1.1000 Cases of Multi-Staged Treadmill Exercise ECG Test: Is It Safe?.

We examined 984 objects for exercise stress test, using CASE I(computer assisted system for exercise). They exercised according to Bruce's protocol, Among them, the mal were 613 and the female were 371. (Male:Female;17:1) The 5th and 6th decades occupied 61% of total objects. The purpose of test was evaluation of chest pain in 84% of total cases. The most frequent end point of test was dyspnea(42%) and chest pain was observed in 13% of total cases. Most of all objects above 8-9 METS. Positive results of exercise stress tert were obtained in 23%. As complications, ventricular tachycardia occurred in 7 cases, supraventricular tachycardia; 8, high degree VPB's; 6, sudden cardiovascular collapse;15, The complication rate was 0.36% but morbidity and mortality were 0. All complicated cases recoved spontaneously only by rest.

3.Dyschondroplasia and Hemangiomata: Maffucci's Syndrome

Yonsei University College of Medicine, Seoul, Korea Maffucci's syndrome is characterized by dysccondroplasia with hemangiomata. Since Angelo Maffucci (1881) first described this entity, 105 cases and some more were reported in the literature till 1976. We recently observed a patient suffering from this disorder. The patient is 21 year-old girl, who has multiple hemangiomes on her right foot, right buttock, left wrist and enchondromas on both proximal phalanx, distal phalanx of both great toes and proximal, middle phalanx of right second toe. The disease started at age of 11 and these lesions grew rapidly last 3 years. The diagnosis was based on clinical, radiological and histological findings. Because of its rarity, we report it with brief review of literature.

4.The Lsolation of Organism and Sensitivity Test in Conjunctivitis Under 1 Year old of Age.

Neonatal and Infantile conjunctivitis are common disease in ophthalmologic outpatient department but, a study of causative organisms and antibiotics sensitivity tests are insufficient till now We divided 117 babies(117eyes) having conjunctivitis into neonatal and infantile group who were visited Sung-Ae General Hospital from Jan, 1989 to May, 1990. We performed bacterial culture and antibiotic sensitivity test. The results were as follows; 1) Culture positive rates are 65% in neonates and 87% in infants. There is no sexual difference of ratio. 2) The incidence of isolated organisms is Staphylococcus aureus, Coagulase negative staphylococcus and Streptococcus. 3) Cephalothin is most sensitive antibiotics and Chloramphenicol shows relatively lower sensitivity.

5.The Efficacy of Mitomycin-C on Glaucoma Filtration Surgery in Rabbits.

The study was carried out to investigate the efficacy of subconjunctivally injected mitomycin-C (MMC) on glaucoma filtration surgery(GFS) in rabbit eyes. Each of 0.05mg, 0.1mg and 0.2mg MMC were injected subconjunctivally into the rabbit eye according to groups I, II, III respectively following GFS. The IOP were returned to preoperative level at postoperative 2 weeks in control group, and at postoperative 12 weeks in group I and II. Histologically the fibrotic reaction appears less intensively in the order of group III, II, I and control group at 2 weeks after GFS. At postoperative 12th week, fibroblasts are more frequently oberved in group I and II than control group. But the proliferation of collagenous and reticulin fibers are less in group I and II than control group. The least amount of fibroblasts, collagenous and reticulin fibers are found in group III. It is suggested that subconjunctival injection of lower concentration of MMC would be effective approval as an adjunctive method to improve more success rate of GFS from this study.

6.Detection or Interleukin 2 In The Urine of Patients with Superficial Bladder Tumors after Intravesical BCG Therapy.

Intravesical Bacillus Calmette-Guerin (BCG) therapy is an effective immunotherapy used for the treatment and prophylaxis of superficial bladder cancer. The mechanism through which BCG exerts its antitumor activity is not fully understood. However, several studies suggest that antitumor mechanism of BCG is associated with T-cell mediated immunity and lymphokines. In this report, the detection of IL-2(a lymphokine produced in response to BCG) in urine and serum specimens of patients treated with intravesical BCG is reported. Urine specimens of 10 superficial bladder cancers were obtained prior to BCG instillation, 4, 8, and 24 hours afterwards. Serum specimens of same patients were obtained before and 8 hours after BCG instillation. No change of serum IL-2 before and after BCG instillation were observed. Urinary IL-2 production increased significantly 4 and 8 hours (p<0.05) and decreased 24 hours after BCG instillation. Urinary IL-2 levels 24 hours after BCG instillation were similar to baseline values. Between urinary IL-2 of healthy men and cystitis patients were similar. These results suggest that BCG induce IL-2 production with highest level at 4 hours after BCG instillation and IL_2 might play a important role in antitumor mechanism of BCG. However, further studies are necessary to elucidate the antitumor mechanism of BCG.

