1.Canalicular inflammatory etiology and the common misdiagnosis study

AIM:To analyze the 34 cases of patients with diagnosis of lacrimal duct disease, to explore the cause and clinical characteristics of lacrimal duct inflammation, and to study the common misdiagnosis to improve the diagnosis rate, to provide clinical experience for medical workers.
METHODS: A total of 34 eyes of 34 patients were selected from 2011-06 to 2015-06 to our hospital for lacrimal duct incision. Clinical data of patients including age and gender distribution, eye and lesion location, duration, canaliculitis clinical symptoms and previous misdiagnosis were collected. The operation of lacrimal duct was carried out, and the secretion of lacrimal secretion was checked.
RESULTS:Patients over the age of 40 accounted for 77%of the population, men accounted for 23. 5%, women accounted for 76. 5%. Left eye in 15 cases, right eye in 19 cases, and were monocular onset, 26 cases ( 76%) of patients with lower lacrimal duct inflammation. Clinical symptoms: increased secretion, canthal conjunctival congestion, swelling, lesion location around the lacrimal puncta swelling and tenderness, slightly raised, epiphora, foreign body sensation. The culture results Showed 34 cases of discharge inspection. A total of 20 cases were fungal growth, 6 cases of bacterial growth rate were 59%, 18%, and 8 cases were not detected, 10 cases of lacrimal duct stone were found by pathological examination. There were 3 cases of Aspergillus, 5 cases of Candida, 2 cases of actinomycetes, 4 cases found in Gram staining microscope. Misdiagnosis of canaliculitis for other eye diseases, including 12 cases of chronic dacryocystitis, 8 cases of chronic conjunctivitis, 6 cases of acute conjunctivitis, stye in 4 cases, the misdiagnosis rate reached 88%.
CONCLUSION:In the middle and old aged people, the lacrimal duct is more than that of the male. The main pathogenic bacteria causing lacrimal duct infection may be fungi, which can be treated with anti-fungal drugs. For the removal of the lacrimal duct stone, we should distinguish the types of pathogenic bacteria for the treatment of the disease. In the case of misdiagnosis, the clinical manifestations of different clinical manifestations from the lacrimal duct infection and other diseases of the eye should be carefully examined and carefully treated.
2.Acupuncture master Xu Yi-nian and his Practical Acupuncture in the Republic of China.

By collecting and studying Practical Acupuncture written by XU Yi-nian, Guangdong acupuncture master in the Republic of China, and using literature methodology, the life story of XU Yi-nian is textually researched and his acupuncture characteristics is analyzed. The results indicate that XU Yi-nian emphasizes on the utility of acupuncture manipulations and acupoint selection, the application of folk experiences in moxibustion and Sha disorders. He pays attention to the co-work of acupuncture and medicine and his work collects the therapeutic experiences of different schools and deserves to be further explored and validated.

3.Brief discussion on the life of ZENG Tian-zhi, a follower of Chengjiang acupuncture school.

ZENG Tian-zhi, as one of the disciples of Mr. CHENG Dan-an, is considered as one of the major successors of the Chengjiang acupuncture school in the South of the Five Ridges. By collecting Mr. ZENG's works and papers as well as the interviews with his followers, his life and achievements are verified and his existing representative works are introduced. It is found out that following CHENG Dan-an's steps, ZENG Tian-zhi has practiced the medicine and performed great deeds, established schools and taught students. Also he established the first educational institution of acupuncture in Hong Kong, namely College of Scientific Acupuncture. He has traveled to many places to promote the acupuncture and cultivate a large number of acupuncturists. His works are characterized by organized edition, bringing in modern medical theory and focusing on clinic applicability.

4.Correcting effect and visual quality after LASlK and lCL implantation in high myopia patients

?AIM: To study correcting effect and visual quality after laser - assisted in situ keratomileusis ( LASIK ) with femtosecond and posterior chamber intraocular lens ( ICL) implantation in high myopia patients.?METHODS: Fifty-five patients ( 106 eyes ) with high myopia from February 2012 to February 2015 in our hospital were analyzed. According to the different operation, patients were divided into the observation group( using ICL implantation, 27 cases with 53 eyes) and the control group (using LASIK, 28 cases with 53 eyes). Postoperative follow-up was 1a, to observe and analyze the visual quality, higher order aberration and complications of two groups.?RESULTS: Uncorrected visual acuity ( UCVA ) , the best corrected visual acuity ( BCVA ) , effectiveness index and security index at 1a postoperatively of observation group, were 1. 04±0. 86(LogMAR), 0. 97±0. 19(LogMAR), 104. 69± 18. 56, 108. 79 ± 17. 68, significantly higher than those of control group 0. 78 ± 0. 11 ( LogMAR ), 1. 04 ± 0. 09 (LogMAR), 93.78±15. 65, 100. 71±11. 68 (P<0. 05). And observation group in the two kinds of light and shade environment at various spatial frequency contrast sensitivity were higher than the control group. Those under the light environment at 1. 5, 3, 6, 12 c/d and under dark environment at 1. 5, 3, 6, 18 c/d compared were different between the two groups (P<0. 05). Spherical aberration and comatic aberration of observation group at 1a after operation were lower than those of control group (P<0. 05 ). The difference of trefoil between the two groups was not significant ( P > 0. 05 ). No severe complications were observed in both groups.?CONCLUSION: LASIK with femtosecond and ICL lens implantation can effectively improve the patient’s visual quality, but for patients with high myopia, ICL lens implantation effect is more significant, the safety index of ICL implantation, as well as the effectiveness index and the visual quality are better than those of LASIK.

