3.A Clinical Study on Benign Bone Tumors at Trochanteric Area of the Femur

Bone tumor in the trochanteric area is not infrequent and it is important to differentiate the tumors in this area for diagnosis, early settings of therapeutic plans and prognostic previsions. Twenty-one cases of benign bone tumors in the trochanteric area treated at Dept. of Orthopedi Surgery, College of Medicine, Ewha Womans University Hospital, from January 1975 to December 1983 were analysed and following results were obtained. l. of 21 cases, 10 cases were fibrous dysplasia, 6 cases were simple bone cyst, 3 were giant ce tumor and 2 were aneurysmal bone cyst. 2. Pathologic fractures were accompanied in 6 cases; 3 of fibrous dysplasia, 2 of simple bone cy: and 1 of aneurysmal bone cyst. 3. After treatment with curettage, bone graft and instrumentation, 3 cases recurred; 2 cases of fibrous dysplasia and 1 of simple bone cyst. 4. As prognostic factors, completeness of curettage, amount and source of bone graft were considered to be important.

6.Risk of Shunt Dependent Hydrocephalus after Treatment of Ruptured Intracranial Aneurysms : Surgical Clipping versus Endovascular Coiling According to Fisher Grading System.

OBJECTIVE: The amount of hemorrhage observed on a brain computed tomography scan, or a patient's Fisher grade (FG), is a powerful risk factor for development of shunt dependent hydrocephlaus (SDHC). However, the influence of treatment modality (clipping versus coiling) on the rate of SDHC development has not been thoroughly investigated. Therefore, we compared the risk of SDHC in both treatment groups according to the amount of subarachnoid hemorrhage (SAH). METHODS: We retrospectively reviewed 839 patients with aneurysmal SAH for a 5-year-period. Incidence of chronic SDHC was analyzed using each treatment modality according to the FG system. In addition, other well known risk factors for SDHC were also evaluated. RESULTS: According to our data, Hunt-Hess grade, FG, acute hydrocephalus, and intraventricular hemorrhage were significant risk factors for development of chronic SDHC. Coiling group showed lower incidence of SDHC in FG 2 patients, and clipping groups revealed a significantly lower rate in FG 4 patients. CONCLUSION: Based on our data, treatment modality might have an influence on the incidence of SDHC. In FG 4 patients, the clipping group showed lower incidence of SDHC, and the coiling group showed lower incidence in FG 2 patients. We suggest that these findings could be a considerable factor when deciding on a treatment modality for aneurysmal SAH patients, particularly when the ruptured aneurysm can be occluded by either clipping or coiling.

7.Renal Hypoperfusion Associated with Splenorenal Shunts in Liver Cirrhosis.

PURPOSE: To determine whether spontaneous a splenorenal shunt can be used as an imaging predictor of early renal hemodynamic changes in patients with cirrhosis. MATERIALS AND METHODS: The study included 82 cirrhotic patients and 41 control subjects. Three-phase CT was performed and CT attenuation values (Hounsfield units) of the renal cortex in three phases were measured to evaluate renal perfusion. Likelihood ratio tests for trend were conducted for age, presence of ascites, and Child's grade. RESULTS: The mean CT attenuation values of the renal cortex in cirrhotic patients were significantly lower than the values of control subjects in three phases: 153.3 +/- 37.9 versus 173.3 +/-25.2 in the arterial phase, 172.6 +/- 41.0 versus 197.6 +/- 26.5 in the portal phase and 136.9 +/- 26.0 versus 152.7 +/- 20.0 in the delayed phase, respectively. The mean CT attenuation value of cortices in patients with renal hypoperfusion was 119.9 +/- 11.8 in the portal phase. Child's class C (aOR: 58.4, 95% CI: 3.6-956.2; p < 0.01) and the presence of a renal shunt (aOR: 7.5, 95% CI: 1.8-30.5; p < 0.01) were associated with renal hypoperfusion. The incidence of renal hypoperfusion was associated with Child's grade (trend: p < 0.01), and not with the grade of ascites or age. CONCLUSION: A dilated spontaneous splenorenal shunt may be a risk factor for renal hypoperfusion in cirrhosis.

The choice of equipment for the administration of anesthesia depends to a considerable extent upon personal preference. Underlying factors must guide this choice, however. Equipment is suited to the individual patient, with especial care to avoid resistance, dead space, fatigue, and trauma, since all of these insults are easily imposed upon small patients. As we have had good experience in pediatric anesthesia with the Ambu valve and the results were satisfactory, we report with discussion of references.

9.Effects of Body Mass on Bronchial Hyperresponsiveness and Atopy in Childhood Asthma.

PURPOSE: For the past two decades, there has been a significant increase in the prevalence of both asthma and obesity. Obesity may be associated with increasing childhood asthma and atopy. So, we performed this study to know the effects of body mass on bronchial hyperresponsiveness (BHR) and atopy in childhood asthma. METHODS: The subjects were, 340 children aged 6-15 years with asthma, visited the Pediatric Outpatient Clinic. There, their heights and, weights were measured and they underwent, pulmonary function tests (PFT), BHR to methacholine and skin prick tests. After determining body mass index (BMI), they were divided into four groups by BMI percentile curve based on gender and age (underweight < or =15th percentile, normal weight 15.1-85th percentile, overweight 85.1th-95th percentile, obesity > or =95.1th percentile). Atopy, allergen sensitization, BHR, respiratory symptoms, PFT were compared between each group. RESULTS: In all subjects, there were 70 (20.6%) and 73 (21.5%) cases in the overweight and obese groups, respectively. Increasing BMI was significantly associated with a higher prevalence of snoring,(P=0.004) wheezing in the perceding three months (P=0.002) and BHR- positive,(P=0.003) but not atopy.(P=0.075) Among the groups classified by BMI percentile, there were significant differences in FEV1 and FEV1/FVC.(P=0.027, P=0.033) Being overweight and/or, obese were significant risk factor for BHR, though only among females (OR 5.1; 95% CI 1.6-16.5, 4.9; 1.8-13.4) not male (OR 1.1; 95% CI 0.5-2.1, OR 1.7; 95% CI 0.7-2.0). CONCLUSION: Among girls, a higher BMI might be a risk factor for BHR. Higher BMI is not associated with atopy on either boys or girls.

10.Evaluation of Elastic Lift for Neck Rejuvenation.

BACKGROUND: The neck is not only the first area to show aging but it also represents the persona of the individual. The understanding of the aging process of the neck is essential for neck rejuvenation. The purpose of this study was to evaluate the outcomes and safety of elastic lift in relation to neck region lifting. METHODS: Forty-seven patients with sagging and laxity of the neck skin treated by elastic lift were enrolled in this study from November 2015 to February 2016. To evaluate the safety of elastic lift, a retrospective chart review was conducted. We used an automatic skin diagnosis system to evaluate the efficacy of elastic lift. RESULTS: Forty-five patients were female and 2 patients were male. The mean age of the patients was 45.3 years and their mean follow-up period was 3 months. There were no major complications requiring the removal of the elastic thread during the follow-up period. The median skin laxity grade score was 5 (4-5) preoperatively; the median skin laxity grade score was 3 at 3 months postoperatively. In comparing the skin grade score at these two points in time, there was a statistically significant difference (P < 0.01). CONCLUSIONS: Neck rejuvenation using elastic lift is a safe and effective procedure in selected patients with moderate sagging and laxity of the neck skin.
