1.A Histopathological Study of 1817 Biopsies of Inflammatory Skin Diseases.

A clinical and histopsthological study were done on 1,817 biopsied inflammatory skin diseases among 58,386 out-patients in Dermatologic department of Korea University Hospital during the period of 24 years from January, 1962 to December, 1985. The results were summarized as follows : 1. According to pattern analysis(Ackerman AB), 1,605 cases(88.3%) of inflammatory skin diseases were classified into 9 patterns and by author's modified pattern analysis, additional 437 cases could be classified. 2. Of the 1605 cases which were classified by 9 pattern analysis, 1185 cases(78.8%) of inflammatory skin diseases were diagnosed as individual disease entity. The ratio of accordance between the histopathological diagnosis and clinical diagnosis on the biopsied inflammatory skin diseases was 44.5%, 3. The ratio of accordance between the histopathological diagnosis and clinical diagnosis by the patterns were '. Vasculitis(75.5%), Subepidermal vesicular dermatitis (73.3%), Nodular and diffuse dermatitis(68.2%), etc. 4. The diseases showing above the 70% accordance were scleroderma, erythema induratum, vitiligo, arthropod reaction, etc; and the diseases showing under the 30% accnrdance were erythema multiforme, erythema annulare centrifugum, neurodermatitis, etc.

3.Macrodactylism Associated with Neurofibroma of the Medial nerve: A Case Report

Macrodactyly is a rare congenital malformation characterized by an increase in the size of all the elements or structures of a digit or digits. The phalanges, tendons, vessels, subcutaneous fat, finger nails, and skin are all enlarged, but the metacarpals are not affected. The condition is most frequently found in the index and long fingers, and its etiology remains unexplained. Macrodactylism associated with neurofibroma of the median nerve in the left middle finger, in a 13 year old girl, is presented with a review of the literature, and diagnosis was confirmed by pathological examination. Chief complaints were enlargement of the left middle finger and a soft tissue mass in the left palm since birth. There was no change in motor and sensory function of the hand. X-ray showed enlargement of phalanges and a diffusely enlarged soft tissue shadow in the middle finger. We performed epiphysiodesis of the proximal, middle and distal phalanges of the left middle finger with exploration of the left median nerve to the wrist joint. An enlarged fibromatous median nerve, left side, was found with very large abundant fat lobules around it.

5.Cavernous Hemangioma of Lymph node: A case report with the review of literature.

Primary hemangioma of the lymph node is an extremely rare vascular neoplasm, and only four cases on the subject have been reported in the literature. We describe a case of cavernous hemangioma in an axillary lymph node that was incidentally found in 70-year-old woman who underwent a modified radical mastectomy for infiltrating duct carcinoma of the left breast. Brief review of the literature regarding vasoformative lesions occupying lymph node is made with special regard to differential diagnosis from reactive-proliferative processes and other true neoplasms.

6.A Case of Toxic Shock Syndrome.

We report a case of toxic shock syndrome in 17-year-old female patient with skin abscess. She had a skin abscess on left inner thigh about 6th days ago, but no treatment was done. And then she presented with the features of high fever, hypotension, generalized rash, myalgia and diarrhea. After the treatment with anti-shock measures, skin wound dressing and systemic antibiotics adminstration, she was completely recovered with generalized desquamation on 10th hospital day.

7.Hemagglutination Inhibition Test in Epidemic Encephalitis of Korea.

Analyzing HI tests of 110 cases of clinical Japanese Encephalitis in 1982, the following results are obtained. 1. The results of HI test are positive in 39 (35.5%), borderline positive in 19 (9.1%), negative in 14 (12.7%) and undetermined in 47 (43.7%) cases. 2. In 49 cases of positive HI test, 14 cases reveal the positive result on the first HI test requested in 5-27 days after the clinical onset of symptoms, and 35 cases show increasing HI titers on the follow-up studies. There is a tendency of increasing HI titers upto 3-4 weeks of onset and sustaining the value for more than two months. 3. In 35 cases with increasing titers on follow-up study, the highest titer is 1:80 in 5 cases, and the half of HI negative cases maintain that value throughout the course. 4. There is no significant statistical differences in clinical characteristics, laboratory and cerebrospinal fluid studies between the patient group of HI positive or borderline and group of HI negative or undermined, except mean hospital day and incidence of coma and death.

8.Absorption, Excretion and Antioxidative Effect of Rebamipide on Reproductive Organ.