6.The screening model for dopamine receptor agonists by a dopamine sensor

Parkinson's disease (PD) is a degenerative disease of the central nervous system due to the loss or death of dopaminergic neurons in the substantia nigra. Clinically, levodopa is the most effective and commonly used drug for PD treatment. However, long-term levodopa therapy is prone to motor complications and other side effects caused by excessive peripheral dopamine production, which has become an urgent problem to be solved in PD treatment. Dopamine receptor (DR) agonists are similar to dopamine. They can directly stimulate postsynaptic dopamine receptors, produce the same effect as dopamine, delay the application of levodopa as much as possible, and reduce complications caused by long-term use of levodopa. Therefore, screening effective dopamine receptor agonists has become a key issue in the study and treatment of PD. In order to establish a rapid, stable and reliable method for dopamine receptor agonist screening, this study used the human dopamine receptor 2 (DRD2) gene fused with a circular permuted EGFP (cpEGFP) to construct a recombinant gene, packaged with lentiviral vector, and the vector replaced the parted inner transmembrane domain of the third intracellular loop (ICL3) of genetically-encoded GPCR-activation based (GRAB) sensors. The fluorescence of GPCR-fused cpEGFP is regulated by conformational changes mediated by the interaction of dopamine receptor agonists with GPCRs without altering GPCR activity. The HEK293T cells were infected with viral vector, screened by puromycin to select highly expressed cells. Dopamine receptor agonists (including dopamine, bromocriptine mesylate, cabergoline, pramipexole) were used as positive drugs to explore the best screening and detection conditions, establishing a stable model to evaluate the dopamine receptor agonist. The results showed that the optimal filter for the dopamine receptor agonist in this study was the cell seeding count of 7×104, and the effective concentration of the positive drug was 1-100 µmol·L-1. In addition, pretreated with 10 µmol·L-1 dopamine receptor antagonists (including chlorprothixol hydrochloride, domperidone, and sulpiride), the positive fluorescence signal of overexpressed DRD2-cpEGFP HEK293T cells could not be detected when exposed to 10 µmol·L-1 dopamine receptor agonists, which proved that dopamine receptor antagonists could block the activity of dopamine receptor agonists, so they cannot activate dopamine receptor allosteric, indicating that the model has good specificity and can also be used for the screening and detection of new dopamine receptor antagonists. In summary, the study constructs a stable dopamine sensor detection system, which can effectively screen potential dopamine receptor agonists. The operation procedures are simple and rapid. And it can be used for a large-scale screening providing a fundamental methodology for drug development and PD treatment targeted on DRD2.

Comprehensiveness of care is one of the core attributes of primary health care, and it has a great impact on the first diagnosis,accessibility,coordination and continuity to form the primary health care high perform-ance mechanism. Comprehensiveness of care has been playing an increasingly significant role in case when faced with the challenges of the aging of Chinese population,the high prevalence of chronic diseases,and the rapidly rising cost of health care. It is a fundamental problem for researchers,policy makers,and practitioners to clarify the definition of comprehensiveness of primary health care,which is the basis for managing comprehensive services. This article has systematically reviewed the current researches on definitions of comprehensiveness of basic health care, ascertained the key elements of comprehensiveness,and pinpointed the relationship between the dominant and recessive elements. Then,it defined the conception and the boundary of comprehensiveness of care in the context of primary health care,ac-cording to three principles-controllability, exhaustion, and exclusiveness, and finally conceptualized the primary health care comprehensiveness for further study on the operational definition of primary health care attributes.
8.Effect of phenylephrine on alveolar fluid clearance in ventilator-induced lung injury.

