1.The value of spectrum CT in the diagnosis of lung cancer and lymph node metastases

Objective To analyze the change of CT value,iodine (water) density in lung cancer and lymph node metastases under different single-energy,and investigate the clinical value of spectrum CT in the diagnosis of lung cancer and lymph node metastases.Methods The clinical and pathological diagnosis of 42 patients with lung lesions,the clinical data of spectrum CT imaging were retrospectively analyzed.Nineteen cases with benign diseases (benign group),23 cases with lung cancer(lung cancer group).Results Thex2 value of spectrum CT imaging in the diagnosis of lung lesions and lymph node metastases was 26.1 and 153.8,there was no significant difference(P > 0.05).Malignant lung lesions lower keV,the greater difference in CT value.The arterial phase standard iodine density in benign group was lower than that in lung cancer group [(0.13 ± 0.08) g/L vs.(0.20 ± 0.15) g/L],there was significant difference (P < 0.05).The arterial phase standard water density between benign group and lung cancer group had no significant difference [(1.01 ± 0.01) g/L vs.(1.00 ±0.01) g/L] (P> 0.05).Spectrum curve slope in lymph node metastases and lung cancer had no significant difference (1.10 ± 0.07 vs.1.07 ± 0.08)(t =1.83,P > 0.05).The spectrum curve slope of squamous carcinoma,adenocarcinoma and small cell lung cancer under different energy imaging had no significant difference (P > 0.05).Conclusion Spectrum CT has obvious advantages in detection of lung cancer and lymph node metastases,and its quantitative analysis is helpful in the diagnosis and differential diagnosis of benign and malignant lung tumor and lymph nodes,in the differential diagnosis of clinical difficult to diagnosis lesions has significant value.
2.Application value of multi-slice spiral CT angiography in liver cancer patients interventional treatment

Objective To investigate the application value of multi-slice spiral CT angiography (MSCTA) in liver cancer patients interventional treatment.Methods Fifty-four liver cancer patients were selected,all patients underwent MSCTA examination,after the image post-processing,and with 17 cases of patients with the same period digital subtraction angiography (DSA) examination results for comparison,in order to analyze the application value of MSCTA in liver cancer patients interventional treatment.Results The patients with liver cancer were seen MSCTA arterial phase varying degrees of tumor staining,2 cases had hepatic arteriovenous fistula.Some patients in the portal vein and hepatic vein were found vein tumor thrombus.MSCTA showed 10 cases of hepatic artery variation,variation rate of 18.5% (10/54),in line with the DSA rate of 100.0%.MSCTA showed 11 rami liver cancer parasitic artery,1 case of DSA was not found for the right phrenic artery of liver cancer,there was no significant difference in showed liver cancer parasitic artery between the two methods (P > 0.05).Conclusion MSCTA as a minimally invasive diagnostic method of tumor blood supply,can clearly show the origin of the hepatic artery and its branches and walk the line,liver cancer feeding artery,liver cancer parasitic artery with DSA showed highly consistent results,interventional treatment of liver cancer with important guiding significance.
3.Application and nursing of contrast medium on displaying pathological changes of gastric cancer by spectral CT

Objective To investigate the nursing highlight of different contrast medium to detect gastric lesions by spectral CT.Methods A total of 150 patients with gastric cancer were averagely divided into experimental group and control group according to coin toss.The 20 ml iohexol mixed with 1 500 ml warm water were used in the experimental group,but only oral warm water was used in the control group.The monitored result of gastric cancer,adverse reactions and appropriate care were evaluated.Results The corrective rate of spectral CT for gastric cancer detection in the experimental group and control group were 96.0% and 85.3% respectively,and the difference was significant (x2 =5.042,P < 0.05).Meanwhile,the proportion of adverse reactions in the experimental group and control group were 10.7% (8/75) and 6.7% (5/75),but the difference was not statistically significant (x2 =0.785,P > 0.05).However,73.3% patients in the experimental group underwent anxiety or depression,which was significantly higher than that in the control group (45.3%),with a statistically significant difference (x2 =12.185,P < 0.01).Conclusions Iohexol can be used as a good developer spectrum of spectral CT,which may also improve the detection rate of gastric cancer.Meanwhile,corresponding nursing is also important during the process.
4.AppLication of comprehensive nursing care in enhanced CT scan

bjective? To expLore the effects of comprehensive nursing care on adverse reactions of contrast media in enhanced CT scan. Methods? From November to December 2016, we seLected 4 413 patients with enhanced CT scan in Cancer HospitaL of Peking Union MedicaL CoLLege. ALL of the patients were divided into controL group and observation group with the random number tabLe. There were 2 010 cases in controL group with routine nursing, whiLe there were 2 403 patients in observation group with comprehensive nursing. We observed the adverse reactions and satisfaction with nursing among patients with enhanced CT scan between two groups. ResuLts? The incidence of adverse reactions of observation group was Lower than that of controL group;the patients' satisfaction with nursing of observation group was higher than that of controL group; the differences were aLL statisticaL (P<0.05). ConcLusions? Comprehensive nursing care can reduce the incidence of adverse reactions of contrast media and improve satisfaction with nursing among patients with enhanced CT scan.
