1.Tuberculous Uveitis in a Hemodialysis Patient.

Tuberculous uveitis is a rare manifestation of tuberculosis infection. Although early diagnosis is important to saving the sight of patients, it is difficult. In dialysis patient, the diagnosis of tuberculosis is often delayed because of non-specific symptoms and extrapulmonary involvements. We report a case involving a 37-year-old man receiving hemodialysis who was presented with persistent fever and left- sided blurred vision. At ophthalmic examination, active papillitis, vitritis, and peripapillary retinal detachment were observed. Neck computed tomography showed multiple necrotic lymphadenopathies (LAPs) in right lateral neck, which was proven as tuberculous lymphadenitis on histological examination. After anti-tuberculosis therapy, fever was subsided and LAPs were improved. Visual acuity was also improved.

It has been known that right side bypass tract ablation is more difficult and has higher recurrence rate than that of left side bypass tract, and often associated with atrioventricular block in patients with septal bypass tract. Multielectrode basket catheter (MBC) allows simultaneous rapid acquisition of numerous electrical signals generated by a selected surface area or volume of myocardium. We experienced two cases of successful catheter ablation of atrioventricular reentrant tachycardia by right septal bypass tract which were mapped by MBC with greater ease.

6.A Case of Generalized Eruptive Syringoma.

Syringoma is a common, benign, adnexal tumor of eccrine origin. Generalized eruptive syringoma, however, is a rare variant of syringoma which presents as a widespread papular eruption. It occurs in large numbers on the anterior parts of the neck, chest, abdomen, axillae, periumbilical region, or sometimes proximally on the extremities. It is as frequent in prepubertal as in postpubertal age. A 20-year-old woman presented with multiple asymptomatic papules on the face, neck, abdomen, back, upper and lower extremities. Family history showed that her mother had similar lesions on the abdomen. Histologically, numerous ducts are embedded in a fibrous stroma and their walls are lined by two rows of epithelial cells. We report a case of generalized eruptive syringoma.

7.Measurement of the excessive stimulus time after the sensory threshold level during electric pulp testing.

Use of electric pulp testing elicits painful response in vital teeth. In this study, we examined the excessive time from pain feeling to stimulation disconnection in clinical situation. D626D (Parkell Inc., USA.) scan type electric pulp tester was used in total of 23 young healthy individuals. Each of the right central incisors and first premolars were used as testing teeth. Stimulation disconnection was achieved by EMG in anterior belly of digastric muscle, finger span, and voice and the excessive stimulation time over the sensory threshold was recorded. As a result, we found that the short responses before the stimulation disconnection appeared following order; EMG, finger span, and voice. The EMG disconnection is suggested to be used to reduce the excessive stimulus time in electric pulp testing.

Intravascular papillary endothelial hyperplasia (IPEH) is a rare benign vascular tumor. It represents a reactive proliferation of endothelial cells arising in an organized thrombus. And it is histologically characterized by the development of endothelial-lined papillary projections in a vascular lumen. It occurs most frequently in the head and neck region or on the extremities. We report a case of intravascular papillary endothelial hyperplasia developed on the finger.