OBJECTIVETo investigate the effect of phenylephrine (an α-adrenergic agonist) on alveolar fluid clearance (AFC) in ventilator-induced lung injury and the possible mechanism involved.
METHODSA total of 170 male Wistar rats were randomly allocated into 17 groups (n=10) using random number tables. Short-term (40 minutes) mechanical ventilation with high tidal volume (HVT) was performed to induce lung injury, impair active Na+ transport and lung liquid clearance in the rats. Unventilated rats served as controls. To demonstrate the effect of phenylephrine on AFC, phenylephrine at different concentrations (1×10(-5), 1×10(-6), 1×10(-7), 1×10(-8), and 1×10(-9) mol/L) was injected into the alveolar space of the HVT ventilated rats. To identify the influence of adrenergic antagonists, Na(+) channel, and microtubular system on the effect of phenylephrine, phenylephrine at 1×10(-5) mol/L combined with prazosin (an α1-adrenergic antagonist, 1×10(-4) mol/L), yohimbine (an α2-adrenergic antagonist, 1×10(-4) mol/L), atenolol (a β1- adrenergic antagonist, 1×10(-5) mol/L), ICI-118551 (an β2-adrenergic antagonist, 1×10(-5) mol/L), amiloride (a Na+ channel blocker, 5×10(-4) mol/L), ouabain (a Na(+)/K(+)-ATPase blocker, 5×10(-4) mol/L), colchicine (a microtubular disrupting agent, 0.25 mg/100 g body weight), or β-lumicolchicine (an isomer of colchicine, 0.25 mg/100 g body weight) were perfused into the alveolar space of the rats ventilated with HVT for 40 minutes. AFC and total lung water content were measured.
RESULTSBasal AFC in control rats was (17.47±2.56)%/hour, which decreased to (9.64± 1.32)%/hour in HVT ventilated rats (P=0.003). The perfusion of phenylephrine at 1×10(-8), 1×10(-7), 1×10(-6), and 1×10(-5) mol/L significantly increased the AFC in HVT ventilated rats (all P<0.05). This effect of phenylephrine on AFC was suppressed by prazosin, atenolol, and ICI-118551 in HVT ventilated rats by 53%, 31%, and 37%, respectively (all P<0.05). The AFC-stimulating effect of phenylephrine was lowered by 33% and 42% with amiloride and ouabain, respectively (both P<0.05). Colchicine significantly inhibited the effect of phenylephrine (P=0.031).
CONCLUSIONPhenylephrine could increase the AFC in HVT-ventilated rats and accelerate the absorption of pulmonary edema.

9.Effect of beta3-adrenergic agonists on alveolar fluid clearance in hypoxic rat lungs.

BACKGROUNDRecent research suggests that beta(2)-adrenergic agonists increase alveolar fluid clearance (AFC) under physiologic and pathologic conditions. It is unknown whether beta(3)-adrenergic agonists also increase AFC under pathologic conditions. The aim of this study was to investigate the effect of beta(3)-adrenergic agonists on AFC following hypoxic lung injury and the mechanisms involved.
METHODSHypoxic rats were exposed to 10% oxygen. BRL-37344 (beta(3)-adrenergic agonist) or CGP-12177 (selective beta(3)-adrenergic agonist) alone or combined with beta receptor antagonists, sodium channel blockers, or Na(+)/K(+)-ATPase blockers were perfused into the alveolar space of rats exposed to 10% oxygen for 48 hours. Total lung water content (TLW) and AFC were measured.
RESULTSAFC did not change for the first 24 hours but then decreased after 48-hour exposure to 10% oxygen. The perfusion of BRL-37344 or CGP-12177 significantly increased AFC in normal and hypoxic rats. The AFC-stimulating effect of CGP-12177 was lowered with amiloride (a Na(+) channel blocker) and ouabain (a Na(+)/K(+)-ATPase inhibitor) by 37% and 49%, respectively. Colchicine significantly inhibited the effect of CGP-12177.
CONCLUSIONSThese findings suggest that beta(3)-adrenergic agonists can increase AFC during hypoxic lung injury in rats and accelerate the amelioration of pulmonary edema.

10.Correlation of lens density and phacodynamics parameters in phacoemulsification

Objective To evaluate the correlation of lens density (LD) measured by opacity lensmeter (OLM) and Pentacam with phacodynamics in patients with age-related nuclear cataract.Methods A prospective study was conducted in 78 patients (78 eyes) with age-related nuclear cataract.The lens density was measured with OLM and Pentacam respectively before operation,and the phacoemulsification kinetic parameters in phacoemulsification,including ultrasound time (UST),cumulated dissipated energy (CDE) and balanced salt solution (BSS) usage,were recorded for the analysis of the correlations of LD values with phacodynamics.Results LD measured by OLM and Pentacam was 32.83 ± 14.41 and 15.26 ±5.82,respectively.LD from OLM and Pentacam was positively correlated with both UST (r =0.645,0.807,respectively;both P < 0.05)and CDE (r =0.669,0.828,respectively;both P < 0.05).UST and CDE were more strongly correlated with LD from Pentacam than from OLM (Z =2.153,2.283;both P <0.05).Conclusion LD from both OLM and Pentacam are correlated with UST and CDE,and a stronger correlation is observed in Pentacam than OLM.
