1.A Clinical Study of 14 Cases of Sarcoma of the Uterus.

Uterine sarcoma whieh is originated from uterine muscle and/or connective tissues, is very rare malignant tumor and is the most lethel of all primary uterine tumors. This study was undertaken to correlate the clinieal findings, diagnoses, managements and ultimate outcome of each particluar grouy of uterine sarcoma at Depart,ment of Obstetrics and Gynecology in Korea University Hospital. The reaults were as follows, 1. The distribution of uterinesarcomaby histologic type was 5 cases (35.7%) for leiomyosarcoma, 5 cases (35.7%) for endometrial strornal sarcoma and 4 eases (28.6%) for mixed Mullerian tumor, 2. The mean age and yarity were 50.8 years and 3.1. 3. The most common syrrlptorn was irregular vaginal bleeding (64.3%), and lower abdominal pain (21.4%), abdominal palpable mass (14.3%) in order of frequency. 4. The distribution by YIGO clinical atage was 35.7% for stage I, 35.7% for stage II, 7.2% for stage IE and 21.4% for stage lV. The average survival time of each stage of disease was decreased with increasing stage. 5. The mean survival time was decreased with inereasing numbers of mitotic figure per 10 high power fields. 6. The mean survival time according to histologc type was 14.5 months for leiomyoaarcoma, 21.5 months for endometrial stromal marcoma, 5.8 months for malignant mixed Mullerian tumor, respectively.

2.Stercoral Colitis Leading to Fatal Peritonitis and Systemic Inflammatory Response Syndrome.

The stercoral colitis is an inflammatory process involving the colonic wall related to fecal impaction. This rare condition usually has a poor prognosis. We experienced a death case where an 84 years old woman died of stercoral colitis complications after total knee arthroplasty surgery. The fatal complications were peritonitis with systemic inflammatory response syndrome, not accompanying bowel perforations. We would like to show the autopsy case of stercoral colitis and suggest the importance of early suspicion and treatment.

3.Obstetric Outcomes in Patients with Systemic Lupus Erythematosus.

OBJECTIVE: To analyze the effect of clinical and serological variables of SLE on pregnancy outcome and to analyze the effects of pregnancy on the disease course of SLE. METHODS: We studied retrospectively about 91 pregnancies in 41 female patients with SLE, who had visted to Kangnam St Mary Hospital from January,1990 to May,1996. We divided the patients into two groups, who had been pregnant before SLE was established versus who were pregnant after SLE had been established. We considered the former as control group. We compared the fetal or maternal outcomes after divided the latter into subgroups according to our purposes such as cases with lupus flare versus without lupus flare, autoantibody (+) cases versus (-) cases, cases with renal disease versus without renal disease. RESULTS: The rate of fetal loss, prematurity was 19. 7%, 46. 9% respectively in 63 cases of 32 patients who were pregnant after SLE had been estabished. The number of pregnancy loss and premature delivery was higher in pregnancy after SLE was established than before SLE was established. Lupus flare was associated with the positivity of antibody to ds DNA and negativity of antibody to Ro. The frequency of fetal loss was higher in pregnancies of antiphospholipid (+) women than those of antiphospholipid (-) women. Pregnancy with renal involvement was associated with short duration of gestation and small body weight of neonate. CONCLUSION: Lupus pregnancy remains highly risky from a maternal standpoint in terms of increased lupus activity and from fetal outcome standpoint in terms of fetal loss and preterm birth, especially in the antiphosphospholid positive mother.

4.The Relationship between Serum Lipid Levels and Suicidality in Suicidal Attempters.

OBJECTIVES: Many studies have demonstrated that suicide is related to low serum cholesterol level, whereas conflicting results have also reported. The aims of the present study are to determine whether suicidal attempters have low lipid concentration and to identify the relationship between suicidal attempt severity and serum lipid levels. METHODS: Subjects were 50 suicidal attempters who visited the emergency room in Korea University Medical Center between July 1998 and June 1999. All subjects had been interviewed by psychiatrist and evaluated with Risk-rescue rating, HDRS and BPRS. They were diagnosed as major depressive disorder (n=29), personality disorder (n=19), and schizophrenia (n=2). Serum lipid levels in 50 suicidal attempters were compared with those in 50 nonsuicidal psychiatric inpatients corresponding to the age, sex and diagnosis of the suicidal attempters. We also examined the Spearman's rank correl-ations between the serum lipid levels and risk-rescue score. RESULTS: The serum total cholesterol level (t=-3.29, p=.001), total lipid level (t=-2.62, p=.01), and LDL level (t=-2.64, p=.011) in suicidal attempters were significantly lower compared with non-suicidal controls. In major depressive patients, total cholesterol level, total lipid level, and LDL level in suicidal attempters were significantly lower than those of nonsuicidal controls. In personality disorder patients, however, only serum total cholesterol level was significantly lower. Risk-rescue rating score was negatively correlated with serum total cholesterol level (t=.-293, p=.039), and positively corre-lated with BPRS (t=.544, p< 001), and HDRS (t=.488, p=.001). CONCLUSION: The lipid levels in suicidal attempter were significantly lower. Suicidal attempt severity was also significantly correlated with serum total cholesterol level. These findings suggest that low serum cholesterol should be related with severe violent suicidal attempt.

