1.The Results of Benign Prostatic Hyperplasia Treatment by Transurethral Resection, Open Prostatectomy, and TUMT(Transurethral Microwave Thermotherapy).

We noticed the treatment results of each of them after having treated the seventy-nine cases of benign prostatic hyperplasia with TUMT (N=32, Term ; 92.6-93.3), TUR-P (N=31, Term; 91.4-93.3), open prostatectomy (N=16, Term; 91.4-93.3). However, there was a difficulty to get to statistical reliability, for there was a little gap between the term of survey and the period of treatment of three groups. The results are following. In the TUMT, the maximal flow rate was improved from 8.2ml/s to 13.3ml/s and Madsen score dropped from 13.6 to 5.3, in the TUR-P they were 8.4ml/s to 16.5ml /s and 14.5 to 5.3 respectively, and in the open prostatectomy 6.8ml/s to 20.0ml/s and 15.8 to 5. 3 respectively. The degree of satisfaction of the patients themselves with each prescription was 46.9% in the TUMT. 67.7% in the TUR-P and 81.2% in the open prostatectomy. But 18.8% and l6.l% of the patients thought TUMT and TUR-P respectively ineffective. The conclusion is that open prostatectomy presented the better results than TUMT or TUR-P in the degree of satisfaction of patients and uroflowmetry during the survey period. And even though TUMT have presented lesser degree of satisfaction then TUR-P up to now, if the exact indications for the use of TUMT are found and the mechanical improvements undergo, then a number of benign prostatic hyperplasia cases seem to be treated in more safety and with lesser cost and time by TUMT instead of TUR-P.

The pelvic abscess communicating with the appendix is relatively rare, however, it could be diagnosed preoperatively by various diagnostic modalities such as ultrasonography, computed tomography or laparoscopy. We experienced a case of pelvic abscess presented as a pelvic mass invading vesical wall, and report this unusual presentation with a brief review of literature.

5.Clinical Features and Prognosis of Patients Who Started Hemodialysis Treatment over 60 Years Old.

BACKGROUND: Observation on the hemodialysis treatment; whether there are any different effects on mortality rate, side effects, or effectiveness of the treatment depending on patients' age. METHODS: Chronic renal failure patients who started hemodialysis treatment between 2002 and 2003 were separated in two groups, 33 patients of 60 and older and 76 patients under 60. We investigated mortality rate difference, A-V shunt re-operation, and total protein and albumin level. Also, comparing and analyzing the urea reduction ratio, we examined whether effectiveness is different depending on the patients' age. RESULTS: The 3 year survivor rate of patients who initially started hemodialysis treatment over 60 was 66.7%, and that of the patients under 60 resulted as 73.1%. The average total protein lefel for over 60 was 6.48g/dL and for under 60 was 6.80g/dL, and albumin for over 60 was 3.15g/dL and under 60 was 3.46g/dL. We identified that the patients who repeated shunt operation in over 60 group was 4(12%) and in under 60 group was 4(5.2%). Effectiveness of the treatment, Urea Reduction Ratio(URR), for both groups was 71%. CONCLUSION: The 3 year survivor rate of patients who initially started hemodialysis treatment over 60 was lower than the control group, but considering the remaining life expectancy, it seems to be no significant difference between the groups; and the treatment effectiveness was same in both groups.

6.Interference of Detection Rate of Lumbar Disc Herniation by Socioeconomic Status.

STUDY DESIGN: Retrospective study. PURPOSE: The objective of the study is to evaluate the relationship between the detection rate of lumbar disc herniation and socioeconomic status. OVERVIEW OF LITERATURE: Income is one important determinant of public health. Yet, there are no reports about the relationship between socioeconomic status and the detective rate of disc herniation. METHODS: In this study, 443 cases were checked for lumbar computed tomography for lumbar disc herniation, and they reviewed questionnaires about their socioeconomic status, the presence of back pain or radiating pain and the presence of a medical certificate (to check the medical or surgical treatment for the pain) during the Korean conscription. RESULTS: Without the consideration for the presence of a medical certificate, there was no difference in spinal physical grade according to socioeconomic status (p=0.290). But, with the consideration of the presence of a medical certificate, the significant statistical differences were observed according to socioeconomic status in 249 cases in the presence of a medical certificate (p=0.028). There was a lower detection rate in low economic status individuals than those in the high economic class. The common reason for not submitting a medical certificate is that it is neither necessary for the people of lower socioeconomic status nor is it financially affordable. CONCLUSIONS: The prevalence of lumbar disc herniation is not different according to socioeconomic status, but the detective rate was affected by socioeconomic status. Socioeconomic status is an important factor for detecting lumbar disc herniation.

7.Role of Transvaginal Ultrasonography in Stress Urinary Incontinence.

