1.A Mycoparasitic Ascomycete Syspastospora parasitica on the Entomopathogenic Fungus Paecilomyces tenuipes Growing in Bombyx mori.

While studying Bombyx mori "Zipnuae" inoculated with an entomopathogenic fungus, Paecilomyces tenuipes, a mycoparasitic hymenoascomycete with a long neck was isolated from the silkworm rearing room. The fungus was identified as Syspastospora parasitica which has been recognized as a hyperparasite on various moniliaceous hyphomycetes. A descriptive illustration is presented in this study.

2.Primary Signet Ring Cell Carcinoma with Widespreand Metastasis.

We present an autopsy case of signet ring cell carcinoma of urinary bladder which underwent widespread metastsis through nearly entire organ. The patient, 45-year old male, suffered from hematuria, dysuria and flank pain for 6 months. Under the diagnosis of the signet ring cell carcinoma by the cystoscopic biopsy, radical cystectomy and ileocutaneous ureteral division were performed. But he died after 11-months after the diagnosis. The autopsy confirmed the widespread metastasis at the lung, kidney, adrenal gland, parathyroid gland, heart, stomach, ileum, spine, rib and meninges. The preveously resected urinary bladder discolosed the well defined fungating mass at the dome and anterio-lateral portion without apparent evidence of the remnant of the urachus. Histologically, the signet ring cells were predominent and focally mucinous pool floating the signet ring cells was noted. We could find several foci of the well differentiated adenocarcinoma and poorly differentiated adenocarcinomas. The above findings may reflect that the origin of carcinoma may be from the multipotential transitional cell epithelium.

3.Benign Osteoblastoma: A Case Report

Osteoblastoma is a rare benign bone tumor and is characterized hitologically by an abundant number of osteoblasts and by the presence of s large amount of osteoid tissue in a highly vascular matrix. The lesion is frequently encountered in children and adolescents with most of the cases occuring between the ages of 10 and 25 years. There is significant sex prevalence (M.: F.=2: 1). Up to now there has been few reports of a benign osteoblastoma occuring in the cervical spine. This report describes a boy of a benign osteoblastoma which involved the cervical vertebral arch and spinous process, presenting with pain and limitation of motion. He was given en-bloc resection of tumor mass and confirmed by histological examination. Postoperation result was good and he goes school without any problem in about 12 months of follow up.

7.Inflammatory Pseudotumor of the Kidney.

Inflammatory pseudotumor, originally described in the lung, is a relatively rare tumor-like lesion that occurs in various organs and tissues. It is usually well demarcated from the surrounding tissue, however it can be unfortunately resected as a malignant tumor. A few inflammtory pseudotumor in the kidney have been reported in English literature, but there have been no reports in Korea. We report a case with inflammatory pseudotumor of the kidney. A 48 year old woman had an intermittent flank pain on the right side. An ultrasonographic study suggested a renal cell carcinoma and a nephrectomy was done. Grossly, there were two separate masses with a well demarcated yellowish appearance, measuring 2.3 cm and 1.3 cm in diameter, respectively. Histologically, they were composed of smooth muscle actin positive spindle cells and a large number of foamy histiocytes, lymphocytes, and plasma cells in the fibrotic backgound.

8.Perception Types of Nursing Students to Clinical Education: Q Methodological Approach.

The purpose of this study is to explore perception types and to understand the nature of experience of nursing students' clinical education by using the Q methodology. A Q sample was developed through a review of the literature and descriptions about nursing students' experience in clinical practice. Thirty-six statements made up the finalized Q sample. The P sample consisted of 33 third grade nursing students in K university. Q statements were written on separate cards and were given to the 33 subjects to sort according to degree of agreement or disagreement. The Q-sorts by each subject were coded and analysed with the Quanl PC program. A a result, three major perception types, namely, 'alienation of ideal and reality'

10.Anticardiolipin Antibody (ACA) and Lupus Anticoagulant (LA): Association with Vascular Access Occlusion in Hemodialysis (HD) Patients.

OBJECTIVES: Anticardiolipin antibody (ACA) and lupus anticoagulant (LA) are acquired antiphospholipid antibodies (APAs), which are regarded as important risk factors far vascular thrombosis and recurrent fetal loss. Although the clinical relevance of APAs in dialysis patients is uncertain, recent studies have suggested that APAs are involved in bioincompatibility and thrombogenic complications in hemadialysis (HD) patients. METHOD: We performed a cross sectional study of ACA and LA in 50 stable HD patients and their 68 vascular accesses (52 native arteriovenous fistulae and 16 synthetic arterovenous grafts), with the analysis of factors associated with the presence of APAs and the retrospective evaluation of vascular access occlusion (VAO). LA was assessed by platelet neutralization method whereas IgG-ACA was measured by a solid phase ELISA. Values higher than 23GPLU/ml (IgG phospholipid units) were considered to be positive for IgG-ACA and positive values for LA was more than 8 seconds in prolongation of the clotting time with human platelet lysate. Vascular access survival was assessed by Kaplan- Meier method, RESULTS: The mean age of the subject (M:F 21:29) was 46 years and the mean duration of hemodialysis was 49 months. The frequency of VAO in entire subjects was 0.45+/-0.98 episodes/patient year. The median value of IgG-ACA was 16.0 GPLU/ml with a distribution from 2.7 to 46.1GPLU/ ml. The median titer of I.A was 4.5 (3.1-45.6) seconds. Fourteen patients (28%) were found to have at least one episode of VAO. In spite of comparable clinical and biochemical data according to the presence of VAO, the titers of IgG-ACA (13.6+/-7.7 vs, 20.3+/-8.7GPLIJ/ml, P<0.05) and LA (4.5+/-2.9 vs. 11.7 +/-12.6sec, P<0.05) were significantly higher in VAO group. Six out of 50 patients(12%) had an increased titer of IgG-ACA and LA was found in 11 patients(22%). No patients were positive for ACA and LA simultaneously. There was no significant difference in sex, etiology of ESRD, diabetic status, the dosage of heparin during HD or the amount of erythropoietin administered according to the presence of APAs. We could not find any significant correlation between the titer of APAs and age, duration of dialysis, blood pressure, platelet count and biochemical parameters. In the patients with positive ACA, the frequency of VAO was 1.05+/-0.12 episodes/patient year, which was significantly higher than patients without ACA (0.33+/-0.17 episodes/ patient year, P<0.05). In the patients with the presence of LA(1.06+/-0.43 vs. 0.12+/-0.06 episodes/ patients year, P<0.01). The median vascular access survival time in IgG-ACA positive patients (32.7 months) was significantly decreased compared to 66.8 months in IgG-ACA negative group. CONCLUSION: Our data suggest that the presence of APAs (ACA and/or LA) affects the event-free vascular access survival in HD patients. Therefore the evaluation of APAs status have to be included in the diagnostic strategies for the patients with recurrent VAO. Further studies are necessary to explore the pharmacologic intervention method to decrease APAs and prevent VAO in HD patients.