6.Gastric Necrosis after Gastric Dilatation in a Patient with Bulimia.

Acute gastric dilatation leading to gastric necrosis is rare but potentially fatal condition that can occur in patients with bulimia. It usually develops after a bulimic episode and it is not diagnosed with sufficient rapidity it may lead to gastric perforation followed by peritonitis, sepsis, shock, and death. Because of the rarity of this condition and the patients in whom it occurs, the clinician must maintain a high degree of suspicion when treating patients with eating disorders who present with abdominal pain after a binge. This case report describes a 24-year old woman with acute gastric dilatation due to binge eating, who had the gastrotomy and died of complications such as gastric necrosis and disseminated intravascular coagulation.

7.Duodenal Lymphangioma.

A lymphangioma is comparatively rare in the elimentary tract, and is a benign tumor of the lymphatic system. A duodenal lymphangioma is especially rare, and has an obscure etiology. Clinically, and in the laboratory the are non-specific, and are incidentally found by endoscopy, and their treatment is total excision. Recently, the authors experienced a 45 year-old woman, who had a duodenal lymphangioma, but with no symptom. An endoscopic examination revealed a single, about 2 cm, polypoid lesion below the ampulla. The lesion was successfully excised after a duodenotomy.

8.A Case of DiGeorge Syndrome with Metopic Synostosis.

PURPOSE: We report a patient with DiGeorge syndrome who was later diagnosed as mild metopic synostosis and received anterior 2/3 calvarial remodeling. METHODS: A 16-month-old boy, who underwent palatoplasty for cleft palate at Chungnam National University Hospital when he was 12 months old of age, visited St. Mary's Hospital for known DiGeorge syndrome with craniosynostosis. He had growth retardation and was also diagnosed with hydronephrosis and thymic agenesis. His chromosomal study showed microdeletion of 22q11.2. On physical examination, there were parieto-occipital protrusion and bifrontotemporal narrowing. The facial bone computed tomography showed premature closure of metopic suture, orbital harlequin sign and decreased anterior cranial volume. The interorbital distance was decreased(17mm) and the cephalic index was 93%. RESULTS: After the correction of metopic synostosis by anterior 2/3 calvarial remodeling, the anterior cranial volume expanded with increased interorbital distance and decreased cephalic index. Fever and pancytopenia were noted at 1 month after the operation, and he was diagnosed as hemophagocytic lymphohistiocytosis by bone marrow study. He however, recovered after pediatric treatment. There was no other complication during the 12 month follow up period. CONCLUSION: This case presents with a rare combination of DiGeorge syndrome and metopic synostosis. When a child is diagnosed with DiGeorge syndrome soon after the birth, clinicians should keep in mind the possibility of an accompanying craniosynostosis. Other possible comorbidities should also be evaluated before the correction of craniosynostosis in patients as DiGeorge syndrome. In addition, postoperative management requires a thorough follow up by a multidisciplinary team of plastic surgeons, neurosurgeons, ophthalmologists and pediatricians.

9.Fatal Rectal Bleeding after Prostate Biopsy.

Transrectal ultrasound (TRUS)-guided biopsy of the prostate is a common outpatient procedure for diagnosing prostatic cancer. It is a relatively noninvasive, safe, and effective method, as the majority of post-biopsy complications are mild and self-limiting. Fatal complications including infection or massive hemorrhage are rare, but can be life threatening. We herein present an autopsy case of fatal rectal bleeding after prostate biopsy and an analysis of similar cases.

The incidence of human immunodeficiency virus (HIV) infections continue to increase throughout the world. Although neurologic complications are frequent in individuals with HIV infection or acquired immunodeficiency syndrome (AIDS), vestibulocochlear neuritis is still a relatively rare manifestation. We report the first case of vestibulocochlear neuritis occurring in an AIDS patient in Korea.