7.The Relationship with the Density of Lens Nucleus and Phacoemulsification Time.

To investigate the relationship of the density, thickness of lens nucleus and cortex with phacoemulsification time, we measured the preoperative density and length of lens nucleus and cortex of 100 eyes with a Scheimpflug camera. We performed phacoemulsification on the same phacoemulsification technique using a phacoemulsifier aspirator by the same surgeon. The preoperative mean values of the density of lens nucleus and cortex were 127. 09+/-57.44 and 30. 33+/-15. 24, respectively. The mean values of the thickness for both were 3.05+/-0.42mm and 0.65+/-0.17mm, respectively. The mean value of phacoemulsification time was 92.68+/-56.98 seconds. The phacoemulsification time was increased significantly as the density of lens nucleus was increased (P<0.0001). However, the thickness of the lens nucleus and cortex and the density of cortex had no significant effect on the phacoemulsification time (p=0. 235, p=0.811). Preoperative evaluation with a Scheimpflug camera is particularly valuable for cataractous patients with corneal endothelial disease deciding on phacoemulsification.

8.The Effect of Extracapsular Cataract Extraction in Patients with Chronic Angle-Closure Glaucoma combined with Cataract.

14 eyes of 11 patients with chronic angle-closure glaucoma(CACG) undergoing extracapsular cataract extraction(ECCE) with posterior chamber intraocular lens(PCIOL) implantation(group 1) and 14 eyes of 12 patients without glaucoma undergoing ECCE/PCIOL(group 2) were studied to assess the effect on anterior chamber depth, anterior chamber angle, and intraocular pressure (IOP). We measured the anterior chamber depth and angle using the EAS-1000 Scheimpflug camera. Following ECCE/PCIOL in group 1, the anterior chamber depth increased 1.00 +/- 0.25mm(p=0.0001), the anterior chamber angle widened 15.36 +/- 5.61 degrees(p=0.0001), and the postoperative IOP drop of 4.7mmHg(23.9%) showed statistical significance. Group 2 showed the increase of anterior chamber depth 0.22 +/- 0.44mm(p=0.0964), the widening of anterior chamber angle 2.51 +/- 2.65 degrees(p=0.0047), and the decrease of IOP 0.9mmHg(6.2%, p=0.0933). The effects on anterior chamber depth, anterior chamber angle, and rap were statistically significant between the two groups. Based on these results, we recommend that eyes with cataract and CACG may be considered for ECCE/PCIOL implantation as a first procedure rather than combined cataract and filtering surgery.

9.Clinical Comparision of the ProTon and the Goldmann Applanation Tonometer.

The recently developed ProTon tonometer is a small hand-held instrument which has less chances of infectious ocular diseases transmission and corneal abrasion than other types of tonometer. We compared the intraocular pressures taken by the ProTon tonometer with those by the Goldmann applanation tonometer in 290 eyes of 145 patients. The results were as follows: 1. The difference of the intraocular pressures between the ProTon tonometer and the Goldmann tonometer is less than 3mmHg in 91.03% of the estimation, and less than ImmHg in 64.83%. 2. The average intraocular pressure measured by the ProTon tonometer is lower than that by the Goldmann applanation tonometer. In the higher intraocular pressure zone, the difference of intraocular pressures between the two is more increased. 3. The correlation between the intraocular pressures measured by the ProTon tonometer and those by the Goldmann tonometer is very high(correlation coefficient 0.98). According to the above results, we conclude that the ProTon tonometer can be used more accurately and conveniently for both patient and examiner than other classic instruments.

10.Oculomoter Nerve Palsy Due to Arachnoid Cyst in the Newborn Infant.

A 22 days old newborn infant with left total ophthalmoplegia involving manifestations of palpebral ptosis was examined and treated. An arachnoid cyst located in the temporo-parietal lobe as the very rare incidence of etiological lesion was detected on brain C/T. Following the successful cystoperitoneal shunt, the ophthalmoplegia improved clinically.
