3.A Case of Localized Darier's Disease.

Localized Darier's disease (DD) is a rare variant of DD. The disease is characterized by multiple hyperkeratotic papules in a unilateral, linear, zosteriform or Blaschkoid distribution with the histological features of classical DD. Unlike DD, which presents as a generalized condition, localized DD lacks family history and other clinical findings suggestive of DD such as distinctive nail abnormalities and keratotic papules on the palms and soles. Herein, we describe a case of localized DD in a 31-year old Korean man on the perianal area that was treated with topical retinoid cream.

4.Habitual Thumbsucking-induced Koilonychia in Infancy.

Koilonychia is dystrophy characterized by eversion of the marginal portion of the nail. As a result, the central portion of the nail is concave. It has been associated with systemic or skin-related conditions. Hereditary koilonychia is inherited as an autosomal dominant trait. Koilonychia can also be caused by exogenous factors such as certain toxic chemicals or repeated mechanical trauma. In this case, koilonychia on the fingernail of the left thumb was caused by repeated trauma resulting from continuous suction since birth. Herein, we report a rare case of koilonychia induced by habitual thumbsucking.

6.A Case of Congenital Ectopic Toenail.

A congenital ectopic nail is a rare anomaly and usually manifests itself as a small abnormal nail appearing alongside the normal nail. In most of the cases, abnormal nails have been found on the palmar surface of the fifth finger. The pathogenesis of the ectopic nail remains unclear. It has been postulated to be a kind of polydactyly or generation from stray germ cell or inoculation of the nail matrix after trauma. We observed a case of a 5-month-old boy with congenital ectopic toenail on the right second toe. The ectopic toenail was removed by surgical excision and the histopathological finding was consistent with ectopic toenail. We report a case of congenital ectopic toenail on the right second toe of a 5-month-old boy.

8.Nonsurgical Nail Avulsion in treatment of Onychomycosis.

The important prerequisite for treatrnent of onychomycosis is the removal of as rnuch fungus-infected nail material as possible. Nincty-five patients with anychomycosis were treated with using urea preparations under occlusive dressings. The urea preparations were quite successful in maceraing the nail plate from the nail bed, allowing the easy removaI of diseased nails in all cases except one. The urea preparations removed only abnormal nail. Completely normal nail was unaffected. After nail avulsion, 95 patients were treated for 6 months with combination clotrimazole and ketoconazcile(group A), or with clotrimazole only(group B). Of the 38 patients in group A who complcted the treatment, 25(65.8%) were cured 6 months later by clinical and mycological criteria, compared to 20(47.6%) of 42 patieries in group B.

Pterygium inversum unguis (PIU) is a rare nail abnormality in which the distal nail bed adheres to the ventral surface of the nail plate, with obliteration of the distal groove. Because of the rarity of this condition, its exact origin is unknown. This disorder can be either congenital or acquired, with or without a family history. The acquired forms may be idiopathic or secondary to systemic connective tissue diseases or other causes such as stroke, neurofibromatosis, leprosy, or the use of nail fortifiers. We present an unusual case of acquired idiopathic PIU of the 10 fingernails in a 22-year-old man.

10.Are Psoriatic Nails Predisposed to Fungal Infection?.

BACKGROUND: Onychomycosis is a very common and widespread disorder, however the incidence of onychomycosis in psoriatic patients is not accurately assessed. OBJECTIVE: : The present study was undertaken to evaluate the frequency of fungal infections of the nails changed in the course of psoriasis and to provide an answer whether psoriatic nails are specially predisposed to fungal infections. MATERIAL AND METHODS: 83 psoriatic patients with nail changes participated in the study. Among them there were 25 females and 58 males, aged 18~76 years. 59 patients (71.1%) had psoriasis vulgaris, the remaining ones - 24 subjects (28.9%) suffered from arthropathic psoriasis. In any case of nail abnormalities clinically suspected of fungal infection the further mycological investigations were performed. Fungal infections were finally diagnosed on the base of direct microscopy and positive culture. The Chi2 test and the Mann-Whitney U-test were used for statistical analysis. RESULTS: Positive mycological cultures were obtained from 15 patients (18%). The most commonly isolated fungi were moulds cultured in 6 patients (37%), followed by dermatophytes - 5 patients (31.5%) and yeast-like fungi - also 5 subjects (31.4%). Moulds and yeast-like fungi were found in both fingernails and toenails; all dermatophyte infections were diagnosed on the toenails. Psoriatic patients with fungal nail infections were older than those with negative mycological examination and the duration of psoriasis in this group of patients was also longer. Although the above differences between two studied groups were evident, they did not reach the statistical significance. CONCLUSIONS: Based on the achieved results it is difficult to assess definitely whether psoriasis is a predisposing factor to the development of fungal infections of the nails.