OBJECTIVE: Rebamipide is a propionic acid derivative that has an action of the inhibition of superoxide production and removal of hydroxyl radical with the sperm incubation and cryopreservation. In the present study, to investigate whether rebamipide is useful to treat male infertility and sterility, the author observed the antioxidative effects in patient with male infertility and also examined its absorption and distribution in rat genital organ. METHODS: To measure the distribution of rebamipide in reproductive organ in the rat, carbon indicated rebamipide, 14C-OPC-12759, was orally administered to 10 Spraque-Dawley rats and its organ concentration in serum, liver, kidney, stomach, duodenum, colon, urinary bladder, seminal vesicle, epididymis and testicle were measured each time after 0.5, 1, 2, 4, 8 and 24 hours by using HPLC fluorescent method. The concentrations in semen were measured by HPLC fluorescent method in a sample of 50 infertile males who took 900 mg of rebamipide daily for 3 months. To measure the antioxidative effect and fertility rate for 3 months, each month before and after the treatment, sperm motility, vitality, the oxygen free radical formation, level of peroxidation, fetilizing capacity of semen sample which were obtained from infertile male patients by masturbation after at least 48 hours abstinence were analyzed by computer assisted semen analyzer, eosin-nigrosin stain, chemiluminescence, thiobarbituric acid method and hypo-osmotic swelling test. Simultaneously in a sample that wanted baby, both pregnancy and delivery were researched. RESULTS: The 14C-OPC-12759 concentration in the body of white rats was highest in gastrointestinal organ like stomach, smal intestine and duodenum and followed by genital organ like seminal vesicle, testis and epididymis. The rebamipide concentration in semen of infertile males was 220.77+/-327.84 ng/mL (SD) which showed a large deviation but it was higher than serum which was 126+/-76 ng/mL (SD). In the infertile males, after the treatment with rebamipide, the level of seminal reactive oxygen species (ROS) and lipid peroxidation have significantly decreased in duration of the treatment (p<0.05) and sperm vitality and fertilizing capacity except sperm motility significantly improved on post treatment of 2~3 months (p<0.05). Out of the 41 cases who hoped for pregnancy, 15 cases (36.6%) became pregnant and 12 cases had childbrith, 2 cases had miscarriage and one case is ongoing. The side effect was observed in 1 case (2%) which experienced diarrhea but it was lost spontaneously. CONCLUSIONS: We conclude from this study that rebamipide showed relatively high tendancy of absorption and excretion in the genital organ. In infertile males who had elevated ROS in semen, by specifically inhibiting the cell damage from the antioxidation, a way to preserve sperm motility, vitality and fertilizing capacity was confirmed.

9.Clinical charateristics of elderly patients with plmonary tuberculosis.

BACKGROUND: The prevalence of pulmonary tuberculosis among the elderly is increasing in Korea and in the developed countries due to the increased elderly population and their predispositions to chronic disease, poverty and decreased immunity. To define the characteristics of pulmonary tuberculosis in the elderly, we evaluated the clinical spectrum of pulmonary tuberculosis. METHODS: We analyzed 92 patients retrospectively that were diagnosed as active pulmonary tuberculosis over the age of 65. The analysis involved patient's profiles, clinical manifestations, coexisting diseases, diagnostic methods, anti-TB medications and their side effects, and treatment outcomes. RESULTS: The results were as follows:- 1) The ratio of male to female was 2.1:1(62:30 cases) 2) Chief complaints were a cough (47.8%),dyspnea(40.2%), sputum(38.0%), chest pain(12.0%), anorexia(10.9%), and fever(9.8%). 3) 38(41.3%) of cases had a past history of pulmonary tuberculosis. 4) The coexisting diseases were:-COPD, 25 cases(27.2%);pneumonia, 17 cases(18.5%);DM, 13 cases(14.1%);and malignancy, 10 cases(10.9%). 5) The positivity of Mantoux test(5 TU, PPD-S) was 82.7%. 6) Pulmonary tuberculosis was diagnosed using the following methods : sputum AFB (Acid Fast Bacillus) smear 42.4%, sputum TB(M.Tuberculosis) culture 15.2%, sputum TB PCR (Polymerase Chain Reaction) 10.9%, bronchial washing AFB smear 2.1%, chest radiology only 25.0%. 7) Locations of radiologic lesions were RULF, 50 cases;RLLF, 50 cases, mostly, then LLLF;26 cases were leastly involved. 8) The coexisting tuberculosis were endobronchial TB(8.7%), TB pleurisy(7.6%) miliary TB(5.4%), intestinal TB(2.2%), renal TB(1.1%) 9) The proportion of treatment regimen with 1st line drug and 2nd line drug were 92.3% and 7.6%, respectively. 10) The outcome of treatment were as follows:cured 31.5%, expired 13.0%, no return 47.8%, follow-up now 7.6%. CONCLUSION: The pulmonary tuberculosis in the elderly has atypical patterns with chronic coexisting diseases. Therefore, the possibility of pulmonary tuberculosis should be considered in elderly patients with pulmonary symptoms.

10.A case Report of Oculocraniosomatic Neuromuscular Disease with "Ragged-red-fibers".

This is a case report of oculocraniosomatic neuromuscular disease with "ragged-red fibers". The patients is 36 years old female who is revealing slowly progressive ptosis, external ophthalmoplegia, general weakness and mild right hemiparesis and hemianesthesia. The EMG findings are suggestive of diffuse myopathic process and the frozen muscle specimen from the left deltoid muscle shows the characteristic "ragged-red fibers" with classical myopathic changes.
