1.The Effect of Preoperative Subconjunctival Triamcinolone Administration on Glaucoma Filtration.

The trabeculectcmies were performed on 12 eyes with uncontrolled glaucoma which had not been treated by the maximum use of tolerated antiglaucoma medication. We assumed that all of these eyes would be at increased risk by episcleral cicatricial closure in the patient with one of the following cases; congenital glaucoma, age of 40 year or less, previously failed glaucoma filtration surgery, aphakia and uveitis. A standard operation was performed except tbat triamcinolone acetonide(4mg) was injected subconjunctivally at the intended trabeculectcmy site one week before surgery in 10 eyes, and cn the day of surgery in 2 eyes. During the followup period of 5 months or more, 11 of 12 eyes had an intraocular pressre(IOP) of 19mmHg or less. The average IOP for these 11 eyes was 14.5 +/- 3.4mmHg(mean +/- SD), and only 2 of the 11 eyes required antiglaucoma medication. One eye was a surgical failure. All eyes with controlled IOP showed micro cystic filtration blebs on slit lamp examination. The postoperative visual acuity was within one line of the preoperative level in all eyes, except for the one surgical failure. The use of triamcinolone acetonide does not seem to have added any additional risks to the postoperative period.

2.CSP0510 Lotion as a Novel Moisturizer Containing Citric Acid and Trisodium Phosphate Relieves Objective and Subjective Symptoms of Atopic Dermatitis.

BACKGROUND: Moisturizers with anti-inflammatory or anti-itch activity should be developed for the safe and effective management of atopic dermatitis (AD). OBJECTIVE: This study evaluated the efficacy of a newly developed moisturizer, CSP0510 lotion (Twolines Inc., Korea), containing citric acid (CA) and trisodium phosphate (TSP) as active ingredients, in mild to moderate AD. METHODS AND RESULTS: CSP0510 lotion applied twice daily for 4 weeks to eczematous lesions improved objective and subjective (itch) symptoms of AD. The physician's global assessment (PGA) score for objective symptoms decreased from 2.5±0.6 before application to 1.3±0.5 after application in the CSP0510-treated group (n=42, p<0.001). Also, the PGA score decreased from 2.3±0.6 to 1.9±0.5 by vehicle-treated (without CA and TSP) control group (p=0.001), but there was no statistical difference between CSP0510-treated and vehicle-treated groups (p=0.089). The visual analogue scale score for itch decreased from 4.8±1.3 to 2.0±0.9 in the CSP0510-treated group (p<0.001), and from 4.6±1.1 to 3.5±0.9 in the control group (p=0.075), showing a statistical significance between two groups (p=0.002). Our results in humans were further supported by in vitro and animal experiments. In HaCaT cells treated with compound 48/80 (7.5 µg/ml), CA:TSP (1:1, vol:vol) synergistically suppressed the compound 48/80-induced upregulation of thymic stromal lymphopoietin, nerve grow factor, and calcitonin gene-related peptide. Application of CSP0510 to the dorsal skin of hairless mice for 3 weeks suppressed the oxazolone-induced allergic skin inflammation. CONCLUSION: In conclusion, CSP0510 lotion has anti-itch and anti-inflammatory activity in the skin, which improves both objective and subjective symptoms of AD.

3.A Clinical Evaluation of Combined Trabeculectomy and Intracapsular or Extracapsular Cataract Extraction.

Trabeculectomy with intracapsular cataract extraction(Group I) on 14 eyes of 14 patients, and trabeculectomy with extracapsular cataract extraction(Group II) on 17 eyes of 17 patients were perfonned during the period of March 1986 to March 1988. The follow-up periods ranged from 3 monthes to 5 monthes. The mean intraocular pressure was reduced by approximately 54%, from 35 mmHg preoperatively to 16 mmHg postoperatively Group I, and was reduced by approximately 50%, from 34 mmHg preoperatively to 17 mmHg postoperatively in Group II. The visual acuity improved in all cases of both groups. The mean intraocular pressure reduction, operative and postoperative complications, and failure rates were not significantly different between Group I and Group II.