Hypermobility of the bladder neck in response to increased intraabdominal pressure is the anatomical cause of female stress urinary incontinence ( SUI) and the degree of bladder neck movement has been used to classify SUI patients and to guide management decisions. We performed transvaginal ultrasonography in 14 SUI patients and 20 normal female as control to assess its role on the demonstration of the anatomical features associated with SUI. We adopted three anatomical factors in the sagittal plane which affect the bladder neck mobility , vertical (JY) and horizontal(JX) distance difference between pubic symphysis and bladder neck, and rotation angle difference(JZ) composed of by pubic symphysis and bladder neck during rest and stress states. There were significant differences in Y and Z between the two groups. Three months after corrective surgery for SUI these two factors showed significant improvement in all patients. It could be concluded that transvaginal ultrasonography is a safe and reliable method to diagnosis and evaluate the postoperative outcome for SUI.

8.Clinical Correlations between Transrectal Ultrasonographic Findings of the Seminal Vesicles and Ejaculatory Ducts, and Symptoms.

Transrectal ultrasonography provides an accurate assessment of the prostate, ejaculatory ducts and seminal vesicles, which can be helpful in the evaluation of the patients with variable genitourinary symptoms. We studied 65 symptomatic patients and 20 age-matched controls who underwent transrectal ultrasonography from Feb. 1993 to Jul. 1993. The broad spectrum of genitourinary signs and symptoms was segregated into 4 groups. Group 1 was hematospermia. Group 2 was pains including ejaculatory pain, pain on erection. perineal pain, scrotal pain, tenderness on prostate grand, suprapubic pain, lower back pain, inguinal pain, and penile pain. Group 3 was bladder outlet obstructive symptoms including hesitancy, intermittancy, terminal dribbling, weak stream, and residual urine sensation. Group 4 was bladder irritative symptoms including frequency, nocturia, urgency, and dysuria. The major abnormal findings on the transrectal ultrasonography were cysts in seminal vesicle, cysts in ejaculatory duct, prostatic calculi, ejaculatory duct calculi, and seminal vesicle dilation. Compared with age-matched controls, symptomatic patients had significantly increased prevalence of the abnormal findings on the transrectal ultrasonography (p <0.01 ). Ejaculatory duct calculi were significantly associated with bladder outlet obstructive symptoms and bladder irritative symptoms (p <0.05), and seminal vesicle dilation was also significantly associated with pain (p <0.05). Prostatic calculi and ejaculatory duct calculi were closely associated with voiding dysfunction, however, cyst in seminal vesicle and ejaculatory duct were closely associated with pain. Transrectal ultrasonography offers clinical insights into the causes of distressing genitourinary symptoms, and may be helpful in their management.

9.A Case of Undifferenciated Large Cell Carcinoma of Lung Associated with Neurofibromatosis.

Neurofibromatosis also referred to as von Recklinghausen`s disease, is an autosomal dominant disease, which is characterized by cutaneous neurofibromas, cafe-au-lait spot, and axillary freckles. Although neruofibromatosis is a congenital disorder, the pulmonary manifestation become evident in adulthood. Approximately 15percent of patients with cutaneous lesions have intrathoracic neurofibromas. In some patients with neurofibromatosis, the lungs are the seat of interstitial fibrosis, leiomyoma, and bullous lesion, meningocele or less commonly lung cancer. The association of neurofibromatosis and lung cancer is rare, A 47-year-old man, who was a heavy smoker, was admitted to our hospital because of hemoptysis. Since cafe-au-lait sopts, diffuse pigmentation and multiple neurofibromatosis were observed on the skin, he was considered to have von Recklinghausen`s disease. Chest X-ray film showd extensive hazy density affecting all of his lung field, and minimal peribronchial infiltratin in his left lower lung field. Chest CT showed that near total consolidative lesions were present of right middle lobe. Also, some patchy infiltration were present in his right lower lobe and some of left lower lobe. Histoathological examination of the percutaneous lung biopsy specimen demonstrated that he had undifferentiated large cell carcinoma. The Authors have experienced a case of neurofibromatosis combined with undifferntiated large cell carcinoma. A brief review of related literature was conducted.

10.Hepatic Failure Due to Hepatitis E Virus Infection in a Patient with Necrotic Hepatocellular Carcinoma

In patients with hepatocellular carcinoma (HCC) or liver cirrhosis (LC) accompanied by hepatitis E virus (HEV) infection, hepatic failure often leads to debility. Here, we report about a 63-year-old man with alcoholic LC who was referred to our hospital with jaundice and abdominal distension 10 days earlier. Abdominal computed tomography showed necrotic HCC accompanied by left lobe shrinkage without tumor progression. Laboratory and imaging findings revealed no acute infection focus. The patient reported no herbal medicine or alcohol consumption, and there was no evidence of acute viral hepatitis. One month later, HEV immunoglobulin M positivity was confirmed, and deterioration of liver function due to HEV infection was suspected. The patient often ate raw oysters and sashimi, as well as boar meat, which is a well-known risk food for HEV infection. His umbilical hernia deteriorated due to tense ascites and infection by skin abrasion. The patient progressed to hepatorenal syndrome and eventually died. Liver function preservation is important when treating HCC patients. Therefore, clinicians should pay more attention to the prevention of HEV and others causes of direct liver injury.
