Povidone-iodine (PVP-I) is a widely used antiseptic for the skin and mucous membrane. It is known that povidone-iodine rarely induce contact sensitization. Recently, we have experienced 2 cases of povidone-iodine contact dermatitis. Standard patch test with PUP-I, PVP and, iodine were performed.

2.A Case of Eosinophilic Pustular Folliculitis.

A case of eosinophilic pustular folliculitis in a 46-year-old woman is presented. The patient showed repeatedly developed erythematous, indurated plaque studded with srnall yellowish pustules on flush area of the left cheek with blood eosinophilia and biopsy revealed histopathological findings of eosinophilic pustular folliculitis. The patient responded well to systemic administration of corticosteroid and dapsone.

3.The Clinical Study of Hemoptysis in Lung Disease.

BACKGROUND: Hemoptysis is a common clinical symptom responsible for 11% of admission to the hospital chest service. In KOREA pulmonary tuberculosis is still the most common cause of hemoptysis and the incidence of hemoptysis due to neoplasia has increased. Bronchoscopy and high resonance CT are essential for diagnosis of the cause of hemoptysis. We studied the causes, diagnostic tools and treatment treatment of hemoptysis. METHODS: We conducted a retrospective analysis of clinical profiles, radiologic and bronchoscopy findings and treatments of hemoptysis for 220 patients who were admitted to our hospital with hemoptysis between 1994 and 1998. RESULTS: The mean age at diagnosis was 49.3 years and male to female ratio was 2.1:1. The main causes were active pulmonay pulmonary tuberculosis in 72 cases(32.7%), inactive pulmonary tuberculosis with sequlae in 69 cases(31.4%) lung cancer in 43 cases(19.5%), bronchiectasis in 10 cases(4.5%), and chronicbronchitis in 10 cases(4.5%). The mean amount of hemoptysis for 24hrs was 120cc. The mean duration of bleeding was 25 days. The number of cases with a past history of pulmonary tuberculosis were 128 cases, in which 24 were relapsed tuberculosis cases, 25 chronic tuberculosis cases, 69 inactive tuberculosis cases, and 10 lung cancer cases. High resonance CT was the most useful method for structural etiologic evaluation of hemoptysis developed in patients with inactive tuberculosis, bronchiectasis and aspergilloma. sputum study and bronchofiberscopy were the confirmative diagnostic tools for active pulmonary tuberculosis and lung cancer. The treatments of hemoptysis medical in 152 cases(71.7%), bronchial artery embolization in 39 cases(17.8%), and operation in 9 cases(4.0%). The mean following up duration was 22.4 months. The overall outcomes of hemoptysis were controlled in 77 cases (43.5%), rebleeding in 100 cases (56.5%) and expired in 9 cases (4.0%). The outcomes of hemoptysis in pulmonary tuberculosis were controlled in 21.6%, rebleeding in 78.4%, and expire in 14.7%. CONCLUSION: The most common cause of hemoptysis was related with pulmonary tuberculosis. HRCT was an important diagnostic tool in AFB smear negative active pulmonary tuberculosis and inactive tuberculosis with sequelae. Early, proper management of pulmonary tuberculosis is important for prevention of hemoptysis in Korea.

4.A Study on the Angle Between the Abdominal Aorta and the Superior Mesenteric Artery by 3D Image Reconstruction.

SMAS(Superior Mesenteric Artery Syndrome) is a disease caused by a chronic obstruction of the duodenum(transverse portion), which is hardly detectable. However, it is known that when the superior mesenteric artery and abdominal aorta form a narrow angle, that the transverse portion of the duodenum is pressed down between the superior mesenteric artery and the abdominal aorta, and that this can lead to obstruction of the duodenum. Measuring this angle is a complicated job using conventional angiography, and results often turns out to be inaccurate. In addition, no attempt has been made to determine the value of this angle in Koreans. In this study, we conducted abdominal CT angiography using MIP(maximum intensity projection) on patients with no clinical evidence of SMAS in order to determine the angle at which the superior mesenteric artery branches from the abdominal aorta by using PC based software(Rapidia ver. 1.2) for the image reconstruction. Accordingly, we found that the mean angle between the abdominal aorta and the superior mesenteric artery was 50.05+/-15.87 degrees on average, and that the angle in men(53.64+/-16.57 degrees) is higher than in women(46.46+/-14.98 degrees). We hope that the angles determined by our study will serve as an important indicator for detecting SMAS.

7.QT and RR interval variability and spectral characteristics in response to physiologic autonomic stimulation.

Purpose: The purposes of this study were to compare the magnitude and phase between the RR interval and QT interval variability in the frequency domain. METHODS: Twenty four, 12-13 year old healthy males were randomly selected. At resting state and for 5 minutes, ECGs were obtained, and they were digitized to 1000Hz. After measurement of RR interval, QT interval variability was measured using template matching strategy. After normalization of the RR and QT interval time series, power spectral and cross spectral analysis were performed. From each of the time series, low- (0.04-0.15 hertz) and high- (0.15-0.4 hertz) frequency power were measured. From the phase spectrum, the phases and time lags between the two time series at each of the two frequency range were calculated. RESULTS: The average of RR interval and QT interval was 616.0+/-71.0, 364.0+/-47.0 msec, respectively. Their normalized low- and high- frequency power was 4.4+/-7.9 NU(normalized unit), 0.1+/-0.1 NU(p<0.005), and 11.0+/-30.0 NU, 0.3+/-0.3(NU, p<0.005), respectively. The phase differences and resulting time lags between the two interval were -0.5+/-0.4 pi radian(-0.9 seconds) and -0.2+/-0.3 pi radian(-0.4 seconds) in the low- and high-frequency range, respectively. CONCLUSION: During resting state, when compared to RR interval, QT interval oscillates in significantly lower amplitude in both low- and high- frequency ranges. However, the oscillations precede those of the RR interval 0.9 seconds and 0.4 seconds, respectively.

8.A Study on the Development of Self-help Program with Care Burden for Families with Demented Elderly Persons.

PURPOSE: This study were to understand the caregiving situation of families with the demented elderly and to develop a self-help program for them. METHODS: A self-help program was designed to increase caregivers' confidence and skills in problem solving. In addition, it was designed to provide information about the demented elderly. The self-help program of this study was composed of nine sessions. RESULTS: Family caregivers of elderly patients with dementia obviously experience high level of stress. which often leads the caregivers to negative outcomes such as depression, interference with social activities and health difficulties. That is, caring is stressful for caregivers because the demands of the caregiving situation tend to overwhelm the caregivers' coping resources. Therefore, families with demented elderly persons need assistances in order to be relieved from their care burden. CONCLUSION: The self-help program includes the understanding of dementia, the family caregiving experiences of the demented elderly persons, the time for self-care, the search for helping, the improving of interpersonal relations and communications.