4.Two Cases of Malignant Melanoma in the Bulbar Conjunctiva.

Two cases of malignant melanoma which are located in the bulbar conjunctiva are presented. A case of 56 years old male revealed melanosis in his medial palpebral conjunctiva and wedge shaped resection was performed. The other case of 76 years old male had dark brownish strawberry like pedunclated mass in his lateral bulbar conjunctiva and limbal area with history of progressive enlargement. This tumor mass was excised radically. Above two cases were confirmed as malignant melanoma histopathologically.

5.The effect of hyaluronic acid on expression of extracellular matrix proteins and bone formation in rabbit mandibular distraction osteogenesis.

Distraction osteogenesis is a new bone formation technique. There is a advantage of the environmental adaptation when distraction force is applied to the gap between osteotomy lines. But it has a disadvantage of long-term wearing of the appliance and long consolidation period. Therefore we make an effort to reduce it and repair normal function. Extracellular matrix proteins have a function to control the cellular growth, migration, shape and metabolism. In these, hyaluronic acid is a member of polysaccharide glycosaminoglycans (GAGs) and has a important function as bone formation and osteoinduction property. PURPOSE: In this experimental study in rabbit mandibular distraction osteogenesis, we investigated the bone enhancing property of hyaluronic acid and the expression of extracellular proteins such as osteocalcin and osteonectin. MATERIALS AND METHODS: The experimental study was carried out on 24 Korean male white rabbits (both mandibular body, n=48). Distraction group was divided to distraction experimental (A, n=16) and distraction control (B, n=16) by the application of hyaluronic acid (Hyruan, LGCI, Seoul, Korea). Normal control group (C, n=16) was only osteotomized. After 5 days latency, distraction devices were activated at a rate of 1.4 mm per day (0.7 mm every 12hours) for 3.5 days. Animals were sacrificed at postoperative 3, 7, 14, and 28 days. HandE stain and immunohistochemical stain was done on decalcified section. Additionally RT-PCR analysis was done for the identification of the expression of osteocalcin and osteonectin. RESULTS: The bone formation in distraction experimental group was much more than that in distraction and normal control group at postoperative 28 days. In immunohistochemical stain, osteocalcin was enhanced at only postoperative 14 days, but osteonectin was not different at each post-operation days. In RT-PCR analysis, osteocalcin was not different at each post-operation days, but osteonectin was strongly expressed in distraction experimental group at postoperative 7 days. The expression of osteocalcin and osteonectin was elevated during the healing period. CONCLUSION: We found the good bone formation ability of hyaluronic acid in distraction osteogenesis through the immunohistochemistry and RTPCR analysis to osteocalcin and osteonectin, known as a bone formation marker. The application of hyaluronic acid in distraction osteogenesis is a method to reduce the consolidation period.

Epithelial-myoepithelial carcinoma is a rate low-grade malignant salivary neoplasm that usually occurs in the parotid gland but can also arise in minor salivary glands. We report a case of a primary epithelial-myoepithelial carcinoma of the lung neoplasm. The patient was 48-year-old women who presented with dry cough of 1 month duration. A right middle lobe endobronchial lesion was identified bronchoscopically. The bilobectomy of RML & RLL was performed, the pathologic result was epithelial-myoepithelial carcinoma.

7.A Female Patient with Schizophrenia Treated with Risperdal Consta during the Entire Period of Pregnancy.

Long acting noble antipsychotic, Risperdal Consta, has been reported to be efficacious and well tolerated in treatment of schizophrenia. However there was no report about the safety in using during pregnancy. Here we present the case of female patient with schizophrenia treated with risperdal consta during the entire period of pregnancy and delivered normal child. This case report highlights the benefit/risk of risperdal consta during pregnancy.

