauds dextrose media. For the complementary study Candida albicans strain isolated from vagina were plated on both Sabouraud's dextrose media and 80 mg/cc ginseng root powder. Microscopic examination, germ tube test and sugar fermentation test were then performed to compare the production of budding cells, the growth of pseudo-hyphae and the characteristics of the yeast on each media. Results from this study show that ginseng saponin and roots of Panax ginseng have no significant effect on growth and multiplication and the characteristics of Candida albicans in vitro.

Carcinoid tumors are arisen from enterochromaffin cells, which are scattered throughout the body but occur primarily in the submucosa of the intestine and brpnchi. Carcinoid tumors synthesises numerous bioactive amines such as serotonin, and cause the carcinoid syndrome. Rectal carcinoids present as asymptomatic submucosal nodules less than 2cm in size. In contrast to other carcinoid tumors, distant metastasis are rare(less than 15%). Malignant potential is seen almost exclusively in patients with tumors larger than 2cm. Transanal local excision suffices for tumors less than 2cm, with radical surgery reserved only for larger tumors and those with histological evidence of invasion of the muscularis propria. I have experienced two cases of rectal carcinoid tumors less than 2cm, which do not invade the muscular wall of the rectum. These carcinoids were treated with endoscopic polypectomy.

The incidence of the spondylolysis is very low before five years but during the adolescence the incidence increase. The etiologic factors of spondylolisthesis are congenital and acquired. The acquired factors are traumatic and stress fracture. Then there are many congenital properties of the etiologic factors for the spondylolisthesis because several congenital anomalies are combined with spondyolysis or isthmic spondylolisthesis. But there are few articles about relationship between the spondylolysis or isthmic spondylolisthesis and the congenital anomalies around the lumbosacral area. The purpose of this particular study is to search the etiologic factors that increase the degree of the vertebral slippage and relationship between the spondylolysis or ishtmic spondylolisthesis around the lumbosacral area. The plain X-ray and computed tomograms were taken in the 48 patients and the 26 control group that have not chronic low back pain previously. The patients were divided into three groups, such as the spondylolysis, grade I spondylolisthesis, and grade II spondylolisthesis. The parameters measured from the plain X-ray were the incidence of congenital anomalies and degree of vertebral slippate. The parameters measured from the computed tomograms were fact angles and the degree of pseudodisc. Tropism were present for 5 cases, and 16 cases at L3-4 facets, 12 cases, and 22 cases at L4-5 facets, 10 cases, and 28 cases at L5-S1 facets in control and patients group. There was no correlation between the presence or absence of tropism and the vertebral slippage. The sacralization was related with the vertebral slippage but other congenital anomalies were not related to the degree of vertebral slippage. There was a increment of vertebral slippage according to the increase of facet angle, but the linear correlation was absent on regression analysis. So statistical significance was absent among the control group and 3 patients groups. And the vertebral slippage was not significantly different among the groups that were divided according to the difference of facet angle. There was a linear correlation between the degree of the pseudodisc and the degree of vertebral slippage of square=0.60 on regression analysis. At present study, there was a trend of increase of vertebral slippage according to increase of facet angle and presence of the sacralization. And there was a linear correlation between the degree of the pseudodisc and the vertebral slippage.