8.Changes of Body Weight, Blood Glucose and Lipid Levels in Patients with Schizophrenia after Long-Term Clozapine Treatment.

OBJECTIVE: This study was designed to investigate the effects of long term clozapine treatment on changes of weight, glucose and cholesterol levels and their relation to clozapine and its metabolite blood levels in outpatients with chronic schizophrenia. METHODS: Among outpatients diagnosed with schizophrenia according to the DSM-IV criteria, 19 consented subjects receiving long-term treatment of clozapine, its dosage level had been constant for last one month, were selected for the study. The serum level of clozapine, metabolites as well as body weight, BMI, glucose level, cholesterol level, insulin, and c-peptide were gathered and analyzed before and after the use of clozapine. RESULTS: Glucose increase after clozapine treatment was statistically meaningful but it was due to two patients who got diagnosed with diabetes. Glucose levels of other patients are all below 120 mg/dl. Cholesterol level showed significant increase after the treatment. Weight and BMI changes over the treatment are not statistically meaningful overall, but 8 out of 17 showed more than 7% increase. The changes of weight and BMI were positively correlated with weight and BMI of pre-treatment. Mean serum level of clozapine, metabolites were not correlated with glucose, cholesterol level, insulin, and C-peptide. CONCLUSIONS: Results indicate that long term treatment of clozapine is correlated with increase of glucose and cholesterol level and weight gain of the patients. Clinicians should be aware of the potential risks of diabetes, hyperlipidemia, and weight gain in patients taking clozapine.

9.Botulinum Toxin Treatment on Upper Limb Function in School Age Children With Bilateral Spastic Cerebral Palsy: One Year Follow-up.

OBJECTIVE: To prospectively investigate the long-term effects of botulinum toxin treatment on the upper limb function and performance of school age children with spastic bilateral cerebral palsy, who have limitations in performing activities of daily living and school activities, due to spasticity of the upper extremities. METHODS: Botulinum type A toxin (BoNT-A) was injected into 24 spastic upper limbs of 15 children. We used a Modified Ashworth Scale and a Modified Tardieu Scale for the evaluation of upper limb spasticity, and Quality of Upper Extremity Skills Test (QUEST), Canadian Occupational Performance Measure (COPM), and Test of Visual-Motor Skills-Revised (TVMS-R) for the evaluation of upper limb function and performance. RESULTS: Upper limb spasticity continuously decreased until the end of the one-year follow-up. Upper limb function on QUEST and COPM showed the best performance at 3 months and deteriorated slightly, but still showed a significantly better performance at 9 and 12 months than at pre-injection. In more functional nine subjects who could perform TVMS-R, the performance enhancement effects remained constant after 12 months, suggesting that the reduced spasticity led to the learning effect acquired by the repeated use of the affected upper limb. CONCLUSION: For school age children with bilateral spastic cerebral palsy whose upper limb functions are important, BoNT-A injections seem to be of help in the performance of school activities and activities of daily living.

10.Reversible Pituitary Dysfunction in a Patient with Cushing's Syndrome due to Adrenal Adenoma.

A 45-year-old woman who complained of weight gain and irregular menstruation was diagnosed as having Cushing's syndrome due to a 3 cm sized left adrenal adenoma. She underwent left adrenalectomy, and she also underwent combined anterior pituitary tests before and 9 months after the surgery. The growth hormone and adrenocorticotropic hormone levels failed to respond to hypoglycemia before the surgery, but their responses recovered after the surgery. Cortisol and thyroid stimulating hormone failed to respond to hypoglycemia and thyrotropin releasing hormone (TRH) before the surgery, respectively, but these were improved after the surgery. Luteinizing hormone, follicle stimulating hormone, and prolactin adequately responded to gonadotropin-releasing hormone and TRH, respectively, before and after the surgery. However, the basal levels of these hormones were higher after adrenalectomy, suggesting that hypercortisolemia had a significant influence on all the pituitary hormones.
